Muscle Infarction and Calcification of the Semitendinosus Tendon: A Case Report (반건양건내에 발생한 근육 경색 및 석회화: 증례 보고)

  • The most common anatomic location of calcific tendinitis is the suprasupinatus muscle of the shoulder joint. However, it is known to develop in any joint including the hip, knee. Infarction of skeletal muscle in the distal areas of the limbs due to vascular occlusion is a well recognized systemic condition in patients who have diabetes. The author experienced mass-like lesion combined muscle infarction and calcification within pure semitendinosus tendon without diabetes in posterosuperior area of distal thigh in old age.

  • the primary diagnosis of synovial chondromatosis of temporomandibular joint is extremely rare. often many months, or even years pass before the diagnosis is confirmed. synovial chondromatosis is an uncommon disease of cartilage transformation of synovial membrane with formation of loose bodies within the joint space. a 28-year-old women had experienced pain of the left TMJ area at mastication. Radiographs of the left TMJ revealed calcified loose bodies in superior temporomandibular joint space. treatment consisted of removal of calcified loose bodies and synovail membrane. after surgery, pain of Lt temporomandibular area at mastication disappeared. until present after surgery there have been no recuurence of symptoms.

  • 견관절의 안정요소 중 정적 안정요소는 관절와 상완 연골 인대 복합체나 관절 일치성 등으로서 재활 과정 중 신장 운동에 관여하며, 회전근 개 근육과 각 관절 사이의 운동 조화 등으로 이루어진 동적 안정요소는 근력 강화 운동으로 재활이 이루어진다. 견관절 불안정성에 대한 수술 후 재활은 복원된 조직을 보호하면서 동시에 주위 조직과의 유착을 방지하고 관절 운동 범위를 향상하기 위해 통증을 유발하지 않는 범위 내에서 부드러운 수동 관절 운동을 먼저 실시한다. 능동적 관절운동은 수술한 조직이 치유될 충분한 시간이 경과되었거나 환자가 통증없이 독립적인 견관절 운동이 가능할 때 실시한다. 견갑와 상완 관절뿐만 아니라 견갑 흉곽 운동과 견갑골 부착 근육에 대한 재활이 제대로 이루어져야 수술 후 만족할 만한 결과를 얻을 수 있다. 끝으로 본문에 상술한 모든 재활 프로그램은 환자의 개개인의 특성에 맞춰 운동 내용과 시기를 조정하여 개별화해야 효과적인 재활이 이루어질 수 있다.

A Study on Symptoms and Clinical Findings of TMJ Dysfunction (악관절 기능장애증의 증상 및 임상소견에 관한 연구)

  • 악관절 기능장애증의 증상 및 임상소견에 관해 많은 연구가 보고되었으나 국내에서는 이에 대한 연구가 미비한 편이며, 이들간의 상호 관련성을 조사한 연구는 별무하였다. 이에 저자는 1983년 3월부터 1984년 7월 사이에 서울대학교 병원 구강진단과에 내원한 악관절 기능 장애증 환자 367명 중 방사선 사진상으로 기질적인 변화를 보이지 않는 327명을 대상으로, 증상 및 임상소견에 관한 조사와 아울러 악관절 동통이 하악 운동에 미치는 영향에 관해 연구한 결과 다음과 같은 결론을 얻었다. 1. 악관절 기능장애증은 약 3:1의 비율로 여자에게 빈발하였고, 15세에서 29세까지의 청장년 층에 많았다. 2. 악관절 기능장애증 환자가 경험한 주 증상은 동통, 관절잡음, 개구장애 등이었다. 3. 관절잡음은 편측성인 경우가 많았고, 좌우 발현 빈도는 비슷하였으며 말기의 관절잡음이 많았다. 4. 악관절 기능장애증의 발병 기간은 1개월 이하인 경우가 많았다. 5. 악관절 동통은 개구와 하악 전방운동을 제한하는 것으로 추정된다. 6. 악관절 동통은 동통이 있는 쪽으로 하악 개구로의 편위를 일으키는 것으로 추정된다.

Advantages of Intra-Articular Tranexamic Acid Injection Following Simultaneous Bilateral Primary Total Knee Arthroplasty (퇴행성 관절염 환자에게 동시에 시행하는 양측 인공 슬관절 전치환술에서 관절강 내 Tranexamic Acid 주입의 장점)

  • Purpose: This study examined the difference between the drainage volume, blood loss, transfusion rate, volume, and complications with or without an intra-articular (IA) tranexamic acid (TXA) injection among patients who underwent simultaneous bilateral total knee arthroplasty (SBTKA) with bilateral knee joint osteoarthritis, including patients who have contraindications of intravenous administration. Materials and Methods: Among patients who underwent SBTKA from April 2016 to December 2018, 139 patients injected with 3 g of TXA in each side through a drainage tube after joint capsule repair (group T), and 57 patients (group A) who underwent the procedure without TXA between October 2007 and August 2010 were tested. No significant difference in age and sex was observed between the two groups (p=0.572, 0.474). TXA was injected in patients with contraindications of intravenous administrations. Patients who underwent SBTKA with inflammatory arthritis were excluded from this study. The average amount of drainage, blood loss, transfusion rate, volume and daily average transfusion rate, and hemoglobin (Hb) change by the postoperative day were compared. Complications, such as deep vein thrombosis, pulmonary thromboembolism, myocardial infarction, cerebral infarction, and infection, were investigated. Results: The average total blood losses in groups A and T were 2195.32±1175.63 ml and 1145.09±382.95 ml, respectively, and the average total drain volume was 1,178.30±48.59 ml and 774.19±310.06 ml, respectively; both were significantly lower in group T (p=0.002, <0.001). The transfusion rates were 77.2% (44/57) and 0.7% (1/139), which were significantly lower in group T (p<0.001). The total average transfusion volume in groups A and T were 735.44±550.83 ml and 4.60±54.28 ml, respectively, which were significantly lower in group T (p<0.001). Hb tended to increase for three or four days after surgery in group A and group T. Regarding complications, deep vein thrombosis was encountered in two cases (1.4%), and pulmonary thromboembolism was noted in three cases (2.2%) in group T, but there were no cases in group A. No infections, cerebral infarction, or myocardial infarction occurred. Conclusion: In SBTKA, IA injections of TXA reduced the average drain volume, blood loss, transfusion rate, and volume significantly and did not increase the incidence of complications, even in patients with contraindications of intravenous administration.

Loose Body in the Interphalangeal Joint of the Big Toe (무지 지절에 발생한 관절내 유리체 - 증례보고 -)

  • A 20-year-old patient who got injured on his right big toe and complained pain visited to our hospital 9 months ago. Before the patient transferred to our hospital, plain radiographs were obtained at a private hospital and the patient underwent conservative treatment with uncertain diagnosis. However, the symptom was not improved, and he continued to complain intra-articular pain on his interphalangeal joint of big toe during plantarflexion or dorsiflexion. On plain radiographs of our hospital, we observed small bone fragment in his interphalangeal joint of the big toe. On the ultrasonographic images, two loose bodies in the interphalangeal joint were found. Then, we removed the loose bodies as surgical treatment, and the symptom was subsided completely. The purpose of this study is to report the ultrasonographic finding of the loose bodies of interphalangeal joint of the big toe.

Rice Bodies Presenting as Intra-Articular Masses in Pediatric Idiopathic Arthritis: A Case Report (소아기 특발성 관절염 환자에서 관절 내 종양형태로 나타난 쌀소체: 증례 보고)

  • Rice bodies are commonly detected in adults with rheumatoid arthritis and rarely occur in children. An 11-year-old female adolescent who visited our hospital for evaluation of knee pain underwent a MRI scan, which revealed an intra-articular mass. Arthroscopic examination of the mass confirmed conglomerated rice bodies. We report a case of rice bodies that clinically presented as intra-articular masses.
