Spinal Stability Evaluation According to the Change in the Spinal Fixation Segment Based on Finite Element Analysis (유한요소해석 기반 척추 고정분절 변화에 따른 척추 안정성 평가)

  • In this study, we evaluated spinal stability based on the change in the thoracolumbar fixation segment using finite element analysis (FEA). To accomplish this, a finite element (FE) model of a normal thoracolumbar spine (T10-L4), including intervertebral discs (IVD), ligaments, and facet joints, was constructed, and the material properties reported in previous studies were implemented. However, L1 was assumed as the lesion site, and three types of posterior fixation, namely, L1-L2, T12-L2, and T12-L1-L2, were implemented in the thoracolumbar FE model. In addition, the loading conditions for flexion, extension, lateral bending, and axial rotation were adopted. Through the series FEA, the deformation, equivalent stress, range of motion, and moment on the pedicle screws, vertebrae, and IVD were calculated, and the spinal stability was evaluated based on the FEA results.

A Case Study of a 53-year-old Female Patient with Frozen Shoulder for Effect of Scapular Stabilizing Exercise on Shoulder Pain, Active Range of Motion and Position of Shoulder Blade (오십견이 있는 53세 여성환자에게 견갑골 안정화 운동 적용 후 견부 통증, 능동관절가동범위, 견갑골의 위치 변화에 미치는 영향-사례연구)

  • Background: The research examined the effect of a scapular stabilizing exercise on shoulder pain, active range of motion (AROM) and changes in the position of the shoulder blade of patients with frozen shoulder. Methods: General physical therapy and a scapular stabilizing exercise were applied to a 53-year-old female patient with frozen shoulder three times a week for four weeks. Before and after the therapy, measurements were made on the visual analog scale (VAS) and of the AROM, and a lateral scapular slide test was conducted. Results: Following the therapy, VAS fell from 7 to 3. Before therapy, flexion, abduction, internal rotation, and external rotation recorded $133^{\circ}$, $102^{\circ}$, $45^{\circ}$, and $53^{\circ}$ respectively; after the therapy they increased to $150^{\circ}$, $123^{\circ}$, $55^{\circ}$, and $65^{\circ}$ respectively. The lateral scapular slide test showed sliding distances before therapy of 8.5cm at $0^{\circ}$, 9.2cm at $45^{\circ}$, and 11.3cm at $90^{\circ}$; after therapy, the distances decreased to 6.8 cm at $0^{\circ}$, 8.2 cm at $45^{\circ}$, and 9cm at $90^{\circ}$. Conclusion: The scapular stabilizing exercise was effective in alleviating shoulder pain, increasing AROM, and reducing changes in the position of the shoulder blade of frozen shoulder patients.

The Effect of Functional Postural Trunk Exercise on Pain, Activities of Daily Living, Range of Motion, Deep Cranio-cervical Flexor Muscle Endurance in Neck Pain Patient by Acute Whiplash-Associated Disorders (체간의 기능적 자세운동이 급성 편타성-관련손상에 의한 경부통 환자의 통증과 기능장애수준, 관절가동범위, 두경부 심부굴곡근 지구력에 미치는 영향)

  • PURPOSE: The main objective of this study is observing the effects of the functional postural trunk exercise (FPTE) on deep cranio-cervical flexor (CCF) muscle endurance applied on neck pain patients suffering from the acute whiplash-association disorder (WAD). METHODS: The study was tested with 47 patients with neck pain. All patients were equally treated with the ordinary therapy, only experimental group (n=23) was treated with FPTE: assumption of an upright lumbo-pelvic and spinal postural position, adding a neck lengthening manoeuvre in addition. Patients attended physical therapy for 3 times a week, for 4 weeks. Visual analogue scale (VAS) for pain, neck disability index (NDI), range of motion (ROM), were recorded both before and after the intervention. Also muscle activity in the CCF test was employed to analyze the changes between before and after. RESULT: After 4 weeks of training intervention, a remarkable increase in muscle endurance, rotation, extension ROM (p<.05) and decrease in pain and NDI (p<.05) in both groups. Further, the differences between groups were muscle endurance (F=60.350, p<.01), pain, and the left rotation ROM (p<.05). CONCLUSION: From the results, the experimental group had significant the increase in muscle endurance, and the significant decrease in pain. Whilst further research in this category is necessary, these observations suggest that applying exercise in early diagnosis can be of help to treat the neck pain patients suffering from the WAD.

  • The purpose of the present study was to the investigate the effect of sports massage, kaltenborn-evjenth orthopedic manipulative therapy and electrical therapy, general exercise on the limitation of range of motion(ROM) and on the pain(VAS) in patients with frozen shoulder. Fiftheen frozen shoulder patients between 50 and 60 years of age(females) were selected and were divided equally by random distribution into A group(sports massage, kaltenborn-evjenth orthopedic manipulative therapy, N=7)and B(electrical therapy, general exercise therapy, N=7) group. The results obtained were as follows : 1) The range of motion between two groups are significantly different in the treatment times, the ROM of A group increase in after-treatment(2 week 4 week) in comparison with ROM in before-treatment, it is significant increase. And it is significant difference in B group. 2) The pain level between two groups are not significantly different in the treatment times, the pain level of A group decrease in after-treatment(2 week, 4 week) in comparison with before-treatment, it is significant decrease. And it is significant difference in B group. Although the pain level of the A group decrease in after 2 week treatment in comparison with before-treatment but it is not significant decrease. Although the pain level of the B group decrease in after 4 week treatment in comparison with after 2 week treatment, but it is not significant decrease. 3) The A group is more effective in increasing the ROM and decreasing the pain level than B group during treatment times. The results showed that both A group method and B group method are effective ROM increase and pain reduce, but A group method is superior to B group method in ROM increase and pain reduce.

The Effect of Lower Trapezius Strengthening Exercises on Pain, Disability, Cervical Range of Motion and Strength of Lower Trapezius in Patients With Unilateral Neck Pain : A Controlled Randomized Trial (하승모근 강화운동이 편측 경부통 환자의 통증, 기능장애, 경부 관절가동범위, 하승모근 근력에 미치는 영향 : 무작위 할당 대조군 실험)

  • This study aimed to determine the effect of lower trapezius muscle strengthening exercises on pain, neck disability index (NDI), cervical range of motion (ROM), and lower trapezius muscle strength in patients with unilateral neck pain. Following baseline measurements, the subjects (N=40) with unilateral neck pain were randomized into one of two 5 weeks exercise intervention groups: a experimental group (EG, $n_1=20$) that received strength training of the lower trapezius muscles or a control group (CG, $n_2=20$) that received routine physical therapy program. Each group participated in the intervention for 30 minutes, 3 times a week, for 5 weeks. All participants performed 2 repetitions of each intervention per day. The numeric pain rating scale for pain, NDI, ROM, and lower trapezius strength were recorded both pre- and post-intervention for both groups. Paired t-tests were used to determine significant changes post-intervention compared with pre-intervention and independent t-tests were used to analyze differences in the dependent variables between the 2 groups. After the 5-weeks intervention, both groups experienced significantly decreased pain and disability level (p<.05) and significantly increased cervical flexion, extension, lateral flexion, and rotation ROM (p<.05). The EG that received strength training of the lower trapezius muscles showed greater improvements in pain and functional disability level, cervical rotation, and lower trapezius strength than the CG (p<.05). These results suggest that a lower trapezius strengthening exercises reduce neck pain and neck disability level and enhance cervical ROM and lower trapezius strength level in patients with unilateral neck pain.

  • Frozen shoulder was diagnosed with this study 14 patients were enrolled. Patients who volunteer to visit the configuration is more than six months and pain were studied in a chronic illness. Frozen shoulder to patients based on these results the effect of KTO treatment is a diet that is valid for a unified model and the exact effect can be enhanced, be combined with other treatments and therapies are deemed necessary. When applied to patients with Frozen shoulder KTO with an increased range of motion of the shoulder can result in a reduction of pain.

Effectiveness of Upper Extremity Exercise and Bandage on the Edema and ROM of Patients with Lymphedema (상지운동과 붕대법이 상지림프부종 환자의 부종과 관절가동범위에 미치는 영향)

  • Purpose : The purpose of this study was to examine the effect of upper extremity exercise and Bandage on the edema and range of motion. Complex decongestive physiotherapy was one of the latest methods to deal with the limitation of range of motion and the edema that were the prime problems of patients with lymphedema. Methods : Twenty patients undergone mastectomy were participated in this study voluntarily. They had lymphedema on upper extremity and partial limitation of range of motion. The subjects had been treated with upper extremity home exercise and complex decongestive physiotherapy with Bandage for 4 weeks. The measure of these patients with upper extremity edema was included: the volume, arm circumfenence and range of motion. These were measured two times: before the treatment, after 4 weeks of the treatment. Results : The upper extremity edema of patients treated home exercise group and complex decongestive physiotherapy group with Bandage was definitely decreased (p<0.05). Moreover, the upper volume of those who were the same condition also significantly shrank and patients' arm circumferences of upper arm breathtakingly diminished as well (p<0.05). But home exercise group patients' arm circumferences of forearm was not remarkable(p>0.05). However, patients' range of motion who were treated with upper extremity exercise were evidently increased when compared to that of the before treatment (p<0.05). Conclusion : Complex decongestive physiotherapy and bandage performed by physical therapist increased the ROM of upper arm and reduced the edema in patients with lymphedema.

Effects of Therapeutic Exercise on Pain, Range of Motion, Strength and Balance Ability in a Patient with Anterior Ankle Impingement: A Case Study (치료적 운동이 앞 발목 충돌 증후군 환자의 통증, 관절가동범위, 근력 및 균형능력에 미치는 영향: 증례보고)

  • PURPOSE: The purpose of this study is to determine the effect of therapeutic exercise on pain, range of motion and strength, and balance ability in a patient with an anterior ankle impingement (AAI). METHODS: A-32-year-old male presented limited ankle motion and pain with forced dorsiflexion at both ankle joints. In response, a therapeutic exercise program consisted of ankle joint mobilization, strength exercises using an elastic band, and proprioceptive exercises including semi-squats and a one-legged standing exercise with open and closed eyes. The program was performed for 40 min/day, twice per week, for 8 weeks. Pain, range of motion (ROM), and muscle strength (ankle dorsiflexion, plantarflexion, inversion, and eversion), as well as a one-legged standing test of both ankles, were measured before and after 4 and 8 weeks of therapeutic exercise. RESULTS: VAS decreased in both ankles after 8 weeks of exercise, respectively, compared to baseline levels. Range of motion and strength increased in both ankles for dorsiflexion, plantarflexion, inversion, and eversion after 8 weeks of exercise compared to baseline levels. In addition, the ability to perform a one-legged standing test with eyes opened and closed improved in both legs after 8 week of exercise compared to baseline levels. CONCLUSION: These results suggest that therapeutic exercise improves pain, ROM, muscle strength, and balancing ability in patients with AAI.

The Effect of Therapeutic Massage and Mckenzie Exercise on Pain and Range of Motion in Chronic Neck Pain Patient: a Case Study (치료적 마사지와 맥켄지 운동이 만성 경부통 환자의 통증과 관절가동범위에 미치는 영향: 단일사례연구)

  • Purpose : The purpose of this study was to examine the effect of therapeutic massage and Mckenzie exercise on pain and range of motion in chronic neck pain patient: a case study. Method : A Twenty-year old female subject with chronic neck pain was selected, she received therapeutic massage and Mckenzie exercise for 30 minutes a day, three days a week for five weeks. Pain was measured visual analog scale(VAS), neck disability index(NDI) and the Copenhagen neck functional disability scale (CNFDS). Range of motion was measured using a goniometer(shoulder joint) and the CROM Deluxe(cervical range of motion instrument). Result : VAS, NDI and CNFDS were decreased 3 score, 4 score and 6 score(respectively) than before the training. The range of motion was increased in shoulder joint and neck movement than before the training. Conclusion : The massage and Mckenzie exercise increased range of motion in shoulder joint and neck movement, and decreased neck pain in chronic neck pain patient. Additional research on therapeutic massage and Mckenzie exercise for improving pain and range of motion is need.

The Effects of Kaltenborn Orthopedic Manual Therapy with Evjenth-hamberg Stretching on Range of Motion of Ankle Joint and Balance Ability in Patients with Chronic Stroke (칼텐본 정형도수 물리치료와 에비안스-함베르크 뻗침 운동의 결합이 뇌졸중 환자의 발목관절 가동범위와 균형능력에 미치는 영향)

  • PURPOSE: Stroke patients have limited ankle range of motion and balance problems. The purpose of this study was to determine the effects of Kaltenborn orthopedic manual therapy, Evjenth-hamberg stretching, and combination of both on ankle dorsiflexion range of motion and dynamic balance ability in stroke patients. METHODS: Thirty patients were placed in three groups, each of which received different treatments: Kaltenborn orthopedic manual therapy (15 mins), Evjenth-hamberg stretching (15 mins), or Kaltenborn orthopedic manual therapy with Evjenth-hamberg stretching (30 mins). Each group received three exercise sessions per week during four weeks. To determine its effectiveness of interventions an evaluation was carried out. ankle dorsiflexion range of motion was gauged using a tiltmeter application, and dynamic balance ability was measured using the Berg balance scale. RESULTS: All groups revealed significant improvements in ankle dorsiflexion range of motion, Berg Balance Scale (BBS) score before and after the intervention, and Kaltenborn orthopedic manual therapy with Evjenth-hamberg stretching group showed a significant improvement in ankle dorsiflexion range of motion and BBS score in comparison to other groups. CONCLUSION: We found out that each individual method was effective in ankle dorsiflexion range of motion, dynamic balance ability in stroke patients, and the combined method was more effective.
