Increased cervical lordosis after deep acupuncture in patients with neck pain: nonrandomized clinical control trial (경추통 환자에게 있어서 내경(內徑)의 심자(深刺)방법이 경추 전만의 각도 변화에 미치는 영향)

  • 목적 : 경추통에 대한 침구치료의 효과여부가 현재까지 시행된 무작위 대조군 시험의 검토에서 논란이 되고 있는데, 경추통에 대한 치료 효과의 검증에 최근 들어 경추 전만각도의 변화를 측정하는 것이 점차 많이 사용되고 있는 실정이다. 하지만 아직까지 변형된 형태의 경추 견인 등 몇몇 요소들이 경추전만을 변화시켰다는 보고는 있었으나, 침치료 중 深刺의 방법이 경추의 만곡을 변화시켰다는 보고는 없었다. 방법 : 자침의 방법 중 深刻(五刺法 중 合谷刺 또는 關刺)방법이 경추 전만의 변화를 주는가에 대하여 검증하기 위하여 심자 치료를 시행한 19명의 실험군과 치료를 시행하지 않고 단순히 방사선 촬영만 시행한 21명의 대조군의 단순 방사선 사진을 후향적 연구의 방법으로 비교하였으며, 아울러 심자 자침군에서는 VAS를 측정하여 통증의 정도의 변화를 함께 연구하였다. 결과 : 연구결과 자침을 시행하지 않은 대조군에서는 VAS와 경추 전만의 Cobb각도에서 유의성 있는 변화가 나타나지 않았으나, 심자의 방법으로 자침한 군에서는 치료 전후의 VAS가 유의한 차이를 나타내었고, 경추의 전만 각도에서도 경추 2번과 7번 사이의 Cobb의 각도에서 유의성 있는 변화를 나타내었다(p < 0.05). 결론 : 이러한 연구 결과들은 심자의 자침방법이 경주의 통증을 줄여줄 뿐만 아니라 경추의 구조적 변화를 일으킬 수 있음을 나타내 주는 것이다.

The effect of cervical stabilized exercise and joint mobilization on maximum muscle strength and static muscle endurance of cervical region (경추안정화운동과 경추관절가동술이 경부의 최대근력과 정적근지구력에 미치는 영향)

  • The purpose of this study was to identify cervical stabilized exercise and joint mobilization, the difference between them, and the maximum muscle strength and static muscle endurance of each group after the enforcement to general people. Cervical joint mobilization group refers to interarticular exercise (traction, pressure, glide) using with Kaltenborn technique. 3 classes were divided into 20 people each, cervical joint mobilization and stabilized exercise, and the comparison groups were randomized for the study. Both cervical stabilized exercise and joint mobilization increased maximum muscle strength and static muscle endurance. Patients should be able improve muscle stabilization and deep cervical muscle by using joint mobilization when the patient is unable to exercise on their own.

  • 사회가 발달함에 따라 교통사고 등에 의한 외상 환자는 20 -30 년전에 비해 기하급수적으로 증가 하고 있으며 그러한 외상 중에 경추부 상해의 빈도는 매우 높다. 경추부 상해 원인을 규명하기 위한 임상학적 시도는 한계성을 지니고 있다. 본 논문에서는 실제 경추부 형상을 바탕으로 근육과 인대 등 을 비선형 스프링, 댐퍼 등으로 수치 모델링하여 후면 충돌시 경추부의 거동과 경추부에 미치는 상해를 구현하였다. 이를 이용하여 편타성 상해와 에어백으로 인한 경추부 골절의 생리학적 원인을 분석하고 자 한다.

Maturation of cervical vertebrae in relation to menarche (초경 전후 경추골 성숙도에 관한 연구)

  • Objective: The purpose of this study was to investigate the relationship between menarche and cervical vertebral maturation. Methods: Lateral cephalograms of 67 young korean girls within the range of 1 year before or after their menarche were gathered. The concavity of the cervical vertebrae base and the ratio of the base length to the 3rd and 4th cervical vertebrae anterior height were measured and analyzed. Results: The mean measured values were as follows, concavity of the 3rd cervical vertebrae base: 1.27(${\pm}0.18$) mm, concavity of the 4th cervical vertebrae base: 1.06(${\pm}0.15$) mm, ratio of the base length to the 3rd cervical vertebrae anterior height: 0.73(${\pm}0.06$) and ratio of the base length to the 4th cervical vertebrae anterior height: 0.70(${\pm}0.05$). There was a significant increase in the ratio of the base length to the 3rd vertebrae anterior height and the base concavity of the 3rd and 4th cervical vertebrae during the period of 1 year before to 1 year after their menarche. Conclusions: These characteristics of the 3rd and 4th cervical vertebrae on the lateral cephalogram can provide useful clues on evaluating the growth stage.

Measurement Method of Deep Cervical Flexors from Cervical Ultrasound Images (경추 초음파 영상에서의 심부 경부 굴곡근 두께 측정 방법)

  • 본 논문에서는 경추 초음파 DICOM 영상에서 심부 경부 굴곡근의 하단 후보 경계선을 이용하여 경추가 위치하는 후보 영역을 추출한다. 추출한 경추 후보 영역에 퍼지 시그마 이진화 기법을 적용하여 경추 객체를 추출한다. 심부 경부 굴곡근의 두께를 측정하기 위한 측정 기준점을 정확하게 추출하기 위해 심부 경부 굴곡근의 하단 경계선의 위치 정보를 이용하여 심부 경부 굴곡근내의 지방이 경추 객체로 추출되는 문제점을 개선하고, 개선된 영상에서 경추 객체들이 가진 픽셀 좌표를 이용하여 측정 기준점을 설정한다. 그 후, 객체들의 위치 정보를 이용하여 측정 기준점을 다시 설정한다. 심부 경부 굴곡근 영역과 측정 기준점을 이용하여 심부 경부 굴곡근의 두께를 측정한다. 제안된 방법을 경추 초음파 DICOM 영상에 적용하여 심부 경부 굴곡근의 두께를 측정한 결과, 기존의 심부 경부 굴곡근 두께 측정 방법보다 정확한 결과를 얻을 수 있는 것을 확인하였다.

  • Perineural cysts are usually discovered incidentally; only approximately one percent of such cysts are symptomatic. Symptomatic cervical perineural cysts, of which only five case reports have been found in the literature, are extremely rare and there is no consensus regarding treatment. When a cervical perineural cyst is discovered in a patient with cervical radiculopathy without the cause of other apparent compression, the cyst can be considered as a possible cause of cervical radiculopathy. In such cases, non-surgical conservative treatment can be considered before any surgical intervention. We reported the successful treatment of case of a 40-year-old woman with a symptomatic cervical perineural cyst by oral steroids and transforaminal epidural steroid injection.

  • 본 연구에서는 유한요소 해석 방법을 이용하여 경추 유한요소 모델을 개발하고, 척추 인공디스크 수술 후 경추 분절의 생체역학적 특성을 평가하였다. 반구속 및 비구속 개념의 인공디스크가 삽입된 경추 유한요소 모델의 제 7 경추체를 완전히 고정시키고 추적 경로 방향의 50 N의 압축력을 가한 상태에서 1 Nm의 굴곡과 신전, 측면 굴곡, 비틀림 모멘트를 가하였다. 모든 방향의 하중 조건에서 인공디스크가 삽입된 경추 모델들의 회전량이 정상 경추 모델의 회전량에 비하여 크게 나타났다. 또한 인공디스크를 삽입한 운동 분절에서 정상에 비하여 후관절의 접촉력과 여섯 가지 주요 인대에 걸리는 응력이 높게 나타났다. 본 연구의 결과는 척추 인공디스크 수술 시 수술 방법의 선택뿐만 아니라 새로운 경추 수술용 임플란트 개발을 위한 인체정보 콘텐츠를 구축하는데 유용하게 활용될 수 있을 것으로 기대된다.

Visualization of the Origin of the Vertebral Arteries with Color Doppler Sonography (색도플러 초음파검사에 의한 경추골동맥 기시부 관찰)

  • Background/aim : Atherosclerotic disease at the origin of the vertebral arteries is one of the risk factors for vertebrobasilar ischemic disease. Assessment and visualization of the origin of the vertebral arteries with color doppler sonography is a non-trivial task. The aim of this study is to increase the visualization rate of the origin of the vertebral arteries with color doppler sonography. Materials and Methods : Color doppler sonography for the vertebral arteries included carotid arteries was performed to 198 patients. We first examined the vertebral artery in the upper neck in the direction of the subclavian artery to distinguish its origin more easily. If the vertebral artery origin was not visualized in natural position, the examiner pushed the transducer toward a clavicle or pushed the shoulder of patient by the other hand. The technical methods for visualization of the vertebral artery origin were classified into three grades: natural position, pushing the transducer, and pushing the shoulder of patient according to the depth (3.0 cm and shallower, deeper than 3.0 cm) of the origin. Results : The origin of the vertebral arteries could be visualized in 97% on the right and in 92% on the left. The origin of the vertebral arteries could be visualized in 98.6%, 1.4%, and 0.0% in natural position, pushing the transducer, and pushing the shoulder of patient, respectively, at shallower than 3.0 cm on the right side. The origin of the vertebral arteries could be visualized in 81.2%, 14.6%, and 4.2% in natural position, pushing the transducer, and pushing the shoulder of patient, respectively, at deeper than 3.0 cm on the right side. The origin of the vertebral arteries could be visualized in 85.4%, 10.7%, and 3.9% in natural position, pushing the transducer, and pushing the shoulder of patient, respectively, at shallower than 3.0 cm on the left side. The origin of the vertebral arteries could be visualized in 55.7%, 30.4%, and 13.9% in natural position, pushing the transducer, and pushing the shoulder of patient, respectively, at deeper than 3.0 cm on the left side. Conclusion : If the examiner pushes the transducer toward a clavicle or pushes the shoulder of patient by the other hand, when the vertebral artery origin during the color doppler sonography is not visualized in natural position, visualization rate of the origin of the both vertebral arteries is increased.

  • In this study, a multibody dynamic model of the cervical spine was developed for a virtual in-vitro cadaveric experiment. The dynamic cervical spine model was reconstructed based on Korean CT images and the material properties of joints and soft tissue obtained from in-vitro experimental literature. The model was validated by comparing the inter-segmental rotation, multi-segmental rotations, load-displacement behavior, ligament force, and facet contact force with the published in-vitro experimental data. The results from the model were similar to published experimental data. The developed dynamic model of the cervical spine can be useful for injury analysis to predict the loads and deformations of the individual soft-tissue elements as well as for virtual in-vitro cadaveric experiments.

  • Whiplash injuries often occur in motor vehicle collision accidents. This injury frequently occurs in the cervical region. However, the reason for this has not yet been clarified. In this study, a multi-body neck model with muscles was designed. Some muscles in the model were preloaded; these were previously determined using the concept of the follower load. Cervical spinal vertebrae, discs, and muscles were designed in accordance with the human cervical spine. The purpose of this study was to investigate the effect of preloads on muscles. The results imply that the whiplash model with preloaded muscles simulates practical situations more closely than models without preloads.
