Penetrating Injuries by Foreign Body in the Head and Neck Region (두경부의 이물질 삽입에 의한 관통성 외상)

  • Penetrating injuries in the head and neck region are not common but can pose difficult situations to manage properly. In small cross-sectional area, the neck housed many vital structures, such as carotid artery, internal jugular vein, cervical spines, esophagus, laryngotracheal complex and nerves. Because each vital structure is located within the fascial compartments, bleeding into these closed spaces can give rise to compression of surrounding structures, which may result in compromised airway. Therefore, management of the penetrating injuries should be based on the fully understanding of anatomical relationships, accurate clinical examinations, a careful history taking and the proper treatment planning. We present two cases of penetrating injuries in the head and neck region and discuss on the clinical considerations for the proper management with the literature review.

Subclavian artery pseudoaneurysm of 10 days after a traffic accident: A Case Report (교통 사고 10일 후 발생한 쇄골하 동맥 가성동맥류 1례)

  • The subclavian artery pseudoaneurysm in blunt trauma is uncommon and rarely occurs secondary to penetrating injury. Subclavian artery injuries represent an uncommon complication of blunt chest trauma, this structure being protected by subclavius muscle, the clavicle, the first rib, and the deep cervical fascia as well as the costo-coracoid ligament, a clavi-coraco-axillary fascia portion. Subclavian artery injury appears early after trauma, and arterial rupture may cause life-threatening hemorrhages, pseudoaneurysm formation and compression of brachial plexus. Most injuries were related to clavicle fracture, gunshot, other penetrating trauma, and complication of central line insertion. The presence of large hematomas and pulsatile palpable mass in supraclavicular region should raise the suspicion of serious vascular injury and these clinical evidences must be carefully worked out by physical examination of the upper limb. Since the first reports of endovascular treatment for traumatic vascular injuries in the 1993, an increasing number of vascular lesions have been treated this way. We report a case of subclavian artery pseudoaneurysm 10 days after blunt chest trauma due to traffic accident, treated by endovascular stent grafting.

Drinking Pattern and Nonfatal Injuries of Adults in Korea (성인에서 AUDIT와 손상의 연관성)

  • As alcohol use is one of the most important risk factors for injuries, this study was intended to clarify and evaluate any relationship between drinking patterns and the incidence rates/specific characteristics of injuries in adult populations, using a widely accepted tool, the Alcohol Use Disorders Identification Test (chronic alcohol drinking behaviors measurement, hereinafter the AUDIT) developed by the World Health Organization to help to assess the behaviors in a more accurate and reliable manner. This study used the data collected from the 2009 Korea National Health and Nutrition Examination Survey (KNHANES), in which 7,511 of 7,893 adult participants aged ${\geq}19$ years answered the questions about injuries, and excluding 104 non-respondents, 6,258 of participants in the questionnaire survey of drinking patterns were finally analyzed. The incidence rates and specific characteristics of injuries as classified by the AUDIT categories (i.e., body regions, types and mechanisms) were assessed and estimated in terms of their relative risk using t-test, ANOVA, and logistic regression. SPSS 19.0 statistical package software was employed for statistical analyses. These analyses indicate that the incidence rates of overall injuries were significantly higher in male respondents than in female respondents. The risks of alcohol use related injuries were 8.3 times higher in male respondents than in female ones. Regarding educational background, high school graduates showed the highest rates in the AUDIT with significant difference from the other groups. The married group and the group of respondents having monthly income estimated at KRW 2.01 to 3 million also showed the highest rates in the AUDIT compared to the other groups, indicating statistically significant difference. Significantly increased in problematic drinkers and those with alcohol dependence, the incidence rate of injuries body regions was 0.0371 in the head/neck, and with respect to the AUDIT and the mechanisms of external causes of injuries, transport accidents ranked first, followed by slippage, others, crash and fall. In regard to the classified types of injuries, it was statistically significant in others (e.g., laceration, contusion, addiction, or penetrating wound). In conclusion, the mechanisms of external causes of injuries as well as injuries attributed to alcohol use are very important, and a strategy is required to reduce such the injuries in the manner of decreasing the frequency of drinking after motivation by professional counsellors.

  • In this study, a series of processes for evaluating the effective protection against barriers that should be equipped in institutions that perform reliability tests on high-risk ammunition, such as high-explosive ammunition, were introduced. The impact that high-explosive bombs can have on personnel includes damage to the eardrum and lungs caused by explosion overpressure and penetrating wounds that can be received by fragments generated simultaneously with the explosion. Therefore, a high-explosive with COMP B explosives as its contents were set up, and an explosion protection theory investigation to calculate the degree of damage, numerical calculations and simulations were performed to verify the protection power. A numerical calculation revealed the maximum explosion overpressure on the protective wall when the high-explosive exploded and the penetration force of the fragment against a 50 mm-thick protective wall to be 77.74 kPa and 41.34 mm, respectively. In the simulation verification using AUTODYN, the maximum explosion overpressures affecting the firewall and personnel were 56.68 kPa and 18.175 kPa, respectively, and the penetration of fragments was 35.56 mm. This figure is lower than the human damage limit, and it was judged that the protective power of the barrier would be effective.

  • Laceration of soft palate and oropharynx is relatively common in children. Soft palate laceration has been reported patients of all ages, ranging from new born to geriatric patients. However, young children often place objects their mouth, they may fall on the object or receive a direct force on the object which then perforates the soft palate tissue. Most frequently affected site is the left supra-tonsillar area. Lesions are predominately in the soft palate with-out perforation. Linear and superficial wounds are frequent. A typical injury is the flat- U-, or V-shaped with apex directed anteriorly. In those cases without any through-and-through lacerations or any tissue loss, suture is not necessary. Furthermore, suture of the affected site may hinder wound healing. Healing of the wound should be complete by three weeks with minimal scarring. There have been reports of carotid artery injury due to soft palate laceration causing neurologic complications. For such reasons, traumatized child must be in close observation for 2-3days, and if symptoms of complication are noticed consultation with oromaxillofacial surgeons, E.N.T., or neurologist is required. These are two reports of boys 2 and 3 years of age who had soft palate laceration caused by a falling-down-in jury with an object in their mouth. They were treated non-surgically and neurologic evaluation was carried out for 1 week with complete healing.

Clinical Characteristics of Patients with Traumatic Diaphragm Injury and Comparison of Scoring Systems as Predictors (외상성 횡격막 손상환자의 임상적 고찰과 예후 예측 인자의 유용성 비교)

  • Because of high morbidity and mortality, traumatic diaphragm injury remains a diagnostic challenge. In this study, we evaluate that some factors and scores can be used as predictors. Material and Method: From May 1995 to June 2003, 23 patients with traumatic diaphragm injury were enrolled. We examined the clinical features of patients. RTS, TRISS, ISS and APACHE II scores for each patient are calculated for analyzing the relationship of mortality and ICU duration. Result: The study identified 15 men $(65.2{\%})$ and 8women $(34.8{\%})$. There are right sided diaphragmatic injury in 11 patients $(47.8{\%})$, left sided in 11 $(47.8{\%})$, and both sided in 1 ($0.4{\%}$). Plain X-ray, CT, upper GI contrast study and esophagogastroscopy were used as diagnostic tools. Age, hemodynamic status, early diagnosis are not associated with outcome. As prognostic factor, RTS and ISS are associated with mortality and there was negative relationship between RTS and ICU duration (r=0.737, p=0.026). Conclusion: An early diagnosis of traumatic diaphragm injury can frequently be missed in the acute trauma setting. So high index of suspicion and a careful examination are important in multiple trauma patients. An RTS can probably be used effectively as a predictor for the severity and prognosis in patients with traumatic diaphragm injury.

Analysis of Human Body Injury by Non-penetrating Ballistic Impact Using a Finite Element Model of the Head and Neck (근육 모델이 고려된 두부 및 경추 유한요소모델을 이용한 비관통 피탄 충격에 의한 인체 상해 해석)

  • Ballistic impact on a soldier wearing a helmet can induce fatal injury, even if the helmet is not penetrated. Although studies on this type of injury have been performed, most of them have used an analytical model focused on head injury only. The injury of the neck muscles and cervical vertebrae by non-penetrating ballistic impact affects the survivability of soldiers, despite not inflicting fatal injury to the human body. Therefore, an analytical model of the head and neck muscles are necessary. In this study, an analysis of human body injury using the previously developed head model, as well as a cervical model with muscles, was performed. For the quantitative prediction of injury, the stress, strain, and HIC were compared. The results from the model including the cervical system indicated a lower extent of injury than the results from the model excluding them. The results of head injury were compared with other references for reliability.

Acceleration Test Method for Failure Prediction of the End Cap Contact Region of Sodium Cooled Fast Reactor Fuel Rod (소듐냉각 고속로 연료봉단의 접촉부 손상예측을 위한 가속시험 방법)

  • This paper reports the results of an acceleration test to predict the contact-induced failure that could occur at the cylinder-to-hole joint for the fuel rod of a sodium-cooled fast reactor (SFR). To incorporate the fuel life of the SFR currently under development at KAERI (around 35,000 h), the acceleration test method of reliability engineering was adopted in this work. A finite element method was used to evaluate the flow-induced vibration frequency and amplitude for the test parameter values. Five specimens were tested. The failure criterion during the life of the SFR fuel was applied. The S-N curve of the HT-9, the material of concern, was used to obtain the acceleration factor. As a result, a test time of 16.5 h was obtained for each specimen. It was concluded that the $B_{0.004}$ life would be guaranteed for the SFR fuel rods with 99% confidence if no failure was observed at any of the contact surfaces of the five specimens.

  • With the exception of gun shot wound, the incidence of penetrating injury of face and neck areas nonorganic foreign bodies is relative low. But the diagnostic evaluation and therapeutic management of penetrating facial wounds need careful decision, when the anatomic proximity of the major vessels and nerve is considered. Penetrating facial trauma with concomitant vascular injury present challenging problems, the immediate complication of this vascular injury are severe bleeding, hematoma formation, shock, obstruction of airway. The vascular injury is conformed by angiography. In this report, a industrial tool(long tack) fired by explosive air is penetrated into face and to neck. In angiograms penetrating injury of the vertebral artery is detected. We performed the embolization of the vertebral artery with coils and manual removal of the foreign body without any complication was followed.

  • Lightning causes physical damage to aircraft, such as melting, burning and arcing, and magnetic field that occurs on the aircraft's outer body during the penetration of a lightning stroke causes voltage and current transients in the electronics and wiring within the aircraft. This effect will cause induced lightning strikes in the aircraft's internal airborne electronic systems, preventing safe flight. This paper introduces protection circuit design techniques, and the test results that meet the requirements for certification of criteria.
