Effect of Stroke Prevention Program for The Elderly in The Rural Area (농촌지역 노인을 위한 중풍예방 프로그램의 효과)

  • The goal of this study was to examine how effective stroke prevention program is for body mass index, blood pressure, blood glucose, and serum lipids of the elderly in the rural areas. The stroke prevention program included health care education, nutrition guidance and exercise. The program was provided for 12 weeks(36 sessions) from May to August, 2014. Participants were 55 elderly people in the rural areas; 28 people in the experimental group and 27 in the control group. The results of before and after the experiment showed that body mass index(p<.001), TC(p<.001), TG(p=.047), and LDL(p=.010) were significantly better in the experimental group compared to the control group. Also, after the program, the figures of body mass index, fasting blood glucose, TC, TG, and HDL in the control group were decreased, while those of LDL were increased. Study findings indicate that the stroke prevention program is effective and can be recommended as a precaution against the stroke of the elderly in the rural area by preventing risk factors beforehand.

The Association of Metabolic Syndrome and Vitamin D and Urine Microalbumin in Korean Adults (대한민국 성인에서 대사증후군, 비타민 D 및 뇨 중 미세알부민의 관련성)

  • The aim of this study was to assess the association of metabolic syndrome (MetS) and vitamin D and urine microalbumin (MA) in Korean adults. The study subjects were Korean adults 20 years or older (n=4,948). A covariance test adjusted for covariates was performed for urine MA levels in relation to vitamin D status (vitamin D deficiency, vitamin D insufficiency, vitamin D sufficiency). The key study results were as follows: First, in subjects without MetS, after adjusting for related variables (age, gender, smoking, drinking, regular exercising, wast measurement, body mass index, total cholesterol, triglycerides, high density lipoprotein cholesterol, fasting blood glucose, and glomerular filtration rate), vitamin D levels were inversely associated with Log-MA levels (p<0.001). Second, in subjects with MetS, after adjusting for related variables, vitamin D levels were not associated with Log-MA levels (p=0.892). In conclusion, vitamin D was inversely associated with urine MA levels in Korean adults without MetS, but not in Korean adults with MetS.

The Effects of an Cardiocerebrovascular Disease Prevention Program on Cardiovascular Risk Factors and Quality of life in Elderly (심뇌혈관질환 예방프로그램이 지역사회 노인의 심혈관 위험요인과 삶의 질에 미치는 효과)

  • The purpose of this study was to investigate the effect of cardiocerebrovascular disease prevention program on cardiovascular risk factors and quality of life in the elderly. A non-equivalent control group pretest-posttest design was used. Participants were 44 elderly who registered at the public health center. The experimental group(n=22) received the cardiocerebrovascular disease prevention program for the elderly 8 times. There were significant differences in the systolic blood pressure(t=-2.419, p= .020), diastolic blood pressure(t=-2.226, p= .031), blood glucose(t=-2.071, p= .045) and quality of life(t=2.511, p= .016) between the experimental group and the control group. The results suggest that cardiovascular disease prevention program can be an effective strategy to reduce cardiovascular risk factors and improve the quality of life of the elderly.

Meta-analysis on the Effect of obesity and blood lipid levels in Salicornia herbacea (함초의 비만 및 혈중 지질에 미치는 영향에 대한 메타분석)

  • This study presents a meta-analysis research to test the improve effects of obesity and blood lipids on Salicronia herbacea L. (S. herbacea) intake in rats. Individual research materials were selected from database such as PubMed, Embase, RISS, DBpia, and NDSL. The Review Manager 5.3 version was used to estimate effect size through meta-analysis. A total of 9 studies were finally included. Body weight gains were decreased in S. herbacea-treated groups as compared to the high fat diet groups. But, no significant effect was observed on FER, liver index, and total cholesterol. In addition, the effect size of triglyceride was reduced, whereas high density lipoprotein (HDL)-cholesterol was increased in S. herbacea-treated group. In this meta-analysis, the results indicated that S. herbacea have a strong effect on anti-obesity through the blood lipids improvement.

The Usefulness of Visceral Fat Thickness Measured by Ultrasonography as an Abdominal Obesity Index (초음파로 측정된 내장지방두께의 복부 비만지표로서의 유용성)

  • Purpose : Abdominal obesity with visceral fat accumulation have been known to be intimately associated with the development of metabolic syndrome. Therefore, it is important to estimate the precise amount of visceral fat. Ultrasonography has been reported that it is a simple and noninvasive method for visceral fat evaluation. Purpose of this study is to evaluate the association of ultrasonographic visceral fat thickness, anthropometric indexes, and risk factor of metabolic syndrome, and to investigate the cut-off value of abdominal visceral fat thickness leading to increased risk of metabolic syndrome. Materials and methods : The subject included 200 men and 200 women who visited D healthcare center in Daejeon from January to April 2008. The subcutaneous fat thickness and visceral fat thickness were measured by ultrasonograph. As anthropometric index, we measured body mass index, waist circumference and waist/height ratio. As for the risk factor of metabolic syndrome, we measured blood pressure, high density lipoprotein cholesterol, triglyceride and fasting serum glucose. Results : VFT was significantly correlated with waist circumference, (r=0.683/M, r=0.604/F), waist to height ratio (r=0.633/M, r=0.593/F) and BMI (r=0.621/M, r=0.534/F) in both men and women. In addition it was significantly correlated with Systolic blood pressure (r=0.229/M, r=0.232/F), Diastolic blood pressure ((r=0.285/M, r=0.254/F), high density cholesterol (r=-0.254/M, r=-0.254/F), Triglyceride (r=0.475/M, r=0.411/F), and Fasting blood sugar (r=0.158/M, r=0.234/F) in both men and women. The cut-off value of visceral fat thickness leading to the increased risk of metabolic syndrome was 4.58cm (sensitivity89.2%, specificity 71.2%) in men and 3.50cm (sensitivity61.2% specificity 80.8%) in women respectively. The odds ratio of the risk of metabolic syndrome was dramatically increased with the abdominal visceral fat thickness level over 6cm in men and 5cm in women. Conclusion : The visceral fat thickness using ultrasonography was significantly correlated with anthropometric indexes and risk factors of metabolic syndrome in both men and women. The cut-off value of visceral fat thickness leading to the increased risk of metabolic syndrome was 4.58cm in men and 3.50cm in women.

Can the Sasang Constitutional Type Trait Act as an Independent Risk Factor for Dyslipidemia? (사상체질이 이상지질혈증의 위험인자가 될 수 있는가?)

  • Objectives: This study was designed to investigate the correlation between Sasang constitution and dyslipidemia and the possibility that Sasang constitution can be a risk factor of dyslipidemia. Methods: This study was a retrospective cross-sectional study based on chart review. Seventeen thousand sixty-nine charts which were collected from May 2007 to June 2010 in International Healthcare Service Center, Kyung Hee University Medical Center, were reviewed for the study. Fifteen thousand two hundred fifty charts including Sasang constitutional diagnosis and lipid profiling were selected and we examined the correlation between Sasang constitution and dyslipidemia. Results: After adjusted for body mass index, the levels of total cholesterol and triglyceride were highest in Taeeum group, and lowest in Soeum group. High-density lipoprotein was highest in Soeum group, and lowest in Taeeum group. After adjusted for age, sex, and body mass index, the odds ratios for 'high triglyceride' were 1.716 (1.411~2.087) in Soyang group and 2.021 (1.650~2.475) in Taeeum group compared to Soeum group. The odds ratio for 'high low-density lipoprotein' was 1.229 (1.006~1.501) in Soyang group compared to Soeum group. The odds ratios for 'low high-density lipoprotein' were 1.195 (1.033~1.381) in Soyang group and 1.414 (1.212~1.649) in Taeeum group compared to Soeum group. Conclusions: The lipid profiles and dyslipidemic risk differed across constitutional types, showing constitution type-specific patterns, so that we could infer the possibility that Sasang constitutional types can be a risk factor of dyslipidemia.

Upregulation of Connexin43 Expression in Mitral Valves in a Rabbit Model of Hypercholesterolemia (고콜레스테롤혈증을 유발한 토끼의 승모판막에서 Connexin43 발현의 증가)

  • Background: Connexin 43-mediated gap junctional communication plays an important role in atherosclerosis. Numerous studies have demonstrated a correlation between mitral valve annular calcification and atherosclerotic disease. However, the relevance of connexin 43 to mitral valve disease remains unclear. We hypothesized that the mechanism contributing to mitral valve disease is associated with alterations in cell-to-cell communication mediated by changes in Connexin 43 expression. Material and Method: Twenty male New Zealand rabbits were divided into two groups: animals in group 1 (n=10) were fed a normal chow diet, whilst those in group 2 (n=10) received a diet containing 1% cholesterol for 12 weeks. After sacrificing the animals, the mitral valves were excised and analyzed with immunohistochemical staining and Real-time Reverse Transcriptase polymerase chain reaction (real time RT-PCR). Result: Myofibroblasts and macrophages were found concentrated within the endothelial layer on the ventricular side of the leaflet in the cholesterol diet group. Immunohistochemial staining showed elevated expression of connexin43 in the cholesterol diet group. Real-time RT-PCR revealed increased connexin43 mRNA levels in mitral valves from hypercholesterolemic animals. Conclusion: Our finding that connexin43 expression is increased in mitral valves of hypercholesterolemic rabbits suggests that alterations in cell-to-cell communication via connexin43 containing gap junctions play a role in the development of mitral valve disease in hypercholesterolemia.

A Study on the Analysis of Risk Factors and Correlations of Coronary Artery Disease of the Examinee taking Coronary Computed Tomography Angiography in a Comprehensive Health Improvement Center (종합검진 수검자의 관상동맥 전산화단층 혈관조영술 검사에서 관상동맥질환의 위험요인과 관련성 분석)

  • The purpose of this study is to evaluate the usefulness of coronary computed tomography angiography of coronary artery disease for the purpose fo health screening according to gender and age. In addition the association between hematological factors (Glucose, total cholesterol, visceral fat, body mass index, triglycerides, high density lipoprotein (HDL), low density lipoprotein (LDL)) and coronary artery disease is investigated. A retrospective analysis of coronary computed tomography angiography with 299 subjects showed that the detection rate of coronary artery disease was higher in men over 50 years of age and it was statistically significant. In particular, the prevalence rate of men (37.9%) was about 2 times higher than that of women (17.0%). Glucose and HDL as hematological factors associated with coronary artery disease were statistically significant. The prevention and management of coronary artery disease seems to require the control of glucose and high density lipoprotein (HDL). Although it is not statistically significant with other hematological factors, the need for management of coronary artery disease was identified. the coronary computed tomography angiography of coronary artery has higher radiation doses than other CT scans. Therefore, for the purpose of screening, coronary computed tomography angiography should be considered in consideration of the sex and age of the examinee, and detection of coronary artery disease through other non-invasive tests should be prioritized over coronary computed tomography angiography.

Effects of Line Dance Exercise on HOMA-IR and Energy Metabolic Factors in Postmenopausal Obese Women (라인댄스 운동이 폐경 후 비만여성의 인슐린저항성지수와 에너지대사조절인자에 미치는 영향)

  • The purpose of this study was to analyze the effects of line dance exercise on HOMA-IR and energy metabolic factors in postmenopausal obese women. The subjects were 24 obese women composed of the line dance group(n=12) trained for 12-week and the control group(n=12). The variables of body composition, HOMA-IR and energy metabolic factors were measured in all the subjects before and after the 12-week line dance exercise. The results of the study in the line dance group were as follows; The body weight, %fat, BMI, WHR and VFA had significantly decreased, but LBM had increased. The ghrelin, HDL-C had significantly increased, but leptin, insulin, glucose, HOMA-IR, TC, TG, LDL-C, SBP and DBP had decreased. And ghrelin, leptin, insulin, and glucose were the energy metabolic factors affecting the HOMA-IR in the line dance group. Therefore, regular and continuous line dance exercise will be effective for preventing the life-style diseases with improvement of the energy metabolic factors and insulin resistance by decreasing the body fat.

Correlation between Serum Osteocalcin and Hemoglobin A1c in Gwangju General Hospital Patients (광주 소재 한 종합병원을 방문한 성인에서 혈중 Osteocalcin과 HbA1c의 상관관계)

  • Recently, there have been many studies on the relationship between the bone metabolism and the mechanism of diabetes. In those studies, it was shown that osteocalcin can be involved in the treatment of type 2 diabetes. Therefore, this study examined the correlation between osteocalcin and HbA1c. From January 1, 2015 to December 31, 2016, a total of 714 adults aged 40 years or older ($70.8{\pm}10.4years$, 452 males), in whom both osteocalcin and HbA1c checked in a general hospital in Gwangju, were enrolled in the study. The serum calcium, total cholesterol, triglyceride, HDL-cholesterol, LDL-cholesterol, and 25-(OH) vitamin D were measured and basic information, such as the height and weight, were recorded. There was a weak negative correlation (r=-0.183, P<0.001) between the osteocalcin and HbA1c levels in diabetic patients but a negative correlation (r=-0.251, P<0.001) when adjusted by all other study variables. The present study showed that there was a negative correlation between the osteocalcin and HbA1c levels in diabetic patients. The relationship between the bone metabolism and the incidence of diabetes mellitus should be studied based on the influence of biological mechanisms and associated factors of bone and glucose metabolism.
