AN EXPERIMENTAL STUDY ON THE EFFECT OF PROSTAGLANDIN $E_2$ ON ALVEOLAR BONE RESORPTION INDUCED BY TOOTH MOVEMENT IN RATS (Prostaglandin $E_2$가 백서의 치아이동시 치조골 흡수에 미치는 영향에 관한 실험적 연구)

  • This experiment was performed to study the effect of $PGE_2$ on the bone resorption at the tooth movement by orthodontic force. The experimental animals were the Sprague-Dawley strain rats. The orthodontic force was applied by the insertion of separating clamp made of 0.014' (0.356mm) wire to the interproximal site between the 2nd and the 3rd upper right molars. In experiment I, $0.2{\mu}g,\;0.4{\mu}g,\;0.8{\mu}g,\;and\;1.0{\mu}g\;PGE_2$ were locally injected at the submucosa near the 2nd molar of the maxilla each. The effect was detected by the count of the number of osteoclasts appeared at the compressed surface of interradicular bone. In experiment II, 1.0 mg/kg indomethacin (a specificc inhibitor of prostaglandin synthetas.) was subcutaneously injected. The effect was examined by the count of the number of cateoclasts appeared at the compressed surface of interradicular bone. The obtained results were follows; 1. The number of osteoclasts on the compressed surface of the interradicular bone increased in proportion to the increased dosage of $PGE_2$ administered. The number of osteoclasts increased significantly at the administration of $0.8{\mu}g\;and\;1.0{\mu}g\;PGE_2$ in contrast to the control (P<0.05). 2. The administration of 1.0 mg/kg indomethacin decreased the number of osteoclasts at the compressed bony surface significantly (P<0.01).

  • To study bone resorption mechanism, effect of LPS on the $^{45}Ca$ release from fetal rat ulnae and radii, and effects of carbonic anhydrase inhibitors on the LPS-induced bone resorption in organ culture were studied. Ulnae and radii were removed from 19 day old fetal rats, prelabelled by subcutaneous injection of $200{\mu}Ci\;^{45}CaCl_2$ into their mother on the 17th day of gestation. Radioactivities of $^{45}Ca$ released into media were determined after 24, 48 and 72 hours. Effects of LPS and carbonic anhydrase inhibitors were observed by the ratio of $\%$ release of $^{45}Ca$ between paired control and experimental group. The observed results were as follows : 1. $LPS(1{\mu}g/ml)$ supplemented in media for 72hours increased the $^{45}Ca$ release significantly after 48 and 72 hours of culture and $LPS(10{\mu}g/m1)$ increased the $^{45}Ca$ release significantly after 72 hours of culture. 2. LPS-induced $^{45}Ca$ release was not inhibited significantly by 1mM sulfanilamide but inhibited significantly by 10mM sulfanilamide after 48 and 72 hours of culture. 3. LPS-induced $^{45}Ca$ release was not inhibited significantly by 0.1mM dichlorphenamide but inhibited significantly by 1mM dichlorphenamide after 48 and 72 hours of culture. 4. LPS-induced $^{45}Ca$ release was not inhibited significantly by 1mM acetazolamide but inhibited sighificantly by 5mM acetazolamide after 72 hours of culture.

  • Exfoliation of the primary teeth and eruption of the permanent teeth is a normal and predictable physiologic process. The accurate prediction of tooth development is of major importance in treatment planning for children. Norms that correspond to the child's age and development have been determined by many investigators. However, there are a variety of factors that may hasten, retard, or otherwise affect the normal developmental process. Among the factors that influence tooth eruption is the premature loss of primary teeth. An abscess of a primary molar, followed by bone resorption and early extraction or exfoliation may lead to premature eruprion of the permanent succesor, and a variety of clinical sequelae. I, hereby present the patients who have prematurely erupted permanent successor in Seoul National University Hospital Pediatric Dentistry, and report the clinical and radiologic characteristics, its sequelae, proper managements, and followed up findings.

  • Loss of multiple teeth by multiple dental caries, traumatic influence or hereditable disease can cause many problem in children. Using removable appliance can be used for treatment of this problem. But conventional removable appliance has limits of retention by wire-clasp, non esthetic appearance, difficulty in pronunciation by its volume. Flexible denture using $Valplast^{(R)}$ system could be a excellent partial denture for restorarion in child because of various advantage such as retention from soft tissue, natural esthetic aspect, biocompatibility, durability to compare with conventional acrylic appliance. The present case report describe properties and consideration of flexible denture using Valplast $system^{(R)}$ in comparison with conventional acrylic appliance and report successful restoration of child using Valplast $system^{(R)}$.

  • Supernumerary tooth is developed from overgrowth of dental laminar and is mainly observed in the maxillary anterior area, named 'mesiodens. Mesiodens cause many problems such as malposition of permanent anterior tooth, diastema, formation of cyst, and eruption to oral or nasal cavity. Extraction, therefore, is generally recommended choice of treatment. This case report describes that the patient is a 9 years and 8 months old boy, whose unilateral maxillary incisor was missing due to trauma, and he had a mesiodens. We practiced autotransplantation of the mesiodens to the missing maxillary central incisor area and then restored with prosthetics, substantially kept out alveolar bony resorption and provided esthetic appearance. Mesiodens was too small to be used as an abutment because its length is totally 14mm, so that poor prognosis was anticipated due to bad crown/root ratio. However, we observed clinically acceptable result during the observation period of 8 months after operation.

  • Management options for impacted maxillary canines can include (1) continued observation, (2) extraction of the primary canine to aid spontaneous eruption, (3) uncovering and bonding of the impacted tooth and its eruption using orthodontic traction, (4) autotransplantation, and (5) extraction followed by prosthetic replacement. Autotransplantation should be considered when the degree of malposition is too severe to correct by orthodontic alignment. The present report describes the management of an ectopic eruption of the left maxillary canine in an 10-year-old girl. The treatment included the extraction of primary maxillary left canine and the autotransplantation using a Rapid Prototyping model. By using RP model to contour the recipient bone and check for fitting in the prepared socket, the extra-oral time can reduce. The autotransplanted canine showed mobility within normal limit, negative response to percussion and positive to electric pulp test after 6 months.

  • Aspects of structural differentiation of the free -floating hydrophyte, Spirodela polyrhira, has been investigated. The hydrophyte exhibited highly reduced structures having only fronds, connective stalks, and roots as a mature plant. The fronds were major photosynthetic and vegetatively reproducing organ during the growth. Daughter fronds which developed early in the mother frond were also chlorenchymatous, but they remained within foliage sheaths of the mother frond before separation from connecting stalks. Although the stalks and roots originated from the same meristematic region of the frond, they exhibited the distinct polarity showing former lateral growth and latter axial growth. Air chambers were formed only in the fronds and roots. Cellular components were scattered throughout the diffuse cytoplasm in most of the actively growing stalk cells. Root cells protected by the root cap demonstrated relatively complex organization, showing dense cytoplasm with Golgi, rER, mitochondria, and chloroplasts in the cortical cells. Cells in the root cap were highly vacuolated within the peripheral cytoplasm. Such reduction and differentiation of the plant body in Spirodela polyrhiza most effectively contributes to the better adaptation of smaller plants to superficial aquatic environments, while also enabling the rapid growth.

  • This study was attempted to analyze the proximate composition content of Jangduck in order to find out usefulness as a stored food. Sex different group of Jangduck were assigned according to the contents of three different glutinous rice(10, 20, or 30%) with or without 30fo beef supplematation. The contents of moisture, protein, fat, carbohydrate, fiber and crude ash of Jangduck were periodically analyzed four times during 60 days. The results wee summarized as follows: 1. The content of mois-ture in jangduck decreased as the level of glutinous rice flour increased in 15days. The moisture content slightly decreasedafter 30days of storage time. 2. The content of protein, fat, carbohydrate, crude fiber and ash of Jangduck were shown to be unchanged. 3. The crude protein of Jangduck was relatively high. 4. There was little change of proximate composition contents of Jangduck throu-ghout the total storage period.

Clinical Study of Vascularized Osteocutaneous Fibular Transfer to the Tibia (경골에 시행한 생비골 및 생피부편 이식의 임상적 연구)

  • Recent advances in microsurgery have made it possible to provide a continued circulation of blood to the grafted bone so as to ensure viability. With the nutrient blood supply preserved, healing of the graft to the recipient bone is facilitated without the usual replacement of the graft by creeping substitution. We reviewed 34 cases of vascularized osteocutaneous fibular transfers to the infected tibial defect complicated with skin defect, which were performed from May, 1982 to January, 1992, and the following results were obtained: 1. Despite of uncontrolled bone infection with skin defect, the vascularized osteocutaneous fibular flap transfer could be performed. 2. In the vascularized osteocutaneous fibula transfer, the patency of anastomoses could be indirectly monitored by observing the color of the skin flap. 3. The vascularized fibula had been hypertrophied with bony union during the follow-up period of 13 months to 6 years and 4 months(average, 30 months) and there was no resorption of the grafted fibula. 4. There was no fracture of the grafted fibula in parti resection of involved tibia. 5. The hypertrophic potentiality of grafted fibula could be inhibited by the infection status as operation site.

STABILITY OF SIMULTANEOUS IMPLANTATION WITH AUTOGENOUS ILIAC BONE GRAFT IN THE SEVERELY ATROPHIC MAXILLA (심하게 흡수된 상악골에서 자가 장골 이식술과 동시에 시행한 임프란트 치료의 안정성에 대한 연구)

  • A severely atrophic maxilla may disturb the proper implant placement. The various bone graft techniques are required for simultaneous or delayed implantation in the cases of atrophic alveolar ridges. We present 11 consecutive patients treated with simultaneous implantation using the autogenous inlay and/or onlay bone grafts from iliac crest to the floor of the maxillary sinus and the alveolar crest. In the cases of atrophic maxilla, a total 69 implants were simultaneously placed with autogenous iliac bone graft. 40 fixtures were inserted in the sinus floor simultaneously with subantral block bone graft, the other 29 fixtures were placed in the anterior or premolar areas with block or particulate bone graft. The vertical alveolar bone height was measured with Dental CT at the preoperation and 6 months postoperation. Moreover, the implant stability quotients (ISQ) were measured by $Osstell^{TM}$ during second implant surgery at 6 months later of first implantation. All implants were obtained successful osseointegration with the grafted bone. The mean vertical increases were 3.9mm in the anterior ridges and 12.8mm in the posterior ridges. During the second implant surgery, mean ISQ were 62.95 in the anterior ridge and 61.32 in the posterior ridge. We concluded that the simultaneous implantation with autogenous iliac bone graft were stable and available methods for severely atrophic maxilla.
