Real-Time Localization of Parathyroid Glands with Near Infrared Light during Thyroid and Parathyroid Surgery (갑상선·부갑상선 수술 중 근적외선을 이용한 실시간 부갑상선의 국소화)

  • Intraoperative identification and localization of parathyroid glands are crucial step in preventing postoperative hypocalcemia during thyroid and parathyroid surgery. If there is a method to predict the parathyroid's location rather than detecting and verifying with naked eye, it would make the operator easier to find and identify the parathyroid. Recently, near-infrared light imaging technologies have been introduced in the fields of thyroid and parathyroid surgery to predict the localization of the parathyroid. These are being conducted in two ways: autofluorescence imaging with a unique intrinsic fluorophore in the parathyroid tissues and fluorescence imaging with external fluorescence materials specially absorbed into parathyroid tissues. We are suggest that parathyroid glands can be detected by surgeon with NIR autofluorescence imaging even if they are covered by fibrofatty tissues before they are detected by surgeon's naked eye. These novel techniques are very useful to identify and preserve parathyroid glands during thyroidectomy. In this article, we reviewed the latest papers that describe autofluorescence imaging and exogenous ICG fluorescence imaging of parathyroid glands during thyroid and parathyroid surgery.

Surgical Outcome of Extracapsular Dissection of Benign Parotid Gland Tumor: A Comparative Study to Superficial Parotidectomy (이하선 양성종양의 피막 외 절제술의 수술 결과: 이하선 천엽 절제술과의 비교)

  • Background/Objectives: Extracapsular dissection has the advantage of reducing complications by minimizing tissue loss of the parotid without intentionally exposing the facial nerve in patients with benign parotid tumor. However, there has been controversy over the surgical results. Thus, the surgical outcomes of extracapsular dissection for benign parotid tumor was compared to those of superficial parotidectomy. Materials & Methods: A retrospective chart review was conducted with 132 patients who received surgery for benign parotid tumor in our center from January 2014 to December 2018 retrospectively. Results: A total of 132 people were enrolled, with 62 people receiving extracapsular dissection, 38 people receiving partial superficial parotidectomy and 32 people receiving superficial parotidectomy. No significant difference was found between the three groups regarding complications such as facial nerve palsy, Frey's syndrome, or first bite syndrome. Operation time and hospital stay was significantly short in extracapsular dissection group. Conclusion: For well-selected cases, extracapsular dissection can be considered as an option for surgery of benign parotid tumor.

Nursing students' experiences of adapting to clinical practice in the COVID-19 pandemic (COVID-19 감염병 시대, 간호대학생의 임상실습 적응 경험)

  • Purpose: This study was performed to explore and describe the overall clinical practice adaptation experiences among nursing students during the COVID-19 pandemic. Methods: This qualitative study applied the grounded theory method by Corbin and Strauss. Data were collected from May to August 2021 through individual in-depth interviews with 14 nursing students from three universities in B metropolitan city. Results: From open coding, 20 sub-categories and 10 categories were identified. Analysis revealed that the core category was the process of "keeping the place of learning while adhering to the restrictions of the era of pandemic" and that it consisted of four phases: confusion, withdrawal, adjustment, and growth. Through this process, participants utilized various action/interactional strategies such as "shifting to positive thinking," "building a shield to protect oneself," and "focusing on learning." The consequences of these strategies were "adapting to the reality of the infectious disease situation" and "strengthening a foothold to grow as a future nurse." Conclusion: An in-depth understanding of nursing students' experiences of adapting to clinical practice during the COVID-19 pandemic will guide nursing educators to promote effective teaching strategies to better support nursing students in a time of infectious disease crisis.

Influences of positive psychological capital, interpersonal competence, and character on caring efficiency in nursing students (긍정심리자본, 대인관계 유능성, 인성이 간호대학생의 돌봄효능감에 미치는 영향)

  • Purpose: The purpose of this study was to identify the factors influencing nursing students' caring efficiency. Methods: This study included 212 nursing students from three University nursing departments in Busan metropolitan city. Data were collected from September 1 to September 29, 2021, using self-report questionnaires. For data analysis, descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation coefficient, and hierarchical multiple regression analyses were conducted with SPSS version 24.0. Results: Positive psychological capital and interpersonal competence on the relationship and character of nursing students were positively correlated with caring efficiency. The variables affecting the caring efficiency of the subjects were character (β=.60, p<.001), amount of participation in a character development program (5 times or more) (β=.16, p=.023), levels of stress (moderate β=.13, p=.037; low β=.15, p=.015), motivation to apply to a nursing program (β=.12, p=.024), and volunteer experience (β=.11, p=.038). The total explanatory power of the variables was 50.0% (F=14.69, p<.001). Conclusion: Character was one of the biggest influential factors on caring efficiency. In order to improve nursing students' caring efficiency, above all, it is necessary to make efforts to raise the level of individual character.

Bacteremia caused by Leuconostoc species : 6-case series

  • Leuconostoc species are Gram-positive coccobacilli and are used in dairy products and are intrinsically resistant to vancomycin. Leuconostoc infections are rare in humans, usually occurring in immune-compromised patients. We describe 6 patients with Leuconostoc bacteremia at Dong-A university hospital between 1990 and 2015. One isolate (L. lactis) was identified to species level using 16S rRNA gene sequencing analysis. All patients had underlying diseases and 5 patients underwent procedures that interrupted the normal integumentary defense. Four patients died within 30 days after being identified as carrying Leuconostoc species.

  • Spontaneous and isolated dissection of the limb arteries without involvement of the aorta is extremely rare, and has been reported previously in pregnant patients in association with collagen vascular disease, and in cases of high-energy trauma or intensive activity in athletes. There is no consensus yet on indications for medical or surgical therapeutic modality. Due to the rarity of spontaneous dissection of external iliac artery, its natural history has been poorly described. A healthy 50-year-old male with normotension was admitted with an acute onset of left flank pain. Left external iliac artery dissection was diagnosed by abdominal computed tomography.

  • Tracheobronchopathia osteoplastica (TO) is a rare benign disease in which the anterior inner wall of the tracheobronchus changes because of abnormal chondrogenesis or ossification, while the posterior wall of the trachea is spared. The etiology is not clearly understood, but may relate with chronic infection, inflammation, and trauma. In some case studies, it has also been reported to be accompanied by other chronic diseases such as atrophic rhinitis and amyloidosis. However, Coexistence of TO and tuberculosis has rarely been reported, and has never been reported in Korea. Here, we report a case of a 70-year-old male patient who complained of hemoptysis and whose case was diagnosed as TO and pulmonary tuberculosis through bronchoscopy with bronchial washing and biopsy.

Delayed Treatment of Foot Compartment Syndrome: A Case Report and Literature Review (족부 구획증후군의 지연치료: 증례 보고)

  • Acute compartment syndrome occurs when pressure increases within closed compartments due to injuries causing soft tissue damage. Delayed treatment can lead to undesirable consequences. This paper reports a three-year-old patient in whom a fasciotomy was performed successfully despite the potential side effects. Fasciotomy may be considered when the diagnosis and clinical symptoms of delayed compartment syndrome are clear. This study determined that the three-year-old patient would undergo a relatively smooth recovery compared to elderly patients of advanced age. Because the dorsalis pedis artery pulse was palpable, an emergency surgical treatment was performed to restore the damaged tissues and prevent further necrosis. The patient has shown a satisfactory recovery.

  • Ceruminous adenoma is a benign neoplasm of the ceruminous gland, and its development in human external auditory canal is very rare. Due to the limited number of cases, controversy still exists about nomenclature, classification, diagnosis and its treatment. Recently we experienced a 55 years old woman who was diagnosed with ceruminous adenoma of the external auditory canal, and who was treated with wide excision. So we report this uncommon case in combination with a review of the literature.

Mallory-Weiss Tear After Cardiopulmonary Resuscitation in a Patient Suffering From Acute Myocardial Infarction

  • A report of a 79 year old male patient suffering from acute myocardial infarction with Mallory-Weiss tear after successful cardiopulmonary resuscitation(CPR) by emergency medical technician in the swimming pool is presented. Successful percutaneous coronary intervention(PCI) was done after appropriate transfusion. The patient survived and discharged without major complications after admitting 11days in the hospital. Importance of CPR in AMI patient is reiterated as complication such as Mallory-Weiss tear may arise.
