The Effect of Cushion Insole on the Flexibility and Exercise Ability of Lumbar Spinal Stenosis (쿠션인솔이 요추 척추관협착증 환자의 유연성 및 운동능력에 미치는 영향)

  • The purpose of this study was to investigate the effect of cushion insole on lumbar flexibility and motor fitness in patients with lumbar spinal stenosis. 26 patients with lumbar spinal stenosis were randomly assigned to the experimental group 13 and the control group 13. Volunteers were allowed to wear cushioned insoles for two hours a day for six weeks. The experimental group was 8 mm and the control group was 4 mm, wearing a cushion insole as a blinded experiment. There was a significant increase(p<0.05) in the flexion test of the experimental group ($3.38{\pm}3.12$), but not statistically significant in the other tests(p>0.05). In conclusion, cushion insole was not suitable for intervention for lumbar spinal stenosis. There is a need to study the height and material of the insoles in the future.

Clinical disease characteristics according to karyotype in Turner syndrome (터너증후군에서 핵형에 따른 임상질환의 발병양상)

  • Purpose : Turner syndrome (TS) is a disorder in which various anomalies can be accompanied, especially cardiovascular, renal, thyroid and auditory problems. The aim of this study is to identify the incidence of these disorders in patients with TS according to karyotype. Methods : We reviewed medical records of 90 patients with TS diagnosed by chromosomal analysis in 4 hospitals from Jan 1998 to Dec 2007. We evaluated these cases by prepared protocol of 4 medical problems.Results : The distribution of karyotype was 45,X (47.8%), mosaic pattern (34.4%) and structural aberration group (17.8%). Renal anomalies, cardiovascular anomalies, thyroid disorders and auditory problems are accompanied in 4.4%, 10.0 %, 11.1% and 5.6%, respectively. 45,X group had renal anomalies (7.0%), cardiovascular anomalies (18.6%), thyroid disorders (9.3%) and auditory problems (11.6%). Mosaic group had renal anomalies (3.2%), thyroid disorders (12.9%), no cardiovascular anomalies and auditory problems. Structural aberration group had cardiovascular anomalies (6.3%), thyroid disorders (12.5%) and no other 2 problems. Patients with 45,X group had a significant higher incidence of cardiovascular anomalies (P =0.025). Conclusion : Our results indicate that there are differences clinically according to karyotype of TS, especially in incidence of cardiovascular anomalies.

A new strategy for transcatheter closure of patent ductus arteriosus with recent-generation devices (경피적 동맥관 폐쇄술에서 최근의 기구들의 전략적 이용과 결과)

  • Purpose : The aim of this study was to assess the efficacy and safety of recent-generation patent ductus arteriosus (PDA) closure devices applied by a new selection strategy according to the characteristics of each PDA. Methods : From February 2003 to January 2006, 138 patients underwent transcatheter closure of PDA (study group). According to the size and morphology of each ductus, a COOK Detachable Coil or 'flex' PFM Nit-Occlud was used for a small ductus (group 1, n=43); 'medium' PFM Nit-Occlud (group 2, n=49) for a moderate ductus; and an Amplatzer Duct Occluder (group 3, n=46) for a large ductus. The 83 patients who underwent transcatheter closure of PDA from February 2000 to January 2003 were defined as the comparison group. The Qp/Qs ratio, pulmonary/aorta pressure ratio, and MD of the ductus were compared. Immediate and follow-up results including residual shunts and complications were also evaluated and compared among groups. Results : In all 138 patients, complete occlusions were confirmed without major complications, while procedure failure (n=2, 2.2%), device embolization (n=1, 1.1%), and persistent residual shunt (n=4, 4.5%) were documented in the comparison group. Total complication rates were lower in the study group than in the comparison group (study group, 1.4%; comparison group, 9.0%; P<0.05). Conclusion : A novel strategy adopting the merits of various recent-generation devices for transcatheter closure of PDA provides excellent clinical results with minimal risk.

Evaluation of Experimentally Induced Lumbar Spinal Cord Injury by Somatosensory Evoked Potentials(SEPs) in Dogs (개에서 Somatosensory Evoked Potentials (SEPs)을 이용한 척수기능장애의 평가)

  • Changes in somatosensory evoked potentials (SEPs) which accompanied by insertion of foreign body in spinal canal were evaluated with clinical signs and positive contrasted myelography in dogs. Foreign bodies occupied 20∼50% of spinal canal. Foreign bodies occupying about 50% of spinal canal were inserted into the animals of group II and III for 1 week and 2 days, respectively. Foreign bodies occupying about 20% of spinal canal were inserted into the animals of group IV, V, and for 1 week, 2 days, and 8 weeks, respectively. in group I (control group), sham operation (lateral laminectomy) was performed. Group III, IV and V did not severely affect on SEPs latencies and clinical signs. Group VI affects on SEPs latencies but not on clinical signs. After foreign body removal, SEPs latencies showed similar recovery patterns with clinical signs. However, group II induced severe abnormalities in SEPs latencies and clinical signs. In group III, IV and V, thoracic potentials (TN1) were abnormally recorded after foreign body insertion and firstly/normally recorded on the 6th 9.5th and 3.5th day after foreign body (removal following) insertion. In group VI, TN1 was abnormally recorded after foreign body insertion and firstly recorded on the 7.7th day and normally recorded on 34th day after foreign body insertion. In group I, TN1 was not recorded from the 3rd day after foreign body insertion and until the 8th week after foreign body removal. In group I, TN1 was firstly recorded on the 1st day after laminectomy.

Comparison of the Results of Ultrasound-guided Caudal Epidural Block - Herniated Intervertebral Disc vs Spinal Stenosis - (초음파를 이용한 미추 경막외 차단술의 결과 비교 - 추간판 탈출증과 척추관 협착증 -)

  • Purpose: Ultrasound-guided epidural caudal block for low back pain and radiating pain is often performed in the treatment of outpatients. However, this procedure has a failure rate of up to 25% even when it performed by an experienced physician. The authors investigate the effectiveness of Ultrasound-guided epidural caudal block in patients related to disc herniation or spinal stenosis. Materials and Methods: Ultrasound-guided caudal epidural block was performed in 55 outpatients with LBP and radiating pain. Patient was placed in the prone position and sonographic image of sacral hiatus was obtained using linear probe. A 22-gauge needle was advanced into the sacrococcygeal membrane under ultrasound guidance and then medication was injected into the caudal epidural space. There were 31 cases of disc herniation, and 24 cases of spinal stenosis. Patients were evaluated by Visual Analog Scale (VAS) pain score at pre-treatment, post-treatment, 2 weeks and 4 weeks by telephone interviews. Results: 53 of the 55 cases (96.4%) of needle insertion into the sacral canal under ultrasound guidance were successful. Gender was not significantly different between disc herniation group and spinal stenosis group. But there was a significant age difference between disc herniation group ($42.3{\pm}10.8$), and spinal stenosis group ($62.8{\pm}15.1$) [p<0.001]. The VAS score at pre-treatment, post-treatment, 2 weeks, 4 weeks in disc group were 6.84, 3.1, 1.8 & 1.77. The VAS score at pre-treatment, post-treatment, 2 weeks, 4 weeks in spinal stenosis group were 6.88, 3.58, 4.33 & 4.88. The VAS score in both groups was significantly improved after the procedure (p<0.001). Over time, the two groups were statistically significant differences in VAS score after adjusting for age (p<0.001). Conclusion: Ultrasound-guided caudal epidural block seems to provide a high success rate and a significantly better response in disc group than spinal stenosis group.

Interrelationship of Tension-type Headache and Oral Parafunction (긴장성 두통과 이갈이 습관의 상호관계)

  • This study was designed to confirm the association between the pain of Tension-Type Headache(TTH) patients and their Oral Parafunction(OPF). Patients with TTH(n=58) visited the Department of Oral Medicine, Kyung Hee University Dental Hospital during two months were recruited to this study. 20 patients with OPF(bruxism, clenching, bruxism & clenching) are the experimental group and 38 patients without OPF are the control group. Both groups were asked to answer the questionnaire based on the diagnostic criteria of TTH(IHS). Then it was analyzed statistically. This study was designed to confirm the association between the pain of Tension-Type Headache(TTH) patients and their Oral Parafunction(OPF). There was no difference in sex(p=0.91) and age(p=0.73) between two groups. 2. In the experimental group, dull pain was presented more frequently than in the control group. But, there was no difference between two groups(p=0.69). 3. In the experimental group, severe pain was presented more frequently than in the control group. But, there was no difference between two groups(p=0.40). 4. In the experimental group, pain shown bilaterally was presented more frequently than in the control group. But, there was no difference between two groups(p=0.52). 5. In the experimental group, pain was more increased by physical activities than in the control group. But, there was no difference between both groups(p=0.74). The pain of TTH patients with OPF was presented to be non-dull pain frequently and more bilaterally and severely, also increased more by physical activities than the pain of TTH patients without OPF. But, there was no significant difference between two groups. Therefore, it is considered that the pain of TTH is not influenced by OPF.

The Effects of Regular Pilates Exercise on Blood pressure and Pulmonary Variables (규칙적인 필라테스 운동이 심혈관 및 호흡 변인에 미치는 영향)

  • The purpose of this study was to investigate the effects of Pilates exercises on the improvement of blood pressure and respiratory function. 36 subjects which were randomly divided into Pilates exercises group(n=18) and control group(n=18). Subjects in the Pilates exercises group were regularly participated in Pilates exercise program for 8 weeks. We were measured the blood pressure and respiratory function such as blood pressure, VO2max, METs, and Anaerobic threshold at before and after the experiment. After 8 weeks, in Pilate exercises group, there were significant improvement on blood pressure and respiratory function except the diastolic blood pressure. And subjects participated in Pilates exercise group were significantly improved compared to control group on systolic blood pressure, VO2max, METs, and Anaerobic threshold. But in control group, there were no significant differences. In conclusion, regular participation in Pilates exercises improved the blood pressure and respiratory function. These results suggest that Pilates exercise is the method that replace aerobic exercise.

A Study on the Effect of Basic Life Support Training on the First Responsive Police Officers

  • The study was conducted to verify the effect of basic life support training on the skill ability of police officers. The subjects of this study were 10 experimental group and 10 comparative group with voluntary consent after explaining the theory and significance of the training experiment at the police station located in K. The education program used in this study consists of theoretical education and practical training, and the theoretical education is 60 minutes and the practical training is 30 minutes. The measurement tool for basic resuscitation performance was measured based on the 'CPR and ECG Guidelines for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care' presented by the American Heart Association. The results are as follows. The experimental group showed higher performance skills than the traditional control group in field confirmation performance skills, primary evaluation performance skills (A, B, C, medical evaluation), and BLS performance skills (heart compression, artificial respiration, medical evaluation) which are the basic resuscitation performance skills. In conclusion, this study confirmed that the theory and practice education program is more effective in improving the clinical performance of police officers than the traditional lectures and practice education, so it is possible to apply this simulation education program to the cardiac arrest patient emergency treatment.

The Association between cardiovascular disease and Periodontal Disease on Convergence Study in Adults over Age 40. (40세이상 성인의 심혈관질환과 치주질환 관련성에 관한 융합적 연구)

  • The purpose of this study was to investigate the relationship between cardiovascular disease and periodontal disease. The subjects were 3,149 adults over 40 years of age using the third year National Health and Nutrition Examination Survey (2015). Data were analyzed using the SPSS 22.0 program. As a result, the relationship between cardiovascular disease and periodontal disease was 1.27 times higher in obesity group compared to normal group, when adjusted for disturbance variables (age, smoking status, drinking status, income) In hypertensive patients, the hypertension group had a 1.32-fold higher risk of periodontal disease when the disturbance variables (age, smoking status, drinking status, income) were adjusted compared to those without hypertension. Therefore, cardiovascular disease is associated with periodontal disease, and it can be used as a good basis for educational and preventive measures to reduce or prevent the incidence of cardiovascular disease and periodontal disease in the future.
