The effect of horizontal microgap location on the bone loss around 2-piece implants (2-piece 임플란트에서 미세 간극의 수평적 위치 변화에 따른 골 소실 비교 연구)

  • 2-piece 임플란트에서는 초기 치유 기간 동안 0.9-1.6mm의 골 소실이 일어나는데 2-piece 임플란트의 미세 간극과 그에 따른 생물학적 폭경의 형성이 중요한 원인이다. 최근 수직적으로 미세 간극의 위치를 변화시킴으로 골 소실의 양을 줄일 수 있다는 보고가 있다. 이번 실험의 목적은 미세 간극의 수평적 위치 변화에 따른 골 소실의 양을 비교하는데 있다. 하악에서 인접하여 최소 2개의 임플란트를 식립할 수 있는 7 무치악 부위에 총 15개의 Osseotitie XP 4/5를 식립하였다. 이때 임플란트의 상연이 주위 치조골과 일치하게 식립하였고 무작위로 선택하여 한 그룹(W군)에서는 wide diameter healing abutment를 연결하였고 다른 한 그룹(S 군)에서는 standard diameter healing abutment를 연결하였다. 3개월의 치유 기간후 보철 과정을 시작하였으며 이 때 healing abutment와 같은 크기의 prosthetic component를 이용하였다. 임플란트 식립 직후, 3개월의 치유 기간이 지난 보철 직전(Interval I)에, 보철 과정 직후(Interval II)에 각각 치근단 방사선 사진을 찍어 각 단계에서의 골 소실 양을 비교하였다. W 군의 경우 골 소실의 양이 Interval I에서 $1.60{\pm}0.78$, Interval I+II에서 $2.36{\pm}0.29$이었고 S 군에서는 Interval I에서 $1.5810{\pm}0.3030$, Interval I+II에서는 $1.7346{\pm}0.4199$이었다. W군에서는 Interval I와 I+II에서의 골 소실 양이 통계학적으로 유의할 만한 차이를 보였으며, Interval I+II에서의 W 군과 S 군에서의 골 소실 양도 통계학적으로 유의할 만한 차이를 나타내었다. Interval I에서는 두 그룹에서 골 소실의 차이가 없었는데 이는 1 stage surgery시 healing abutment 주위로 mucoperiosteal flap 접합의 어려움 때문으로 생각된다. 한편 Interval II에서는 abutment manipulation 등의 과정이 추가적인 골 소실을 야기한 것으로 생각된다. Interval I+II에서 W 군과 S 군 사이의 골 소실 양 차이는 미세 간극의 수평적 위치 변화의 양과 유사한 결과를 나타내었는데 이로 미루어 미세 간극의 수평적 이동은 임플란트 주위의 골 소실 양에 영향을 미칠 수 있다고 생각된다.

Radiographic evaluation of computer aided design/computer aided manufacturing (CAD/CAM) customized abutment of implant (CAD/CAM으로 제작된 임플란트 맞춤형 지대주의 방사선학적 평가)

  • Purpose: In this study, the retrospective radiographic study is executed to evaluate amount of bone loss of various conditions in patients using customized abutment for 4 years of follow-up. Materials and methods: The subjects of this study were implant fixed dental prosthesis using CAD/CAM customized abutments. CAD/CAM customized abutment and fixed dental prosthesis were manufactured by the Prosthodontics Department of Chosun University Dental Hospital from August 1, 2011 to July 31, 2012. Radiological assessments were performed on the patients who were treated by the fixed prosthodontics. After each treatment, a retrospective study was performed for a total of 4 years at 3 months, 6 months, 1 year, 2 years, 3 years, and 4 years. Results: As a result of the study, the customized abutment using CAD/CAM showed less bone loss than the results of existing research. There was no statistically significant differences at alveolar bone loss between splinting group and non-splinting group (respectively 0.27 mm, 0.5 mm). Also, there were statistically significant differences at alveolar bone loss in mx. anterior, mx. posterior, mn. anterior and mn. posterior part (respectively 1.37 mm, 0.39 mm, 0.00 mm, 0.30 mm). Conclusion: The customized abutment using CAD/CAM showed less bone loss than the results of existing research, there were statistically significant differences at alveolar bone loss in implant positions.

A Convergence Study of age-related Bone Loss and Peak BMD in Korean (한국인에서 연령에 따른 요추 및 대퇴부에서의 최대 골밀도 및 골소실률에 관한 융합 연구)

  • We investigated the age-related BMD, accumulated bone loss rate and peak BMD at Lumbar spine, total hip in Korean using data from KNHANES (the 1st(2010), 2nd(2011) and year at the 5th survey). We found that the cubic regression model was the best for describing age-related changes in BMD. Lumbar spine, total hip in bone mineral density difference were analyzed using ANOVA. This showed that the peak BMD was at the age of 20-24 years at lumbar spine, total hip and the bone loss rate was the highest in the lumbar spine at 75-79 years and the total hip was 80 years or older in the men. This showed that the peak BMD was at the age of 40-44 years at lumbar spine, total hip and the bone loss rate was the highest in the lumbar spine at 70 years or older and the total hip was 75-79 years older, 80 years old, 55-59 years old in the women. Therefore, in men, 75 years or older to increase the rate of osteoporosis screening, and women in their 50s and older menopause related management strategies to manage osteoporosis will be needed.

Retrospective study on marginal bone loss around maxillary anterior implants with or without bone graft (상악 전치부에서 골 이식 유무에 따른 임플란트 변연골 소실에 관한 후향적 연구)

  • Purpose: This study is to evaluate the clinical significance of implantation with simultaneous bone graft by comparing the marginal bone loss around maxillary anterior implants with or without bone graft. Materials and methods: Patients treated with implant-retained restorations on maxillary anterior region at Implant Center, Dental Hospital, Wonkwang University between June 2011 and May 2014 were included in this study. Date of implant placement, implant diameter, implant length, implant-abutment connection type and whether the bone graft was done were investigated. The patient's periapical radiographs taken immediately after implantation and at the most recent visit were compared. Marginal bone loss was measured using Emago advanced v5.6 program (Oral diagnostic systems, Amsterdam, Netherlands). Statistical analysis was done in independent t-test by using SPSS 22.0 program. Results: As a result of observing on 83 implants (without bone graft: 44, with bone graft: 39) of 52 patients for 6 - 45 months (average: 18.4 months), implants without bone graft showed $1.42{\pm}0.42mm$, implants with bone graft showed $1.28{\pm}0.45mm$ of marginal bone loss. Conclusion: In limitations of this study, implants with simultaneous bone graft had significantly less marginal bone loss than implants without bone graft.

  • Alveolar crest is the section of interproximal alveolar bone which includes the free edge of the alveolar process. An increase of the normal forces within limits of tolerance leads to deposition of new bone. If forces are beyond the limits of tolerance, resorption of bone will result whether the force produces pressure or tension. This study was designed to evaluate changes of alveolar bone levels in mesial and distal surface of the left, right first molar, by using pre-treatment, post-treatment panorama films. Two hundreds sixteen subjects were divided into adolescent group of 104 subjects and adult group of 112 subjects, to which orthodontic treatment with a bicuspid extraction (adolescent group-50 subjects, adult group-50 subjects) or without a nonextraction (adolescent group-54 !subjects, adult group-62 subjects) was applied by fixed appliances. Pre- and post-treatment Panorama films were traced, and alveolar crest height was measured. Amounts of changes in alveolar crest height by treatment were calculated md compared in terms of side of tooth, extraction, age. The results were as follows ; 1. When pre-treatment alveolar crest bone levels were compared, levels of adult group were significantly lower than those of adolescent group. 2. Post-treatment alveolar crest bone levels were significantly lower than pre-treatment levels. 3. When changes of alveolar crest height were compared, between adolescent and adult group were not significantly. 4. When changes of alveolar crest height were compared, significantly larger changes were noticed in ex윤action than nonextraction cases. 5. When changes of alveolar crest height were compared, significantly larger changes were noticed in maxilla than mandible. 6. When mesio-distally compared, significantly larger changes were observed in the distal than mesial sides of adult group.

The use of granulation tissue for the esthetic implant restoration for missing tooth due to alveolar bone loss (치조골 소실로 발치하게 된 치아의 심미적인 임플란트 수복을 위한 granulation tissue의 활용)

  • When maxillary anterior tooth is extracted due to alveolar bone loss, the augmentation of alveolar ridge is very important for esthetic implant restoration. Because alveolar bone loss increases after extraction, the ridge preservation performed right after tooth extraction is meaningful for esthetic implant restoration. However, no achievement of primary closure during ridge preservation can negatively affect bone regeneration. To overcome this problem, we can use granulation tissue in the extraction socket for primary closure. This case report confirmed that primary closure using granulation tissue resulted in not only ridge preservation but also ridge augmentation by providing an environment more advantageous of bone regeneration than the open wound.

Reduction Loss after Extension Block Kirschner Wire Fixation for Treatment of Bony Mallet Finger (골성 추지 신전제한 K 강선 고정술 시행 후 정복소실)

  • Purpose: We investigated occurrence of reduction loss after extension block (EB) Kirschner wire fixation or additional interfragmentary fixation (AIF) and clinical results including extension lag of the distal interphalangeal joint for treating bony mallet finger. Methods: Forty-six patients were included with a mean follow-up of 28 months (range, 12-54 months). Twenty-seven patients were treated with EB K-wire fixation (Group A) while 19 patients were treated with AIF (Group B). We checked radiologic factors, such as amount of articular involvement, volar subluxation, mallet fragment angle, reduction loss, range of motion including extension lag, and functional outcomes using Crawford's criteria. Results: Reduction loss occurred in eight patients (17%). Differences in mean extension lag, age, preoperative volar subluxation and mallet fragment angle between patients with reduction loss and those with reduction maintaining were significant. However, there were no significant differences in gender, hand dominance, amount of articular involvement, AIF, or further flexion between reduction loss and reduction maintaining. As for patterns of displacement, there was a significant relationship between gap or step-off and extension lag. Using Crawford's evaluation criteria, functional outcomes were excellent in 31, good in 10, fair in 3, and poor in 2 patients. Conclusion: Reduction loss should be careful in older age, smaller mallet fragment angle and preoperative volar subluxation.

Optimized Implant treatment strategy based on a classification of extraction socket defect at anterior area (전치부에서 발치와 골결손부에 따른 최적의 심미를 얻을 수 있는 수술법)

  • It is considered an implant failure when there is esthetic problems in the anterior area although the prosthesis function normally. In 2003, Dr. Kan et al stated that implant bone level is determined by the adjacent teeth. After that many scholars have studied how can achieve the esthetics result on adjacent teeth bone loss cases. In 2012, Dr. Takino published an article in Quintessence. He summarized previous articles and reclassified the defects from class 1 through 4. Class 1 and 2 depicts a situation where there is no bone loss on adjacent teeth. In Class 3 and 4, interproximal bone loss extends to the adjacent tooth. If one side is involved, it is Class 3. If both sides are involved, it is Class 4. The clue for esthetic implant restoration is whether bone loss extends to adjacent tooth or not. If the bone level of adjacent tooth is sound, we can easily achieve the esthetic but the bone level is not sound, the surgery will be complicated and the esthetic result will be unpredictable. So regenerative surgery for adjacent tooth is necessary for long-term maintenance. But the options and process were so complicated, the purpose of this article is to report the method simplify the surgery and gain a similar outcome.

Age-Related Bone Mineral Density, Accumulated Bone Loss Rate at Multiple Skeletal Sites in Korean Men (한국 남성의 연령에 따른 부위별 골밀도 변화 및 골 소실률)

  • This study examined the age-related bone mineral density (BMD), accumulated bone loss rate (ABLR) at different skeletal sites in Korean men using the data from the Korea National Health and Nutrition Examination Survey (KNHANES)(the 1st (2010), 2nd (2010) and the year at the 5th survey). The cubic regression model was found to be the best for describing the age-related changes in BMD. The lumbar spine, total hip, femoral neck, trochanter, Ward's triangle in the bone mineral density difference were analyzed by ANOVA. The peak BMD was at 20-24 years at the lumbar spine, total hip, femoral neck, trochanter, Ward's triangle, and the 75-79 years of age group had the highest Accumulated Bone Loss Rate. Therefore, intensive management will be necessary for men over 75 years, and a diagnosis of osteoporosis in Korean men should be made according to The International Society for Clinical Densitometry; ISCD.
