교합조정은 비가역적인 술식이므로 예방적인 목적으로 사용하는 것은 바람직하지 않으며 환자가 증상을 호소하거나 환자의 구강검사시 교합간섭으로 인한 증상이 확인된 경우라도 자연치에서의 교합조정술은 반드시 신중을 기해야 한다. 교합조정술을 시행하기 전에 먼저 하악의 위치가 안정되어야 한다는 것을 명심하고 이를 위해 치과의사는 하악의 중심위에 대한 이해와 중심위로의 하악유도 방법에 대한 숙지도 필요하다. 또한 심한 악관절 장애나 통증 및 불안정한 하악의 위치를 보이는 경우 구강내 교합상이나 교합 안정장치가 선행되어야 하는 경우가 있다. 교합조정술을 시행해야 하는 치과의사 자신이 교합조정술에 대한 지식을 충분히 습득하고 있어야 하며, 교합조정술의 적응증을 분명하게 확인한 뒤 조심스럽고 정확하게 시술을 시행해야 한다. 즉, 잘못된 교합조정으로 오히려 교합간섭을 야기할 수도 있음을 명심해야 한다. 그러나 교합조정에 대해 올바로 이해하고 일반적인 교합조정의 원칙을 지켜 정확히 시행한다면 교합조정은 결코 환자에게 해를 주지 않고, 자유로운 하악운동을 가능케 하며, 교합의 안정성을 유지해 줄 것이다.

In case of prosthesis fabrication by CAD/CAM, location, area and contour of occlusal contacts can be adjusted so more functional occlusion can be acquired. Also, errors in a manufacturing process is reduced compared to cast metal prostheses and porcelain fused metal prostheses fabricated by conventional methods such as casting and porcelain build up. Therefore, prostheses by CAD/CAM show superior occlusion accuracy. Recently, virtual articulator function has been introduced to CAD/CAM system, which reproduces mandibular movement against maxilla. Thus, it is possible to consider occlusal interference in anterior/lateral movement as well as closing movement. There have been many studies on the marginal and internal fit of prostheses using zirconia but the occlusal fit of zirconia crown fabricated by CAD/CAM has not been researched as much. In this case report, 7 zirconia crowns were designed and fabricated by CAD/CAM for total 5 patients. The models of zirconia crowns before and after occlusal adjustment during intraoral try-in were scanned for occlusal contacts, which were compared to evaluate accuracy of prostheses and understand patterns of occlusal adjustment. Most of the occlusal adjustments were done on functional cusps and slopes of zirconia crown, and the magnitude of occlusal adjustment ranged from $15{\mu}m$ to $60{\mu}m$. In the zirconia crown fabricated with CAD/CAM systems, the occlusal adjustment is a necessary procedure, so additional procedures will be needed for compensating reduced mechanical properties.

Open bite is accompanied by decrease in tooth contact and overbite, and causes collapse of occlusal plane, mastication difficulties, speech disorders, changes in appearance, and lower occlusal force than normal. Open bite caused by temporomandibular joint disorder in adults with complete occlusion must be corrected after removal or stabilization of the causative factors. Orthodontic treatment, occlusal adjustment, prosthetic treatment, and surgical treatment can be the option of occlusal correction. This report describes about estimating the cause of occlusion change in two patients who developed an open bite due to mandibular displacement in adults with complete occlusion and different treatment approaches accordingly. In one patient, satisfactory result was obtained in functional and esthetic aspects through occlusal adjustment after stabilization of the temporomandibular joint.

교합조정이나 대부분의 보철치료는 비가역적이어서 명확한 치료 계획이 없이는 섣불리 치료를 시작하지 말아야 한다. 명확한 치료 계획을 위해서는 증상의 원인 인자를 파악해야 하나, 때때로 그 원인이 불분명하여 환자에게 무어라 설명해야 할지 난감한 경우가 많은 것이 바로 이 분야이다. 교합 재구성을 함에 있어서 생체 역학과 근육의 활동량을 고려해야 하는 것은 이미 주지하고 있는 바이다. 즉, 악관절이나 치아에 대한 하중을 조절함으로써 근조직의 안정을 도모하여 적정한 하악위와 원활한 하악운동을 획득하는 것이 교합 재구성의 큰 목적 중 하나이다.
