Arthroscopic Management of Septic Coxitis (화농성 고관절염의 관절경적 처치)

  • Purpose : The purpose of this study is to evaluate the usefulness of arthroscopy in the management of septic coxitis in adults. Materials and Methods : The subjects were 9 patients ranging from 46 to 61 years of age. All patients were diagnosed to have septic coxitis by arthrocentesis and laboratory tests. Operative arthroscopy of the hip has been performed on the patients under supine position. All patients had a follow-up period of more than 12 months by checking ESR and CRP, and by evaluating the function with Harris hip scoring system. Results : The laboratory abnormalities returned to normal level in average of 11.9 days after the surgery. Twelve months after the surgery, all patients showed normal functional hip without any discomfort. Conclusion : Arthroscopic debridement and irrigation technique seem to have more advantages than open drainage in the management of septic coxitis minimizing the morbidity of the hip joint, while it obtains the same results as open technique.

Efficacy of Ultrasonogram for the Diagnosis of Biceps Tendon Pathology (상완 이두건의 병소에 대한 초음파 검사의 유용성)

  • Purpose: We wanted to assess the accuracy of ultrasound for detecting abnormality of the long head of the biceps tendon in patients with rotator cuff disorders. Materials and Methods: Between January 2006 and March 2007, we reviewed the arthroscopic findings of biceps tendons in 67 patients with rotator cuff disorder and who underwent ultrasonography pre-operatively. The patients' average age was 58 years and there were 41 males and 26 females. The statuses of the biceps tendons were described as 'normal', 'dislocation', 'subluxation', 'partial or complete tears', and 'tendinopathy'. We investigated the correspondence between the arthroscopic and ultrasonographic findings. Results: On ultrasonography, the biceps tendons were 'normal' in 37 patients and 'abnormal' in 30 patients. On arthroscopy, 5 biceps tendons turned out to have partial tears, which were 'normal' on ultrasonography. On the other hand, 8 biceps tendons were 'normal' which were 'abnormal' on ultrasonography. Three dislocations and 4 complete tears of biceps tendons were identified on arthroscopy, and all of them were detected by ultrasonography. Of the 20 cases of 'tendinopathy' seen on ultrasonography, 15 patients had partial tears and the remaining 5 patients were normal on arthroscopy. Conclusion: Ultrasonography is accurate for detecting some biceps pathologies, such as dislocation or complete tear, but it is not accurate for the detection of partial tear.

Diagnostic Correlation between Ultrasonography and CT Arthrography in Rotator Cuff Disease (회전근 개 질환에서 초음파 검사와 관절 조영 컴퓨터 단층 촬영의 진단적 가치 비교)

  • Purpose: The aim of this study wasto evaluate the comparison of CT arthrography and ultrasonography, confirmed by the arthroscopic finding in patients with rotator cuff disease. Materials and Methods: We evaluated fifty seven patients with rotator cuff disease underwent CTA and arthroscopy, and twenty eight patients had taken ultrasonographyadditionally. The diagnostic value and prediction for tear size between CTA and ultrasonography were evaluated, as compared to arthroscopic findings. Results: CTA showed a sensitivity of 86.2% and a specificity of 100% in full thickness tear ofsupraspinatus, a sensitivity of 58.3% and a specificity of 87.8% in partial-thickness tear. CTA demonstrated good diagnostic value for full thickness tear, but there was relatively lower value for partial-thickness tear. Ultrasonography showed a sensitivity of 84.6% and a specificity of 86.7% for diagnosing in full thickness tear, a sensitivity of 84.6% and a specificity of 73.3% in partial-thickness tear. Ultrasonography provided good diagnostic value, but, there is lesser accurate result for prediction of tear size. Conclusion: CTA showedgood diagnostic tool of detection full-thickness tear of rotator cuff disease and predicting of tear size. Comparing with ultrasonography, CTA was inferior for detection of partial-thickness tear, but, provided better estimation for tear size.

Association between Oral Health Status and Rheumatoid Arthritis (구강건강상태와 류마티스 관절염의 관련성)

  • The purpose of this study was to examine the association between oral health status and rheumatoid arthritis (RA). The study used a nationally representative sample of Koreans (2013 Korea National Health and Nutrition Examination Survey) aged 19 years over (n=6,113). Dependent variable was RA, which was assessed with oral health status. Independent variable was oral health status (periodontal status, missing tooth). The chi-square test and logistic regression analysis were performed to identify the association between oral health status and RA. Results of logistic regression analysis for association between periodontal status and RA was no significant. Results of logistic regression analysis for association between missing tooth and RA was statistically significant. The odds ratio (OR) for RA participants was 3.03 (95% confidence interval [CI], 1.47~6.23) in missing tooth 19~28 than missing tooth 0~8. The OR for RA participants was 2.08 (95% CI, 1.06~4.08) in missing tooth 9~18 than missing tooth 0~8. After adjustment for confounders (socio-demographic factors, health behaviors), results of logistic regression analysis was no significant. More missing tooth among adults was greater the risk of RA. By promoting the improvement of oral hygiene and oral health would contribute to reduce the risks associated with systemic diseases. Future study is needed to examine the detailed causal relations between oral health status and RA bidirectionally.

Arthroscopic Treatment for Painful Hip in Young Patients (젊은 연령의 동통성 고관절에 대한 관절경적 치료)

  • Purpose : We analyzed the effectiveness of arthroscopic diagnosis and treatment for painful hip in young patients caused by labral tear or synovial disease. Materials and Methods : From May 1997 to June 1998,7 young patients who had painful hip were diagnosed by history, physical examination, radiologic studies(including plain films, MRI, MR arthrogram) and arthroscopy. We treated the lesions by arthroscopic surgery and followed up for more than 1 year. Results : On arthroscopy, the labrum was torn in 6 cases and chronic synovitis was found in 1 case. After arthroscopic surgery, pain was eliminated completely in 5 cases and mild pain remained in 2 cases, respectively. But there was no limitation in activities of daily living and athletic performances. Conclusion : Arthroscopy is effective in diagnosis and treatment of labral tears and synovial diseases in young patients.

Intra-articular Corticosteroid Treatment of Biceps Tenosynovitis in a Dog (개에서 두갈래근 힘줄 윤활막염의 관절강 내 코티코스테로이드 치료 1예)

  • A 3-year-old, 35kg, neutered male Borzoi was admitted with a history of intermittent weight-bearing left forelimb lameness. Physical examination revealed pain response on left bicipital tendon during palpation of the tendon and shoulder flexion and extension. Radiographic findings of the left shoulder joint included intertubercular grove osteophytes and periarticular changes. Arthrograms revealed a roughened and irregular appearance to the biceps tendon. Synovial fluid analysis is consistent with degenerated joint disease. It was diagnosed as biceps tenosynovitis on the basis of history, physical examination, radiography and arthrograms. The dog was treated with medical management. Medical treatment included an aseptic intra-articular injection of 40mg methylprednisolone acetate in the left shoulder and strict exercise restriction. Three weeks later, the dog responded well to glucocorticoid therapy.

  • Benign fibrous histiocytoma of the infrapatella fat pad is very rare. We report one case of the benign fibrous histiocytoma involved between the patella fat pad and synovium. Diagnostic arthroscopic procedure was performed to investigate the retrobulging shape of synovium without specific synovial changes, and there was no meniscal structural deformity and abnormality of joint motion. The lesion was completely resected. At 24 monthes follow-up was performed, and the function of knee was to be sufficient.

  • Headache disorders, one of most common disease in general population, have been developed according to many versions of international classifications. The primary headaches are those in which no consistently identified organic cause can be determined. It is divided into the following categories: (1) migraine, (2) tension-type headache, (3) cluster headache and other trigeminal autonomic cephalalgias, (4) other primary headaches. This review described a diagnosis of primary headache disorders based on International Classification of Headache Disorders (ICHD)-3 beta criteria.

  • The shoulder is a complex joint and, by virtue of having a large range of motion, is inherently unstable, relying on the surrounding soft tissue structures for stability. The bony joint consists of the glenoid, acromion, and humoral head, while the soft tissues include the glenoid labrum, the glenohumeral ligaments. and coracoacromial ligament as well as the muscles of the rotator cuff, the long head of the biceps, and the scapulothoracic muscles. Dysfunction in any one of these components can cause shoulder problems. The throwing motion involves a series of phases that stress to their limits the dynamic and static restraints of the glenohumeral and scapulothoracic joints. . Therefore, maintaining a balance of proper biomechanical forces is essential to avoiding shoulder injuries in throwing athletes. Over the last decade, signficant advances have been made in the study and understanding of the shoulder mechanics, and pathophysiology of injury. Additionally, advances in surgical techniques, particularly arthroscopy , have aided in the diagnosis of and the developement of less invasive surgical treatments for injuries that do not respond to nonoperative measures. In this article, we reviewed the pathophysiology of injuries , diagnostic techniques, and surgical management of shoulder injuries in throwing athletes .

  • Toinsul consists of breathing method and exercising method by stimulation of acupuncture point and meridian. It activates the circulation of energy and blood and helps excretion of noxious vapor and it improves elasticity and vitality of muscles, tendon md ligament. It sharpens both secretion of synovia .and circulation around joints and also controls the whole body through meridian. This report introduces some about the exercise methods for treatment of lumbago and shoulder joint diseases among medical, surgical and gynecological diseases. It can be used for health care, treatment, relaxation muscles and recovery from fatigue in an aspect of prevention. It has been widely used for treatment of diseases and promotion of health so far even though it is not yet demonstrated scientifically. Therefore, it should be used mere efficiently as an important part of the oriental physical therapy.
