The purpose of this study was to evaluate the effect of splinting methods on the rearrangement of periodontal fibers after experimental tooth movement. Orthodontic force was applied by placing closed coil spring between upper third incisor and canine in seven dogs, weighing 20 kg or more. After 3 weeks of force application, 0.0215 inch multistrand wire and polyethylene ribbon were bonded to each side, as a flexible and rigid splinting respectively in 6 experimental animals. The remaining one served as a control. Each two animals were sacrificed at 4, 8 and 12 weeks after splinting respectively and prepared histologically for hematoxylin-eosin and Masson's trichrome staining. The results of this study were obtained as follows: 1. After tooth movement, periodontal space was narrowed and periodontal titers were thick on pressure side while elongated fibers were observed on tension side. 2. After 4 weeks of retention, the rearrangement of periodontal fibers was observed in the flexible splinting group, but not in the rigid splinting group. 3. After 8 weeks of retention, the rearrangement of periodontal titers was observed in both groups, but the difference could not be detected between two groups. 4. During the retention period, the rearrangement of periodontal fibers was faster in tension side than in pressure side. These results show that the rearrangement of periodontal fibers is also obtained by rigid splinting after tooth movement. It is suggested that the rigid splinting by polyethylene ribbon can be used as a way of postorthodontic retention.
This study examined the relations between degree of posterior dental compensation and skeletal discrepancy in Class III malocclusion. The pretreatment lateral cephalogras and dental casts of 87 skeletal Class III adults were selected to provide a random sampling of skeletal Class III malocclusion. Skeletal discrepancy was described with ANB angle, Wits appraisal, SN-Mn plane angle, FMA and ratios of basal arch width. Degree of posterior dental compensation was described with maxillary intermolar angle, mandibular interolar angle and sum of intermoloar angle. The relationships between skeletal discrepancy and degree of posterior dental compensation were analyzed with simple correlation analysis, stepwise multiple regression analysis. The results were as follows 1. A strong association was found between the variation in the anteroposterior measure, ANB angle and the variation of posterior dental compensation measures, sum of intermolar angle and mandibular intermolar angle in skeletal Class III malocclusion. 2. There was no statistically significant relationship between the variation in the vertical measures and the variation of posterior dental compensation measures in skeletal Class III malocclusion. 3. There was no statistically significant relationship between the variation in the anteroposterior and vortical measures and degree of basal arch width discrepancy.
The Purpose of this study was to investigate the stress distribution and tooth displacement at the initial phase produced by 5 types of molar uprighting springs using finite element method. The three dimensional finite element model of lower dentition, bone and springs was composed of 5083 elements and 2071 nodes. The results were as follows: 1. In case of helical spring and root spring, intrusion of lower canine and first premolar were observed md distal tipping, translation and extrusion of lower second molar were observed. 2. In case of T-loop, modified T-loop and box loop, intrusion and distal translation of lower second premolar were observed, and the largest crown distal tipping and translation of lower second molar were observed in T-loop and the smallest were observed in box loop. 3. In case of T-loop with cinch-bact crown distal tipping and translation of lower second molar were decreased, but extrusion was also decreased. 4. With increase of activation in T-loop, mesial translation and won distal tipping of lower second molar were increased and edentulous space was closing, but distal translation of second premolar was also increased. 5. With increase of tip-back bend in T--loop, distal tipping and translation of lower second molar were increased, but extrusion was also increased more largely.
The purpose of this study was to obtain the angulation and inclination of FACC of all teeth to FH plane. Study models of 31 persons with normal occlusion were selected and mounted on the semiadjustable articulator for this study. Using T.A.R.G. with a little modified method, the angulation and inclination of FACC of all teeth to FH plane were measured and then the measurements were analyzed statistically. The obtained results were as follows. Mean, standard deviation, maximum value, and minimum value of the angulation and inclination of FACC of upper and lower teeth were obtained. $\cdot$The FACC in both upper and lower arch was progressively lingually-inclined from anterior teeth to posterior teeth. $\cdot$In the angulations of FACC of upper teeth, central and lateral incisor showed similar value. Yet, the FACC of the rest was progressively distally-angulated from the canine to posterior teeth. The FACC in lower arch was progressively mesially-angulated from anterior teeth to posterior teeth. $\cdot$The angulation and inclination of FACC of any tooth in both upper and lower teeth correlated strongly and positively with the angulation and inclination of FACC of adjacent tooth.
Objective: The purpose of this study was to determine whether an exogenous electric current to the alveolar bone surrounding a tooth being orthodontically treated can enhance tooth movement in human and to verify the effect of electric currents on tooth movement in a clinical aspect. Methods: This study was performed on 7 female orthodontic patients. The electric appliance was set in the maxilla to provide a direct electric current of $20{\mu}A$. The maxillary canine on one side was assigned as the experimental side, and the other as control. The experimental canine was provided with orthodontic force and electric current. The control side was given orthodontic force only. Electrical current was applied to experimental canines for 5 hours a day. The amount of canine movement was measured with an electronic caliper every week. Results: The amount of orthodontic tooth movement in the experimental side during 4 weeks was greater by 30% compared to that of the control side. The amount of increase in tooth movement in the experimental side was statistically significant. The amount of tooth movement in the experimental side during the first two weeks was !Bleater than that in the following two weeks. The amount of weekly tooth movement in the control side was decreased gradually. Conclusions: These results suggested that the exogenous electric current from the miniature electric device might accelerate orthodontic tooth movement by one third and have the potential to reduce orthodontic treatment duration.
Objective: The purpose of this study was to investigate the horizontal skeletal pattern and dental arch differences between Class III malocclusion patients and normal occlusion patients. Methods: Twenty skeletal Class III malocclusion patients and ten normal occlusion patients were selected and 3D facial CT were taken to analyze the horizontal skeletal differences between the two groups. Results: In the horizontal comparison of the maxilla, skeletal width and perimeter were significantly smaller in skeletal Class III patients on ANS and A point reference planes. The difference between maxillary width of ANS and A point reference planes showed that there was greater constriction of the first and second premolar in skeletal Class III patients. In the horizontal comparison of the mandible, the widths of the canine and premolar area were significantly larger in skeletal Class III patients on B point reference plane. The differences between width of the upper and lower jaws (comparison of A and B reference planes) were significantly large in the canine and premolar area. Conclusions: From this study, the characteristics of Class III malocclusion patients were shown through horizontal constriction of the maxilla. But to make clear further detailed characteristics of Class III malocclusion patients, additional studies are necessary.

Ectopic eruption means the eruption of the tooth in an abnormal position due to multiple factors, which found most frequently in maxillary fist permanent molars, mandibular lateral incisors and maxillary permanent canines. Ectopic eruption of the maxillary first permanent molar occurs when the molar erupts with a more mesial angulation than normal, and locks itself in an atypical resorption on the distobuccal root of the second primary molar. The maxillary first permanent molar plays important roles for mastication and occlusion, so ectopically erupted maxillary first permanent molars should be relocated into proper position. Treatment options are separation by insertion of the brass wire or elastic rings, preparation of distal aspect of the maxillary second primary molar, using fixed or removable appliance with finger spring, and placement of space maintainer or space regainer after extraction of the maxillary second primary molar. We report three cases treated of ectopically erupted maxillary first permanent molar by re-setting of stainless steel crowns, placement of brass wire and using active plate. We could find out distal movement of maxillary first permanent molars into proper position and normal occlusion.

The purpose of this study was to examine the prevalence of developmental anomalies of permanent lateral incisor and eruption disturbances and analyze the association between two components. Panoramic radiographs of 3984 patients (aged 5 to 13 years) who visited the Department of Pediatric Dentistry of Seoul National University Dental Hospital between November 2016 and October 2017 were screened. The prevalence of developmental anomalies of permanent lateral incisors was 10.2%. The most common developmental anomalies were congenitally missing teeth(66.1%), followed by peg lateralis(33.5%). The prevalence of eruption disturbances was 16.5%. Among the patient with developmental anomalies of permanent lateral incisors, associated eruption disturbances were appeared on 31.1% of patients. Peg lateralis(p < 0.001), underdeveloped lateral incisors(p < 0.001) and dens invaginatus(p = 0.004) were associated significantly with eruption disturbance of permanent teeth. Eruption disturbance in patients with peg lateralis and dens invaginatus was most prevalent in maxillary canine. For patients with underdeveloped lateral incisor, eruption disturbance of maxillary central incisor was most frequent. It is important for patients with developmental anomalies of lateral incisors to detect associated eruption disturbance early through regular checkup. Diagnosis and treatment plan in view of such relationships is important in order to treat appropriately at the optimal time.

Alveolar bone resorption after extraction impairs the necessary bone volume and complicates the case for implant surgery and aesthetic implant prosthesis. Immediate implant surgery after extraction decreases the number of surgical procedures and the duration of treatment, while allowing minimum alveolar bone resorption and preserving the residual bone volume. Although immediate implant holds many advantages such as preservation of hard and soft tissue around the extraction socket, greater implant survival rate and higher patient satisfaction, various complications and high failure rate are discouraging factors for the clinicians. In this case report, severe alveolar bone resorption with soft tissue changes were predicted after the extraction of prolonged retained deciduous incisors and impacted maxillary canines and thus decided on immediate implant procedure. Immediate implant surgery after extraction was carried out with minimal bone reduction and tapered wide-neck implant to establish initial stability. Simultaneous bone graft was done by filling the defect area with iliac cancellous bone with additional onlay-type bone graft and absorbable membrane on the labial bone for upper lip support. A stable and esthetic result was obtained with shortened treatment period.

Tooth impaction is a frequently observed eruption anomaly in pediatric dental practice. Young patients with impacted or unerupted teeth have more prediction for dentigerous cyst formation. Dentigerous cyst presents radiographic features, unilocular or multilocular radioluscency. Cysts occur most frequently in the premolar region except third molar. Dentigerous cysts can grow to a considerable size, and large cysts may be associated with a painless expansion of the bone in the involved area. Extensive lesions may result in facial asymmetry, osseous destruction, root resorption of proximal teeth and displacement of associated tooth. The nature of the causative tooth influences the type of surgical treatment required for the dentigerous cyst. If the cyst is associated with a supernumerary or wisdom tooth, complete enucleation of the cyst along with extraction of tooth may be the first treatment choice. Otherwise, preservation of the associated teeth should be considered to prevent a young patient from psychological and mental trauma because of the loss of tooth. We should consider the degree of tooth displacement, osseous destruction and growth pattern of oromaxillofacial area when planning treatment. Thus a proper and logical treatment planning can help a proper growth and development of oromaxillofacial area and can save the patient from a psychological and mental trauma. This report describes 4 cases of the management of impacted premolars and molars associated with dentigerous cysts in children.
