  • 본 논문에서는 디지털 X-ray 영상에서 자동으로 손가락의 골단판을 추출하는 알고리즘과 이를 이용한 자동 뼈 나이 측정 방법을 제안한다. 골단판은 뼈의 골화 정도 및 성장 정도에 따라 변화되기 때문에 뼈 나이 평가 시스템에 적합하여 많이 사용된다. 골단판을 자동으로 추출하는 알고리즘을 이용해 골단판을 추출하고 영상인식 분야에서 사용되는 eigen analysis를 사용하여 뼈 나이를 자동으로 측정하였다.

  • 뼈 나이 평가는 소아 뼈의 골화정도, 내분비선 장애 등을 쉽게 알아 볼 수 있어 소아 방사선 의학에서 자주 사용되는 방법이다. 뼈 나이 평가를 위해서는 골단판과 손마디 뼈의 길이 넓이 등 뼈 정보가 필요하기 때문에 골단판 영역의 추출이 선행되어야 한다. 하지만 골단판의 성장이 많이 진행되어 손마디 뼈 부분과 붙어 있는 경우 골단판 추출이 어려운 점이 있다. 본 논문에서는 골단판 성장 여부와 상관없이 다양한 나이의 디지털 X-ray 영상에서 손가락의 골단판을 추출하는 알고리즘을 제안한다. 손가락 경계선의 레이블링 처리를 이용하여 정확한 손가락 영역을 추출하고 골단판 위치의 통계적 특성을 사용하여 골단판의 후보 지역을 생성한다. 그리고 골단판 영역에서는 손가락 영상의 수직 투영 미분값이 크기 때문에 후보 지역 내에서 수직 투영 미분값의 변화량으로 골단판의 위치를 정확하게 추출한다. 다양한 나이에 대해 실험해 본 결과 제안한 방법은 골단판의 성장 여부와 상관없이 골다판과 손가락 뼈가 붙은 곳에서도 골단판의 통계적 특성을 사용해 정확한 골단판 영역을 추출할 수 있었다.

Longitudinal Alterations on Lumbar Vertebral Trabecular Bone Qualities during Pregnancy (임신기간 중 척추 해면골의 골질(bone qualities) 변화)

  • The aim of this study was to detect longitudinal alterations on lumbar vertebral trabecular bone quality (microarchitecture and degree of mineralization) and bone mineral density (BMD) during pregnancy Virginal eighteen mice were used. Then, twelve mice were mated. Mice lumbar vertebrae were scanned before mate, at 7 days of pregnancy (early pregnancy, 6 mice) and immediately after delivery (late pregnancy, 6 mice) by using in-vivo micro-computed tomography Structural parameters, degree of mineralization and BMD were measured. During early pregnancy, there were no significant alterations on structural parameters, degree of mineralization and BMD. At late pregnancy, Tb.Th (11.8%) and BMD (12.7%) were significantly decreased and Tb.N (6.3%), Tb.Pf (43.0%) and BS/BV (15.1%) were significantly increased (p<0.05). Additionally, the lower degree of mineralization was increased, although, the higher degree of mineralization was decreased. These results indicated that the quality and BMD might be not affected during early pregnancy. At late pregnancy, however the bone quality and BMD were likely to be negatively affected.

Outcomes and Complications of Total Elbow Arthroplasty (주관절 치환술의 임상 결과와 합병증)

  • Purpose: To describe the recent clinical results and complications of total elbow arthroplasty based on the literature review. Materials and Methods: The indications of total elbow arthroplasty include rheumatoid or inflammatory arthritis, posttraumatic arthritis, anklylosed elbow, tumor resection which cannot recover elbow function by other reconstructive procedures, and comminuted distal humerus fracture in elderly patients. Complications are aspetic loosening, infection, prosthesis fracture, periprosthetic fracture, ulnar neuropathy, ectopic ossification, triceps insufficiency, dislocation, and bushing wear. Results and Conclusion: Mean 10 year survival rate following total elbow arthroplasty has been reported 85% on the basis of revision. The prognosis in patients with an inflammatory arthritis is reported to be best, and loosening rate in patients with a posttraumatic arthritis tends to be high. Complication rate is known to be higher than that of other joint arthroplasty. In particular, deep infection occurs in 3~5% of the patients. Total elbow arthroplasty provide satisfactory results when it is performed properly in selected patients who have an elbow joint with irreversible dysfunction and low level activities.

  • Chondroblastoma is a benign chondroid-forming tumor usually originating in the epiphysis. The base of metatarsal bone contains neither an epiphysis nor a secondary ossification center and so is the rare site of chondroblastoma. Here, we present a case of chondroblastoma of the base of fifth metatarsal bone in 34-year-old man. Histologically, the osteoclast-like giant cells were abundant enough to simulate a giant cell tumor. And the chondroid intercellular matrix was intermixed but scanty. However, the background mononuclear cells showed irregular and indented nuclei with longitudinal clefts and positive immunoreactivity for S-100 protein, as the evidence of chondroblasts.

A Novel COMP Gene Mutation in a Korean Kindred with Multiple Epiphyseal Dysplasia

  • Multiple epiphyseal dysplasia (MED) is a clinically and genetically heterogeneous chondroplasia, characterized by delayed development of the ossification centers and, deformities of the extremities that involve only the epiphysis and result in mild short stature. Mutations in the cartilage oligomeric matrix protein (COMP) gene are most commonly found, and most of the mutations are located in the calmodulin-like repeats and the C-terminal domain. We report a Korean kindred of 12 family members with MED in four generations who were found to have a novel mutation in the COMP gene. A pedigree showed early onset osteoarthritis requiring arthroplasty that was an autosomal dominant inherited trait. Radiological examinations demonstrated the presence of osteochondral defects in the medial femoral condyles, and the knee and hip joints showed variable degrees of precocious degenerative changes. Mutation analysis of the COMP gene in the proband and five other affected family members identified a novel missense mutation, c.1280G>C (p.Gly427Ala) in exon 12, which was not found in three unaffected family members. Direct sequencing of the COMP gene may yield pathogenic mutations in dominantly inherited MED cases, and may provide opportunities of carrier detection among high-risk family members, leading to genetic counseling for early diagnosis and intervention before the onset of complications.

Teratological Evaluation of Azinphos-Methyl in Rat Fetuses (Azinphos-methyl이 랫트 태아에 미치는 기형학적 연구)

  • This study was carried out to investigate the teratological potential of azinphos-methyl in the rat fetuses and to establish the nature of the effects on organogenesis and intrauterine development. The Sprague-Dawley female rats (180-210g) without previous litter were used in this study. Azinphos-methyl dosages of 0.094mg/kg, 0.4mg/kg, 1.5mg/kg were selected based on the acute intragastric $LD_{50}$ of 15mg/kg in the rat. Azinphos-methyl in water (Treatment Group), non-treatment control (Negative Control), water control (Sham Control), were administered by oral route and aqueous solution of acetyl salicylic acid (Positive Control) was administered by gavage at rate of 10 ml/kg of body weight from day 6 through 15. The results obtained were summarized as follows. 1. Decreased body weight of dams was observed in animals treated with aspirin and azinphos-methyl 1.5 mg/kg from day 7 through 14. (P<0.01) 2. There was an apparent decrement in the absolute liver weight in the azinphos-methyl 1.5 mg/kg treated group (P<0.05). However, the absolute and relative kidney weight in aspirin group (P<0.05, P<0.01) and the absolute and relative ovary weight in aspirin, azinphos-methyl treatment groups (P<0.01, P<0.05) were increased. 3. Decreased protein contents of dam's liver was observed in the aspirin and high dose azinphos-methyl treated group of animals (P<0.01). 4. The number of male-female ratio per dam increased in azinphos-methyl 1.5 mg/kg group but there was an apparent decrement in the body weight of fetuses in aspirin and high dose azinphos-methyl group (P<0.01, P<0.05). Total immature and resorbed fetuses were increased in aspirin group and the number of dead fetuses were also increased in azinphos-methyl 1.5mg/kg treated group of animals. (P<0.01, P<0.05). 5. In soft tissue defects, diaphragmatic hernia in diaphragm, anophthalmia, enlarged olfactory bulb, hydrocephalus, absence of third and lateral ventricle in skull, hydronephrosis in kidney, atrophy of left ventricle wall, enlarged apex in heart were observed. Especially, defects of diaphragm, heart and eye ball showed peak incidences in the high dose azinphosmethyl and aspirin group. (P<0.01). 6. Variations in the ossification patterns of skull, sternebrae, tail, forelimbs and hindlimbs showed peak incidences in the aspirin and high dose azinphos-methyl group. (P<0.01). 7. In the developmental indices of offspring, the mortality of aspirin and azinphos-methyl 1.5mg/kg treated group was higher than that of negative control. And, there was an apparent decrement in the body weight of fetuses (P<0.01) and considerable differences were obtained in pivoting, development of fur, auditory function, vision, quadrupled muscle development and testes descent in aspirin and azinphos-methyl 1.5mg/kg group. Effect of Backfat Thickness on the Carcass Grade Factors and Carcass Price in Hanwoo Cows and Steers (등지방 두께가 한우 암소와 거세우의 도체 등급 요인 및 도체가격에 미치는 영향)

  • Analyses were conducted to estimate the effect of backfat thickness (BFT) on carcass grade factors and carcass price in Hanwoo cows and steers. Data on grade factors and prices were collected from 69,159 Hanwoo carcasses (n = 13,376 cows, n = 55,783 steers) abtained at the Nonghyup Seoul slaughter house in 2009. Carcass BFTs were divided into nine categories from ${\leq}$ 3 mm to ${\qeq}$ 25 mm. The average BFTs were 11.93 mm in cows and 12.57 mm in steers. An increase in the BFT resulted in an increase in carcass weight, loin-eye area, skeletal maturity, fat color, but decreases in the yield index and yield grade (from grade A to C) in cows and steers. Carcass unit price (Won/kg) improved if the carcass was sold with a BFT of 13-15 mm in cows and 10-12 mm in steers. Marbling score and quality grade increased significantly to 21 mm BFT but decreased from a BFT of 22 mm in cows and steers. BFT was significantly correlated with yield index (cow, r = $-0.97^{**}$; steer, r = $-0.96^{**}$), marbling score (cow, r = $0.20^{**}$; steer, r = $0.14^{**}$), and carcass price (cow, r = $0.03^{**}$; steer, r = $-0.11^{**}$). These results could be used as a fundamental dataset for further research to increase the productivity of Hanwoo carcasses.

The Immunohistochemical Expression of Collagens and the Morphogenesis in the Developing Mandible of Human Embryos and Fetuses (배자와 태아에서 하악골의 형태발생 및 교원질 발현에 관한 면역조직화학적 연구)

  • Underlying malocclusions and dentofacial deformities are often related to variations in the craniofacial development. Type I and type II collagens are considered the major collagens of bone and cartilage respectively. Monitoring the patterns of those protein expressions during development will Provide a basis for the understanding of normal and abnormal growths. This study was undertaken to investigate the morphogenetic changes and the expression patterns of type I and II collagen proteins involved in the developing mandible of human embryos and fetuses. 50 embryos and fetuses were studied with Hematoxylin and Eosin, Alcian, blue-PAS, Masson Trichrome, md Immunohistochemical stains. The results were as follows : 1. A 13.5 mm embryo showed the stomatodeum with dental lamina, maxillary and mandibular processes. Meckel's cartilage appeared in the mandibular arch of a 20.5 mm embryo. New bone formation was bilaterally initiated at the outer side of middle portion of Meckel's cartilage of 22-38 mm embryos. 2. Meckel'cartilage was resorbed at the 15th week fetus. The endochondral ossification was observed where there was direct replacement of cartilage by bone. Meckel'cartilage disappeared and membraneous ossification were observed at the 25th week. 3. Before the appearance of Meckel's cartilage, the expression of type I collagen was moderate at the odontogenic epithelium of maxillary & mandibular process, but mild for the expression of type II collagen. 4. During the appearance of Meckel's cartilage and new bone formation, the immunoactivity of type II collagen was more expressed than type I collagen at the Meckel's cartilage and new bone. 5. During intrarmembranous bone formation, the expression of type II collagen was rare in the bony trabeculae. There was a switch for the expression of collagens from type II to type I during the appearance of Meckel's cartilage.
