Delayed Diagnosis of Volar Dislocation of the Distal Ulna after Treatment of the Radial Shaft Fracture (요골 간부 골절 치료 후 지연 발견된 원위 척골의 전방 탈구)

  • Delayed treatment of volar dislocation of the distal radioulnar joint can result in wrist pain, limited rotation of the forearm, and degenerative arthritis that could be managed only by salvage procedures. A 24-year-old male patient presented with wrist pain and a loss of forearm rotation after surgery for a radial shaft fracture. The shaft of the radius was fixed with a plate and screws with a volar angulation of 7°. The ulnar head was dislocated volar to the distal radius, and the bone defect in the ulnar head was impacted into the volar rim of the sigmoid notch of the radius, preventing the head from being reduced in the joint. Corrective osteotomy of the malunited radial shaft and sliding osteotomy of the proximal ulnar head were performed to fill the distal bone defect. Pain and range of the forearm rotation were improved at postoperative 19 months.

  • Isolated fracture dislocation of the tarsal navicular bone is extremely rare. The mechanism of injury of this fracture dislocation is known as a horizontal or axial load on plantar flexed foot. Closed or open reduction is recommended for displaced navicular fracture. We report one case of isolated fracture dislocation of the tarsal navicular which was treated with closed reduction and percutaneous K-wire fixation.

  • A 1 year 4 months old, male Poodle was presented with a history of non weight bearing lameness and pain of the right forelimb by trauma. Orthopedic examination and radiographs were revealed medial shoulder luxation and scapular fracture. Biceps tendon was translocated to the lesser tubercle and secured to the humerus. Despite of it could be a potential cause of degenerative change of the joint, a favorable result was achieved for more than 3 years follow-up.

  • 본 연구는 1987년 1월부터 1991년 12월까지 연세대학교치과병원 구강악안면외과에 과두 골절이 의심되어 내원한 환자중 임상 및 방사선학적 소견에서 하악과두 골절로 진단되어 비외과적 치료를 받은 12세 미만의 13명의 소아환자를 연구 대상으로 하여 임상 및 방사선학적 소견을 분석하여 다음과 같은 결론을 얻었다. 비외과적인 보존적 또는 기능적 치료를 받은 후의 결과는 양호하였으며, 골의 재생과 개조는 신속하여 골 구조는 기능 중에 가해진 힘에 반응한다는 Wolff의 법칙과 일치하는 듯이 보였다. 두 예의 골절탈구에서 과두의 단축 및 과두가 골절된 악관절에서 clicking음을 보였으나 저작하거나 개구시에 큰 어려움은 없었다. 술후 관찰기간이 불충분하였으나 본 예의 대부분에서 비외과적인 술식에도 만족스러운 치료결과를 보였다.
