  • In General, popliteal cyst found in children does not present a symptom and also it rarely causes a complication. The researcher observed an entrapment of tibial nerve and popliteal artery in a child with popliteal cyst without intra-articular lesion, and treated it by using early decompression and prolotherapy, which is presented in this study with literature review.

  • 본 논문에서는 동물의 십자인대 질환의 진단지표인 경골고원각도(TPA)를 자동으로 측정하는 딥러닝 소프트웨어 기법을 제안한다. 동물 X-ray 영상에서 나타나는 피사체의 위치와 형태에 대한 다양한 변이는 TPA(Tibial Plateau Angle) 지표 산출에 필요한 특징점 검출과정에서 학습 효율을 현저하게 저하시킨다. 이에 본 연구에서는 YOLO(You Only Look Once) 기반 모델을 사용하여 일차적으로 경골영역의 분할 단계를 수행하고, 이어서 경골 상단부의 과간융기와 복사뼈의 중심점을 찾는 과정을 Resnet 기반의 특징점 추출 모듈로서 구현함으로써 학습의 효율과 지표 검출의 정확도를 향상시켰다. 총 201 개의 실제 X-ray 영상을 사용하여 학습 속도와 영역 분할 및 특징점 추출의 정확도 측면을 고려함으로 제안된 이론의 타당성을 실험적으로 평가하였다.

  • The development of different entities of soft tissue sarcoma in one patient is rare. It usually affects head and neck or abdominal region, whereas those affecting the extremities are much rarer. We describe a patient with double primary presentation of liposarcoma and Ewing's sarcoma in extremity. This case implies that sarcoma patients are at increased risk of a second malignancy, and this implies a need to search for occult tumors during follow up.

  • The leading surgical method for correcting the misalignment of the varus and valgus in the knee joint is the high tibial osteotomy (HTO). In the opening wedge HTO (OWHTO), there is no concern about damaging the peroneal nerve on the lateral tibia of the proximal fibula. OWHTO has been the preferred choice, as the opening of the correction angle can be modulated during the operation. The correction of the varus and valgus on the coronal plane are performed adroitly. Nevertheless, there have been numerous reports of unintended changes in the medial tibial plateau and posterior slope angle (PSA). The authors have developed an HTO method using computer-assisted surgery with the aim of addressing the abovementioned problems from an engineer's perspective. CT images of the high tibia were reconstructed three-dimensionally, and a virtual osteotomy was performed on a computer. In addition, this study recommends a surgical method that does not cause changes in the PSA after OWHTO. The results of the study are expected to suggest a clear relationship between the anteromedial cortex oblique angle of each patient and the PSA, and an optimal PSA selection method for individuals.

  • Brodie abscess is a localized form of subacute or chronic osteomyelitis which is common in children but may also occur in adulthood. When Brodie abscess is located in the posterior metaphysis of the proximal tibia, open biopsy and curettage have a difficulty in approach to the lesion and can cause neurovascular injury or soft tissue contamination. We report a case wherein a novel surgical technique was used to treat a Brodie abscess in the posterior proximal tibial metaphysis in 48 year-old-male with endoscopic-assisted curettage by commercial anterior cruciate ligament targeting device(Rigid Fix; Mitek, Johnson & Johnson, Norwood, MA). Two portals were created toward the abscess site and, through each portal interchangeably, the granulation tissue and sclerotic bone could be excised. We believe that endoscopic-assisted curettage presents safe technique, decreased morbidity, accurate assessment of the extent of the abscess and possible improvement in long-term outcomes.

  • Ganglion cysts that occurred within sheaths of peripheral nerves have been documented in literatures, but it is relatively rare condition. The peroneal nerve is the most common site of involvement. Other reported sites of involvement are the radial, ulnar, median, sciatic, tibial and posterior interosseous nerves. We report a case of the intraneual ganglion cyst within peroneal nerve and another case of the intraneural ganglion cyst within a medial plantar nerve that were successfully excised without neurologic complication.

  • Objectives The purpose of this study was to report the effect of Korean medicine treatment, including the application of joint mobilizing chuna, by reporting two cases after tibial plateau fracture surgery. Methods Two patients with tibial plateau fractures were treated using joint mobilizing chuna, myofascial chuna, acupuncture, and herbal medication. The effect of the treatments was evaluated using the range of motion, manual muscle test, numeric rating scale, and Korean Knee Injury and Osteoarthritis Outcome Score. Results In both cases, the range of motion, muscle strength, and pain were significantly improved. In particular, the range of motion for knee joint flexion increased by 47° in case 1 and 30° (right) and 42° (left) in case 2. Conclusions Korean medicine treatment, especially joint mobilizing chuna, may be an effective intervention for rehabilitation after tibial plateau fracture surgery.

  • It is a well-accepted tenet that wide surgical margins are necessary for the treatment of soft-tissue sarcomas, and it is still true that the rate of recurrence depends on the adequacy of the surgical margins. Sarcomas that rest directly against bone pose a dilemma for the surgeon. A wide margin is not possible in the literal sense without excision of the bone. Whereas reconstruction of skeletal defects is possible, it adds to the complexity of the surgery and increases potential complications. We report the experience of treatment in a case of synovial sarcoma which located at popliteal fossa adjacent to proximal tibia.

Comparison of the Effects of an Adductor Canal Block and Periarticular Multimodal Drug Local Injection on Pain after a Medial Opening High Tibial Osteotomy (내측 개방 근위 경골 절골술 후 통증 조절에서 관절 주위 다중 약물 국소 주사와 내전근관 차단술의 효과 비교)

  • Purpose: The efficacy of periarticular multimodal drug injection and adductor canal block after a medial opening-wedge high tibial osteotomy was compared in terms of the postoperative pain level. Materials and Methods: From November 2016 to March 2017, 60 patients underwent a medial opening-wedge high tibial osteotomy under spinal anesthesia. Preemptive analgesic medication, intravenous patient controlled anesthesia were used for pain control in all patients. Thirty patients received a periarticular multimodal drug injection (group I), and 30 patients received an adductor canal block (group II). These two groups were compared regarding the postoperative pain level, frequency of additional tramadol injections, total amount of patient-controlled analgesia, and number of times that the patients pushed the patient-controlled analgesia button at each time interval. Results: The visual analogue scale scores over the two-week postoperative period showed no statistical significance. The frequency of additional tramadol hydrochloride injections was similar in the two groups over time. The mean number of times that patients pushed the patient-controlled analgesia button was similar in two groups over time. The total amount of patient-controlled analgesia was similar in the two groups over time. Conclusion: This study shows that intraoperative periarticular multimodal drug injections and adductor canal block may have a similar effect on postoperative pain control in patients who have undergone a medial opening-wedge high tibial osteotomy for unicompartmental osteoarthritis of the knee.
