수포성, 궤양성 및 백색구강점막질환

  • 협점막, 치은, 구순막, 혀, 경구개, 연구개 및 구강저등으로 구성된 구강점막은 다양한 형태의 외상성 자극, 면역반응 이상, 세균감염, 유해한 환경등으로 인해 여러가지 질환이 발생된다. 이러한 질환들은 만성적이고 다른 질환에 비하여 발생빈도가 낮으며 병인이 불확실한 특성을 가지고 있다. 구강점막에 발생되는 질환들은 다양하게 분류될 수 있지만 여기에서는 크게 수포성, 궤양성 질환과 백색 구강점막 질환으로 나누어 그중 비교적 호발하는 몇몇 질환들에 대해 기술한다.

Personality Type Test(MBTI) of Korean College Students with Symptoms of Oral Mucosa Disease (구강점막질환증상자의 성격유형검사(MBTI))

  • The purpose of this study is to investigate the relationship between personality type and symptoms of oral mucosa disease. 393 college students completed the MBTI(Myers-Briggs Type Indicator) and a questionnaire and collected data were analyzed by SAS 9.2 program. The obtained results were as follows : 1. Dysgeusia(p<0.05) and xerostomia(p<0.01) occurred significantly more frequently in I type than E type. 2. Herpetic stomatitis, recurrent aphthous ulcer, glossitis, dysgeusia, burning mouth syndrome and xerostomia seemed to occur more frequently in S type than in N type. 3. Most symptoms of oral mucosa disease seemed to occur the most frequently in the type including NF among 16 types of personality of MBTI. 4. Most subjects had negative attitude in curing their symptoms of oral mucosa disease(p<.0001). Significantly more subjects with I type than E type had negative attitude in curing herpetic stomatitis(p<0.01) and dysgeusia(p<0.05). 5. Most symptoms of oral mucosa disease appeared to occur the most frequently in nervous or sensitive subjects. Therefore a guideline for the individual cure of oral mucosa diseases meeting personality type is necessary.

종양성 병변으로 인한 구강점막질환

  • 구강점막에 발생하는 종양성 병변으로는 대표적으로 구강점막암(oral mucous membrane cancer), 유두종(papilloma),혈관종(hemangioma), 임파관종(lymphangioma),섬유종(fibroma)등이 있는데 이들 병변들은 구강내 협점막, 치은, 경구개, 연구개, 구강저, 혀 등에 궤양이나 증식성 병소로 나타나서 동통, 출혈, 치아동요, 종창 등의 증상을 유발시킨다. 이러한 병변들에 대한 명확한 치료와 예후를 결정하기 위해서는 무엇보다도 이들 병변들에 대한 정확한 소견에 근거한 진단이 우선되어야 할 것이다. 따라서 본 장에서는 정확한 진단과 치료를 위해 이들 병변들에 대해 소개하고자 한다.

Consideration for the Importance of Diseases Associated with Oral Medicine by Analyzing the Dental Cyber Consultation (사이버 치과 진료 상담을 통해 살펴본 구강내과 질환의 중요성에 대한 고찰)

  • The moderners of industrial society suffer from various stresses, which bring about increase in prevalence of temporomandibular disorders, oral mucosal disease and chronic neuropathic pain, therefore, the number of patients seeking help of those symptoms tend to increase. The purpose of this study was to discuss the importance of oral medicine related disease by investigating questions that appeared in cyber consultation of Kangnung National University Dental Hospital web site. Among the nearly 2000 questions, the rate of questions related to oral medicine was 20.92%, and the rate of questions related to other departments were oral and maxillofacial surgery 16.87%, conservative dentistry 16.67%, orthodontics 14.02%, prosthodontics 12.25%, periodontics 8.36%, pediatric dentistry 4.93%, preventive dentistry 2.08% and otherwise questions 3.90%. Among the 403 questions related to oral medicine, the frequent questions were oral mucosal diseases 44.17%, temporomandibular disorders 41.19%, halitosis 4.47%, xerostomia 3.23%, other orofacial pain 2.23%, forensic dentistry 1.49% and otherwise questions 2.98%. From the higher rate of questions related to oral medicine compared with other fields of dentistry, we would consider that the people are considerably concerned about the oral medicine related disease, such as oral mucosal diseases and temporomandibular disorders.

Impact of physical stress symptoms and psycho-emotional stress symptoms on oral health in adults (성인의 신체적 스트레스 증상과 심리적 스트레스 증상이 구강건강에 미치는 영향)

  • Stress is closely associated with oral diseases, being considered to be one of important variables to affect the improvement of oral health and the quality of life. In this study, a survey was conducted on 550 adults from July 1, 2012, to February 28, 2013. A path analysis was carried out to determine the influence of physical stress symptoms and psycho-emotional stress symptoms on dry mouth, oral mucosal disease and halitosis symptoms. The findings of the study were as follows: Oral mucosa affected halitosis in a direct effect and dry mouth exerted a direct influence on that as well. When stress symptoms affected halitosis, dry mouth and oral mucosa had an indirect impact on that as parameters. Dry mouth exercised a direct influence on oral mucosa. The above-mentioned findings suggested that the physical stress symptoms and psycho-emotional stress symptoms of the adults affected their dry mouth, oral mucosal disease and halitosis both in direct and indirect effects. As the stress of adults is closely bound up with their oral diseases, how to properly cope with stress should carefully be considered in order for them to lead a better life.

  • Lichen planus is an immune-mediated mucocutaneous disease, affects more frequently middle-aged Caucasian women and makes signs and symptoms in the oral mucosa. Cutaneous lichen planus lesions cause itching but they are self-limiting, oral lichen planus lesions are usually chronic, recalcitrant to treatment and potentially premalignant in some cases. Although, oral lichen planus is non-plaque related disease, they possess particular problems because plaque control is complicated by pain and bleeding and might cause plaque-related disease. The resulting condition comprises accumulations of plaque, which again can influence the progress of oral lichen planus with burning sensation, spontaneous gingival bleeding. Thus, it should be noted that both medication and supportive periodontal treatment are essential for the remission of the lesions. This case report introduces topical corticosteroid therapy and supportive periodontal treatment including intensive oral hygiene procedures to obtain an improvement of subjective symptoms and objective changes and to prevent relapse the lesions.

  • Painful, ulcerative lesions of various systemic disease can affect the oral mucosa membrane at first. If you don't consider the skin lesion, followed or accompanied by oral mucosa, you are likely to fail in differential diagnosis. In this cases, we introduced erythema multiforme and Stevens-Johnson syndrome(SJS) patients with painful, ulcerative lesions on oral mucosa and skin. Also we review oral mucosal diseases come with the skin lesions.
