소아에서 구강내 정상세균총에 대한 채취부위별 항균제 감수성 차이에 관한 연구 (ANTIMICROBIAL SUSCEPTIBILITY TEST ON ORAL FLORA FROM DIFFERENT SAMPLING SITES IN CHILDREN)

  • The most proper antibiotic must be selected after antimicrobial susceptibility test. If difference in antimicrobial susceptibility was significant between oral sampling sites, it is rationale to use the most susceptible antibiotic agent respond to dental procedure and object of treatment. This study examined sampling site variation from saliva, supragingival plaque and subgingival plaque of 16 children's oral microbes. The cultured bacterial isolates, which were Streptococcus viridans and Neisseria, were examined for 10 antimicribial drugs with the Bauer-Kirby agar disk diffusion method. The used drugs were Penicillin, Ampicillin, Oxacillin, Cephalothin, Imipenem, Gentamicin, Erythromycin, Vancomycin, Ciprofloxacin, Clindamycin. There was no significant difference between three sampling sites for antimicrobial susceptibility test of S. viridans and Neisseria and the sequence of susceptibility was agreed among them. In conclusion, it was suggested that antimicrobial susceptibility test from saliva, supragingival plaque and subgingival plaque of children have no significant sampling site variation.

구강종양에서 p53 종양억제 유전자의 돌연변이에 관한 연구 (A STUDY ON MUTATIONS OF P53 TUMOR SUPPRESSOR GENE IN ORAL TUMORS)

  • Nowadays, there are a lot of evidence that mutation of the p53 tumor suppressor gene is one of the most common genetic abnormalities in neoplastic progression. In this study, we analyzed 20 specimens of oral tumors(squamous cell carcinoma 14 cases, ameloblastoma 3 cases, adenoid cystic carcinoma 2 cases, malignant schwannoma 1 case)using polymerase chain reaction and direct sequencing which used an automated DNA sequencer and software for detection of mutations. Polymerase chain reactions were performed with 4 sets of primers encompassing exon 5, 6, 7, 8, and direct sequencing method was employed. The results were as followings. 1. We detected 10 point mutations out of 20 specimens (50%). 2. The genetic alterations included 7 mis-sense mutations resulting in single amino acid subtitutions, 2 silent mutations, 1 non-sense mutations encoding a stop codon. 3. Mutations were mostly in exon 7(7 out of 10 mutations, 70%) and involved codons 225, 234, 235, 236, 238, 247. 4. Therse were 4 cases of $T{\rightarrow}A$ transversion, 2 cases of $C{\rightarrow}A$ transversion, $A{\rightarrow}G$ transition, 1 case of $C{\rightarrow}G$, $T{\rightarrow}G$ transversion respectively. 5. We could find out point mutations more conveniently using PCR - Automated Direct Sequencing method.

구강편평세포암종에서의 Cisplatin 유도 아폽토시스에서의 NF-$\kappa$B의 활성화 (ACTIVATION OF NF-$\kappa$B IN THE CISPLATIN-INDUCED APOPTOSIS OF ORAL SQUAMOUS CELL CARCINOMA)

  • Purpose: This study was done to confirm the role of NF-$\kappa$B in cisplatin-induced apoptosis of oral squamous cell carcinoma. Materials and Methods: Five cell lines originated from different oral cancer patients were tested for the apoptosis by the treatment of cisplatin. These cells showed different degree of cisplatin-resistance and the order is OSCC-2>OSCC-3>OSCC-5> OSCC-1>OSCC-4. OSCC-2 and OSCC-4 cells were assayed for the apoptosis by measuring DNA fragmentation and TUNEL staining after cisplatin treatment. While OSCC-4 cells showed apoptosis, OSCC-2 cells showed no or very slight apoptosis by cisplatin treatment. Next, It was determined whether NF-$\kappa$B activation is required in mediating cisplatin-induced apoptosis of OSCC-4. Result: The result was that elevated NF-$\kappa$B activity mediated cisplatin-induced apoptosis. Conclusion: In conclusion, these findings suggest that NF-$\kappa$B activation is essential to cisplatin-induced apoptosis and it may be involved in cisplatin resistance in OSCC cells.

구강내에 발생한 평활근종 (A CASE OF ORAL LEIOMYOMA)

  • Leiomyoma is a benign smooth muscle tumor that usually arise in the uterus, skin and gastrointestinal system. Only 2 percent are located in the head and neck. Leiomyomas of oral cavity are uncommon as only two cases have been reported in the Korean literature. The common location of oral cavity has been tongue, but other sites include buccal mucosa, lips, palate. mouth floor, and gingiva. Our patient was 30 years old female who complained of swelling and discomfort in the mouth floor. Microscopically this tumor showed bundles of intertwining spindle cells within fibrous connective tissue stroma. The nuclei were generally pale staining and blunt ended. Masson's trichrome stain was positive for muscle, and immunohistochemical study for ${\alpha}$-smooth muscle actin revealed strong positivity. It was treated by surgical excision. We experienced a case of leiomyoma of oral cavity, so we report with literature reviews

  • Several investigators have shown that human papillomavirus(HPV) appear to play an etiologic role in oral and paranasal sinus carcinoma. It was known that 15-25% of head and neck squamous cell carcinoma(SCC) showed HPV-positive infection. Among them, HPV 16 was the most common type but HPV 18 was observed only 2-4% of HPV-positive head and neck cancers. In recent, we treated uncommon 2 oral SCC cases that associated with HPV infection. One is a case of tongue SCC after bone marrow transplantation(BMT), and the other is a case of SCC occurring with aspergillosis in the maxillary sinus. After surgery, HPV 16 and 18 were detected in the surgical specimens by the histological and polymerase chain reaction(PCR) examination. In this report, we present these cases with a review of literature.

구강외과 영역에 있어 Chymoral 임상응용 치험예 (THE CLIMICAL EFFICACY OF CHYMORAL IN ORAL SURGERY)

  • The authors have reported on the evaluation of remarkable therapeutic results from the use of chymoral against postextracted and post operative swelling, pain, 2nd hemorrhage and muscle spasm. The drug was administered to 8 tablets per day in the oral method and to 47 cases of 9 different kinds of oral diseases. The following are the results of the clinical efficacy of the drugs. 1) As results of the use of chymoral preparation, marked efficacy was observed in 30 cases(70%), moderate efficacy in 14 cases(24%), total efficacy in 47 cases (94%) and obscure efficacy in 3 cases(6%). 2) Postoperative infection by extraction and its complications were nearly all cured within a short time by use of chymoral. 3) The comparatively satisfactory results obtained from the use of antibiotic drugs (especially, penbritin or tetracycline) combined with chymoral.

피질골절단술을 이용한 상악협소증환자의 치험례 (CORTICOTOMY IN PATIENT WITH NARROW PALATAL ARCH)

  • 상악협소증으로 인하여 골격관계의 수평적인 부조화와 부정 교합을 보인 상기환자의 경우 상악확대를 위해 구개확대장치를 이용하여 치료하였으나 원하는 치료결과를 얻을 수 없었다. 방사선 평가상 안면골의 봉합부골화가 이미 끝난 것으로 보여 외과적인 피질골절단술을 시행하였다. 외과적인 피질골절단술은 상악확대시 주위안면골의 저항을 줄이기 위한 목적으로 시행하는데 좋은 결과를 얻을 수 있었다. 술후 2주경과후와 12개월 경과후 교합관계 및 확대된 상악궁의 재발상태를 평가한 결과 술후 상태로 유지되고 있었다. 전반적인 교합상태도 많이 개선되어 있었다. 그러나 피질골절단술은 사전에 악궁의 형태나 길이, 수술방법의 선택, 술후 합병증에 대한 충분한 평가를 하여야 한다. 외과적인 피질골절단술은 치료비와 치료기간을 줄일 수 있고 술후 재귀가능성을 줄일 수 있다. 이에 본과에서 피질골절단술을 이용하여 양호한 결과를 얻었기에 본 학회지에 보고하는 바이다.

  • 치아종은 외배엽성 상피세포와 중배엽성 세포의 과성장으로 형성되는 혼합종양이다. 대부분 정상 치아 발생 시기 동안 형성되며 어느 정도 크기까지만 자라므로 진성 종양이 아닌 과오종으로 여겨진다. 형태에 따라 복합 치아종과 복잡 치아종으로 구별된다. 복합 치아종은 주로 구강 전방부에서 발견되며 작은 치아와 비슷한 방사선 불투과성 구조물을 함유하고 있는 병소이다. 치아종의 치료는 대부분 보존적인 외과적 적출술이 추천되며 재발은 거의 없다. 본 증례는 유치 및 영구치의 맹출 장애를 일으킨 복합 치아종으로 진단된 세 어린이에서 병소의 외과적 적출술을 시행하고 약 1년간 관찰한 결과 자발적 맹출을 보여 이에 보고하는 바이다.

DNA 염기손상 치유유전자의 변이와 두경부암 발생 위험성 (THE EFFECT OF GENETIC VARIATION IN THE DNA BASE REPAIR GENES ON THE RISK OF HEAD AND NECK CANCER)

  • DNA 손상 치유 유전자 연구를 기초로 한 임상적 접근이 새로운 치료방법으로 떠오르고 있다. 많은 연구들이 중요한 DNA 수복유전자의 다형성을 찾아내어 각각의 단백질의 활동성에 대한 영향을 알아내고 특정한 치료법을 찾아내고 임상적 적용을 시도하고 결과를 평가하였다. 그 결과 암 치료에서 정상 세포와 암세포에서 DNA 수복 유전자의 발현 분석은 화학요법이나 방사선 치료에서 개인맞춤형 치료법을 가능하게 하고 있다. 예를 들어, NER이 결핍된 종양은 cisplatin 치료에 민감성을 나타내고, MMR 결핍세포는 알킬화 화학요법 약제에 높은 내성을 나타낸다. 선천성 비폴립성 결장암과 같은 MMR 결손종양 또한 알킬화 화학요법 약제에 의한 치료에 내성을 가진다. 신경교종(glioma)에서 MGMT 유전자 프로모터가 흔히 메틸화되는데 이것은 유전자 발현이 억제되고 알킬화 화학요법제에 대한 반응성을 증가시킨다. 향후 구강악안면외과 영역에서도 구강암의 발생의 위험성을 증가시킬 수 있는 더 많은 DNA 수복 유전자의 다형성을 발굴하고 임상적으로 개인맞춤형 치료법을 개발하고 적용할 수 있는 많은 연구가 필요할 것으로 사료된다.

  • We studied 1809 oral cancer patients who visited and were treated in 2002 at the 148 institutions certified as training facilities by the Japanese Society of Oral and Maxillofacial Surgeons, which is composed of 39 dental university hospitals, 44 medical university hospitals, 64 general hospitals, and 1 unknown institution. The patients consisted of 1071 (59.2%) males and 738 (40.8%) females (male:female ratio, 1.45:1), who had a mean age of 65.2 years old. The tongue (40.2%) was the most common site affected, followed by the gingiva (32.7%), buccal mucosa (10.1%), and oral floor (9.0%). There were 6 cases of intraoral multiple cancer. In histopathological examinations, squamous cell carcinoma (88.7%) was the most common type found, followed by adenoid cystic carcinoma (2.1%), and mucoepidermoid carcinoma (1.7%). In addition, non-epithelial tumors comprised 1.8%, among which malignant melanoma was the most common type. Cases classified as T2N0 were the most common (32.1%), followed by T1N0 (21.4%), T4N0 (8.0%), and T2N1 (7.6%). Distant metastasis occurred in 17 patients (1.0%). The sizes of the non-epithelial malignant tumors ranged from 1.0 to 7.0 cm, with a mean size of 3.7 cm.
