The Convergence Effect of Histamine and Atropine on Intestinal Contractility (위장관 수축성에 대한 Histamine과 Atropine의 융합성 조절 효과)

  • The aim of the study was to observe the influence and related mechanism of histamine and its analogues used for hypersensitivity tests and used as an indicator of impurities in drugs on the tissue-specific intestinal contraction. Intestinal contraction includes the activation of thick or thin filament regulation. However, there are few reports addressing the question whether this regulation is involved in histamine-induced regulation. We hypothesized that histamine plays a role in tissue-dependent regulation of intestinal contractility. Denuded ileal/colonic longitudinal and circular muscles of male rats were used and isometric contractions were recorded using a data acquisition system. Interestingly, histamine alone didn't increase the contraction of the circular muscle but increased the contraction of the longitudinal muscle. Histamine together with atropine (M3 receptor antagonist) didn't inhibit the contraction of the longitudinal and circular muscle. Therefore, histamine alone and together with atropine increases the ileal longitudinal muscle contraction suggesting that additional mechanisms (decreased receptor density, postreceptor signaling or distribution of agonists) might be involved in the regulation of ileal muscle contractility. In conclusion, histamine and/or atropine has some effect on the regulation of the longitudinal contractility regardless of M3 receptor and the simpler test would be preferred as the drug impurity test compared to more complicated tests.

Hospice and Palliative Care for the Terminal Patients with Colorectal Cancer (말기 대장직장암 환자의 호스피스 완화의료)

  • Purpose: Colorectal ranter is the 4th leading cause of cancer death in Korea and the prevalence is increasing continuously. This study was aimed to figure out the problems through the clinical consideration about terminal colorectal ranter patients who had died in hospice unit. Methods: We retrospectively reviewed the medical records in 78 patients with colorectal ranter who had admitted, received palliative care, and died in a hospice unit between April 2003 and November 2006. Results: The median age of patients was 59.6 years with 45 men (58%) and 24 women (42%). The median survival in hospice and palliative care was 36 days. The median hospitalization was 22 days. The most prevalent reason for admission was pain (38 patients, 49%), and the most common symptom was also pain (70 patients, 90%). Forty eight patients (62%) took analgesics before hospice referral. Twenty seven patients (65%) of 45 patients with intestinal obstruction have been performed palliative procedures. Median survival of patients with palliative procedure was higher than that of no palliative procedure group (47 days vs 19 days, P-value=0.005). Conclusion: The duration of hospice and palliative care was not enough to care the terminal colorectal cancer. Therefore, we suggest that proper education and information should be provided to physician, patients and their family members for effective hospice and palliative care.

The Role of Colonoscopy in Children with Hematochezia (소아 선혈변에서 대장 내시경 검사의 역할)

  • Purpose: This study was performed to evaluate the role of colonoscopy in children with hematochezia. Methods: We retrospectively reviewed the medical records of 277 children who underwent colonoscopy because of hematochezia between January, 2003 and July, 2010. Results: The mean age of the patients was $6.0{\pm}4.4$ (7 days~17.8 years) years. The male to female ratio was 2.2:1. The duration between the 1st episode of hematochezia and colonoscopy was $4.9{\pm}12.1$ months. Characteristics of hematochezia included red stool (65.1%), blood on wipe (12.8%), bloody toilet (11.9%), and blood dripping (10.2%). The most proximal region of colonoscopic approach was terminal ileum (84.5%), cecum (9.5%), hepatic flexure (2.8%), and splenic flexure (3.2%). Eighty five patients (30.6%) had no specific abnormal findings. Major causes of hematochezia were polyp (26.4%), food protein induced proctocolitis (6.9%), infectious colitis (5.4%), lymphofolliculitis (5.7%), non specific colitis (5.7%), and vascular ectasia (5.1%). The hemorrhagic sites included the rectum (24.0%), rectosigmoid junction (18.1%), sigmoid colon (13.5%), ascending colon (14.2%), transverse colon (11.3%), descending colon (7.8%), cecum (8.1%), and terminal ileum (3.1%). The recurrence rate of hematochezia after colonoscopy was 19.1%. Colonoscopy was performed in 262 patients (94.6%) with conscious sedation. Endoscopic hemostasis was performed in 5 patients. Complications of colonoscopy or sedation were not found. Conclusion: The causes and lesional localization of pediatric hematochezia were diverse. Colonoscopy has an important role in the diagnosis and treatment of hematochezia in children. Total colonoscopy is recommended to detect the cause of hematochezia.

Protective Effect of Kefir Grain Against Dextran Sodium Sulfate-Induced Colitis in Rats (Dextran Sodium Sulfate로 대장염을 유도한 흰쥐에서 캐피어 원말의 장보호 효과)

  • Probiotics have emerged as a potential treatment modality for numerous gastrointestinal disorders, including IBD. However, few probiotics have undergone appropriate preclinical screening in vivo. Kefir is considered a probiotic, benefiting the host through its effects in the intestinal tract. Despite numerous studies examining the action of probiotics on the host organism, few have analyzed the effects on intestinal environment. We assessed the protective effect of kefir for three weeks before inducing colitis with 2% dextran sodium sulfate for five days. The DSS loads were similar in all DSS treatment group. The results of the experiment are as follows. Food intake and FER of experimental groups were not significantly different each other, but water consumption tended to be higher in all DSS treatment groups as compared with the normal control. And visual inspection of feces revealed mild diarrhea in rat given 2% DSS. The anti-inflammatory activity of kefir was determined by myeloperoxidase activity during the DSS treatment, and there was no significant difference in any group. The levels of thiobarbituric acid reactive substances (TBARS) as a colonic lipid peroxidation were significantly lower in the kefir intake groups than in rats treated with 2% DSS alone. The DNA % in tail and tail moment values as a DNA damage level of the blood lymphocytes in kefir intake groups tended to be lower than 2% DSS treatment alone, especially tail lengths were significantly diminished. According to the colonic histopathological assay, there were a severe inflammation of lamina propria and submucosa and mild edema in mucosa and sub mucosa in DSS alone treated group. We found a slight regenerative change in kefir treatment groups. In our experiments, this means that ulcerative colitis related to oxidative injury might be prevented by kefir as a probiotic. Further studies of the potential benefits of kefir as a probiotic in inflammatory condition are encouraged.

The Significance of Small Polyp of Colon in Koreans (한국인에 있어서 대장의 작은 용종의 의의)

  • Background: There arc two theories in the development of colon cancer. One is the adenoma-carcinoma sequence theory and the other is the de novo cancer theory. Western countries believe in the adenoma-carcinoma sequence theory, however there are many recent reports from, Japan about cancers developing from small adenomas. Methods: The present study analyzed 408 polyps from 508 cases that were taken by colonoscopic polypectomy at the Department of internal Medicine, Yeungnam University Hospital. Results: The percentage of patients who had a polyp was 41.3% (210 cases out of 526 cases), and the peak incidence was noted in patients in their 50s and 60s, There was no difference between the sexes, but we noted significant increase in the incidence of polyps in patients over the age of thirty. We found 395 polyps below 1cm and 13 polyps above 1cm. Among 408 polyps, 5 cases were cancerous polyps and 3 cases showed polyp size of less than 1cm each. The first case was a polyp of 0.4cm in size with elevated mucosa at the ascending colon. The second was 0.5cm in size with round elevation and hyperemic mucosa in the rectum. The third was 0.6cm in size with tubular elevation at the hepatic flexure. Conclusions: Colon polyp is a common disease in Koreans, Even small polyps can have cancer tissue, which should be removed if discovered during colonoscopy. We believe that not all colon cancer originates in the manner described by the adenoma-carcinoma sequence theory. However further studies with a larger sample population arc needed to determine the exact role of colon polyps in the development of colon cancer.

Study of Radiation dose Evaluation using Monte Carlo Simulation while Treating Extrahepatic Bile Duct Cancer with High Dose Rate Intraluminal Brachytherapy (간외 담도암 고선량률 관내근접방사선치료 시 몬테카를로 시뮬레이션을 통한 주변장기의 선량평가 연구)

  • The relative dose calculated by MCNPX and the relative dose measured by ionization chamber and solid phantoms evaluated the accuracy comparing with Monte Carlo simulation. In order to apply Monte Carlo simulation the intraluminal brachytherapy of extrahepatic bile duct cancer, 192Ir sealed radioactive source replicate, Bile duct and surrounding organs were made using KMIRD phantom based on a South Korea standard man. To check the absorbed dose of normal organs around bile duct, we set the specific effective energy and initial radioactivity to 1 Ci using MCNPX. Evaluation of the accuracy of the Monte Carlo simulation, the difference of the relative dose is the most 1.96% that satisfy the criteria that is the relative error less than 2% suggested by MCNPX code. In addition, The specific effective energy and absorbed dose of normal organs that were relatively adjacent to bile duct such as right side of kidney, liver, pancreas, transverse colon, spinal cord, stomach and small intestine were relatively high. on the contrary, the organs that were relatively distant to bile duct such as left side of kidney, spleen, ascending colon, descending colon and sigmoid colon were relatively low.

Inhibitory Effect of Persimmon Leaves on the Mutagenicity in Spore Rec Assay and on the Growth of Human Cancer Cells (감잎의 용매별 추출물의 돌연변이 유발 억제 및 암세포 증식억제 효과)

  • The inhibitory effects of persimmon leaves on th e mutagenicity in spore rec assay and on the growth of HT-29 human colon cancer cells and AZ-521 human gastric cancer cells were studied. Methanol extract of persimmon leaves inhibited the mutagenicity induced fly N-methyl- N'-nitro-N-nitrosoguanidine(MNNG) in spore rec assay. The hexane, chloroform and ethylacetate fraction from the methanol extract exhibited strong antimutagenicity against MNNG in spore rec assay The methanol extract of persimmon leaves also revealed the inhibitory effects on the growth of HT-29 human colon cancer cells and AZ-521 human gastric cancer cells. Among the solvent extracted fraction from the methanol extract, the chloroform fraction was most effective and inhibited the growth of HT-29 and AZ-521 cells by 100 percent.

Immunostimulatory Effects of Traditional Doenjang (전통된장의 면역증강 효과)

  • We investigated the immunostimulatory effects of doenjang, a famous Korean traditional food made from fermented soybean paste, on the immunohistochemical reaction in the gastrointestinal (GI) tract and immune response in mice. Male C57BL/6N mice (6 weeks-old) were divided into 4 experimental diet treatment groups and a basal diet (control) group, and fed with different diets for 4 weeks. The immunoreactive density of $CD4^+/CD8^+$ lymphocytes were strongly stained in the jejunum and colon in Group III. The immunoreactivity of universal nitric oxide synthase (uNOS) was strongly stained in the myenteric plexus in the colon of all doenjang-fedgroups (I, II and III). The colonic immunoreactive density of protein kinase C-${\alpha}$ (PKC-${\alpha}$) was strongly increased in Groups II and III, while that of stem-cell factor (c-kit) was increased in colonic mucosa of all doenjang-fedgroups (I, II and III) and especially increased in the colonic muscle layer of Group III. These morphological and immunological results indicated that the intake of doenjang could improve the mucosal immune reaction, gastrointestinal motility, blood circulation in the GI tract, and the immuneactivity of the body. These results provide experimental evidence about the health benefits of doenjang.

Effect of Solvent Extracts from Sargassum hemiphyllum on Inhibition of Growth of Human Cancer Cell Lines and Antioxidant Activity (짝잎모자반(Sargassum hemiphyllum)의 암세포주 증식 억제 및 항산화 효과)

  • This study was carried out to determine the inhibitory effects of solvent extracts from Sargassum hemiphyllum on growth of cancer cell lines (AGS human gastric adenocarcinoma and HT-29 human colon cancer cells) and production of lipid peroxides. Inhibitory effects of acetone with methylene chloride extract from S. hemiphyllum on the growth of AGS and HT-29 cancer cells were increased as dose dependent patterns (p<0.05). The methanol extract was more effective on inhibition of growth of AGS. The treatments of hexane, 85% aq. methanol, butanol and water fractions significantly inhibited the growth of cancer cells (p<0.05) and the inhibitory effect was stronger in HT-29. In DCFH-DA (dichlorodihydrofluorescin diacetate) assay, acetone with methylene chloride and methanol extracts showed a stronger inhibitory effect on the production of cellular lipid peroxides (p<0.05) compared with the butanol and hexane fractions. These results indicate that the consumption of S. hemiphyllum may be recommended as a potent functional food for preventing cellular oxidation and cancer.

The Study for Incidence Trends of Colorectal Cancer in Jeju-do (제주도 대장암 발생률 추세에 대한 연구)

  • The age-adjusted incidence rate (AAR) of colorectal cancer in Korea peaked in 2011 and then began to decline. However, the AAR of colorectal cancer in Jeju increased continually from 1999 to 2013. Compared with the 16 major cities in Korea, the inhabitants of Jeju-do have a low cancer screening program participation rate, high alcohol consumption rate and high BMI. The present study aimed to provide a statistical basis for the lowering of the incidence of colorectal cancer in Jeju-do. The Jeju regional cancer institute data from 1999 to 2013 was used to analyze the between Jeju-do and Korea using the Joinpoint Regression program (Statistical Methodology and Applications Branch, Surveillance Research Program, National Cancer Institute) Version 4.2.0 - April 2015. The AAR of colorectal cancer in Jeju-do has being increasing because of the increase in the rates of male colon cancer (average annual percent change 8.422%, p-value<0.000), female colon cancer (AAPC 6.136%, p-value<0.000), male rectal cancer (AAPC 4.221%, p-value 0.003) and colon cancer in people aged over 50 years (AAPC 7.986% p-value<0.000). The results of this study suggest that the treatment of precancerous lesions of the male rectum and lowering of the incidence of colon cancer in people aged over 50 years are necessary to reduce the colorectal cancer incidence rate in Jeju-do.
