Implant restoration considering maintenance for a patient with excessive crown height space (과도한 치관높이 공간을 가진 환자에서 유지관리를 고려한 임플란트 수복증례)

  • Complications increases when crown height space is excessive and one should be careful of choosing type of fixed prosthesis in case crown height space is over 12 mm. In this condition, tooth seems to be longer, therefore, it is needed to use gingival shade in aesthetically important area for fixed prosthesis. In this case, fixed prosthesis which has inappropriate maintenance and hygiene could hold food, plaque, calculus, thus increase incidence of inflammation and infection. Moreover, it could lead to chronic inflammation and infection and in that case surgical improvement could be necessary. In present case report, gingival inflammation and swelling developed around prosthesis after having made it considering articulation and aesthetic view in patient who had excessive crown height space. Finding out that design of prosthesis is a factor to make it hard to maintain oral hygiene, a temporary prosthesis which has enough interproximal space and proper emergence profile was fabricated. After two months of observation, significant abnormal symptoms are not found and oral reconstruction is performed using fixed prosthesis with zircornia infrastructure and porcelain build-up based on convenient self hygiene design. Through serial treatment, inappropriate soft tissue response is not observed and satisfactory result in functional aspect comes out.

The Effect of Splinting Methods on the Rearrangement of Periodontal Fibers after Tooth Movement in Adult Dogs (치아이동 후 고정방법이 성견 치주인대 섬유의 재배열에 미치는 영향)

  • The purpose of this study was to evaluate the effect of splinting methods on the rearrangement of periodontal fibers after experimental tooth movement. Orthodontic force was applied by placing closed coil spring between upper third incisor and canine in seven dogs, weighing 20 kg or more. After 3 weeks of force application, 0.0215 inch multistrand wire and polyethylene ribbon were bonded to each side, as a flexible and rigid splinting respectively in 6 experimental animals. The remaining one served as a control. Each two animals were sacrificed at 4, 8 and 12 weeks after splinting respectively and prepared histologically for hematoxylin-eosin and Masson's trichrome staining. The results of this study were obtained as follows: 1. After tooth movement, periodontal space was narrowed and periodontal titers were thick on pressure side while elongated fibers were observed on tension side. 2. After 4 weeks of retention, the rearrangement of periodontal fibers was observed in the flexible splinting group, but not in the rigid splinting group. 3. After 8 weeks of retention, the rearrangement of periodontal titers was observed in both groups, but the difference could not be detected between two groups. 4. During the retention period, the rearrangement of periodontal fibers was faster in tension side than in pressure side. These results show that the rearrangement of periodontal fibers is also obtained by rigid splinting after tooth movement. It is suggested that the rigid splinting by polyethylene ribbon can be used as a way of postorthodontic retention.

  • In order to deliver fixed partial denture to a patient successfully, dentist must take into many considerations beforehand. Patient's occlusal pattern should be fully understood before delivering any type of prosthesis, whether it's canine guidance or group function occlusion. In order to do so, movement of mandible should be recorded precisely. Introduced by Meyers in 1933, functionally generated path technique (FGP) has been successfully used to record mandibular movement with various materials, such as wax, and utilize it in fabricating precise prosthesis. In the following cases, patients showed secondary caries or endodontic-periodontic lesion on maxillary molars. Root canal treatment and subgingival curettage were done for each patient. Since the long term prognosis of each tooth was questionable, lateral force should be minimized. In order to do so, FGP technique was used to record mandibular movements precisely. Instead of using conventional plaster impression, intra-oral scanning of wax tracing was done to fabricate prosthesis efficiently. After delivery of fixed partial denture, favorable outcome was obtained.

  • For a full-mouth fixed prosthetic treatment of the edentulous patient, it is essential to confirm the proper tooth position and thorough evaluation of the remaining alveolar bone and soft tissue before surgery. CAD-CAM dentistry and guided implant surgery have such advantages of providing simultaneous planning of surgery and prosthetic treatment to ensure pre-knowledge of the treatment. In this clinical case, using the digital technology, digital temporary denture fabrication, esthetic evaluation before fixed prostheses treatment, and guided surgery planning was possible. After the surgery, previously obtained data was used for fabricating fixed temporary prostheses. Definitive zirconia prostheses transferred from the provisional prostheses were fabricated and functionally and esthetically satisfying results were obtained.

  • 고정식 연료전지 발전시스템 설치 시 고려해야 할 잠재적 위험성과 설치 방법상의 문제점을 도출하고 그에 대한 해결책을 제시함으로써 운전시의 안전성을 확보하는데 본 연구의 목적이 있다. 연구 대상은 가정용 고정식 고분자 연료전지 발전시스템으로 실증 연구 중에 있는 FCP(Fuel Cell Power)의 1kW급 연료전지 시스템으로 하였다. 본 연구를 위해 위험 요소를 파악하고 위험 요소를 제거하기 위해 필요한 설치 시 요구사항을 도출하여 실제 설치 시에 적용함으로써 신뢰성과 적용 용이성을 확인하였다. 본 연구를 통해 가정용 고정식 연료전지 발전시스템의 설치 시 고려해야 할 요소들을 찾아낼 수 있었고 위험 요소를 제거하여 설치 안전성을 확보할 수 있었다.

Preparation, Characterization and Catalytic Performance of Ionic Liquid Immobilized onto Polystyrene-based Polymer for the Synthesis of Allyl Glycidyl Carbonate (폴리스티렌계 고분자에 고정화된 이온성 액체 촉매의 제조와 알릴글리시딜카보네이트 합성 반응 특성)

  • In this study, imidazole-based ionic liquid on polystyrene was prepared and its catalytic performance in the cycloaddition of $CO_2$ with allyl glycidyl ether(AGE) to produce allyl glycidyl carbonate was investigated. The ionic liquid was generated on the polystyrene-based polymer through the immobilization of imidazole. The prepared catalyst was characterized using a number of instrumental analysis including EA, FT-IR, TGA and SEM. The immobilized ionic liquid showed very good catalytic activity for the cycloaddition of $CO_2$ with AGE, having 80% of AGE conversion with over 96% of the carbonate selectivity at $120^{\circ}C$ under 1.48 MPa $CO_2$ pressure. The immobilized ionic liquid can be used for the reaction up to four consecutive runs without significant loss of its catalytic activity.

  • This report describes the prosthetic treatment of a patient with multiple missing teeth. Installation of five fixtures on maxilla with sinus lift and six fixtures on mandible with ramal bone graft were performed. With implant supported all-ceramic with zirconia core using CAD/CAM technology and porcelain-fused-to-gold prosthesis, treatment with positive outcome which satisfies both functional and esthetical aspect was obtained.

  • The immobilized cell tubular tormentor was prepared by wood chips or alginate gel. Investigations of characterization of the performance of ethanol fermentation in the immobilized cell tubular tormentor were undertaken and the results were compared with those of other tormentors. The immobilized cell tormentor packed with alginate gel showed much higher ethanol productivity than that with wood chip. It was concluded that the immobilized cell tubular tormentor packed with alginate gel might offer better perspectives for continuous ethanol production than that with wood chip.

  • We present a method for spatially selective immobilization of functional materials, such as proteins and nanoparticles, onto pre-activated silicon surfaces by electrochemical reaction. Carboxymethylbenzendiazonium (CMBD) cations, being adsorbable on silicon surfaces through electrochemically reductive deposition, is used as an anchor molecule to prepare the pre-activated silicon surfaces. It is demonstrated that the use of BD reaction is very efficient for the selective immobilization because the functional materials are immobilized exclusively onto the pre-adsorbed CMBD region. The method is applied to immobilize gold nanoparticles on the selected nanowire of the nanowire array.

A Retrospective Study of Luxation Injuries in Primary Teeth: Prognosis with Splinting (유치의 탈구성 손상에 관한 후향적 분석: 고정술의 치료 결과)

  • This study investigated the prognosis of luxation injuries in primary teeth treated with splinting. This study retrospectively analyzed 92 children with luxation injuries to their primary teeth who were treated with splints between 2010 and 2015. Prognoses were analyzed in patients who had been followed for more than 6 months. The prognoses with splinting were based on clinical and radiographic evaluations performed during the follow-up examinations. The mean patient age was 42.1 months, and 67.4% were male. The most common cause of luxation injury was falling, and the mean splint duration was 2.4 weeks. The success rate of splinting was 58.9%. The highest rate of success was achieved following subluxation, while repositioning and splinting of lateral luxation had the lowest rate of success. Pulp necrosis was the most common unfavorable prognosis in the luxation injuries. Depending upon the type of luxation, splint therapy had acceptable prognoses and might be a feasible treatment option.
