Health-related quality of life in female patients with reumatoid arthritis: a structural equation model (여성 류마티스 관절염 환자의 건강관련 삶의 질 구조모형)

  • Purpose: This study aimed to construct a structural equation model to explain and predict factors affecting the health-related quality of life (QoL) in female rheumatoid arthritis (RA) patients based on the health-related QoL model by Ferrans et al. (2005) and a literature review. Methods: Patients (N=243) who were either registered members of an internet cafe composed of patients with RA or rheumatology outpatients at two tertiary general hospitals in Busan, Korea, were recruited via convenience sampling. Data were collected from July 2 to September 9, 2021, and the survey was conducted using a web-based questionnaire. The data were analyzed by SPSS and AMOS 26.0. Results: The goodness-of-fit statistics of the final model exhibited good results (χ2/degree of freedom=2.68, Turker-Lewis index=.94, comparative fit index=.96, standardized root mean-squared residual=.04, root mean- square error of approximation=.08), and 11 out of 14 paths of the model were supported. The squared multiple correlation, which reflected the explanatory power of the environmental characteristics, symptoms, functional status, and perceived health status on health-related QoL, was 80%. In the hypothesis model, 10 paths had significant direct effects, 6 paths had significant indirect effects, and 12 paths had significant total (direct and indirect) effects. Conclusion: Considering that factors directly affecting the health-related QoL of female patients with RA were social support, symptoms (fatigue and depression), resilience, and perceived health status, and that resilience was the most influential factor, clinicians can encourage resilience. Hence, to improve the health-related QoL of female patients with RA, continuing management is necessary, using various intervention methods that focus on enhancing resilience from the early stage to the end of treatment for RA.

A Study of Relationship of Pain and Fatigue in Elderly Women with Osteoarthritis (골관절염을 가진 여성노인의 통증과 피로의 관계)

  • Purpose: The purpose of this study was to identify the relationship of pain, fatigue, and illness duration in elderly women with osteoarthritis. Methods: The subjects of the study were one hundred and thirty five elderly women (age over 65years) who have an osteoarthritis. Data were gathered from September 2002 to June 2005 using a questionnaire. Data were analyzed with the SPSS Win 11.0 using descriptive statistics, t-test, ANOVA and Pearson's correlation coefficient. Results: Pain in the elderly women with osteoarthritis correlated with fatigue (r= .447, p= .00). Pain showed a significantly positive correlation to illness duration (r= .197, p= .02). Fatigue showed a significantly positive correlation to illness duration (r= .213, p= .01). Conclusion: It is suggested that we need to develop the pain and fatigue control program considering illness duration should be provided for elderly women with osteoarthritis.

Effects of Neuromuscular Electrical Stimulation of the Vastus Medialis on Pain and Muscle Function in Patients with Knee Osteoarthritis (내측광근의 신경근전기자극(NMES)이 퇴행성 슬관절염 환자의 통증 및 근 기능에 미치는 영향)

  • The purpose of this study was to investigate the functional changes of the vastus medialis while the neuromuscular electrical stimulation(NMES) was performed in elderly women with knee osteoarthritis. The subjects of this study were 30 women who were diagnosed degenerative arthritis, 15 in the experimental group and 15 in the control group. The experimental group performed conventional physical therapy and neuromuscular electrical stimulation, and the control group received only conventional physical therapy, five times per week for four weeks. Outcome measures were assessed before and after 4weeks using the VAS(pain), quadriceps femoris strength, thigh circumference, and Q-angle. The data of the quadriceps femoris strength and thigh circumference were significantly increased in experimental group(p<0.05). The data the pain was significantly decreased in both the two groups(p<0.05). There were statistically significant differences in quadriceps femoris strength and thigh circumference between the two groups(p<0.05). As the result, the application of neuromuscular electrical stimulation of the vastus medialis could be more effective for decrease pain and improving muscle function in patients with knee osteoarthritis.

Comparing Effects of Tai Chi Exercise on Pain, Activities of Daily Living, and Fear of Falling in Women with Osteoarthritis and Rheumatoid Arthritis (타이치 운동이 골관절염과 류마티스관절염 여성의 통증, 일상활동 및 낙상공포감에 미치는 효과 비교)

  • Purpose: The purpose of the study was to compare the effects of Tai Chi exercise program on arthritic pain, difficulties in performing activities of daily living, and fear of falling in women with osteoarthritis (OA) and rheumatoid arthritis (RA). Method: A quasi-experimental design with two intervention groups was used. According to the inclusion criteria, 35 women with OA and 22 with RA were recruited and participated in the Tai Chi for arthritis program for 12 weeks. In 12 weeks, 21 in the OA group and 15 in the RA group completed both pretest and posttest measures. Outcome variables consisted of pain, difficulties in performing activities of daily living, and fear of falling for both groups. Results: The women with OA (64 years) were significantly older and perceived better health than those with RA (49 years) with more than 6 years of diagnosis. The pain scores and the performance of activities of daily living were significantly improved in the OA group at the completion of 12 week Tai Chi exercise, but RA group decreased pain only. The improvements in pain and activities of daily living were significantly more in women with OA than those with RA. No significant changes were found in fear of falling for both groups. Conclusion: The 12 week Tai Chi exercise was more effective on pain and activities of daily living for women with OA than those with RA. Considering that the women with RA perceived more pain and had limited activities of daily living, Tai Chi exercise should be applied slowly and gradually for longer period of time than those with OA.

Effects of Physical Activity in the Elderly with Osteoarthritis on Stress and Health-related Quality of Life (EQ-5D) : Using Data from the 2018 National Health and Nutrition Survey (골관절염 노인의 신체활동이 스트레스 및 건강관련 삶의 질(EQ-5D)에 미치는 영향 : 2018 국민건강영양조사 자료 이용)

  • The purpose of this study was to investigate the effects of physical activity in the elderly with osteoarthritis on stress and health-related quality of life (EQ-5D). This study performed data analysis using the SPSS 17.0 program in 104 elderly people with osteoarthritis over 65 years old using raw data from the 7th National Health and Nutrition Survey. The main results of this study are as follows: Males exercised more strength than women, and the better the subjective health status, the higher the rate of walking and aerobic activity. The group that felt stress showed a higher rate of walking and aerobic activity than the group that had no stress, and one group that felt a little stress practiced walking more than 3 days. The exercise ability of the elderly with osteoarthritis was better in the group who practiced walking, strength training, and aerobic activity than in the group without physical activity, and in self-management, the group practicing exercise and aerobic activity had problems with self-management. There was no daily activity, and the group that practiced walking and strength training also showed a high level of daily activity. In the area of pain/discomfort, the group practicing walking showed less pain/discomfort, and in the area of anxiety/depression, the group practicing walking showed less anxiety/depression. In conclusion, it is considered that in order to increase the quality of life of the elderly with osteoarthritis and to manage the stress, active efforts to increase the level of physical activity are necessary.

The Effects of Kinesio Taping on the Pain and Functional Improvement in Patients With Degenerative Arthritis (키네시오 테이핑이 퇴행성관절염 환자의 통증 및 기능회복에 미치는 효과)

  • This study was conducted to investigate the effects of kinesio taping on the pain and functional improvement of patients with degenerative arthritis. 40 female patients were participated in this study. To evaluate the effects of taping therapy we calculated pain, range of motion, and get up and go(GUG) test. The results were as follow: Pain of experimental and control group were significantly decreased after than before treatment, but there was significant difference between experimental and control group. ROM of experimental and control group were significantly increased after than before treatment, but there was significant difference between experimental and control group. Time of GUG of experimental and control group were significantly decreased after than before treatment, but there was significant difference between experimental and control group. And Steps of GUG was significantly decreased only experimental group. So we think that kinesio taping have effectiveness on the pain and functional improvement in patients with degenerative arthritis.

A Survey on Experience and Illness Management of Rural Women with Osteoarthritis (농촌 골관절염 여성의 질병 경험과 관리 실태조사)

  • Purpose: This descriptive survey was done to assess how rural women with osteoarthritis perceive their symptoms, what are their treatment preferences, and how they manage their illness. Method: Individual interviews were carried out with 205 women with osteoarthritis living in rural areas. Structured questionnaires were used and the interviewers were 7 trained research assistants. Descriptive statistics were used for data analysis with SPSS win 11.5 program. Results: Of the respondents, 68.8% were overweight and 15.1% were obese, 86.9% reported pain in the knees, low back, and shoulders and the average pain score was 66.06. Those knowing the exact diagnosis accounted for 73.7% of the respondents but only 49.7% reported receiving medical treatments, and 40.2% who received treatment adhered to their prescription. Folk medicines were used by 14.1% of the respondents, but 65.2% among them reported that folk medicines were not effective. Exercise 2.03 times per week was reported by 17.1% of the respondents and 48.6% reported preferring to walk. Conclusion: Women with osteoarthritis in rural areas need a rehabilitative care program focused on exercise. The community health care system in the rural area should deliver optimal health care for this population. The results of this study can be used as a basis for planning intervention programs for women with osteoarthritis in the rural area.

Pain of Elderly Women with Osteoarthritis (골관절염 여성노인의 아픔경험에 대한 현상학적 연구)

  • The purpose of this study was to determine the meaning of the pain and experiences of elderly women with osteoarthritis, by adopting Colaizzi′s phenomenological method. The participants were 7 elderly women over the age of 65. They were selected using a theoretical sampling technique. The Data was collected by in-depth and open-ended interviews from Dec. 1. 1999. to Feb, 28. 2000. The length of the interviews varied from 120 minutes to 180 minutes. Data was recorded and analyzed by a constant comparative method. From the data, significant statements were extracted and then organized into 48 themes, which resulted in 15 clusters of themes and 6 categories. The final descriptions turned out to be valid through the interviewee′ validation process. Essential themes of the pain experiences emerged : "physical discomfortness(disturbances)", "negative state of mind", "influence of the death", "positive change in life", "Cause of pain perceived", and "change of their personal relationships". From these results, it was found that elderly women need nursing care based on a deep understanding of pain, and a reflection on their past is imperative to overcome their given situations. In conclusion, it is suggested that the care givers provide more support to solve the problems experienced by the elderly. Thus the researchers expect to provide understanding of older people and give basic data of holistic care for them.

  • Osteoarthritis in patients with temporomandibular disorders(TMDs) induces pain, limitation of mouth opening, occlusal problems, and most commonly affects their life quality. Control method and progressive process of osteoarthritis are being extensively researched. The researchers focus on histologic changes, synovial changes, muscular and ligamental changes and observed reaction to pain. Therefore most of them developed the animal model for osteoarthritis in TMD patients. In this study, we applied several methods which induces osteoarthritis of temporomandibular joint(TMJ) in rats or mice. For locally induce osteoarthritis in TMJ, Monosodium iodoacetate(MIA) or interleukin-$1{\alpha}$(IL-$1{\alpha}$) were injected into TMJ joint space for 5 or 3 weeks. Other groups are chosen for osteoarthritis under systemic control including hormonal changes and aging. To observe cellular change, increased collagen, degenerative bony destruction and distribution of proteoglycans (PGs), safranin-O staining and Masson's trichrome staining were used.

Effects of Balance Taping Therapy on the Pain and Range of Motion of the Knee Joint in the Female Elderly with Degenerative Knee Arthritis (밸런스 테이핑요법이 퇴행성슬관절염 여성노인의 통증과 슬관절운동범위에 미치는 효과)

  • Purpose: The purpose of this study was to testify effects of Balance Taping Therapy on the pain and range of motion of the knee joint in the female elderly with degenerative knee arthritis. Method: The research design was one group pretest-posttest design. The data were collected from November 15 to December 25, 2004. The subjects were 23 female elderly with degenerative knee arthritis who were registered at one public health center in Busan. Balance Taping Therapy was conducted twice a week for 5 weeks. The data were analyzed such as the number, percentage, paired t-test, Kolmogorov-Smirnov test using SPSS 10.0 WIN Program. Result: The pain in the female elderly with degenerative knee arthritis significantly decreased from 8.82 to 6.04 after Balance Taping Therapy(t=16.781, p=0.001). The knee flexion joint range of motion in the female elderly with degenerative knee arthritis significantly increased from 119.73 to 123.13 after Balance Taping Therapy (t=-6.003, p=0.001). The knee extention joint range of motion statistically significantly increased from 112.74 to 117.83 after Balance Taping Therapy(t=-8.940, p=0.001). Conclusion: Balance Taping Therapy proved an effective nursing intervention in decreasing pain and increasing the range of motion of the knee joint in the female elderly with degenerative knee arthritis.
