교합조정은 비가역적인 술식이므로 예방적인 목적으로 사용하는 것은 바람직하지 않으며 환자가 증상을 호소하거나 환자의 구강검사시 교합간섭으로 인한 증상이 확인된 경우라도 자연치에서의 교합조정술은 반드시 신중을 기해야 한다. 교합조정술을 시행하기 전에 먼저 하악의 위치가 안정되어야 한다는 것을 명심하고 이를 위해 치과의사는 하악의 중심위에 대한 이해와 중심위로의 하악유도 방법에 대한 숙지도 필요하다. 또한 심한 악관절 장애나 통증 및 불안정한 하악의 위치를 보이는 경우 구강내 교합상이나 교합 안정장치가 선행되어야 하는 경우가 있다. 교합조정술을 시행해야 하는 치과의사 자신이 교합조정술에 대한 지식을 충분히 습득하고 있어야 하며, 교합조정술의 적응증을 분명하게 확인한 뒤 조심스럽고 정확하게 시술을 시행해야 한다. 즉, 잘못된 교합조정으로 오히려 교합간섭을 야기할 수도 있음을 명심해야 한다. 그러나 교합조정에 대해 올바로 이해하고 일반적인 교합조정의 원칙을 지켜 정확히 시행한다면 교합조정은 결코 환자에게 해를 주지 않고, 자유로운 하악운동을 가능케 하며, 교합의 안정성을 유지해 줄 것이다.
저자는 두개하악장애 환자에 대한 교합장치 치료시 group function 교합이 있는 환자에서도 canine guidance교합으로 변화시켜 치료함이 정당한지를 근전도학적 측면과 임상적 측면에서 알아보고자 Group function 교합을 가진 20명의 두개하악장애환자와 10명의 정상인을 대상으로 교합장치치료 전, 도중, 후의 좌우측 교근 및 전측두근의 근전도를 측정하고 두개하악장애지수를 조사한 수 이들 자료를 분석 평가하여 다음과 같은 결론을 얻었다. 1. 정상인군에서 최대이악물기상태의 좌우측 교근 및 전측두근의 근전도는 교합장치치료 도중의 cannine guidance교합시가 교합장치치료전후의 group function 교합시에 비해 낮게 나타났다(P<0.01). 2. 두개하악장애군에서 최대이악물기 상태의 좌우측 교근 및 전측두근의 근전도는 교합장치치료 도중의 canine guidance교합시 교합장치치료전후의 group functin교합시에 비해 낮게 나타났다. (P<0.01). 3. 두개하악장애지수는 교합장치치료 전에 비해 치료 후 감소되는 경향을 나타냈었다(P<0.01)

The objective of this study was to analyze occlusal contact pattern of 33 young adult males with normal occlusion during lateral excursion using T-scan III system which can measure the dynamic occlusion. Occlusal contact patterns were examined and categorized in non-working side disclusion point(more functionally acceptable position) not canine-to-canine position. Disclusion time, occlusal force, and occlusal force distribution ratio of upper canine at disclusion point during lateral excursion movement was also measured and compared among the groups divided according to contact patterns. The results showed that group function was clearly more dominant than canine guidance and other occlusal contact patterns different with existing patterns were found. There were significantly difference between groups in variables of T-scan measurement. T-scan III system is effective tools to analyze and evaluate occlusal contact patterns, disclusion time, occlusal force, and occlusal force distribution ratio of upper canine at disclusion point.

Purpose: The aim of this study was to compare changes of bite force, occlusal contact area, and dynamic functional occlusion analysis after occlusal stabilization splint therapy during sleep for one month in a patient with bruxism. Materials and Methods: From October 2021 to July 2022, sleep bruxism of 30 patients who visited the Department of Oral Medicine at Yonsei University College of Dentistry Hospital were recruited. The participants were divided into two groups: using an occlusal stabilization splint during sleep (treatment; n = 15) and not using an occlusal stabilization splint (control; n = 15). Before using the occlusal stabilization splint and one month after, bite force, occlusal contact area and dynamic functional occlusion analysis (ratio of left/right bite forces, average bite forces, maximum bite forces, and maximum contact areas during lateral and anterior and posterior mandibular movements) were performed. Results: There was no difference in bite force and occlusal contact area between the treatment group using the occlusal stabilization splint and the control group not using the occlusal stabilization splint during sleep for one month. However, there were significant differences in the average bite force and maximum bite force in the lateral and anterior and posterior mandibular movements and the maximum contact areas in the anterior and posterior mandibular movements. Conclusion: The occlusal stabilization splint is helpful for sleep bruxism patients who lateral and anterior and posterior mandibular movements. In addition, further studies are needed a double-blind study with a large population.
다양한 자세에서 교합장치의 두께가 저작근에 미치는 영향을 근전도학적으로 평가하고자, 측두하악장애의 징후와 증상이 없고 정상교합을 가진 성인 8명을 대상으로, 교합장치를 장착하지 않은 상태에서와 4.3, 6.0, 8.1mm 두께를 가진 교합장치를 장착한 상태에서 직립위, 식사위 그리고 앙와위에서 최대 이악물기 상태의 좌, 우측 교근 몇 전측두근의 근활성도를 채득, 분석하여 다음과 같은 결과를 얻었다. 1. 전측두근에서는 교합장치를 장착하지 않은 상태보다 교합장치를 장착한 상태에서 근활성이 유의하게 감소하였고 (p<0.05), 교근에서는 차이가 없었다. 2. 직립위에서 전측두근의 근활성은 교합장치를 장착하지 않은 상태보다 8.1mm 두께의 교합장치를 장착한 상태에서 유의하게 감소하였다 (p<0.05). 3. 식사위에서 전측두근의 근활성은 교합장치를 장착하지 않은 상태보다 8.1mm 두께의 교합장치를 장착한 상태에서 유의하게 감소하였다 (p<0.05). 4. 앙와위에서 전측두근의 근활성은 교합장치를 장착하지 않은 상태보다 4.3, 6.0, 8.1mm 두께의 교합장치를 장착한 상태에서 유의하게 감소하였다 (p<0.05).
치과교정학 분야에 있어서 부정교합자의 다양한 하악골 위치변화에 따른 Hyoid bone 위치변화에 대한 연구는 부족한 감이 있어, 저자등은 Hellman의 치령ⅢA 이후의 부정교합을 가진 남ㆍ녀 97명을 Angle씨 각급 부정교합의 분류에 의해 중심교합위와 안정위시의 두부 X선 사진을 가각 탐득하고 Hyoid bone의 위치변화를 측정하여 다름과 같이 결과를 얻었다. 1. 중심교합위에서의 Cranial base에 대한 Hyoid bone 위치변화에서는 Angle씨 ClassⅢ에서 남ㆍ녀 모두 물징적으로 전방에 위치하며, Mandibular plane에 대한 Hyoid bone위치변화에서는 각급 부정교합사이에 특기할 차이가 없다. 2. 안정위에서의 Hyoid bone위치는 중심교합위에서의 위치와 비슷한 분포를 나타내고 있다. 3. 중심교합위에서는 안정위로싀 위치변화에서 각급 부정교합 똑같이 후ㆍ하방 이동을 나타내고 있다.
This study was undertaken to compare each maximum biting force and to investigate its relationship with the facial skeketal form, number and position of tooth contact between anterior openbite and normal occlusion adults, using the T-scan system and the lateral cephalogram. The subjects of this study consisted of a group of 25 individuals with normal occlusion and another group of 14 with anterior openbite. The obtained results of this study were as follows : 1. The maximum biting force of anterior openbite adults was less than that of normal occlusion adults. 2. In anterior openbite adults, there were negative correlations between the maximum, biting force and SN/MP, FMA, PP/MP mesurement of lateral cephalogram. 3. In anterior openbite adults, as the mesial angulation of lower first molar against the occlusal plane increased, the more the biting force decreased. 4. In both groups, the greater the number of tooth contact, the more the biting force increased. 5. In both groups, the center of effort for anteroposterior occlusal contact was located on the first molar region.
지금까지 여러 가지 형태의 기기를 이용하여 보고된 사람의 교합력은 그 최대치가 매우 다양하다. 본 연구에서는, 교합력을 측정하기 위하여 새로운 측정기기를 개발하였다. 본 기기를 이용하여 전치부 교합력 측정 시에는 대칭적으로 최대한 물도록 하악을 유도하여 최대 교합력을 측정하였고, 구치부는 편측으로 물도록 하악을 유도하여 최대 교합력을 측정하였다. 교합 시 치아간 수직고경은 15.1mm 였다. 측정기기 내 strain gauge를 사용하여 전기저항의 변화를 힘으로 전환하였다. 마이크로 프로세서가 수치를 계산하면 액정화면에 수치가 표시된다. 실험실 내 기기교정 검사에서는 50kg과 100kg의 부하를 가하였다. 개발된 교합력 측정기기의 임상적 신뢰도를 시험하기 위해 건강한 치과대학 재학생 10명을 대상으로 최대 교합력을 측정 하였다. 이 새로운 측정기기로 측정된 교합력은 이전연구에서 기록되었던 것보다 더 높은 수치가 나왔다. 또한 제1대구치와 전치부에서 최대 교합력을 측정한 경우 실험자 내와 실험자 간에 통계적으로 유의한 신뢰성을 얻을 수 있었다. 본 기기는 안정적이고 조작성이 쉽다고 판명되었다. 그러므로, 본 기기는 더 큰 연구 집단의 선별검사에 매우 유용하게 사용할 수 있을 것이며, 또한 치주질환이 있는 치아나 임플란트, 악관절 장애를 가진 환자의 교합력 측정과 같이 특정된 경우의 교합력 측정에도 유용할 것이다.
This study was done to identify the changes of bite force and occlusal contact between before and after stabilization splint therapy. 16 female patients, accepted for orthodontic treatment at the Department of Orthodontics, College of Dentistry, Chosun University, were selected as the sample of this investigation. For comparisons, the samples of 16 were divided into just before stabilization splint, 1 month after stabilization splint, 2 months after stabilization splint, and 3 months after stabilization splint and used the T-scan system to identify the bite force and occlusal contact changes for each group. Statistical analysis of the data was carried out ANOVA tests, and Turkey tests using $SPSS/PC^+$. The results were as follows : 1. Bite force change from just before treatment to 1 month after stabilization splint therapy was statistical significantly decreased (P<0.05). 2. Bite force changes from 1 to 3 months after stabilization splint therapy showed no statistical significance (p>0.05). 3. The changes of anterior occlusal contacts showed no statistical significance regardless of the wearing periods of stabilization splint (P>0.05). 4. The changes of posterior occlusal contacts from just before treatment to 1 month after stabilization splint therapy was statistical significantly decreased (P<0.05). 5. The changes of posterior occlusal contacts Outing 1 to 3 months after stabilization splint therapy showed no statistical significance (P>0.05). 6. Posterior teeth rather than anterior teeth were more influenced by the changes of the number of occlusal contacts. To sum up above results, we may respect to capturing and stabilizing centric relation Position just 1 month after stabilization splint therapy.

This article describes a clinical protocol for the conventional rehabilitation of patient diagnosed with partial anodontia. A combined dental therapy approach was used and included endodontic therapy and root capping on the maxillary central incisors, fabrication of a maxillary overdenture, and fabrication of mandibular konus overdenture supported by 3 konus abutments. Within this protocol, tooth-supported overdenture prostheses are used for 2 purposes: first, to obtain the most rigid retention and function at an established maxillary-mandibular relationship; and second, to continuously maintain function and esthetic appearance applying immediate dentures after teeth extraction. The idea behind this protocol and its associated clinical procedures is presented along with a discussion compared with implant therapy. In the case introduced, and after 7 years of observation, the therapy can be seen as a success. We increased the occlusal vertical height in this case, but it would be more appropriate to see this as recovering the occlusal vertical height that was lost. The process of increasing the occlusal vertical height, that is restoration of the face, modification of the extrinsic occlusion of the incisors, and retraction of the mandible is very difficult and important. Ultimately, class III malocclusion is fixed, adequate occlusal vertical height is gained, and the retracted posterior anodontial portion is restored by prosthodontic dentures based on the rigid support theory. The result of the therapy done on the later-achieved malocclusion with partial anodontia on the posterior portion must consider the following in order to maintain the safety of the esthetics of the tooth and face for a period of time: 1) occlusal restoration with an ideal occlusal vertical height, 2) allowance of the final occlusion induced by the functional relationship of the upper and lower jaw, 3)final occlusion functionally induced by the lip competence limit.
