방사선치료를 위한 고에너지 광자선의 품질관리를 위해 사용하는 TLD의 광자선 선질에 대한 에너지 의존도를 몬테카를로 모사법을 사용하여 평가하였다. IAEA 선량보증사업에 이용되는 LiF TLD 및 홀더를 EGS4기반의 사용자 코드인 DOSIMETER 와 MCNP4C 몬테카를로 코드를 사용하여 기하학구조를 구성하고, Co, 4, 6,10 밑 15 MV 광자선을 시뮬레이션하였다. DOSIMETER계산 결과를 통해 TLD의 에너지 보정인자가 실험 데이터와 일치함을 확인할 수 있었으며, 이와 별도로 캡슐에 의한 교란량도 무시할 수 없음을 발견하였다.
In order to apply the TRS-398 dosimetry protocol developed by IAEA we directly calculated the quality correction factors for high energy photons. The calculations were peformed for seven commercial cylindrical chambers (A12, IC70, N23333, N30001, N30006, NE2571, PR06C/G). In comparison with quality correction factors given by TRS-398 our results were in good agreement within ${\pm}0.3%$ (maximum ${\pm}0.3%$) for all chambers and photon qualities.
For wedged photon beams, the variation of the wedge factor with field size was reported by several authors. However, until now such variation with field size had not been explained quantitatively. Therefore, the variation of the wedge factor was investigated by measuring outputs with field sizes increasing from 4 cm $\times$ 4 cm to 25 cm $\times$ 25 cm for open and wedged 6 and 10MV X-ray beams. The relative outputs for wedged fields to 10 cm $\times$ 10 cm have been obtained. The results show the Increase of the wedge factor caused by the change in fluence of high energy Photon beam with field size, up to 8.0% for KD77-6MV X-ray beam. This increase could be explained as a linear function of the irradiated wedge volume except small field size up to about 10 cm. In the cases of the narrow rectangular beam parallel to the wedge direction, the wedge factor decreases slightly with increasing field size up to about 10-15 cm due to a relatively reduced photon fluence from the change of the wedge thickness. We could explain the causes of a wedge factor variation with field size as the fluences of primary photon passed throughout the wedge, contributing to the dose at the central beam axis and that the fluences were affected by the gradient of the wedge with the change of field size. For clinical use, the formula developed to describe the wedge factor variation with field size has been corrected.

Purpose : To develop a dose calibration program for the IAEA TRS-277 and AAPM TG-21, based on the air kerma calibration factor (or the cavity-gas calibration factor), as well as for the IAEA TRS-398 and the AAPM TG-51, based on the absorbed dose to water calibration factor, so as to avoid the unwanted error associated with these calculation procedures. Materials and Methods : Currently, the most widely used dosimetry Protocols of high energy photon beams are the air kerma calibration factor based on the IAEA TRS-277 and the AAPM TG-21. However, this has somewhat complex formalism and limitations for the improvement of the accuracy due to uncertainties of the physical quantities. Recently, the IAEA and the AAPM published the absorbed dose to water calibration factor based, on the IAEA TRS-398 and the AAPM TG-51. The formalism and physical parameters were strictly applied to these four dose calibration programs. The tables and graphs of physical data and the information for ion chambers were numericalized for their incorporation into a database. These programs were developed user to be friendly, with the Visual $C^{++}$ language for their ease of use in a Windows environment according to the recommendation of each protocols. Results : The dose calibration programs for the high energy photon beams, developed for the four protocols, allow the input of informations about a dosimetry system, the characteristics of the beam quality, the measurement conditions and dosimetry results, to enable the minimization of any inter-user variations and errors, during the calculation procedure. Also, it was possible to compare the absorbed dose to water data of the four different protocols at a single reference points. Conclusion : Since this program expressed information in numerical and data-based forms for the physical parameter tables, graphs and of the ion chambers, the error associated with the procedures and different user could be solved. It was possible to analyze and compare the major difference for each dosimetry protocol, since the program was designed to be user friendly and to accurately calculate the correction factors and absorbed dose. It is expected that accurate dose calculations in high energy photon beams can be made by the users for selecting and performing the appropriate dosimetry protocol.
It is necessarily to evaluate the energy of X-ray emitted from linear accelerator in order to determine the accurate absorbed dose. The method of direct measurement for x-ray energy is very difficult and impractical. Therefore the method of using beam quality index is generally used. Several dosimetry protocols recommend the use of quality indices such as depth of dose maximum at radiation central axis, dose gradient, and dose level. The linear accelerator manufactures follow the recommendation as dosimetry protocols. The study was performed for us to select the most suitable parameter among the Quality indices as described above. For photon beams of 4, 6, 10, 15, and 21 MV nominal energies produced by four kinds of accelerators(Mitsubishi, Scanditronix, Siemens, Varian) in eleven institutions, We evaluated the x-ray energies obtained by the Quality indices as recommended by several dosimetry protocols and manufactures. Results showed that there were energy spreads according to the same accelerators and Quality indices even though nominal energies were same. It appeared that the percent depth dose at 10 cm (D$_{10}$(%)) gave the smallest deviation and spread of energies. As energies increased, the energy deviation increased for all the quality indices. It is desirable for the use of unified quality index to compare the evaluation of beam quality at different institutions.
It is difficult to determine dosimetric characteristics for small field photon beams since such small fields do not achieve complete lateral electronic equilibrium and have steep dose gradients. Dosimetric characteristics of small field 4, 6, and 10 MeV photon beams have been measured in water with a diamond detector and compared to measurements using small volume cylindrical and plane parallel ionization chambers. Percent depth dose (PDD) and beam profiles for 6 and 10 MeV photon beams were measured with diamond detector and cylindrical ion chamber for small fields ranging from $1{\times}1\;to\;4{\times}4cm^2$. Total scatter factors($S_{c,p}$) for 4, 6, and 10 MeV photon beams were measured with diamond detector, cylindrical and plane parallel ion chambers for small fields ranging from $1{\times}1\;to\;4{\times}4cm^2$. The $S_{c,p}$ factors obtained with three detectors for 4, 6, and 10 MeV photon beams agreed well ($\pm1.2%$) for field sizes greater than $2{\times}2,\;2.5{\times}2.5,\;and\;3{\times}3\;cm^2$, respectively. For smaller field sizes, the cylindrical and plane parallel ionization chambers measure a smaller $S_{c,p}$ factor, as a result of the steep dose gradients across their sensitive volumes. The PDD values obtained with diamond detector and cylindrical ionization chamber for 6 and 10MeV photon beams agreed well ($\pm1.5%$) for field sizes greater than $4{\times}4\;cm^2$. For smaller field sizes, diamond detector produced a depth-dose curve which had a significantly shallower falloff than that obtained from the measurements of relative depth-dose with a cylindrical ionization chamber. For the measurements of beam profiles, a distortion in terms of broadened penumbra was observed with a cylindrical ionization chamber since diamond detector exhibited higher spatial resolution. The diamond detector with small sensitive volume, near water equivalent, and high spatial resolution is suitable detector compared to ionization chambers for the measurements of small field photon beams.

A small water phantom (dual-window phantom) was developed to improve the output measurement efficiency of medical linacs. This phantom is suitable for determining the quality index and output dose for high-energy photon beams. The phantom has two opposite windows and two independently rotating axes. The two axes measure the tissue phantom ratio (TPR) and the percentage depth dose (PDD) simply without requiring chamber movement by rotating the phantom around its axis. High-energy photon beams from a Co-60 irradiator and a medical linac were used to evaluate the phantom. The measured quality index is in good agreement with the reference values; the measured and reference values are within 0.2% of each other for the Co-60 gamma rays and within 1.4% for 6 and 10 MV X-rays. This phantom is more practical for routine output measurements, resulting in the prevention of potential human errors.
A radiophotoluminescent glass rod detector (GRD) system has recently become commercially available. We investigate the dosimetric properties of the GRD regarding the reproducibility of signal, dose linearity and energy dependence. The reproducibility of five measurements for 50 GRDs is presented by an average of one standard deviation of each GRD and it is ${\pm}1.2%$. It is found to be linear in response to doses of $^{60}Co$ beam in the range 0.5 to 50 Gy with a coefficient of linearity of 0.9998. The energy dependence of the GRD is determined by comparing the dose obtained using cylindrical chamber to that by using the GRD. The GRD response for each beam is normalized to the response for a $^{60}Co$ beam. The responses for 6 and 15 MV x-ray beams are within ${\pm}1.5%$ (1SD). The energy response of GRD for high-energy photon is almost the same as the energy dependence of LiF:Mg:Ti (TLD-100)and shows little energy dependence unlike p-type silicon diode detector. The GRDs have advantages over other detectors such diode detector, and TLD: linearity, reproducibility and energy dependency. It has been verified to be an effective device for small field dosimetry for stereotactic radiosurgery.

Purpose: A selection of proper energy in treatment planning is very important because of having different dose distribution in body as photon energy. In generally, the low energy photon has been used in intensity-modulated radiation therapy (IMRT) for head and neck (H&N) cancer. The aim of this study was to evaluate the effect of partially used high energy photon at posterior oblique fields on IMRT plan for H&N cancer. Materials and Methods: The study was carried out on 10 patients (nasopharyngeal cancer 5, tonsilar cancer 5) treated with IMRT in Seoul National University Bundang Hospital. CT images were acquired 3 mm of thickness in the same condition and the treatment plan was performed by Eclipse (Ver.7.1, Varian, Palo Alto, USA). Two plans were generated under same planing objectives, dose volume constraints, and eight fields setting: (1) The low energy plan (LEP) created using 6 MV beam alone, (2) the partially used high energy plan (PHEP) created partially using 15 MV beam at two posterior oblique fields with deeper penetration depths, while 6 MV beam was used at the rest of fields. The plans for LEP and PHEP were compared in terms of coverage, conformity index (CI) and homogeneity index (HI) for planning target volume (PTV). For organs at risk (OARs), $D_{mean}$ and $D_{50%}$ were analyzed on both parotid glands and $D_{max}$, $D_{1%}$ for spinal cord were analyzed. Integral dose (ID) and total monitor unit (MU) were compared as addition parameters. For the comparing dose to normal tissue of posterior neck, the posterior-normal tissue volume (P-NTV) was set on the patients respectively. The $D_{mean}$, $V_{20Gy}$ and $V_{25Gy}$ for P-NTV were evaluated by using dose volume histogram (DVH). Results: The dose distributions were similar with regard to coverage, CI and HI for PTV between the LEP and PHEP. No evident difference was observed in the spinal cord. However, the $D_{mean}$, $D_{50%}$ for both parotid gland were slightly reduced by 0.6%, 0.7% in PHEP. The ID was reduced by 1.1% in PHEP, and total MU for PHEP was 1.8% lower than that for LEP. In the P-NTV, the $D_{mean}$, $V_{20Gy}$ and $V_{25Gy}$ of the PHEP were 1.6%, 1.8% and 2.9% lower than those of LEP. Conclusion: Dose to some OARs and a normal tissue, total monitor unit were reduced in IMRT plan with partially used high energy photon. Although these reduction are unclear how have a clinical benefit to patient, application of the partially used high energy photon could improve the overall plan quality of IMRT for head and neck cancer.
