Use of Molecular Identification Analysis in a Case of Intra-familial Transmission of Tuberculosis (결핵균의 유전자 분석법으로 증명된 가족 내 결핵 전파 1예)

  • 결핵균은 공기를 매개로 전파되고 결핵균에 노출된 시간과 최초의 노출 정도가 전파의 위험을 증가 시키는 중요한 인자로 알려져 있다. 활동성 폐결핵 환자와 함께 생활하는 가족 내에서 감염이 현저히 증가하는데 이는 결핵균에 노출되는 기회 및 시간이 증가하기 때문이다. 하지만 고식적인 접촉자 조사만으로는 흔히 결핵 발생률을 과소평가할 수 있기 때문에 접촉자 조사와 결핵균의 유전형 조사를 병행하는 것이 감염의 위험이 높은 집단을 선별해내고 발생빈도 및 전염경로를 밝히는 데 보다 효과적인 것으로 보고되고 있다. 이에 접촉자 조사와 결핵균의 유전형 조사를 통해 가족 내 결핵균 전파를 증명한 1예를 보고하는 바이다.

Distribution of Hospital Airborne Microorganisms in Seoul, Korea (서울시내 종합 병원 공기중의 미생물 분포)

  • This study was carried out to investigate the airborne microbial pollution in hospital environment. Using a mechanical air sampler, microbiological samples were taken from intensive care unit, general ward room, patients wailing room and outdoor of 20 hospitals in Seoul, Korea. The concentration of airborne bacteria and fungi ranged 97-410 cfu/㎥ and 37-77 cfu/㎥, respectively and patients waiting room had highest bacterial count. 10 genera of molds were identified and the most frequently recovered molds were Aspergillus, followed by Penicillium, Alternaria and Cladosporium. Among Staphylococcus species, S. haemolyticus and S. epidermidis were predominant and 47% of Staphylococcus species were isolated from intensive rare unit.

  • The air sterilization systems are investigated experimentally in this paper. The goal is to reduce bacteria, mold and viruses in office air by using a UV sterilizer installed inside a partition panel and wall-mounted unit. These systems allow occupants to turn the system on/off and to control the incoming air speed and direction. The partition air sterilization system conditions and sterilizes the air, and then delivers the clean air into the personal task area through the partition panels, which are connected to the pressurized under-floor plenum. Room air exits through the return grills mounted on the ceiling. The wall-mounted air sterilization system sterilizes the air, and then delivers the clean air to the personal task area from the wall. In this study a full-size experimental environment is established to investigate the immunization performance of these air sterilization systems. A typical office space scale is used in this study in order to find an optimal system to achieve a sterilized healthy micro-environment. Multiple system parameters, including volume flow rate and velocity of supplied air, were regulated during the experiments. The more air contact these air sterilization systems had, the better disinfection performance. Over 90% of eradication ratios were obtained by these two air sterilization systems. The results indicate that these systems can efficiently disinfect office air contamination.

An Experimental Study on the Air Sterilization Performance of A Reflective Electro Magnetic Energy System (Reflective Electro Magnetic Energy(REME)를 이용한 공기살균시스템의 성능평가에 관한 실험적 연구)

  • From the point of view to prevent airborne infection-related diseases such as H1N1, SARS, and MERS, an actual application of air cleaning and purification systems including technologies like UVGI has become increasingly important. Recently, an air purification system using REME (Reflective Electro Magnetic Energy) developed in the U.S. is applied for indoor air purification and sterilization technology to counteract the outbreak of new airborne infections. In this study, an air sterilization performance experiment using REME was carried out. The results verified that air sterilization performance in the case of installing a REME system in a medical center was 31%, namely the number of floating bacteria decreased by 31% after only a five-day operation. In addition, the number of culture collections in the REME operating air conditioning systems using nonpathogenic Geobacillus stearothemophilus as a biological indicator decreased maximally to 67%. A field application of REME technology will be useful to prevent airborne infection-related diseases, especially in response to public health crises due to the advent of emerging diseases.

A Study on the Multizone Modeling for Preventing Transmission of Air Borne Contagion (실내 미생물오염 전파방지를 위한 멀티존 모델링에 관한 연구)

  • In this study multi-zone modeling program CONTAM 2.4 developed by NIST is used for estimating the air disinfection rate of the interior of a room which is set up the indoor air disinfection system with filter and ultra violet germicidal irradiation (UVGI). Developed models those enable to predict the transmission of air home contagion such as bacteria and fungus generated in our daily life are useful model for designning air cleaning & ventilation system of building. Also, results indicate that these models are enable to compute the real situation that is almost impossible of carrying out experiment in an actual condition due to biohazard problems and suggest that engineers will use these models as a design tool for the future immune building system.

  • Purpose: The MERS(Middle East Respiratory Syndrome) outbreaks in Korea highlighted dramatically the failings of traditional hospital environment for controlling or preventing infections among both patients and healthcare workers. MERS is transmitted by droplets that can be airborne over a limited area. The point should be emphasized that MERS in South Korea was predominantly a hospital-acquired (not a community-acquired) infection, because approximately 93% of MERS cases were resulted from exposure in hospital settings. This paper tries to suggest the design guidelines of negative pressured isolation ward for the sake of proper control of severe respiratory infectious diseases. Methods: Literature survey on the design guideline and regulations of airborne infection wards in Korea, Europe U.K. and CDC of U.S. have been carries out. 4 special infection wards in Hongkong, Germany, Japan and Korea have been surveyed in order to make the best use of the experiences related to facility design and operations. Results: Operating system influencing the facility design, space organizations of infectious ward including required space and zoning, and circulations of patients, staffs and materials are proposed. Implications: The results of this paper can be the basic data for the design of the airborne infection ward and relevant regulations. Afterwards in-depth study such as the development of space standards for the single bedroom, locker room and so on could be explored.

Aorta-esophageal Fistula That Developed after Endovascular Stent-grafting of a Mycotic Aneurysm - A case report- (감염성 대동맥류에 혈관 내 스텐트 삽입 시행 후 발생한 대동맥-식도루 - 1예 보고 -)

  • A 74-year-old woman presented at our hospital with hemoptysis. Three months ago, she had endovascular stent-grafting done by a general surgeon for a saccular thoracic aneurysm that was found accidentally following an episode of fever and chills. Despite a lasting fever after the procedure, she was discharged without further treatment and follow-up. She was subsequently admitted to the hospital for evaluation and several exams were performed. Chest CT scans and an esophagoscopy identified an aorto-esophageal fistula at the level of the aorta that was covered by a previous stent-graft. After extensive administration of antibiotics, surgery was done - esophagectomy, cervical esophago-gastrostomy and replacement of the thoracic aorta. She was later discharged uneventfully.

Viability of eggs, filariform larvae and adults of Stronglyloides venezuelensis (Nematoda: Strongyloidea) maintained in vitro (베네수엘라분선충 (Strongvloides venezuelensis)의 충란, 감염자충 및 성충의 실험관 내 배양)

  • The present study was performed to check the viability of eggs, filariform larvae and adults of Strongvloines venezueLensis exposed to various conditions for an in vitro maintenance. The eggs in the feces remained viable for about 25 days at $4^{\circ}C$ and 15 days at room temperature. However, the isolated eggs in sterile saline lost their viability within 24 hr at $4^{\circ}C$. The eggs in morula stage were very sensitive to air drying and rapidly lost their viability (=12 hrs. Filariform larvae survived for a maximum period of 45 days in fecal suspension and 28 days in 0.12% nutrient broth in polyvinyl culture bags maintained at $20^{\circ}C$. On the other hand, those isolated from nutrient broth cultures survived for a maximum period of 32 days in tap water and 22 days in sterile saline at $20^{\circ}C$. The mature adult worms obtained from experimentally infected rats survived maximally for 9 days in serum supplemented (10% rat-serum) 0.12% nutrient broth and 4 days in serum free nutrient broth at $37^{\circ}C$ while the culture media were changed at an alternate day. The adult female worms deposited fertile eggs in serum supplemented and serum free nutrient broth cultures, however, the hatched larvae (Ll) were not able to develop to the filariform stage in the culture media and found to die within 24 hr of maintenance. The present findings on an in vitro maintenance of different stages of 5. uenezueLetis may provide useful information for biological and biochemical studies with Strongyloines species.   • A ventricular assist device (VAD) is used for bridge to heart transplantation and heart diseases. Knowing the status of a pneumatic pulsatile VAD when implanting it into the body is important: when the velocity of blood flow through the VAD is slow, a thrombus may occur, and thrombosis can be fatal to a patient. In order to determine the state of a VAD, various sensors need to be implanted. Because this introduces the risk of infection and difficulties with sensor management, we developed a method for estimating the state of a VAD indirectly via the pressure in an air tube that can be measured in vitro. We compared the measured values to in vitro experimental results. The estimated and measured values showed some errors, but the accuracy can be improved by refining the estimation process to minimize the risk of infection.

Lower respiratory tract infection of positive antigen test for respiratory syncytial virus on children under 2 years of age (2세 이하의 소아에서 respiratory syncytial virus의 항원검사에 양성인 하기도 감염)

  • Purpose : This study was design and performed for evaluations of resent clinical pattern of bronchiolitis caused by RSV infection with children under 2 year of age for 5 years, who were admitted to pediatric ward. Methods : The inclusion criteria of the patients were children under 24 month-of-age, clinical manifestations of lower respiratory tract infection, and RSV antigen that was detected by a direct immunofluorescence test from the nasal secretions. The additional laboratory and simple chest X-ray findings were reviewed from the medical records of children who were admitted Wonkwang university hospital from Jan. 1999 to Dec. 2003. Results : In the 5 year study duration, 127 patients were enrolled and outbreak of RSV bronchiolitis took place in 2001. The 80 cases(63 percent) of RSV infection were concentrated in later autumn and winter. Number of the cases show coughing were 120(94.5 percent), but rale was audible in 78 cases(61.4 percent). Dyspnea, wheezing, and intercostal retraction were noticed in 27(21.3 percent), 21(16.5 percent), and 4(3 percent) cases respectively. The most common chest X-ray finding was hyperinflation of the lung that was noticed in 110 cases(86.6 percent). Care with mechanical ventilator for more than 2 days required in 5 cases. Conclusion : Lower respiratory tract infection by RSV was common in late autumn and winter season but year-round infection was noticed. The severity of RSV respiratory tract infectiontakes in some degree a grave course. So we suggest that population-based surveillance of acute respiratory infection due to RSV is necessary for assessment of prevalence and epidemiology of this disease.
