Clinical Phenotypes and Dietary Management of Hepatic Glycogen Storage Disease Type 0 (간 0형 당원축적병의 임상 표현형과 식사관리)

  • The hepatic glycogen storage disease type 0 (GSD type 0) is an autosomal recessive disorder caused by a deficiency of hepatic glycogen synthase encoded by the glycogen synthase 2 (GYS2) gene, leading to abnormal synthesis glycogen. The clinical findings of GSD type 0 are hyperketotic hypoglycemia at fasting state and accompanying postprandial hyperglycemia and hyperlactatemia. GSD type 0 has only been reported in a very small number so far, and the diagnosis is likely to be missed because symptoms are mild, severe hypoglycemia is rare or asymptomatic, or symptoms gradually disappear with age. Essential management strategies include feeding high-protein meals to stimulate gluconeogenesis, frequent meals to prevent hypoglycemia during the day and feeding complex carbohydrates such as uncooked cornstarch to slowly release glucose during nignt. GSD type 0 has a good prognosis, with appropriate treatment, normal growth can be achieved and no complications occur. Significant hypoglycemia occurs less common in adulthood, but ongoing dietary management may be necessary.

Association between Dietary Sodium Intake and Abdominal Obesity in Pre-diabetes Korean Adults (전당뇨병 성인에서 나트륨 섭취와 복부비만과의 상관관계)

  • The objective of this study was to assess the relationship between dietary sodium intake and prevalence of abdominal obesity in Korean adults. We used data from the Korea National Health and Nutrition Examination Survey V-1 and analyzed data on 4,475 Koreans (${\geq}30$ years old). Subjects were divided into three groups according to fasting plasma glucose (FPG): 1) normal (FPG <100 mg/dL), 2) pre-diabetes ($100mg/dL{\leq}FPG{\leq}125mg/dL$), and 3) diabetes (FPG ${\geq}126mg/dL$ or subjects diagnosed with diabetes). The subjects in each category were stratified by dietary sodium intake as well as index of abdominal obesity. We found that dietary sodium intake was positively correlated with waist circumference (WC) (P=0.002) and was particularly high in the pre-diabetes group. In multiple logistic regression analysis, the normal and diabetes groups showed no association between dietary sodium intake and WC, whereas the pre-diabetes group with a high sodium intake exhibited a significant association (odds ratio (OR)=1.479, P=0.029) between dietary sodium intake and WC. Further, the OR for abdominal obesity in the high sodium intake group with pre-diabetes was 1.590 after adjusting for age and sex (P=0.012). In addition, the ORs for the prevalence of abdominal obesity with fasting glucose, fasting insulin, and homeostasis model assessment of insulin resistance were significantly higher in the pre-diabetes group with high sodium intake compared with low sodium intake. Moreover, these associations were significant even after adjusting for confounding variables (model 2: age and sex; model 3: age, sex, and total energy intake). Our results suggest a strong association between sodium intake and abdominal obesity in pre-diabetes Korean adults.

The Influences of Risperidone and Clozapine on Body Weight and Glucose Level in Patients with Chronic Schizophrenia - Comparison Study with Haloperidol - (만성 정신분열병 환자에서 Risperidone과 Clozapine이 체중과 혈당에 미치는 영향 - Haloperidol과의 비교 연구 -)

  • Object:The goal of this study was to examine the changes in body weight and glucose levels of the patients treated with risperidone, clozapine or haloperidol in order to compare the effect of risperidone or clozapine with that of haloperidol. Methods:For nine months(January to September, 2003), a prospective study was performed in 60 patients with chronic schizophrenia who were in Seoul National Hospital. Two-week period was required for a drug wash-out. The patients were randomly assigned to risperidone, clozapine and haloperidol groups. They were given risperidone(n=20), clozapine(n=20) and haloperidol(n=20), respectively, everyday for 12 weeks. To examine the effects of these drugs on body weight and fasting glucose levels, we measured body weight and glucose levels of all the patients first without the drug treatment and at each end of 4, 8, and 12-week periods with the treatment. And we examined the differences among three groups in the changes of body weight and fasting glucose levels. Results:There were no significant differences in the changes of the body weight and fasting glucose levels between the atypical antipsychotics(risperidone or clozapine) and the typical antipsychotics(haloperidol). Conclusion:The study in the patients with chronic schizophrenia suggests that risperidone or clozapine do not cause any additional effects on body weight or glucose levels compared to haloperidol.

The Effect of Exercise Program in Korea for Controlling Blood Glucose Levels in elderly women patients undergoing type II diabetes mellitus (제 2형 당뇨병 여성 노인 환자의 혈당조절을 위한 국내 운동 프로그램의 효과분석)

  • The purpose of this study was to investigate the effects of exercise program intervention in Korea for the control of blood sugar levels in elderly women patients undergoing diabetes mellitus typeII and to investigate the effects of exercise program intervention. Research methods were searched through RISS, KISS, KoreaMed, and KMbase as domestic databases and limited to domestic papers published from 2010 to May 2017. Diabetes, female diabetes, and diabetes mellitus typeII were used as the search terms related to the patient group. Programs related to intervention, intervention, exercise, blood glucose, glucose insufficiency, glucose control, fasting glucose and HbA1c were used. A total of 12 papers were provided according to the intervention method. The mediation period was 12 weeks(50%), 3 times per week(58.3%), and 50 to 69 minutes(83.4%) was the most. The combined aerobic and anaerobic exercise was found to be an effective exercise method to control not only blood glucose level but also HbA1c. Individualized exercise program intervention should be provided considering the physical fitness level and health status of elderly patients undergoing diabetes mellitus typeII and should be encouraged to maintain effective blood glucose control through continuous exercise rather than one-off exercise.

  • The present study aimed to investigate the effect of whole wheat cookie supplemented with Capsosiphon fulvescens (CF) extract on serum glucose homeostasis in C57BL/6 mice. This study examined whether the same effect was demonstrated for whole wheat cookie in comparison to previous research documenting the glucose-lowering effect of food products combined with CF extract. Mice were divided into three groups depending on the diet administered: normal cookie (NC), whole wheat cookie (WC), and WC blended with CF extract (WCFE). After 4 weeks of administering the experimental diet, the blood glucose level, serum insulin level, and homeostatic model assessment for insulin resistance index were found to be significantly lower in the WCFE group than in the NC and WC groups. These results suggest that whole wheat cookie containing CF extract is effective in preventing insulin resistance and maintaining blood glucose homeostasis.

Antidiabetic Activity of a Chloroform Fraction of Vigna nakashimae in db/db mice (db/db 마우스 동물모델에서 Vigna nakashimae 클로로포름층 분획물의 항당뇨 효능)

  • In this study, we evaluated the antidiabetic effect of a chloroform fraction of a methanol extract of Vigna nakashimae (designated VN) and compared it with that of a water fraction. Both fractions were administrated to eight-week old db/db mice for two weeks, after which the plasma glucose, triglyceride, and total cholesterol levels were measured. The chloroform fraction (VN-C) lowered the fasting glucose and blood glycated hemoglobin in the db/db mice more effectively than those of the water fraction (VN-W). VN-C also improved the glucose tolerance and led to a significant decrease in the plasma levels of free fatty acids and triglycerides. VN-C enhanced the phosphorylation of AMP-activated protein kinase (AMPK) and increased the expression of carnitine palmitoyltransferase-1 (CPT-1) in HepG2 and C2C12 cells more significantly than VN-W. Consistent with AMPK activation, VN-C inhibited cAMP/Dex-stimulated expression of gluconeogenic genes and increased glucose uptake in C2C12. Collectively, these results suggest that VN-C has an antidiabetic effect, which is exerted via AMPK activation, and that this effect is stronger than that of VN-W.

Effect of Curcuma longa L. on the Obesity and Insulin Resistance in Sprague-Dawley Rats and db/db Mice (SD계 랫트와 db/db 마우스에서 Curcuma longa L.가 비만과 인슐린저항성에 미치는 영향)

  • In this study, the effect of Curcuma longa L. on obesity and insulin resistance was investigated in animals fed a moderate high fat diet. The animals used in this study were normal weight Spargue-Dawley (SD) rats and type 2 diabetic obese db/db mice. Accumulation of abdominal adipose tissue and weight gain were inhibited in the animals fed the C. longa extract. C. longa decreased fasting insulin and HOMA-IR in the SD rats, and effectively decreased blood glucose and hemoglobin A1c in db/db mice. C. longa decreased serum free fatty acid (FFA) level in the SD rats. FFA in db/db mice fed C. longa tended to decrease. C. longa significantly decreased serum triglyceride level. Our results collectively represent that C. longa prevented fat accumulation and insulin resistance in both normal weight SD rats and type 2 diabetic obese db/db mice fed a moderate high fat diet.

Relations of Cataract to Metabolic Syndrome and its Components - Based on the KNHANES 2005, 2007 (백내장과 대사증후군의 관련성 - 2005, 2007년 국민건강영양조사 이용)

  • Purpose: This study was performed to assess the effects of metabolic syndrome and its components to cataract. Methods: We investigated the relation of metabolic syndrome and its components to cataract using data for 2,120 adults, aged 60 years or older, from the Korean National Health and Nutrition Examination Survey 2005, 2007. Using multiple logistic regression analysis, we presented significant odds ratio (OR) increase of cataract according to the number of metabolic abnormalities ${\leq}$1, 2, 3, ${\geq}$4). We also analyzed OR by the prevalence of metabolic components, and analyzed the effects of metabolic medication intakes to cataract prevalence using multiple logistic regression analysis. Results: The risk of cataract development was significantly increased according to the number of metabolic abnormalities, after adjusting for age, sex, life style, and social economic status variables (p for trend < 0.0001). In metabolic components, the disturbances of blood pressure (OR(95% Confidence Interval): 1.32(1.05,1.65)) and fasting glucose (1.35(1.09,1.67)) significantly increased the prevalence of cataract after adjusting for age and sex. Among these metabolic components, the significance of fasting glucose (1.26(1.01, 1.58)) was remained after adjusting for the other variables. Medication intake of hypertensive also increased the risk of cataract (1.49(1.14,1.96)). Conclusions: Metabolic syndrome and its components increased the risk of cataract, and some medication for treating hypertension was also associated with the cataract incidence.

Fasting Blood Sugar and Adherence to Diabetes Control Recommendation : Impact of Education Using Short Messaging Service of Cellular Phone (휴대폰 문자메시지 교육이 제2형 당뇨병환자의 공복혈당과 자가관리에 미치는 효과)

  • Purpose : The purpose of this study was to investigate the effects of the education using short messaging service(SMS) on fasting blood sugar(FBS) and diabetes adherence. Method : Forty-five diabetic patients being assessed pre and post intervention was used to assess the effectiveness of the education. Participants were requested to input the FBS and 2 hours post-prandial blood sugar everyday in http://www.biodang.com by cellular phone or wire Internet. The goal of the education was to lower FBS and keep diabetes adherence. The education was applied to the for 12 weeks. The education consisted of continuous education and reinforcement of diet, exercise, medication adjustment, as well as frequent self-monitoring of blood glucose levels. The education performed weekly. All medication adjustments were communicated to the participants' doctor. FBS and diabetes adherence were measured before and after the education. Results : The education using SMS of cellular phone had decrease FBS and increase oral medication taking, exercise, foot care, and hypoglycemia preparation adherence.

Effects of growth hormone treatment on glucose metabolism in idiopathic short stature (특발성 저신장증 환자에서 성장 호르몬 투여가 당 대사에 미치는 영향)

  • Purpose : To study the effects of growth hormone(GH) treatment on glucose metabolism and insulin resistance in children with idiopathic short stature(ISS). Methods : Glucose and insulin concentrations were measured during oral glucose tolerance test (OGTT) before and after GH treatment(0.6-0.7 IU/kg/week) in 20 patients with ISS. Insulin resistance was assessed by homeostasis model assessment(HOMA). Results : During OGTT, the mean blood glucose level did not show any significant changes after GH treatment. However, mean blood insulin levels of fasting and 30 minutes of OGTT showed significant increases after GH treatment, accompanying significant increases of insulin resistance. There was no difference in change of glucose, insulin levels and insulin resistance before and after GH treatment between two groups of body mass indices(BMI) of 25< and >25. There also was no significant difference between two groups of with and without family histories of diabetes mellitus (DM). There was no case of newly developed impaired glucose tolerance, fasting glucose tolerance, nor newly developed DM. Conclusion : GH treatment with doses of 0.6-0.7 IU/kg/week for mean 9.6 months in patients with ISS did not show any significant changes in blood glucose levels during OGTT. However, GH treatments induced considerably higher fasting insulin levels compared to pretreatment, resulting in statistically higher insulin resistance. Higher BMI and family history of DM did not induce any significant changes in glucose, insulin level and insulin resistance after GH treatment than the other groups.
