Many types of occlusal splints are used for treatment of craniomandibular disorders. Most widely used splint among them is flat-type centric relation splint. Insertion of splint into the mouth may cause increasing of vertical dimension, masticatory muscle realignment and rearrangement of maxillo-mandibular relationship, so as a result of splint treatment, occlusal relation may vary whether you like it or not. From this point of view, occlusal state of patient shold be frequently monitored to prevent undesired or harmful effect during occlusal splint therapy. The purpose of this study was to investigate the effect of occlusal splint, especially centric relation splint, on the occlusal contact state after 3 months treatment. 32 patients with craniomandibular disorders who had unilateral symptoms participated in this study. To observe and record occlusal contact state, the author used T-Scan system (Tekscan Co. U.S.A.) at both pretreatment and posttreatment. The recorded date were analyzed with regard to contact number, contact force and contact time, change of anterior tooth contact and coincidence of first contact point with affected side were observed, too. Aan last, the subjects were divided into 2 groups and compared, according to average value of VAS index, with respect to joint pain, sound and limitation of movement, respectively. The collected date were statistically processed with SPSS and the result as follows : 1. Total occlusal contact number and force were not changed by occlusal splint therapy but total occlusal contact time decreased slightly. 2. There was a tendency of increasing number of subjects with anterior tooth contact after treatment and change of first contact point side were observed in as many as 40.6% of subjects. 3. There were no difference between higher and lower group of VAS index, and between pretreatment and posttreatment in each group, either.

Despite of the successful clinical performance of implants, it is still lacking of the knowledge of changes in implant occlusion. The purpose of this study was to evaluate the changes of infraocclusal contact after clinical occlusal function of implant. Twenty patients(38 implants) were recalled during 10 months after placement of implant prosthesis. Occlusion changes were investigated at placement, placement 1 months, 4 months and 10 months serially with silicone bite material and T-Scan II sensor. Bony changes were also evaluated with periapical radiographs. The changes of silicone thickness and T-Scan II sensored areas were statistically analyzed with repeated measured ANOVA and the Scheffe's post-hoc test at the 95% significance level. The following results have been made based on this study: 1. Alveolar bone loss was within 0.20mm and it was generally concluded within physiologic level. 2. There were no statistically significant differences in the thickness changes of silicone material at 1 month and 4 months of occlusal function. However, there was statistically significant difference at 10 months of occlusal function (p<0.05). 3. There was no statistically significant difference in changes of occlusal contact area in T-Scan II at 1 month and 4 months of occlusal function, but there was statistically significant difference at 10 months of occlusal function (p<0.05). Conclusively, as time goes by, implant occlusion to be formed infraocclusion was to be far close and increased occlusal contact. However, it was not observed destructive bone resorption in periapical radiographs and any other side effects.
The purpose of this study was to evaluate an effect of change on head posture initial occlusal contacts with measuring the distances between initial occlusal contacts and maximum intercuspal position at different head posture in TMDs patient. For this study, 24 patients from age 13 to 36 were selected, they were examined health history taken, patients who have sign and symptoms of TMDs were examine before the study. For the normal group, 21 adults from age 23 to 25 were selected. They have normal or class I molar relationship, and have no other prosthetic restorations. Difference on distance between initial occlusal contact and maximum intercuspal position with mandibular kinesiograph$(MKG^R)$(K6 diagnostic system, Myo-tronic Inc, USA) in upright, supine, 45$^{\circ}$ extension, 30$^{\circ}$ flexion position of the head were measured. The Frankfort horizontal plane was used as a reference plane. The results were as follows : 1. There were significant differences between initial occlusal contacts of the normal and patient group on upright position and 30$^{\circ}$ flexion of the head(p<0.05, p<0.01) 2. The position of the initial occlusal contacts have a tendency to place anterior and inferior to maximal intercuspal position in upright position and 30( flexion of the head as well as posterior and inferior in supine position and 45$^{\circ}$ extension of the head in the normal and patient groups. 3. There were significant differences among the initial occlusal contacts between uptight and supine position; upright and 45$^{\circ}$ extension of the head(p<0.05); supine position and 30$^{\circ}$ flexion of the head, .and 30(flexion and 45$^{\circ}$ extension of the head in the patient group(p<0.01) The result have shown that after treatment on the supine position, it may be necessary to check occlusal contact on the upright position as well ass flexion of the head. It may need careful adjustment in occlusal condition on upright position of TMDs patient.
The purpose of this study was to evaluate effect of head posture change on initial occlusal contacts through measuring the distances between initial occlusal contacts and maximum intercuspal position at different head posture. Two special devices were designed and constructed. Mandibular movement replicator was used to assess reliability of the K6 diagnostic system(MKG; Myo-tronic Inc, Seatle, USA) and head posture calibrator was used to maintain the constant head posture during experiment. We measured difference of distance between initial occlusal contact and maximum intercuspal position with MKG in upright, supine, 45 degrees extension, 30 degrees flexion, 30 degrees right and left bending postion of the head. The Frankfurt horizontal plane was used as a reference plane. 21 adults aged from 23 to 25 were selected, who have normal or class I molar relationship, and have no symptoms on TMJ and masticatory muscles, and have restorations less than 3 surfaces on each tooth, and have no other prosthetic restoration. The obtained results were as follows : The mean absolute distances between initial occlusal contact and maximum intercuspal postion were 0.39(0.18mm in the upright position, 0.65(0.37mm in the supine position, 0.59(0.33mm in the 45 degree extension, 0.70(0.53mm in the 30 degrees flexion, 1.12(1.10mm in the 30 degrees right bending and 1.94(0.67mm in the 30 degrees left bending of the head. The positions of the initial occlusal contacts have a tendency to locate anterior, left and inferior to maximal intercuspal position in upright position, posterior and inferior in supine position and 45 degrees extension, anterior and inferior in 30 degrees flexion, right and inferior in 30 degrees right bending, and left and inferior in 30 degrees left bending of the head. There were significant differences among the initial occlusal contacts in each head postures(P<0.0001). Therefore, we need to check initial occlusal contacts in the altered head posture during occlusal analysis and adjustment of occlusal appliance and dental occlusion for diagnosis and treatment of temporomandibular disorder.

Proper management of provisional prosthesis is key to success in prosthodontics. Provisional restoration on maxillary anterior missing area frequently come across a incident of falling off especially in patients with long span pontics and oval arch shape. This is because maxillary anterior teeth are more exposed to horizontal force than the posterior teeth and additional anterior cantilever effect will negatively affect to the retention of provisional prosthesis. Beside that maxillary anterior provisional prosthesis should provide proper incisal guidance during the mandibular functional movements. However occlusal contacts on the prosthesis in maximum intercuspal position are located on opposite side of fulcrum line of prosthesis which will cause removing force against the provisional prosthesis. This case report present that provisional implant prevent pre-described harmful effect on maxillary anterior fixed provisional prosthesis and provide comfort and satisfactory result during post-extraction healing period.
The purpose of this study was to investigate the peak electromyographic activity(EMG) and time intervals in firing sequence of Anterior temporalis(TA) and Masseter(MM) on clenching, the number of tender points in the head and neck muscles, the occlusal contact state on clenching, and their relationship. 78 patients with Temporomandibular Disorders(TMDs) participated and were classified into articular or muscular group by clinical signs and symptoms. BioEMG$\textregistered$ of integrated masticatory function analyzer, Biopak$\textregistered$ system(Bioresearch Inc., USA), was used to measure EMG and related items, and T- Scan$\textregistered$(Tekscan, USA), computerized occlusal analyser, was used to record occlusal contact state on maximum voluntary clenching. EMG and occlusal contact were synchronously recorded and analysed with SAS Statistical program. The results of this study were as follows : 1. In total subjects, EMG of TA was lower than that of MM in articular group but in muscular group, vice versa, As a result, the ratio TA to MM was significantly different between the two groups. no significant differece. 3. The number and force of occlusal contact were more in articular group, but there was no significant difference in Total Left-Right statistics(TLR) between the two groups. 4. In unilaterally affected subjects for muscle function, no significant difference was observed between the affected side and the contralateral side in articular group, but in muscular group, the item of the number of tender points showed significant difference. However, for occlusal contact items, contact force in articular group shows significant difference between the two sides. 5. Rate of coincidence of the first firing side of TA with affected side or preferred chewing side was higher and not different between the two groups, but no significant correlation was showed between the first firing side and the first occlusal contact side.
The understanding the nature of occlusal tooth contacts of natural dentition is important for correct diagnosis and treatment of diseases developed in stomatognatic system. Several investigator have studied the distribution of tooth contacts in maximum intercuspation and have repored contact locations with respect to the tooth position. However, there are few report the variation of the occlusal contact point with change in each head position. This study analysed the number of occlusal contact point with change in each head position. 30 subject(male 17, female 13), who ahad natural occlusion and no symptoms of temporomandibular disorder, were selected. The numbers and patterns of tooth contact were recorded by silicone bite registration on stone model at four different head positions with head anguration gauge(from the supine to the upright position). The results obtained were as follows : 1. The numbers of total occlusal contact point on teeth increased to average 25, 29, 35, 42 points as head angulation was changed from the supine to the upright position against the ala-tragus line, and there was significant difference(P<0.05). 2. In the 19 subject(65%)of total 30 subject, the perforated point of the silicone bite indicated that the locus for the prime contact point moved mesially as the head angulation was changed from the supine to the upright position. 3. On the basis of the fact that the anterior occlusal contact point increase as head angulation changed from the supine to the upright position, we could find that the mandibular position is moved anteriorly.

Excessive teeth wear can lead to reduced chewing efficiency, occlusal plane collapse, and facial changes. Full mouth rehabilitation with a change in vertical dimension may cause pain in the temporomandibular joint, masticatory muscle and teeth, so an accurate diagnosis and treatment plan is required. In this case, a 69-year-old man had excessive teeth wear with a loss of posterior support. We evaluated the degree of adaptation to the increased vertical dimension with the removable occlusal splint and provisional restoration. We report this case because the treatment result has been functionally and aesthetically satisfactory by providing stable anterior guidance, proper posterior teeth disclusion, and even contact of all teeth in centric occlusion.

In previous studies, methods for enhancing cellular response on the Hydroxyapatite coated implant surface were described. In this study, the changes of surface characteristics such as surface roughness, contact angle, surface energy and surface morphology were observed when Hydroxyapatite coated Ti discs were immersed in NaCl solution for various time. Hydroxyapatite coated Ti discs were immersed in 0.9% NaCl solution for 7, 14 and 21 days at $37^{\circ}C$. The control group comprises dry identical discs not immersed in a solution. (n=3) All discs were dried in air completely and the surface roughness was measured using confocal laser scanning microscopy(CLSM). Static contact angle was recorded by video contact angle analyzer after dropping distilled water on the surface. The surface energy was calculated from contact angles of the three liquids. Surface was observed using a field emission-scanning electron microscope(FE-SEM). As a result, the surface roughness of immersed Hydroxyapatite coated Ti discs increased significantly and the contact angle decreased comparing with control group discs. The surface energy of immersed discs increased except for discs immersed for 14 days.

Purpose: This study aimed to analyze stress distribution and maximum von Mises stress generated in intracoronal restorations and in tooth structures of mandibular molars with various types of cavity designs and materials. Materials and Methods: Three-dimensional solid models of mandible molar such as O inlay cavity with composite and gold (OR-C, OG-C), MO inlay cavity with composite and gold (MR-C, MG-C), and minimal invasive cavity on occlusal and proximal surfaces (OR-M, MR-M) were designed. To simulate masticatory force, static axial load with total force of 200 N was applied on the tooth at 10 occlusal contact points. A finite element analysis was performed to predict stress distribution generated by occlusal loading. Results: Restorations with minimal cavity design generated significantly lower values of von Mises stress (OR-M model: 26.8 MPa; MR-M model: 72.7 MPa) compared to those with conventional cavity design (341.9 MPa to 397.2 MPa). In tooth structure, magnitudes of maximum von Mises stresses were similar among models with conventional design (372.8 - 412.9 MPa) and models with minimal cavity design (361.1 - 384.4 MPa). Conclusion: Minimal invasive models generated smaller maximum von Mises stresses within restorations. Within the enamel, similar maximum von Mises stresses were observed for models with minimal cavity design and those with conventional design.
