GARRE'S OSTEOMYELITIS IN CHILDREN (소아에서의 Garre 골수염)

  • Garre's osteomyelitis is associated with bacterial infection and bone necrosis resulting from obstruction of blood supply. The most common cause for Garre's osteomyelitis is odontogenic infection that originates from periodontal tissue or dental pulp. Subperiosteal abscess may also cause Garre's osteomyelitis in the progress of the infection. Mandible is more often affected than maxilla, most commonly in the permanent first molar region of mandible. Clinically, it results in a hard swelling over the jaw, producing facial asymmetry. Meanwhile, radiograph shows a characteristic feature of irregular pulpal cavity, showing new periosteal proliferation located in successive layers to the condensed cortical bone on stimulated site. The treatment method for Garre's osteomyelitis are removal of the infection source, root canal treatment, antibiotic medication, and incision and drainage. This report presents a case of Garre's osteomyelitis under 15 years old. The patient was successfully treated by antibiotic medication accompanied with root canal treatment. Since the symptom of pediatric patients is less severe than adult, careful diagnosis with history taking and clinical examination is necessary. Furthermore long-term follow-up examination is needed to prevent recurrence even after the symptom disapears.

Effect of Brazilin from Caesalpinia sappan L. on the Growth of Streptococcus mutans ATCC 25175 (소목으로부터 분리된 Brazilin이 Streptococcus mutans ATCC 25175의 생장에 미치는 효과)

  • Somok, the heart wood of Caesalpinia sappan is used in traditional Chinese medicine. This study was performed to investigate the effect of growth and culture conditions of brazilin from C. sappan against S. mutans ATCC 25175. The bacteria were cultured in brain heart infusion (BHI) broth, and then incubated under 5% $CO_2$ at $37^{\circ}C$ for 18-24 hours. The effect of brazilin against S. mutans was confirmed under the changes of the culture conditions, such as growth curve and the change of pH, protein, and total carbohydrate. The growth of S. mutans in control medium was the highest at 24 hr, while brazilin-added medium (0.3 mg/ml) showed maximum growth at 32 hr. The pH values of the control medium was 5.25 at 16 hr, but the media supplemented with brazilin (0.3 mg/ml) was 7.0 at 16 hr. The amounts of total carbohydrate of the control medium was 11 mg/ml at 8 hr, but the brazilin-added media (0.3 mg/ml) was 18 mg/ml at 8 hr. In the protein change of the culture medium, the control culture broth and the brazilin supplemented-cultures was 2.4 mg/ml and 2.54 mg/ml at 24 hr, respectively. Polysaccharide contents of the control medium and test media were 3 mg/ml and 2 mg/ml at 8 hr, respectively. Thus, the application of C. sappan can be considered a useful and practical material for the prevention of dental caries.

CARIES-RELATED MICROBIOLOGICAL SCREENING IN CHILDREN UNDER THREE YEARS OF AGE (3세이하의 어린이의 치아우식증과 관련된 미생물학적 선별검사)

  • To evaluate microbial data and salivary measurements from clinically compatible, culture-based screening procedures employed with children younger than 36 months old. Plaque and stimulated saliva specimens were collected from 87 children. The pH of each saliva sample was measured before and after 0.94% lactic acid was added. Specimens were diluted and plated on selective media and non-selective media. Data collected were counts of mutans streptococci (MS) and lactobacilli (LB). In addition, total viable counts (TVC) of specimens, salivary pH and buffering capacity were also assessed. Each variable was compared to caries status of subjects. According to this study, the results were as followed: 1. Highly significant correlation with caries rates were found for counts of MS and LB. 2. The specific counts/ml saliva or plaque above which caries is predicted, or below which caries is not predicted were as follows: 1) Saliva MS; $10^5$ 2) Plaque MS; $2{\times}10^5$ 3) Saliva LB; $10^3$ 4) Plaque LB; $10^3$. 3. Salivary pH and buffering capacity versus caries status were not significant. 4. Microbial screening methods based on mutans streptococci had higher predictive values and odds ratios than methods for lactobacilli. 5. MS counts were clearly the best indicators of caries status in young children. This measurement can easily be obtained in a dental clinical setting both by conventional culture techniques, or commercial kits for MS recovery.

Survey of Recognition on Periodontal Health Inpatients of Some Dental Clinics in Geoje (거제지역 치과 내원환자의 치주 건강에 관한 인식도 조사)

  • The following are the results of questionnaire research targeting 213 people at 4 dental clinics in Geoje region in order to examine recognition on the periodontal health in Inpatients of dental clinic. 1. In the item of recognition on periodontal health, the patients, who know about whether or not recognizing periodontal disease, were indicated to be 62.9%. Women were knowing better than men. The significant difference by gender was indicated(p < .01). As for a route of acquiring information on periodontal disease, a case of obtaining via television and internet was surveyed to be 31.6%. As for recognition on frequency of tooth scaling, a case of recognition as saying of the necessity of doing once a year was indicated to be 43.7%. The significant difference was indicated depending on job and academic background(p < .001). 2. As for the periodontal-health status, the patients of recognizing that the condition in the gums is mostly healthy were indicated to be 75.1%. The more professional job led to showing the significant difference depending on occupation(p < .001). As for a reason for a visit to dentistry, a case of visiting due to smell in the mouth was indicated to be 18.1%. As for kinds of dental treatment, the patients. who received the tooth scaling by visiting a dental clinic. were the largest with 46.5%. The significant difference was indicated depending on job(p < .01), academic background(p < .05), And monthly income(p < .001).

Epidural Abscess Caused by Eikenella corrodens in a Previously Healthy Child

  • Eikenella corrodens rarely causes invasive head and neck infections in immunocompetent children. We report a case of epidural abscess caused by E. corrodens in a previously healthy 13-year-old boy who presented with fever, headache, and vomiting. On physical examination upon admission, there was no neck stiffness, but discharge from the right ear was observed. Brain magnetic resonance imaging (MRI) revealed approximately 4.5-cm-sized epidural empyema on the right temporal lobe as well as bilateral ethmoid and sphenoid sinusitis, right mastoiditis, and right otitis media. During treatment with vancomycin and cefotaxime, purulent ear discharge aggravated, and on follow-up brain MRI, the empyema size increased to $5.6{\times}3.4cm$ with interval development of an abscess at the right sphenoid sinus. Burr hole trephination was performed, and foul-smelling pus was aspirated from the epidural abscess near the right temporal lobe. Pus culture yielded E. corrodens. Endoscopic sphenoidotomy was also performed with massive pus drainage, and the same organism was grown. The patient was treated with intravenous cefotaxime for 3 weeks and recovered well with no other complications. Therefore, E. corrodens can cause serious complications in children with untreated sinusitis.

DISTRIBUTION OF AIRBORNE BACTERIA BY HANDPIECE AEROSOL CONDITIO (핸드피스 분무조건에 따른 부유세균 기균(氣菌) 의 분포)

  • In recent years, cross-contamination has become one of the noticeable issues in dental clinic. Two major routes of contamination are the direct-contamination through blood and oral secretion and the indirect-contamination through dental office equipments. Especially, air-contamination through air-floating pollutant in a confined space like hospital, and also contamination through aerosol ejected from high-speed handpiece in a dental office was interested. The purpose of this study was to understand risk of bacterial infection through aerosol from handpiece in a dental office, which will help the practitioner with prevention of contamination during dental treatment. The main findings are as follows. 1. In a comparative test, the group using handpiece has higher bacterial number than the group not using handpiece with significant statistical difference(P<0.01). 2. The group using handpiece with rubber dam has lower bacterial number than the group using handpiecewithout rubber dam with significant statistical difference(P<0.01). 3. Comparing the group using drainage water with the group using distilled water as a handpiece water source results in 22.4 cfu and 17.0 cfu respectively but the difference is no statistically significant(P>0.05). 4. Measuring cfu at 0.5m and 1.5m distance, 0.5m distance showed higher bacterial number with statistical significance(P<0.01). 5. Classification of bacterial types showed the largest bacterial number came from gram-positive micrococcus(73.9%), and gram-negative micrococcus, gram-negative bacillus, and gram-positive bacillus follow in descending order.

A STUDY OF NI-RESISTANT BACTERIA ON THE RESTORED STAINLESS STEEL CROWN (기성금관 수복어린이에서의 니켈내성균에 관한 연구)

  • Stainless steel crowns are widely used for restoration for primary molars. The material used for the crowns is an alloy of $70\sim80%$ nickel and $5\sim15%$ chromium. Nickel has been known to cause allergic reaction, cancer and cell toxicity. Little is known about nickel with respect to the relationship between Ni-contained SS crown and graining of Ni-resistance in oral microorganisms. The purpose of this study is to examine whether use of Ni-contained SS crown leads to occurrence of Ni-resistant microorganism, especially enterococci. The gingival crevicular fluid of two different groups was taken. Experimental group included patients wearing SS crown, and control group comprised individuals without SS crown. The samples were plated in BHI agar, BHI agar supplemented with nickel chloride at the concentration of 3mM and bile esculin azide (BEA) agar. The cultured enterococci on BEA agar medium were tested their Ni-resistance in nickel-containing media increasing concentrations from 3mM to 50mM. The results were as follows: 1. In experimental group, a total of 507,350 strains were isolated on BHI agar, of which 53,864(10.62%) strains were found to be resistant to 3mM nickel. In control group, of 414,590 isolates on BHI agar, 37,523 isolates were resistant to 3mM nickel. 2. A total of 95 enterococci were isolated on BEA agar in experimental group, while 20 were isolated in control group. of the enterococci, 68 and 12 isolates were found to be nickel-resistant in experimental and control group, respectively. 3. Of 68 nickel-resistant isolates in experimental group, one survived 50mM nickel. In contrast, none of the isolates in control group was observed to grow at the concentrations over 30mM nickel.

  • This study was investigated to observe the effect of Treponema denticola cell sonicates(TDC) and Treponema lecithinolyticum cell sonicates(TLC) on cytokine secretion and matix metalloproteinase-2(MMP-2) activation of cultured human gingival fibroblast. Several experiments were performed including $IL-1{\beta}$, IL-6 ELISA for the effect on the $IL-1{\beta}$, IL-6 secretion of human gingival fibroblast. Also gelatinase zymography and gelatin dissolubility test for the activation of MMP-2 secreted by gingival fibroblast. The results were as follows. 1. The effect of TDC and TLC on IL-6 secretion of human gingival fibroblast showed statistically significant increase of IL-6 secretion in the TDC and TLC treated group compared to no treatment group(p<0.05) . 2. The amount of $IL-1{\beta}$ secretion was below the lower limit and there was no difference in the $IL-1{\beta}$ secretion of gingival fibroblast between TDC, TLC treated group and no treatment group. 3. The active form of pro MMP-2 with 72 kDa molecular weight was activated in both TDC and TLC treated group and clear band was appeared at 62kDa site on the zymography. 4. Gelatin dissolubility of MMP-2 secreted by gingival fibroblast was higher in TDC and TLC treated group compared to no treatment group(p<0.05). 5. In the TDC treated group, serine protease of T. denticola affect gelatin dissolubility. But in the TLC treated group gelatin was degraded by only MMP secreted by gingival fibroblast. Regarding to the above results, TDC and TLC have an effect on the IL-6 secretion increase of human gingival fibroblast and appears to activate pro MMP-2 which degrades collagen.

THE EFFECT OF XYLITOL ON THE LACTOSE FERMENTATION OF STREPTOCOCCUS (Streptococcus의 유당분해에 대한 자일리톨의 효과)

  • Xylitol is a 5-carbons carbohydrate, which can be replaced with sucrose for preventing dental caries. To study the effect of xylitol on the fermentation of lactose in bacteria, the important oral bacteria such as Streptococcus(S.) mutans, S. oralis and S. salivarius were studied. The optical density using spectophotometer and the cell concentration were assessed to evaluate the combined effect of lactose and xylitol against the bacteria. Thin layer chromatography and lactose-PTS activity test were performed to evaluate the effect of xylitol on the fermentation of lactose in S. mutans and by ${\beta}-galactosidase$ with the following results. 1. The optical density of Streptococcus mutans culture was not increased for 8 hours-incubation in the media added with lactose and xylitol, but was increased at 24 hours-incubation. The number of viable cells at 8 hours-incubation was smaller in the media containing lactose and xylitol in comparison with lactose only. 2. The optical densities of Streptococcus oralis culture and Streptococcus salivarius culture were not increased for 8 hours-incubation in the media added with lactose and xylitol but were increased at 24 hours-incubation. 3. When Streptococcus mutars was incubated for 8 hours in the media added with lactose and xylitol, the amount of remained lactose was larger compared with the media added with lactose only But all lactose was fermented in both media after 24 hours-incubation. 4. When Streptococcus mutans was incubated in the media added with lactose and xylitol, the activity of lactose-PTS was higher compared with the media added with lactose only. 5. When ${\beta}-galactosidase$ was incubated in the media added with lactose and xylitol, the amount of remained lactose was larger compared with the media added with lactose only. These results indicated that xylitol inhibited the fermentation of lactose by Streptococcus.

  • Background : Goblet cell hyperplasia is a critical pathological feature in hypersecretory diseases of the airways. A bacterial infection of the lung is also known to induce inflammatory responses, which can lead to the overproduction of mucus. Recently, mucin synthesis in the airways has been reported to be regulated by neutrophilic inflammation-induced epidermal growth factor receptor (EGFR) expression and activation. In addition, it was reported that migration of the activated neutrophils is dependent on the matrix metalloproteinases (MMPs), especially MMP-9. In this study, bacterial lipopolysaccharide (LPS)-induced goblet cell hyperplasia and mucus hypersecretion by EGFR cascade, resulting from the MMPs-dependent neutrophilic inflammation were investigated in the rat airways. Methods : Pathogen-free Sprague-Dawley rats were studied in vivo. Various concentrations of LPS were instilled into the trachea in $300{\mu}{\ell}$ PBS (LPS group). Sterile PBS ($300{\mu}{\ell}$) was instilled into the trachea of the control animals (control group). The airways were examined on different days after instilling LPS. For an examination of the relationship between the LPS-induced goblet cell hyperplasia and MMPs, the animals were pretreated 3 days prior to the LPS instillation and daily thereafter with the matrix metalloproteinase inhibitor (MMPI; 20 mg/Kg/day of CMT-3; Collagenex Pharmaceuticals, USA). The neutrophilic infiltration was quantified as a number in five high power fields (HPF). The alcian blue/periodic acid-Schiff (AB/PAS) stain were performed for the mucus glycoconjugates and the immunohistochemical stains were performed for MUC5AC, EGFR and MMP-9. Their expressions were quantified by an image analysis program and were expressed by the percentage of the total bronchial epithelial area. Results : The instillation of LPS induced AB/PAS and MUC5AC staining in the airway epithelium in a time- and dose-dependent manner. Treatment with the MMPI prevented the LPS-induced goblet cell hyperplasia significantly. The instillation of LPS into the trachea induced also EGFR expression in the airway epithelium. The control airway epithelium contained few leukocytes, but the intratracheal instillation of LPS resulted in a neutrophilic recruitment. A pretreatment with MMPI prevented neutrophilic recruitment, EGFR expression, and goblet cell hyperplasia in the LPS-instilled airway epithelium. Conclusion : Matrix metalloproteinase is involved in LPS-induced mucus hypersecretion, resulting from a neutrophilic inflammation and EGFR cascade. These results suggest a potential therapeutic role of MMPI in the treatment of mucus hypersecretion that were associated with a bacterial infection of the airways.
