오랜 기간 치료에도 불구하고 치료에 반응하지 않았던 결절성 양진 환자에게 장내 세균 검사에 따른 Probiotics 요법과 한의학적 치료를 통해 양호한 치료 효과를 거두었다. 하지만 치료사례가 1례여서 장내 세균의 불균형과 결절성양진의 연관성을 입증하기에는 역부족이고 한의학적인 치료가 양진을 개선하는 구체적인 기전을 밝히기에는 한계가 있다. 향후 난치성 질환인 결절성 양진에 대한 다양한 치료 연구가 필요하리라 생각된다.

저자들은 오랜 기간 경구 스테로이드제 및 항히스타민제와 국소 스테로이드제 사용에도 불구하고 호전이 되지 않은 극심한 소양감을 동반한 결절성 양진 환아에게 한의학적 변증치료와 식이요법을 통하여 유의한 치료효과를 보았다. 하지만 병행한 식이요법이 치료호전에 어떤 영향을 주었는지 확실히 밝히기 어렵고 결정성 양진의 성인형 및 다른 변증치료에 대한 임상례가 없어 향 후 이에 대한 더 많은 연구가 필요하다고 생각된다.

Objective : The purpose of this study is to report about a mid-40's female patient with Prurigo Nodularis, whose symptoms were relieved after Korean medicine treatment alone with Whidam's Su-Gi Therapy. Methods : the patient in this study received only Korean medicine treatment such as herbal medication, acupuncture treatment, cupping therapy and specially Whidam's Su-Gi therapy. Results : After the treatment, the symptoms of Prurigo Nodularis were relieved. Contact Itching Index fell from 10 to 5, and Non-Contact Itching Index fell from 10 to 4. On the whole the Body Heat has risen and skin condition has improved visually. The significance of Meridian Function Check(based on HRV) and skin syndrome is remarkable., Conclusions : This study showed that Whidam's Su-Gi therapy added with Korean Medicine treatment can be an effective choice for Prurigo Nodularis. So, wasting disease, especially immune-reduced syndrome such as Prurigo Nodularis, may also be treated with Korean Medicine alone without side effects.

본 연구는 장기간 스테로이드 주사 및 연고, 항히스타민제 복용 등에도 악화된 중증 각화성 결절 양진 환자 치험례이다. 환자에게 한약 투약, 침, 증기, 습부항, 약침, 한방 외용제 치료를 시행한 결과 결절 과각화 감소 및 소양감, 열감, 통증, 수면 장애 등 제반 증상에 치료 효과가 있음을 확인할 수 있었다.

Objectives : This study was carried out to report the effect of Korean medicine treatment on prurigo nodularis. Methods : We treated a 26-year-old female prurigo nodularis patient. The patient received Korean medicine treatments including acupuncture, herbal-medicine, pharmacopuncture at Daegu Haany University Korean medical hospital. We evaluated the degree of improvement in prurigo nodularis by using the Visual Analogue Scale(VAS) and counting nodules. Results : After treatments, the results of this patient showed improvements on pruritus, nodules. Nodules are removed, and pruritus decreased from VAS 10 to VAS 1. Conclusions : This study suggests that Korean medicine treatment may be effective on prurigo nodularis.

Objectives: This study is to report a case of prurigo nodularis with skin rash that occurred after COVID-19 vaccination improved by Korean medicine treatment. Methods: The patient visited our hospital complaining of prurigo nodularis and skin rashes which occurred in various parts of the body (Total score 17). The patient complained of sleep disturbance (NRS 9) due to nighttime pruritus (NRS 7). Based on the patient's symptoms, herbal medicine Soshiho-tang and acupuncture on the left Triple Energizer Hangyeok was prescribed. Results: Most of the skin symptoms improved significantly leaving only scars, minor scratches and pigmentation (Total score 17 → 6). The patient's sleep condition improved (NRS 7 → 1) as the pruritus decreased from NRS 9 to 2. Conclusions: This study is significant in that the patient with prurigo nodularis accompanied by skin rash effectively improved by the Korean Medicine treatment.

연구배경: 폐 랑게르한스세포 조직구증(pulmonary Lan gerhans cell histiocytosis, PLCH)은 랑게르한스세포의 증식으로 형성된 육아종이 여러 장기에 침범하여 발생하는 질환이다. 주로 폐에 침범하며, 무증상에서부터 호흡곤란등의 다양한 임상증상을 가진다. 국외뿐만 아니고 국내에서도 빈도가 적은 것으로 알려져 있어, 다기관에서 국내 증례를 모아 그 임상양상을 알아보고자 하였다. 방 법: 부위에 관계없이 조직학적으로 진단되고 폐의 병변이 있는 PLCH 환자의 임상증상을 ILD 연구회를 통하여 후향적으로 조사하였다. 결 과: 수집된 증례는 총 56명으로 남자 48명, 여자 8명이었고, 중간나이는 43세(18-67세)였다. 진단 당시 흡연자와 과거 흡연자는 79%로 흡연과 관계가 있었다. 가장 흔한 증상은 기침(39%)이었고, 이어 호흡곤란(38%), 객담(20%), 흉통(20%) 순이었다. 기흉은 16명의 환자에서 관찰되었다. 폐기능 검사에서 정상은 26명(61%), 제한성 7명(16%), 혼합형 7명(16%), 폐쇄성은 3명(7%)에서 관찰 되었다. 방사선 소견은 대부분의 환자에서 병변이 중엽과 상엽에 있었으며, 결절과 낭종이 같이 있는 경우가 59%로 가장 많았다. 결 론: 이 연구는 PLCH 환자에 대한 국가적 그리고 장기적 추적 연구의 필요성을 시사한다.
