A STUDY ON THE INITIAL TISSUE CHANGES TO INTERMITTENNT FORCE IN DOGS (성견에서 전자력을 이용한 간헐적 교정력에 의한 치아이동시 초기 조직학적 변화에 관한 연구)

  • The purpose of this study was to help examining force system, optimal treatment, and prevention of relapse. Orthodontic force was generated by electromagnets, therefore duration could be freely controlled, and applied for 4 days in dogs. Force magnitude was 200gm and 50gm. duration was contious and intermittent. Intermittent duration was divided into 2 kinds of types in on/off, 1 mininute/1 minute, 10 seconds/1 minute. The results were as follows: 1. In the intermittent group to which force was applied for 10 seconds and ceased for 1 minute, osteoclast was not observed. 2. In the intermittent group to which 200gm of force was applied for 1 minute and ceased for 1 minute, there was blood circulation in periodontal space on pressure side, but mild hyalinized zone was observed. 3. More number of total osteoclast was counted in the pressure side of continous group to which 200gm of force was applied than in any other groups. 4. More number of frontal osteoclast was counted in the pressure side of intermittent group to which 200gm was applied for 1 minute and ceased for 1 minute than in any other groups. 5. On tension side, less periodontal space widening, more new bone and secondary cementum formation were observed in the intermittent group to which force was applied for 1 minute and ceased for 1 minute than in the continous group.

  • Crown-root fractures occur throughout both crown and root, and are defined as fractures involving enamel, dentin and cementum. The fractures may be grouped according to pulpal involvement into complicated and uncomplicated one. Crown-root fractures often occur on maxillary anterior teeth and comprise 5% of injuries affecting the permanent dentition and 2% in the primary dentition. To restore crown-root fractured tooth, biologic width must be maintained. For maintaining biologic width, such methods as gingivectomy following osteoplasty or orthodontic extrusion or surgical extrusion are available. Surgical extrusion is a method that extracts the tooth and replants the fractured tooth supragingivally. It is indicated when the length of the crown fragment is less than half the length of the clinical root. In these cases, root canal treatment and crown restoration using light-cured composite resin were performed after surgical extrusion. In following periodic examinations, favorable outcome was observed.

A study on the postoperative stability of occlusal plane in Class III orthognathic surgery patients (제 III급 부정교합자의 양악수술후 교합평면의 안정성에 관한 연구)

  • In Patients with severe skeletal discrepancy, surgical orthodontic treatment must be accompanied, and recently two jaw surgery has become a common procedure, resulting in improved esthetics and function. Choosing the position of the occlusal plane in this two jaw surgery is an important factor in postoperative stability Therefore this must be taken into consideration during the diagnosis and treatment plan. In this study, among patients with skeletal Class III occlusion, 25 patients(8 male, 17 female, average age $23.2{\pm}3.17$) who have undergone two jaw surgery, setting the ideal occlusal plane according to Delaire's architectural and structural cranial analysis. In comparing preoperative($T_1$). postoperative($T_2$, average of 15.4 days), and long-term postoperative($T_3$, average of 32.6 months) lateral cephalometric radiography, the following conclusions have been made. 1. There were no significant changes of the occlusal plane angle after the two jaw surgery, and there were no significant differences between the surgical technique(SSRO and IVRO). 2. The postoperative changes of the occlusal Plane had no relationship with the amount of jaw movement, amount of posterior impaction, nor the time relapse after surgery. 3. After two jaw surgery, in the SSRO group there was significant forward movement of the mandible, and in the IVRO group the lower incisors extruded as the mandible moved backward and downward which makes the genial angle and the mandibular plane angle significantly increased

Periodontal Response According to the Timing of Orthodontic Force Application alter Bone Graft into Angular Bony Defect in the Dog (성견에서 수직적 골결손부에 골이식후 교정력 적용시기에 따른 치주조직의 반응)

  • The aim of this study was to investigate the periodontal response according to the timing of orthodontic force application after bone graft into the angular bony defect. Nine dogs were divided into three groups, 2, 4, and 6 weeks, according to the timing of orthodontic force application after bone graft. Periodontal angular bony defects were created surgically at the distal aspect of both maxillary third incisors. Two weeks later, flap operation was performed to eliminate inflammation and reference notch was made on the root surface at the level of the bottom of each defect. Demineralized freeze-dried bone was implanted on the left side whereas only debridement was done on the other side. Experimental tooth movement was executed during 8 weeks on both graft and non-graft sides. After 2 weeks of retention period, animals were sacrificed for histologic specimens. The results were obtained as follows 1 New bone formation was more pronounced in the graft side than in the non-grad side in all experimental animals. 2. In the 6-week group, new bone and cementum formation was observed in more than half from the notch to the cemento-enamel junction, and the zone of connective tissue attachment was found without apical migration of junctional epithelium. 3. In the 4-week group, the amount of new bone formation was smaller than in the 6-week group whereas the overall remodeling pattern was similar. 4. New bone formation was confined to around the notch and the junctional epithelium migrated apically to the level of the notch with no connective tissue attachment and cementum formation in the 2-week group. The results of the present study suggest that periodontal response may be influenced by the timing of orthodontic force application after bone graft into angular bony defect.

An Immunohistochemical Study on DNA Synthesis in the Periodontium during Tooth Movement in Dog (성견의 실험적 치아이동시 치주조직내 DNA 합성에 관한 면역조직화학적 연구)

  • The purpose of this study was to investigate the changes about the cellular activity on DNA synthesis in the periodontal ligament of dog, in which experimental tooth movement was performed. A control and 5 experimental dogs, one and half year in age, were studied. Light force (50-75g) was applied by placing open-coil spring between left mandibular premolars ; heavy force (250-300g), between right mandibular premolars. Experimental dogs were sacrificed after infusion of bromodeoxyuridine(BrdU), at 12 hours, 1, 3, 7 and 14 days after force application, respectively. And the histologic and the immunohistochemical evaluation were performed on the obtained periodontal tissue around mandibular premolars, using anti-romodeoxyuridine antibody, which can indicate proliferating cells. The results were as follows: 1. The tearing of periodontal ligament and the vascular dilatation at tension side were observed in 12 hours, increasing until 3 days. After then it decreased; Such a finding was more evident in heavy force group than in light force group. 2. The hyalinization of the periodontal ligament and the activity of osteoclast at pressure side were observed in 12 hours, increasing until 3 days. But from 7 days on, it decreased; Such a finding was more evident in heavy force group than in light force group. 3. The BrdU expression in the control group was positive, mainly in the oral epithelium and the fibroblasts in the periodontal ligament, but negative in bone cells in periodontal ligament. 4. The BrdU expression in the experimental group was more positive in tension side than in pressure side; The expression was a little more positive in the periapical area than in the cervical area of tooth. 5. The BrdU expression in light force group was the highest in 1 day, after which it decreased; In heavy force group, it was the highest in 12 hours, after which it decreased. But in 14 days, there was no difference between the experimental group and control group.

  • Tooth movement, the phenomena and mechanisms of which are still controversial, can be considered as part of the result of the inflammatory processes. The purpose of this study was to examine the activity of macrophage and T-cell, playing important roles in the immune reaction, in the periodontal ligament of dog, in which experimental tooth movement was performed. Six one and half year-old dogs, a control and 5 experimentals, were studied. Light force (50-75g) was applied by placing open-coil spring between left mandibular premolars ; heavy force (250-300g), between right mandibular premolars. Experimental dogs were sacrificed at 12 hours, 1, 3, 7 and 14 days since force application, respectively. And the histologic and the immunohistochemical evaluation on the obtained tissue were performed, using $\alpha$-1-antichymotrypsin and CD3 antibodies. The results were as follows : 1. There were more inflammatory cell infiltrations in heavy force group than in light force group until 3 days. But from 7 dsays on, no difference was not observed between groups ; Such an infiltration was more evident at pressure side than at tension side. 2. Osteoclastic activity at pressure side began to be seen in 12 hours, increasing until 7 days. After then it decreased ; Such an activity was more evident in heavy force group than in light force group. 3. Tearing of periodontal ligament and vascular dilatation at tension side began to be seen in 12 hours, increasing until 3 days. After then it decreased ; Such an observation was more evident in heavy force group than in light force group, but there was no difference between groups in 14 days. 4. $\alpha$-1-antichymotrypsin expression in control group was positive, mainly in sulcular epithelium, but negative in periodontal membrane, pulp, bone cells. 5. $\alpha$-1-antichymotrypsin expression in experimental group was more positive in pressure side than in tension side ; The expression was a little more positive in cervical area of tooth until 3 days, but after 7days, it was more positive in apical area. 6. $\alpha$-1-antichymotrypsin expression in light force group began to be observed in 12 hours and reached to the greatest level in 7 days, after which it decreased ; In heavy force group, it was the greatest in 3 days, after which it decreased. 3 Expression in the periodontium was almost negative.

ORTHODONTIC TREATMENT WITH MINISCREWS IN MIXED DENTITION (혼합치열기의 miniscrew를 이용한 교정치료)

  • Anchorage plays an important role in orthodontic treatment. Skeletal anchorage like the miniscrew is considered a more effective method in anchorage control than conventional anchorage which needs much patient's cooperation. The miniscrew offers many advantages. 1) It is easy to insert and to remove. 2) It can endure the force needed for moving teeth. 3) It can be immediately loaded and 4) Patient cooperation is not needed. 5) It is economic compared to other skeletal anchorage systems. In comparison to adult's bones, children's bones have comparatively poor bone quality and quantity. Therefore, it is hard to obtain primary stability in younger patients. However, if the miniscrew can be retained successfully, it will be effective in many orthodontic treatments. In these cases, we used the miniscrew in correcting of diastema, in aligning dental midline, and in rendering a forced eruption of impacted tooth in mixed dentition patient. We obtained satisfactory results.

Camouflage treatment in adult skeletal Class III cases by extraction of two lower premolars (성인 골격성 III급 부정교합환자의 하악 소구치 발치를 통한 보상치료)

  • Objective: The purpose of this study was to evaluate the dentoskeletal and soft tissue profile changes after extraction of two lower first or second premolars in "borderline" adult skeletal Class III cases. Methods: Twenty-eight patients with "borderline" skeletal Class III malocclusion were studied. All of them were treated by extraction of two lower first or second premolars. Lateral cephalometric radiographs taken at the start and end of treatment were analysed. Twenty-five cephalometric variables were calculated and paired $t$-tests were performed. Results: After treatment, no significant changes were noted in the skeletal parameters ($p{\geq}0.05$). Regarding the dental parameters, the L1-MP angle decreased by $8.1^{\circ}$, the U1-L1 angle increased by $7.7^{\circ}$ ($p$ < 0.01), the overjet distance increased by 5.7 mm ($p$ < 0.01), the L1-NB angle decreased by $7.3^{\circ}$ and the L1-NB distance decreased by 4.8 mm ($p$ < 0.01). The soft tissue parameters of Li-E, Li-H and Li-RL2 distance decreased by 3.2 mm, 3.4 mm and 4.1 mm respectively ($p$ < 0.01). Conclusions: Orthodontic camouflage treatment by extraction of two lower first or second premolars provides a viable treatment alternative for "borderline" skeletal Class III cases to achieve a good occlusal relationship.

A CLINICAL STUDY ON ANCHORAGE CONTROL OF MOLAR ANCHORING SPRING(MAS) DURING RETRACTION OF THE MAXILLARY CANINE (상악 견치 후방견인시 MAS(Molar Anchoring Spring)의 저항원 조절에 대한 임상적 연구)

  • In maxillary canine retraction by means of sliding mechanics, we designed MAS(molar anchoring spring) to prevent anchorage loss and uncontrolled tipping of tooth movement and have applied it in clinical cases. The anchorage control of the maxillary first molar and type of tooth movement of the maxillary canine were studied in 31 subjects. The measurements were made on cephalograms, orthopantomograms and dental casts. The obtained results were as follows. 1. In case of the maxillary first molar, there was a little sagittal anchorage loss, but there was no vertical & transverse anchorage loss. 2. In case of the maxillary canine, there was distal tipping movement and also there was a little intrusion tendency.

Nonextraction treatment of Class II division 2 in an adult patient using microimplant anchorage (MIA) (Microimplant Anchorage(MIA)를 이용한 II급 2류 성인 환자의 비발치 치험례)

  • Maxillary anterior teeth were intruded and lingually root torqued with two maxillary anterior microimplants between the lateral incisors and canines. Overerupted maxillary canines were intruded with two other microimplants between the maxillary canines and first premolars. Maxillary posterior teeth and canines were distalized, then the maxillary incisors were retracted with two maxillary posterior microimplants between the first and second molars. The mandibular anterior teeth were intruded and the mandibular posterior teeth were extruded with conventional method such as anterior bite plane, intrusion arch and Class II elastics. The mandible moved slightly forward after the correction of deep bite and retroclination of the upper incisors. Consequently, microimplant anchorage (MIA) provided absolute anchorage for simultaneous correction of Class II canine and molar relationships and deep overbite.
