Multiple Low-grade Osteosarcoma in Children - A Case Report - (소아에서 다발성으로 나타난 저등급 골육종 - 1예 보고 -)

  • Low grade central osteosarcoma is an rare variant of conventional osteosarcoma and generally occurs in older age than conventional. We experienced a case of low grade osteosarcoma occurred in a 12 years old female. Moreover, it occurred multifocally in left tibia, left femoral neck and midshaft, and left acetabulum. We could not sure whether it was from synchronous or metachronous metastasis. Usually the low grade osteosarcoma progresses very slowly. But, despite this case was low grade osteosarcoma with very indolent progression, furthermore, it showed synchronous or metachronous metastasis. So we report this unusual case with review of literature.

  • Hemangiopericytoma is a rare tumor of blood vessels, first described and named by Stout and Murray in 1942, arising from the contractile, elongated cells which surround capillaries originated from the pericytes of Zimmermann. It can develop anywhere there are capillaries and mostly in the connective tissue of the human body. It can also develop at any age from 12 to 90 years old, and the rate of metastasis and prognosis vary with its histologic finding, We report three cases of hemangiopericytoma - one recurred in the right forearm where a tumor was excised 15 years ago, another occurred in the left pubis which metastasized to lung and head, the other metastasized from the lung to the right scapular area - treated at the department of orthopaedic surgery, Hanyang University Hospital from 1989 to 1995.

Alveolar Soft Part Sarcoma (포상 연부 육종)

  • Alveolar soft part sarcoma, named by Christopherson in 1952, is a very rare soft tissue sarcoma. It reportedly accounts for 0.5 to 1.0% of all soft tissue sarcoma and no established treatment principle assists it. The relative lack of symptoms makes it easily overlooked and consequently early diagnosis is frequently impossible. Often, the metastasis to the lung or the brain is the first manifestation of the disease. In some cases wide excision is not performed because it is misdiagnosed as a benign tumor. Authors experienced 7 cases of alveolar soft part sarcoma in 20 years and found the ultimate prognosis grave and the treatment not promising. Therefore, the authors recommend the following; 1. Early diagnosis and early treatment are important for successful management of alveolar soft part sarcoma. 2. Preoperative incisional biopsy is necessary for pathologic diagnosis. 3. The recommended treatment modality is a combination of wide excision, chemotherapy and radiotherapy.

  • Survival rate of osteosarcoma has been improved recently due to the neoadjuvant and adjuvant chemotherapy. Limb-salvaging operation(LSO) has replaced the amputation technique without' lowering the survival rate. And there occurred a lot of patients who are suffering from the high cost of artificial implants and forced to choose amputation due to economic problem. In LSO, usually relatively high cost artifical implant is needed. When a patient and not afford such an expensive implant he had to choose an inexpensive way, amputation. Authors tried bone lengthening by adopting Ilizarov technique after wide resection of tumor in two patients. Bone transportation was successful in one patient and less successful in the other. One case in CDF(continuosly disease free since the surgical procedure) state at follow-up 3 year 4 months after knee joint fusion. And the other was given lobectomy for lung metastasis at postop. 1 year and 9 months, and given osteosynthesis for infected nonunion at the docking site. Bone transportation was thought to be a good method for the bony coverage of dead space caused by wide resection. Bone transportation technique was economical as well as biological We present two osteosarcoma patient who treated with Ilizarov bone transportation.

Treatment of Subungual Glomus Tumor (손톱밑에 발생한 사구종의 치료)

  • Glomus tumor is uncommon tumor with an incidence of about 1% in soft tissue tumors and the single most common location is the subungual region of the finger. Many reports have emphasized that glomus tumors were difficult to diagnose and that the result of surgical removal was good, but nail deformity has received little attention. Most of authors removed the entire nail or partially splitted the nail to exposure the nail bed and tumor. Nobody reported whether nail deformity after operation was present or not. But, an abnormal nail is both a cosmetic and a functional problem in that catches on objects. In this study, our attention was postoperative nail deformity as well as the results of surgical removal. Seven patients(eight cases) who were diagnosed as a subungal glomus tumor in the hand were treated by surgical excision without removal of the nail and followed over twenty two months. And then we report on our experience with 8 cases of subungual glomus tumor which were successfully treated by complete excision without nail deformity.

  • Ganglion cysts that occurred within sheaths of peripheral nerves have been documented in literatures, but it is relatively rare condition. The peroneal nerve is the most common site of involvement. Other reported sites of involvement are the radial, ulnar, median, sciatic, tibial and posterior interosseous nerves. We report a case of the intraneual ganglion cyst within peroneal nerve and another case of the intraneural ganglion cyst within a medial plantar nerve that were successfully excised without neurologic complication.

  • Chondosarcoma of the calcaneus in young child is extremely rare. A thirteen-year old male patient has suffered from right heel pain for several months before. Dominant gross feature was hard mass and swelling on one entire right heel, and increased density and calcification on entire calcaneus with cortical destruction, and calcific densities on soft tissues around calcaneus were seen on roentgenogram of right ankle. It was diagnosed to be chondrosarcoma by bone biopsy. It was treated by below knee amputation and chemotherapy, and no recur and distant metastsis during 12 months after operation.

  • Ameloblastoma is a benign neoplasm originating from odontogenic epithelium. It is the most common neoplasm in the jaws and is characterized by aggressive behavior and local invasion. Unicystic ameloblastoma (UA) has a unilocular feature in radiologic examination and a cystic feature histologically. Decompression and marsupialization are conservative method of treatment of large UA. The purpose of decompression and marsupialization are size reduction of the mass, which makes it easy to handle at total enucleation with protection of nerve damage and facial deformity. Here we report successful conservative treatment of extensive UA using decompression and marsupialization with a review of literatures.

An Image-guided Radiosurgery for the Treatment of Metastatic Bone Tumors using the CyberKnife Robotic System

  • Bone is a common site for metastatic spread from many kinds of malignancies. The morbidity associated with this metastatic spread can be significant, including severe pain. When it comes to spinal metastasis, occupying nearly 40% of skeletal metastases, the risks of complications, such as vertebral body collapse, nerve root impingement, or spinal cord compression, are also significant. Because of the necessity of preserving the integrity of the spinal column and the proximity of critical structures, surgical treatment has limitations when durable local control is desired. Radiotherapy, therefore, is often used as an adjunct treatment or as a sole treatment. A considerable limitation of standard radiotherapy is the reported recurrence rate or ineffective palliation of pain, either clinically or symptomatically. This may be due to limited radiation doses to tumor itself because of the proximity of critical structures. CyberKnife is an image-guided robotic radiosurgical system. The image guidance system includes a kilovoltage X-ray imaging source and amorphous silica detectors. The radiation delivery device is a mobile X-band linear accelerator (6 MV) mounted on a robotic arm. Highly conformal fields and hypofractionated radiotherapy schedules are increasingly being used as a means to achieve biologic dose escalation for body tumors. Therefore, we can give much higher doses to the targeted tumor volume with minimizing doses to the surrounding critical structures, resulting in more effective local control and less severe side effects, compared to conventional fractionated radiotherapy. A description of this technology and a review of clinical applications to bone metastases are detailed herein.

Chondroblastoma of the Patella with Secondary Aneurysmal Bone Cyst (슬개골에 발생한 연골모세포종의 동맥류성 골낭종화 - 증례 보고 -)

  • Chondroblastoma of the patella is a uncommon benign neoplasm in the middle aged person. Furthermore, a hemorrhagic cystic change associated with chondroblastoma is extremely rare. We experienced a case of chondroblastoma which underwent hemorrhgic cystic change. A fiftyyear-old female patient has suffered from right knee joint pain for 1 month before. Dominant gross feature was hemorrhagic cyst which was not involved into the joint space. Soft tissue was curetted and it was diagnosed to be chondroblastoma. The case was treated with curettage and bone graft.
