A Subcutaneous Lipoma in a Male Red Fox (여우에서 피하지방종의 진단)

  • An 8-year-old male red fox (Vulpes vulpes) in Species Restoration Technology Institute of Korea National Park Service (KNPS), revealed nodular growths in its ventro-cervical region. The fox was introduced from Young-Yang Gun in 2012 to KNPS for re-introduction of the red fox. It has been cared in captive facility and showed the mass in August 2013 that was sent to Wildlife Medical Center. For the diagnosis of underlying disease and cervical mass, radiographical and sonographical examinations, complete blood count, serum chemistry analysis, peripheral blood smear examination and surgical removal of the mass were performed. The mass was fixed in 10% neutral buffered formalin and processed routinely for haematoxylin and eosin (HE) stain. Based on hematological and serum chemical examination, the fox showed mild leukocytosis, thrombopenia, increase of creatine kinase MB (CKMB) and uric acid. However, it was considered as no clinical relevance since the fox showed no related clinical signs. Macroscopically, the mass was round shape, whitish and well-demarcated. Microscopically, it was diagnosed as a lipoma consisting of mature adipose tissue. Lipoma is a common benign tumor in most domestic animals, however it has never been reported in the red fox. The present case report provides comprehensive diagnosis of a subcutaneous lipoma in a red fox.

  • A Common peroneal nerve palsy caused by ganglionic cyst is very rare condition but well recognised entities. There have been three previous reports describing the magnetic resonance image (MRI) findings of peroneal nerve entrapment due to a ganglionic cyst. Ultrasonography, MRI, and electromyography (EMG), nerve conduction velocity (NCV), and microscopic examination were taken for diagnosis. A tubular structure near the fibular neck extending longitudinally over several slices with an inferior extension towards the superior tibiofibular joint with high T2 signal intensity was characteristic. The peroneal nerve was exposed and the ganglionic cyst was excised. The nerve was paralysed immediately after operation, but at 4 month after operation, started recovery of the function gradually and has recovered completely at 7 month. MRI is helpful to detect the extent, location, and origin of the cyst. Meticulous surgical excision can provide favorable result.

  • Infective endocarditis (IE) caused by methicillin-resistant Staphylococcus aureus (MRSA) has become a worldwide concern. We present a case of a 12-year-old child with IE of the native mitral valve due to MRSA infection after an invasive dental procedure. Based on the clinical symptoms and the presence of cerebrospinal fluid pleocytosis, the patient was initially diagnosed with presumed bacterial meningitis and treated with empiric antibiotics. On the third day of hospitalization, MRSA was cultured from the initial blood samples and vegetation was observed on the mitral valve during an echocardiogram, findings which are compatible with a diagnosis of IE. The revised guidelines for antibiotic prophylaxis for the prevention of IE advise that IE prophylaxis for dental procedures is reasonable only for patients with underlying cardiac conditions, who are at the highest risk of adverse outcomes from IE. However, in this case, the patient had no high risk factors indicative of IE prophylaxis, except for mitral valve prolapse. She had no recurrence of IE over a follow-up period of 12 months.

  • Objectives The high sensitivity of ultrasound and thyroglobulin determination for follow-up of differentiated thyroid cancer allows early detection of nonpalpable recurrences. Intraoperative localization of these small foci in previously dissected necks is a surgical challenge. We assessed the safety and effectiveness of ultrasound-guided tattooing (US-tattoo) with a charcoal suspension for localizing nonpalpable cervical recurrences after thyroidectomy for thyroid cancer. Subjects and Methods Between March 2009 and December 2010, we retrospectively reviewed 19 patients who underwent US-tattoo with injection of a charcoal suspension for recurrent thyroid papillary cancer on central neck compartment after thyroidectomy. All patients underwent the surgical dissection after US-tattoo. The complications and effectiveness of US-tattoo were evaluated. Results The technical success rate of US-tattoo for suspicious lesions was 100%. There was no complication with regard to US-tattoo. During surgery, all but one tattooed lesions were detected by surgeons. On final pathologic reports, all recurrence lesions but two cases were successfully removed. Conclusion Preoperative US-tattoo is a safe and effective method for successful reoperation of central neck compartment recurrences after thyroidectomy.

Medullary Thyroid Carcinoma in a Maltese Dog (말티즈 개에서 발생한 갑상선수질암)

  • An 11-year-old, 2.67 kg female Maltese dog with 3 weeks history of palpable cervical mass near trachea was submitted to a local animal hospital. Radiography and ultrasonography showed radiopaque mass adjacent trachea and vagus nerve. Surgically excised mass was solitary and approximately $3.5{\times}2{\times}0.8cm$ in size. Histopathologically, there were large neoplastic foci admixed with normal thyroid tissues. These neoplastic foci were composed of small to large packets of the neoplastic cells with plasmacytic morphology, and these packets were divided by fine fibrovascular septa. Immunohistochemically, most neoplastic cells in the thyroid mass showed positive reactions for cytokeratin (AE1/AE3), chromogranin A, neuron specific enolase (NSE) and the negative reaction for vimentin. Based on the gross, histopathologic and immunohistochemical characteristics, this dog was diagnosed as medullary thyroid carcinoma.

Uterine Torsion Associated with Hydrometra and Pyometra in a Domestic Rabbit (애완 토끼에서 자궁수종, 자궁축농증, 자궁염전의 병발 증례)

  • A 1.5-year-old, female domestic rabbit was diagnosed with uterine torsion accompanying hydrometra of the right uterine horn and pyometra of the left horn. The exploratory laparotomy revealed the torsion of bilateral uterine horns in the region of the uterine cervix. No growth was detected in fluid collected from the right horn of the uterus; however, Gram-positive micrococcus and bacilli were cultured from the left horn. The pathophysiology of the simultaneous occurrence of these disorders was suspected considering that abdominal distension progressed over 4 months and the rabbit's condition deteriorated abruptly within 2 days, hydrometra was the initial disease and then infection to the left uterine horn and torsion developed later due to the large volume of fluid within the uterus. This report is the first case showing uterine torsion accompanying hydrometra and pyometra in each uterine horn.

Effect of Dentin Desensitizer on the Hypersensitivity of Teeth with Non-carious Cervical Lesions Subsequent to Ultrasonic Scaling (지각과민처치제가 초음파 스케일링 처리한 비우식성 치경부 병소가 있는 치아의 지각과민증에 주는 영향)

  • The aim of this study was to evaluate the changes in dentinal permeability after application of dentin desensitizer on exposed dentin immediately after ultrasonic scaling to teeth with non-carious cervical lesions. Thirty caries-free extracted molars were fixed to slide glasses after horizontally being sectioned at 5 mm below the cemento- enamel junction (CEJ). The prepared specimen was connected to a fluid flow measuring device (nano-Flow), and a V-shaped cavity was formed at the CEJ to imitate the non-carious cervical lesion. After no fluid leakage was confirmed in the connected system with specimen, tooth surface was treated ultrasonic cleaning with piezoelectric ultrasonic scalers until dentinal tubules were exposed. And 6 different desensitizers were applied on exposed dentin. Real-time measurements of dentinal fluid flow were performed during ultrasonic scaling and application of dentin desensitizer. To evaluate the occlusion of exposed dentinal tubules, tooth surface was examined by SEM. Following results were observed. After ultrasonic scaling, more dentinal tubules were exposed on the tooth with non-carious cervical lesions compared to tooth without lesions. The rate of fluid flow measured with nano-Flow system had correlation with the degree of dentin occlusion observed with SEM after application of desensitizers on exposed dentin. Desensitizers with glutaraldehyde and HEMA did not decrease the rate of fluid flow and did not show dentin occlusion. Desensitizers with oxalate showed the limited effects on the rate of fluid flow and dentinal tubule occlusion. Desensitizer with resin monomer showed the significant effect on the rate of fluid flow and dentin occlusion.

  • The uterus can be largely divided into the uterine corpus and uterine cervix. Diseases that can occur in the uterine corpus, composed of the endometrium and myometrium, vary from benign to malignant tumors. Ultrasound and CT are the primary non-invasive evaluation methods to differentiate between benign and malignant tumors, but in some cases, they are difficult to differentiate due to their non-specific imaging findings. However, magnetic resonance imaging (MRI), which has high resolution, helps not only in locating lesions but also in evaluating histological characteristics and staging of malignant tumors. In this review article, the characteristic MRI findings that radiologists should be aware of regarding various benign and malignant tumors detected in the uterine corpus are summarized with their points of differentiation.

Optimal Imaging Time for Diagnostic I-123 Whole Body Scan in the Follow-up of Patients with Differentiated Thyroid Cancer: Comparison between 6- and 24-Hour Images of the Same Subjects (분화 갑상선 암의 추적 관찰에서 진단적 I-123 전신 스캔의 최적 영상 시점: 동일 환자에서 6시간과 24시간 영상의 비교)

  • Purpose: To determine optimal imaging time for diagnostic I-123 whole body scan in the follow-up of patients with differentiated thyroid cancer(DTC), we compared the image quality of 6- and 24-hour images of the same subjects. Materials and Methods: Four hundred ninety-eight patients(M:F = 55:443, Age $47.6{\pm}12.9$ years) with DTC who had undergone total thyroidectomy and I-131 ablation therapy underwent diagnostic whole body scanning 6 hour and 24 hour after oral ingestion of 185 MBq(5 mCi) of I-123. Serum thyroglobulin measurement and ultrasonography of the neck were performed at the time of imaging. In 40 patients underwent additional I-131 therapy, post-therapy I-131 images were obtained and compared with diagnostic I-123 images. Results: In 440 patients(88.4%), 6- and 24-hour diagnostic I-123 images were concordant, and 58 patients(11.6%) showed discordant findings. Among 58 discordant patients, 31 patients showed abnormal tracer uptake on only 6-hour image, which turned out false-positive findings in all cases. In 12 patients with positive findings on only 24-hour image, remnant thyroid tissue(4 patients) and cervical lymph node metastasis(3 patients) were presented. Among 40 patients underwent additional I-131 therapy, 6-hour and 24-hour images were discordant in 13 patients. All 5 patients with abnormal uptake on only 6-hour image revealed false-positive results, whereas most of 24-hour images were concordant with post-therapy I-131 images. Conclusion: I-123 imaging at 24-hour could reduce false-positive findings and improve diagnostic accuracy, compared with 6-hour image in the follow-up of patient with DTC.

Radiomics Analysis of Gray-Scale Ultrasonographic Images of Papillary Thyroid Carcinoma > 1 cm: Potential Biomarker for the Prediction of Lymph Node Metastasis (Radiomics를 이용한 1 cm 이상의 갑상선 유두암의 초음파 영상 분석: 림프절 전이 예측을 위한 잠재적인 바이오마커)

  • Purpose This study aimed to investigate radiomics analysis of ultrasonographic images to develop a potential biomarker for predicting lymph node metastasis in papillary thyroid carcinoma (PTC) patients. Materials and Methods This study included 431 PTC patients from August 2013 to May 2014 and classified them into the training and validation sets. A total of 730 radiomics features, including texture matrices of gray-level co-occurrence matrix and gray-level run-length matrix and single-level discrete two-dimensional wavelet transform and other functions, were obtained. The least absolute shrinkage and selection operator method was used for selecting the most predictive features in the training data set. Results Lymph node metastasis was associated with the radiomics score (p < 0.001). It was also associated with other clinical variables such as young age (p = 0.007) and large tumor size (p = 0.007). The area under the receiver operating characteristic curve was 0.687 (95% confidence interval: 0.616-0.759) for the training set and 0.650 (95% confidence interval: 0.575-0.726) for the validation set. Conclusion This study showed the potential of ultrasonography-based radiomics to predict cervical lymph node metastasis in patients with PTC; thus, ultrasonography-based radiomics can act as a biomarker for PTC.
