A Prospective Cohort Study of Exercise and the Incidence of Type 2 Diabetes in Impaired Fasting Glucose Group (규칙적 운동과 제2형 당뇨병 발생에 관한 전향적 코호트 연구 -공복혈당장애군을 대상으로-)

  • Objectives : To determine the relationship between exercise and the incidence of type 2 diabetes in an impaired fasting glucose group. Methods : This prospective cohort study was conducted in 19,440 men and 4,297 women, aged 30-69 years, with impaired fasting glucose at baseline who had undergone biennial medical evaluation through the National Health Insurance Corporation from 2000 to 2004. Impaired fasting glucose was defined as fasting glucose of 100 to 125 mg/dl and the subjects were divided into 3 groups depending on weekly exercise frequency. Multivariate logistic regression analysis was used to evaluate the association between the baseline exercise status and incidence of type 2 diabetes. Results : During the 4-year follow-up, a total of 3,239 men and 283 women developed type 2 diabetes, a cumulative incidence of 16.6% for men, and 6.5% for women. Also, 1,688 men (21.2%) and 127 women (15.2%) developed type 2 diabetes in the obese group. The adjusted relative risk (RR) of developing type 2 diabetes in non-exercising men was significantly higher than exercising men regularly (RR= 1.375, 95% CI=1.236-1.529)(p<0.0001), and the RR for non-exercising women was higher than exercising women regularly (RR=1.124, 95% CI=0.711-1.778). The RR for non-exercise men/women in the obese group was 1.571 (95% CI=1.351-1.827)(p<0.0001)/1.869 (95% CI=0.846-4.130). Conclusions : Regular exercise is effective in preventing type 2 diabetes in people with impaired fasting glucose, and particularly in obese people. People with risk factors for diabetes should participate in a regular exercise program.

Associated Factors of Impaired Fasting Glucose in Some Korean Rural Adults (농촌지역 주민의 공복혈당장애와 관련요인)

  • Objective: This study was performed to investigate the prevalence of impaired fasting glucose (IFG) and its related characteristics among healthy adults in some Korean rural areas. Methods: We conducted a cross-sectional study using the data from 1352 adults who were over the age 40 and under the age 70 and who were free of diabetes mellitus (DM), cardiovascular diseases and other diseases and who participated in a survey conducted as part of the Korean Rural Genomic Cohort Study. IFG was defined as a serum fasting glucose level between 100 and 125 mg/dL. Results: The prevalence of IFG was 20.4% in men, 15.5% in women and 12.7% overall. Multivariate logistic regression analysis demonstrated that the independent risk factors for IFG were male gender, having a family history of DM, the quartiles of gamma glutamyltransferase and high sensitive C-reactive protein and the waist circumference. The homeostatis model assessment for insulin resistance was very strongly associated with IFG. The prevalence of metabolic syndrome (MS) and MS components was higher in the subjects with IFG then in those with normal fasting glucose (NFG). Conclusions: The result of study could supply evidence to find the high risk population and to determine a strategy for treating IFG. Further research is needed to explain the causal relationship and mechanisms of IFG.

Study of association of neuralgia with blood parameters and anthropometric indices in Korean adult men and women (한국인 성인남녀에서 신경통과 혈액정보 및 체형정보와의 연관성 연구)

  • Neuralgia is a disease that involves severe pain and has a very strong effect on the quality of human life, and the prevalence of the disease increases with aging. To date, previous studies on neuralgia were mainly focused on associations with mental illness, demographic information, and nutrients, and studies on association with blood information were very rare. Therefore, the objectives of this study are to examine the association between neuralgia and blood parameters and find clinical indicators related to neuralgia. To analyze the data, we used binary logistic regression based on data of the Korea National Health and Nutrition Examination Survey. Our results showed that age tended to have the higher association with neuralgia in both men and women, waist circumference and hematocrit level were associated with neuralgia in women, and fasting blood glucose and hemoglobin levels were associated with neuralgia in men. Also, we found that the association of neuralgia with waist circumference and blood information differed according to gender.

  • Arteriosclerosis is a disease in which blood circulation is impaired due to loss of elasticity as blood vessels become narrower, and is a potential cause of recently increasing cardiovascular and cerebrovascular diseases. Carotid ultrasound is used as a predictor of cardiovascular and cerebrovascular diseases by evaluating the degree of atherosclerosis. Therefore, this paper attempted to investigate the correlation between the increase in the thickness of the inner media and various clinical variables in carotid ultrasound. Patients with carotid ultrasound findings were classified into three stages: mild intima thickening, sclerosis, and significant stenosis. CAVI (Carotid Ankle Vascular Index: vascular age) data measured in the degree and physical characteristics of the carotid artery(gender, age, body mass index, blood pressure), blood test(total cholesterol, triglycerides, high-density lipoprotein, low-density lipoprotein, creatine phosphokinase, fasting blood sugar), and arteriosclerosis test were collected. It was confirmed that the carotid intima thickness was correlated with variables such as hypertension, hyperlipidemia, and BMI, and also correlated with the risk factors of cardiovascular disease as CAVI increased.

An Analysis on Risk Factors of Colon Polyps with Health Examination Examinees (건강검진 대상자에서 대장용종의 위험요인 분석)

  • This study aimed to analyze the occurrence of colon polyps and also risk factors of colon polyps. Among health examination examinees, 508 people were included who underwent colonoscopy and abdominal ultrasonography at the same time. Physical measurements also performed such as height, weight, blood pressure, waist measurement and BMI. General characteristics including age, sex, smoking, drinking and exercise as a risk factors were checked. At the same time, we analyzed various blood tests and fasting blood sugar through blood-gathering. In the results, regarding risk factors of colon polyps, AST, ALT, TC, fatty liver, obesity and smoking were shown significantly high. As a dependent variable with colon polyps, the highest relation was fatty liver(3.4 times), and then T-C(2.3 times). Smoking factor showed 2.3 times higher relation than non-smokers.

A Comparison of Fasting Glucose and HbA1c for the Diagnosis of Diabetes Mellitus Among Korean Adults (공복혈당과 당화혈색소에 의한 당뇨병 진단 비교)

  • Objectives: The American Diabetes Association (ADA) has recently recommended the HbA1c assay as one of four options for making the diagnosis of diabetes mellitus, with a cut-point of $\geq$ 6.5%. We compared the HbA1c assay and the fasting plasma glucose level for making the diagnosis of diabetes among Korean adults. Methods: We analyzed 8710 adults (age 45-74 years), who were not diagnosed as having diabetes mellitus, from the Namwon study population. A fasting plasma glucose level of $\geq$126 mg/dL and an A1c of $\geq$ 6.5% were used for the diagnosis of diabetes. The kappa index of agreement was calculated to measure the agreement between the diagnosis based on the fasting plasma glucose level and the HbA1c. Results: The kappa index of agreement between the fasting plasma glucose level and HbA1c was 0.50. Conclusions: The agreement between the fasting plasma glucose and HbA1c for the diagnosis of diabetes was moderate for Korean adults.

  • We obtained medical test data of 150 male from National Health Insurance Corporation, that included physique, blood chemistry, and lifestyle. ${\chi}^2-test$ and logistic regression, and multiple regression were done to verify how CAD risks and/or lifestyle are different between obese and non-obese person. SPSS(version 18) was used for statistical procedures and p-value of .05 was regarded as good through. ${\chi}^2-test$ showed the frequency of hypertension, htperglycemia, hyperlipidemia were higher in obese person than in non-obese This result told us that lifestyle modification could be a key to prevent cardiovascular disease by controlling and managing body composition, especially abdominal obesity.

Relationship of Colorectal Polyps and Fatty Liver Disease Diagnosed by Ultrasonography (초음파로 진단된 지방간 질환과 대장 용종과의 연관성)

  • This study proposes a more comprehensive approach for FLD by analyzing the relationship of colorectal polyps, which are precursors of colorectal cancer. In Chi-Square tests of FLD and colorectal polyps, the prevalence of colorectal polyps was significantly high in cases of FLD. The polyps and correlation of each factor showed a positive relationship with all factors, and the correlation coefficient with FLD was highest (r = 0.39, p <.001). In multiple regression analysis, FLD(OR 3.80 95% CI 1.93.-7.50), FBS (OR 2.51; 95% CI 1.12-5.62), and older age (OR 2.12; 95% CI 1.27-3.54) were independent risk factors for colorectal polyps. FLD was associated with the prevalence of colorectal polyps. These results show a meaningful influenceof FLD by ultrasonography in the occurrence of colorectal polyps, and that positive consideration of colonoscopy is needed for diagnosed FLD.

Self-rated Health and its Associated Factors in the Elderly (노인의 주관적 건강상태와 관련요인에 관한 연구)

  • This study aimed to identify the self-rated health and its associated factors of the elderly. Study subjects were community-dwelling elderly of 60 or more. Survey was done by using questionnaire including general characteristics, lifestyle, and health related variables. Blood pressure, anthropometric variables, lipid profiles, and fasting glucose were checked. Descriptive statistics showed that good self-rated health was 78.2% in the elderly. Multiple logistic regressions revealed that good self-rated health was associated with disease(OR:10.83, 95% CI:2.47-47.43), depression(OR=2.50, 95% CI=1.20-5.18), and exercise(OR=3.01, 95% CI=1.40-6.47). Therefore, health promotion program considering associated factors of self-rated health should be developed.

Factors Influencing Changes in Fasting Blood Sugar Level of Participants in Primary Care Chronic Disease Management Pilot Project using ICT (ICT를 활용한 만성질환관리 시범사업 참여자의 공복혈당수치 변화에 영향을 미치는 요인)

  • The purpose of this study is to analyze that ICT-based primary health care affects clinical changes of participants in the primary care chronic disease management pilot project using ICT medical care. Customized health information data, provided by National Health Insurance Service, was used for the analysis. The study targeted a total of 676 people that participated in primary care chronic disease management pilot project using ICT medical care from 2017 to 2018. Hierarchical regression was used to test three model. First, there were many subjects who used face-to-face consultation and non-face-to-face consultation(messaging), but less than half of patients using non-face-to-face consultation(telephone). Second, after participating in the pilot project, the fasting blood sugar level decreased. Third, the clinical condition of the subjects appeared to be an important factor in controlling blood sugar levels. Finally, patients using the non-face-to-face consultation(messaging) had reduced blood sugar levels after participating in the project. This results imply that non-face-to-face consultation is effective in reducing fasting blood sugar level with hospital intervention, and there are effects of the primary care chronic disease management project using ICT.
