Calcifying Aponeurotic Fibroma - A Case Report - (석회화 건막 섬유종 - 증례 보고 -)

  • Calcifying aponeurotic fibroma is a benign, aggressive fibrous tumor seen in childhood or adolescence that most commonly affects the palm. We report our experience in treating calcifying aponeurotic fibroma in the hand of fifteen-year-old boy. He was presented with a recurrent mass in hypothenar area of his left hand. He had a history of local excision of the mass 7 years ago in local clinic. The palpable mass was hard, tender and movable. The simple X-ray revealed multifocally scattered microcalcifications in the mass. The excised mass showed the features of dense fibrotic soft tissue tumor with multiple small whitish calcifications. The microscopic sections showed a lobulated and a poorly circumscribed proliferation of dense fibrous tissue, containing islands of metaplastic chondroid differentiation with prominent calcification. The recommended treatment of this lesion is wide excision allowing for sparing of functional or vital structures, but high recurrence rate as this case has been reported to be more than 50 percent.

  • The result of the close analysis of the health status of 549 working students (the experimental group) of a high school attached to an industry and that of 1,259 non-working students (control group) based on the question provided by CMI from 1st, June to 31st, July 1984 has been revealed as follows: 1. In contrast with the control group, the experimental one is statistically noticeably higher (p<0.01) in their average complaints both in terms of the whole items(A-R) and the physical items (A-L) 2. In the experimental group, particularly the item(L) for habits has the highest rate of average complaints, followed by the item(B) for the respiratory system and the item(E) for the musculoskeletal system. 3. The average complaints of the individual class in both the experimental group and the control group have the tendency to be on the increase accordingly as the school year goes up, and the increasing rate of the experimental group has turned out to be higher than that of the control group. 4. The average complaints of the experimental group based on residential types are the highest(34.7%) in the case of cooking food for oneself, followed by a dormitory(30.3%), and one's own house (28.5%). 5. The average complaints based on the individual working place has been found out to be higher than the rest in the item of the miscellaneous disease of the blowing and roving parts, the respiratory system of the carding part, the fatigability of the spinning part, the tension of inspecting part.

Effect of Oryzalin on the Gravitropic Response and Ethylene Production in Maize Roots (옥수수 일차뿌리에서 oryzalin이 굴중성 반응과 에틸렌 생성에 미치는 효과)

  • Oryzalin is a dinitroaniline herbicide, which disrupts the arrangement of microtubules. Microtubules and microfilaments are cytoskeletal components that are thought to play a role in the sedimentation of statoliths and the formation of cell walls. Statoliths regulate the perception of gravity by columella cells in the root tip. To determine the effect of oryzalin on the gravitropic response, ethylene production in primary roots of maize was investigated. Treatment with 10-4 M oryzalin to the root tip inhibited the growth and gravitropic response of the roots. However, the treatment had no effect on the elongation zone of the roots. An application of 10-4 M oryzalin for 15 hr to the root tip caused root tip swelling. The application of 1-aminocycopropane-1-carboxylic acid (ACC), a precursor of ethylene, to the root tip also inhibited the gravitropic response. To understand the role of oryzalin in the regulation of the growth and gravitropic response of roots, ethylene production in the primary roots of maize was measured following treatment with oryzalin. Oryzalin stimulated ethylene production via the activation of ACC oxidase (ACO) and ACC synthase (ACS), and it increased the expression of ACO and ACS genes. Indole-3-acetic acid (IAA) played a key role in the asymmetric elongation rates observed during gravitropism. The results suggest that oryzalin alters the gravitropic response of maize roots through modification of the arrangement of microtubules. This might reduce the distribution of IAA in the upper and lower sides of the elongation zone and increase ethylene production, thereby inhibiting growth and gravitropic responses.

Evolutionary Optimization of Neurocontroller for Physically Simulated Compliant-Wing Ornithopter

  • This paper presents a novel evolutionary framework for optimizing a bio-inspired fully dynamic neurocontroller for the maneuverable flapping flight of a simulated bird-sized ornithopter robot which takes advantage of the morphological computation and mechansensory feedback to improve flight stability. In order to cope with the difficulty of generating robust flapping flight and its maneuver, the wing of robot is modelled as a series of sub-plates joined by passive torsional springs, which implements the simplified version of feathers attached to the forearm skeleton. The neural controller is designed to have a bilaterally symmetric structure which consists of two fully connected neural network modules receiving mirrored sensory inputs from a series of flight navigation sensors as well as feather mechanosensors to let them participate in pattern generation. The synergy of wing compliance and its sensory reflexes gives a possibility that the robot can feel and exploit aerodynamic forces on its wings to potentially contribute to the agility and stability during flight. The evolved robot exhibited target-following flight maneuver using asymmetric wing movements as well as its tail, showing robustness to external aerodynamic disturbances.

The Treatment of the Benign Bone Tumor by Curettage and Fresh Frozen Allograft (양성 골 종양의 절제 및 소파술 후 사용한 신선 동결 동종골 이식의 결과)

  • Purpose: We compared the time how much time spent until the bone unions, when we grafted fresh frozen allograft during therapy of benign bone tumor depends on the degrees of bony unions. Materials and Methods: This study selected 29 cases, in which a curettage on the benign bone tumors was conducted and a fresh frozen allograft was transplanted. The area of the focus, the new bone formation, the recurrence of the focus and complications in the plain radiographs were observed. Results: The average time when we could find out bony unions in the radiographs is eleven weeks. The time when we could observe the unions in the radiographs are 11.4 weeks in allograft group, 10.7 weeks in allograft and autograft groups and 13.6 weeks in allograft and bone substitute. On radiologic findings, the average lesion size is 40 cm3. The time when we could find bony unions are 9.3 weeks in less than 40 $cm^3$ and 12.9 weeks in more than 40 $cm^3$. We could observe recurrences in two cases. Conclusion: The authors could get the similar results between fresh frozen allograft, allograft and autograft after curettage of benign bone tumor. We can think the ideal method which is the transplantation of autograft, but if we can’t get autograft enough, the best way which heal the defects is the transplantation of fresh frozen allograft.

Utilization pattern of health care resources of resident in a designated rural area (일부 농촌 지역주민의 보건의료자원 이용양상)

  • 농촌지역은 도시지역에 비하여 의료자원의 부족으로 보건의료자원의 이용 접근성이 낮다고 할 수 있다. 따라서 일부 농촌지역주민의 질병이환과 이환시 보건의료자원의 이용양상을 분석하는 것은 매우 의의가 있다고 하겠다. 본 연구방법은 포천군 주민의 보건의료자원의 이용과 치료원의 이용 양상을 분석할 목적으로 1995년 8월 4일~20일까지 1,200가구를 대상으로 가구 면접조사를 실시하였다. 분석대상 가구수는 1,019가구였으며 훈련받은 조사원에 의하여 조사되었으며 경기도 포천군 지역은 연세대학교 보건대학원 및 간호대학의 연구사업지역으로 본 분석 자료는 연세대학교 연구자료의 일부를 사용하였다. 연구 결과는 다음과 같다. 성별인구분포는 남자가 49.9%, 여자가 50.1%이고 연령별 분포는 60세 이상이 16.5%로써 전국의 노인인구 비율보다 높았다. 교육상태는 13년 이상 교육이수율이 남자가 여자보다 높았으나 전체 조사 대상 인구는 교육수준이 높았다. 가족 형태는 핵가족이 70.9%로써 농촌지역임에도 매우 높았다. 지난 15일간의 이환상태를 조사한 결과 급성이환율은 5.4%(54/1,000)이며 3개월 이상 만성이환율은 130/1,000으로 나타났다. 급성이환시 증상별 분포는 호흡기계질환이 36.4%, 소화기계가 20.9%, 여러 가지 복합 증상이 33.0%을 나타낸 반면 만성이환율은 관절염 및 류마티즘이 21.2%, 기타 골격계가 12.6%로서 높았다. 급성 이환시 의료이용양상은 포천군관내의 의료기관이용이 62.6%, 약국이용이 15.2%, 보건소 이용이 4.0%였으며 의료기관이 주요 이용자원이었다. 아무 조치를 취하지 않은 비율은 6.1%로서 높지 않았다. 즉 극성질환인 경우 전체이환자의 76.5%가 1회 방문으로 문제를 해결한 것으로 나타난 반면 나머지 23.5%가 2회 이상 의료자원을 이용한 것으로 나타났다. 3개월이상 만성이환자는 포천군관내 의료기관의 이용율이 56.3%, 관외의료관 이용이 19.3%인 반면 한방기관이용이 7.6%로서 급성이환시 한방이용비율인 4.0%보다 높았다. 1회 방문이 67.4%인 반면 2회 이상 보건의료자원을 이용한 비율이 33.6%로서 만성질환 이용시 보건의료자원의 이용 빈도가 높은 것으로 나타났다. 본 분석결과 포천군 주민의 주요 보건의료자원의 이용은 의료기관으로 나타났고 미치료율이 타농촌지역에 비하여 낮았으며 만성질환시 급성이환시보다 한방의료이용이 높았음을 나타났다.

The role of the orthodontist in diagnosis of hyperparathyroidism : rare case with general root resorption (부갑상선 기능항진증 진단에서의 교정의사의 역할 : 치근흡수를 동반한 희귀증례)

  • Excessive production of parathyroid hormone causes bony disorder such as periosteal bone resorption and bone pain due to excessive skeletal demineralization. A Class III facial deformity case with generalized root resorption presented bete was fumed out to be due to hyperparathyroidism. Clinical and cephalometric analysis revealed a straight skeletal profile with a retruded maxilla and a prognathic mandible. The x-ray findings demonstrated generalized root resorption of entire dentition to different degree. There also appeared osteoporosis like immature trabecular structure with the evidence of ground glass appearance. Serum test showed elevated 1evel of parathyroid hormone and growth hormone. Change of cranial growth by hyperparathyroidism can be dependent up(In a decreased bone apposition in viscerocranial growth site and abnormalities in cranial suture growth. It is possible to hypothesize that growth retardation of maxilla at least partially be accounted lot hyperparathyroidism. Therefore, regarding to the definite etiology of skeletal Class III and orthodontic treatment planning considering root resorption and osteoporosis, the early diagnosis for the hyperparathyroidism should be carefully carried by clinical and laboratory studies.

Evaluating Seismic Performance of Steel Welded Moment Connections Fabricated with SN Steel (SN 강재가 사용된 강구조 용접모멘트접합부의 내진성능 평가)

  • This study was programmed to fabricate a beam-to-column connection that is limited to a steel-welded moment connection with full-scale members, using SN steel. A cyclic seismic test was conducted of the nine specimens that were fabricated by choosing the test variable for the weld access hole geometry, connection design method, and RBS. From the test results, failure modes, the moment-drift behavior, and the strain distribution were provided. From the specimen material properties, the beam's nominal plastic flexural capacity and classified qualified connection as a special moment flame were calculated. By analyzing the skeleton part and the baushinger part, a range of strength-raising effects, and deformation ratios were provided, with which the seismic performance of the specimens were evaluated. The test results showed that the specimens eliminated their weld access holes that demonstrated higher seismic performance than the specimens' existing weld access holes, and that the WUF-W connection that was reinforced by the supplemental fillet weld around the shear tap that was fastened by five bolts demonstrated superior seismic performance.

3-dimensional finite element analysis of maxillary molar distalization using R-jig with TADs (TADs와 R-jig를 이용한 상악 구치 원심 이동에 관한 3차원 유한요소 분석)

  • Purpose: The purpose of this study was to investigate the differences of displacement pattern depending on type of sliding jig and application method during maxillary molar distalization with temporary anchorage devices (TADs). Materials and Methods: Maxilla with normal tooth size and arch shape was selected to create a 3-dimensional finite element model, which included the bracket, orthodontic main archwire, removable sliding jig (R-jig). The orthodontic mini-implant anchorage was set 8 mm superiorly from main archwire, buccally between the second premolar and first molar. The base experimental design was Condition 1, which was composed $0.019{\times}0.025$ inch stainless steel (SS) of wire size of R-jig, 200 gm force, un-tied state. And the other designs varied to wire size of R-jig, magnitude of force. The results are as follows. Results: As the wire size of R-jig was increased, the deformation of R-jig was decreased. However, the displacement of second molar wasn't different each other. As the force to second molar was increased, the more displacement of second molar was observed, and the more distal tipping movement, vetical displacement was observed. Conclusion: R-jig can get distal teeth movement in orthodontic treatment without side effects.

The Effects of Treadmill Exercise on Inhibition of Soleus Muscle Atrophy and Improvement Functional Recovery after Spinal Cord Contusion in the Rats (척수타박손상 후 트레드밀운동이 근 위축지연과 기능적 회복에 미치는 효과)

  • Physical activity and exercise can promote sensorimotor recovery from central nerve injury. It has been suggested that the functional recovery promoted by exercise training after spinal cord injury might be associated with insulin-like growth factor-I in the inflicted muscle. To investigate morphological and biochemical change of the soleus muscle after spinal cord injury, all tissues were used for H&E, immunofluorescence staining and Western blot. Also, BBB-test was used to evaluate behavioral improvement after spinal cord contusion. Thirty male Sprague-Dawley rats ($230{\pm}10\;g$; 7week in age) were assigned equally to three different groups; Normal (n=10), SCI (n=10), SCI+TMT (n=10). Every rat in SCI and SCI+TMT groups underwent laminectomy at T9 level and then contusion on the exposed spinal cord site in anesthetized condition. After one week-recovery from contusion, every rat in the SCI+TMT group exercised on a motorized treadmill for 30min/d, 5d/wk for 7wks. TMT followed by injury increased IGF-I induction levels in the soleus muscle and inhibited muscle atrophy. Behavioral scales for 4 and 8 weeks after spinal cord injury were improved in the SCI+TMT group compared to the SCI group. These results suggest that treadmill exercise after spinal cord injury might promote functional recovery along with muscle regrowth through the up-regulation of IGF-1 in muscle tissue.
