Muscle Stiffness and Elasticity Related to Physical Therapeutic Modalities (물리치료에 따른 근육의 경도와 탄성도 비교)

  • The purpose of this study was to quantitatively assess treatment effect of physical therapy on experimentally-induced masseter muscle fatigue by two parameters of muscle stiffness and elasticity. Three physical therapeutic modalities inducing electroacupunture stimulation therapy(EAST), Microwave diathermy and low-level laser therapy(LLLT) were compared. 10 healthy volunteers with normal occlusion (mean age of $26.3{\pm}1.16$ years, M:F=1:1) were participated in this study. All subjects were asked to chew gum on the right side until they felt pain(more and VAS 5 (0 to 10)) and their masseter muscles were examined with a tactile sensor in order to evaluate changes of stiffness and elasticity according to gum chewing and three physical therapeutic modalities. Subjective discomfort or pain was self-estimated by VAS as well. Unilateral gum chewing increased stiffness and decreased elasticity only on the chewing side but VAS increased on the both sides(p<0.05). EAST or Microwave diathermy greatly decreased stiffness and VAS and increased elasticity(p<0.05) but LLLT did not exhibit significant difference. From the results of this study, it is concluded that both EAST and Microwave diathermy have favorable effect on stiffness and elasticity of muscles and pain relief while effect of LLLT is not reliable. In addition, experimental unilateral gum chewing compromises stiffness and elasticity of masseter muscles only the chewing side while subjective discomfort or pain can be felt on the both sides.

  • The bulging of mandibular angle area, frequently, make some difficulties in social life due to its reckless appearence, especially in oriental nations. So, many authors had reported its causes and surgical techniques for correction since late 19th century with diagnosis of masseter hypertrophy. But in spite of its muscular origin, major surgical techniques, in general, should aim complete bony reduction or osteotomy and supplemental myotomy as in hemimandibular hypertrophy or mandibular elongation. We used ultrasonogram for soft tissue depth estimation and expearenced some complications such as incomplete lingual cortical bony reduction, condylar fracture in mandibular angle reduction via intraoral circumvestibular approach. So we reported our surgical technique for angle reduction with possible complications.

  • 두개하악장애는 가장 흔히 발병하는 질병중의 하나이다. 이때 저작계에 흔히 호소하는 주소가 근육의 통증이다. 통증의 정도를 측정하는 방법으로 촉진이 가장 널리 사용되고 있으나 정량화하기 어렵다는 단점이 있다. 따라서 통증 측정기 등과 같이 다양한 시도가 진행되고 있다. 근육이나 관절의 병적 소견은 해당조직의 혈액순환장애나 염증반응과 종종 관련이 있다. 이러한 상태는 신체의 표면과 밀접한 관계가 있어 피부온도에 영향을 미치며, 따라서 thermography로 측정할 수 있다. 피부온도의 측정은 질병의 활성 상태를 평가하는 데 유익하며, 또한 치료결과를 평가하는 데에도 효과적이다. 객관적 피부온도 측정장치가 있다면 턱관절이나 저작근의 이상상태를 평가하는데 매우 도움이 될 것이다. 따라서 본 연구의 목적은 경제적이면 양측성으로 특정 부위의 피부온도를 측정할 수 있는 기기를 사용하여 검사자내 및 검사자간 신뢰도를 측정하고 좌우 온도차이를 확인함으로써 향후 질환 및 치료 겨오가에 대한 임상적 평가시 객관적 측정방법의 하나가 될 수 있는 지를 알고자 하는 데 있다. 구강내 염증이나 이상이 없으며 두개하악장애가 없는 치과대학생 15명을 대상으로 조사하였다. 평균연령은 24.9세, 범위는 24-30세이었다. 사용한 기기는 미국산 YSI Precision 4000이다. 18-2$0^{\circ}C$의 일정한 실내온도에서 두 검사자가 각각의 검사를 알지 못하는 상태에서 별도로 정해진 부위에 검사를 시행하고, 약 1주일후 이전 검사에 대한 기억이 없어진 상태에서 두 검사자중 한 검사자가 다시 동일한 검사를 시행하였다. 측정치들을 이용하여 상관관계를 계산하고 이에 대한 유의성을 검정하였다. 1. 검사자내 상관관계(r)는 좌 .798(p=<.0001), 우 .757(p=<.0001)이었다. 2. 두검사자간 상관관계(r)는 좌 .958(p=<.0001), 우 .951(p=<.0001)이었다. 3. 좌우 측정치간의 차이는 유의한 차이가 없었다. 4. 근육부위별 측정에서는 inferior masseter muscle의 검사자간에서만 유의한 상관관계가 없었을 뿐 anterior masseter muscle, anterior temporalis 모두 검사자내 및 간에 유의한 상관관계를 나타내었다. 이상의 결과로 보아 정상인에 있어 근육내 발통점의 피부온도는 검사자간에는 특정부위에 따라 다소 차이가 있을 수 있으나 일반적으로 높은 재현성을 보여줌으로서 향후 교근 및 측두근의 임상연구 평가에 피부온도조사는 도움이 되리라 사료된다.

  • Human beings have made remarkable advances in medical technology as well as technological advances. However, as was the case in the past, incurable diseases still exist: temporo-mandibular joint (TMJ). The diagnosis of the Korean medical staff, currently called a "medical advance," is adhering to the outdated patient's comments, diagnosis using a doctor's auditory diagnosis and a ruler, and diagnosis of X-ray imaging. Therefore, it is important to have accurate patient symptoms, to have a doctor's own diagnosis and experience, to increase the number of diagnoses due to the severity of the symptoms, and to cover the costs of medical care. To solve this problem, the core conductive signal generated from the bridge was quantified through %MVC. Quantified EMG will be assessed and compared with Cortex to establish a jaw joint condition evaluation criterion.

AN ELECTROMYOGRAPHIC STUDY ON THE CHANGES IN MUSCLE ACTIVITY IN DIFFERENT MALOCCLUSION TYPES FOLLOWING ACTIVATOR THERAPY (Activator 사용전후 부정교합 유형에 따른 각 근육 활성도의 변화에 관한 연구)

  • This study was carried out to study the changing patterns in muscle activities in Angle's II and Class III malocclusion patients following activator therapy, To study the activities of anterior temporal(T.A.), masseter(M.M.), posterior temporal(T.P.), and anterior belly of digastric(D.A.) muscles, surface electrodes were placed on 15 Angle's Class II and 17 Angle's Class III malocclusion patients and following conclusions were obtained after electromyographic recordings were taken at prior to the activator therapy and at some time between 6 -12 month after the start of activator therapy. 1. Class II and Class III malocclusion groups, when compared to normal occlusiongroup, showed no significant differences during resting and swallowing. But significant differences were observed in masseter and posterior temporal muscles during clenching. 2. Most of the muscles studied showed a certain pattern, that is those groups whose pre-treatment E.M.G. were lower than the normal value showed significant increase after activator therapy, whereas those groups whose pre-treatment E.M.G. were higher than the normal value showed significant decrease after activator therapy. This pattern was observed in all of the muscle groups studied except T.A.(swallowing), T.P.(clenching) and D.A.(swallowing, clenching) in Angle's Class III malocclusion group and in Angle's Class II malocclusion group, above tendency were observed only in T.A.(swallowing), M.M.(resting, swallowing) and T.P.(clenching). This pattern was less obvious during clenching. 3. When E.M.G. activity after activator therapy between Angle's Class II and Class III malocclusion group was compared, Class III malocclusion group showed more increase during resting, and Class II malocclusion group showed more increase during swallowing and clenching excepting M.M. and D.A. respectively.

  • The etiology of masseteric hypertrophy is obscure. When the hypertrophy occurs unilaterally it's appearance is more striking because of the resultant facial asymmetry. Two soldiers were admitted with the complaint of a lump on their jaws. The authors obtained good results on the esthetic & functional aspect in two cases of unilateral masseteric hypertrophy with the Adam's method. The etiology was thought to be the combination of unilateral masticatory and jaw clenching habits when emotionally disturbed or under nervous tension in army services.

하악편측절근환자의 교근활성도에 대한 근전도학적 연구

  • The electrical activities of masseter muscle were recorded on 9 subjects with unilateral fracture of mandible. The electromyographic studies were executed with 2 channel RS dynograph recorder for electromyography. The graphs were recorded in the physiologic rest position, incisal occlusion, molar occlusion, left lateral excursion, right lateral excursion, and protraction. The following conclusions were drawn; 1. In the physiologic rest position, incisal occlusion, molar occlusion, and protraction, the electrical potentials of the masseter muscle were greater in the affected side. 2. In the right lateral excursion, the electrical potentials of the masseter muscle were greater in the right side, and in the left lateral excursion, greather in the left side. There is no correlation mutually between the affected side and unaffected side.

  • We had experienced a case of traumatic myositis ossificans arising in right masseter muscle, inferior to zygomatic arch occurred on 25 year old male. He had some trauma on the site one year ago and visited with complaint of mouth opening limitation and swelling. Palpable mass with facial disfigurement was noted. and several ovoid radiopaque masses were revealed on C.T. examination. On histopathologic examination, multiple bone trabeculae with osteoblasts in its periphery was noted in connective tissues and invaded to neighbouring muscles, but any chondroid components were not revealed.

The Effect of Low Level Laser Therapy at the Trigger Points in Masseter and Trapezius Muscles (저출력레이저를 이용한 교근 및 승모근 발통점의 치료에 관한 연구)

  • To investigate the effect of low level laser therapy, the author selected 37 dental students with tender points in both masseters and trapeziuses, also measured maximum comportable opening(MCO), Numerical analog scale(NAS) and pressure pain threshold (PPT). 20 subjects were assigned randomly and were treated with GaAlAs diode laser after ultrasound. The other 17 subjects were treated with ultrasound and laser without irradiation. All the subjects were treated after 2 and 4 day respectively and were examined again after 6 days. And the obtained results were as follows : 1. The MCO of irradiated group increased more significantly after treatment than non-irradiated group. 2. The NAS of irradiated group decreased more significantly after treatment than non-irradiated group. 3. The PPT or irradiated group increased more significantly after treatment than non-irradiated group.
