THE USE OF MINISCREWS FOR TOOTH MOVEMENT IN CHILDREN (성장기 아동에서 miniscrew를 고정원으로 이용한 치아이동)

  • Anchorage control in orthodontic treatment is an important factor affecting treatment results. In the conventional approach, intra-oral anchorage such as application of differential force and moment, Nance holding arch and lingual arch, as well as extra-oral anchorage such as head gear were used for anchorage reinforcement. However, these anchorages may result in undesired tooth movement and require patient cooperation. To overcome these disadvantages, skeletal anchorage system was introduced as orthodontic anchorage. Types of skeletal anchorage include implant, onplant, miniplate and miniscrew. Especially, miniscrew has many advantages such as reduced patient cooperation, low cost and easy placement. Recently, it is successfully used in orthodontic treatment. This cases were treated using orthodontic miniscrews for retraction of ectopically erupting maxillary canine and impacted mandibular canine and intrusion of maxillary incisors.

Mesial Angulation of Upper Anterior Teeth Crown : Positional Relationship between Pupil and Mesial Inclination Line of Upper Canine (상악 전치 치관의 근심경사도: 견치 근심경사선과 동공의 위치적 관계)

  • Statement of problem: Arrangement and angulation of clinical crown is very important for esthetic restoration in the upper anterior dentition. However, there was no clinical criteria to mesial angulation of the crown for Korean. Purpose: This study was undertaken to estimate the mesial angulation of the crown of the anterior teeth and the Oh's E-triangle made of the inter-pupillary line and the mesial inclination lines of the canines. Material and Method: 270 portraits of Korean were used for this study. The mesial angulation of the upper anterior teeth and the relationship of the inter-pupillary line and the mesial inclination lines of the canines were measured with the tools of PhotoShop software on the scanned images. Results: The angulation between the clinical crowns having a same name in the upper anterior dentition were $3.6^{\circ}$ between the central incisors, $8.6^{\circ}$ between the lateral incisors, and $13.6^{\circ}$ between the canines. There was no significance according to occupation and gender( P > 0.05). The ratio of height to base line of Oh's E-triangle was 4.47. 81.6% of the subjects showed the mesial inclination line of the canine passed by mesial border area of pupil. Conclusion: These data for Korean would be useful clinically to give the esthetic arrangement and to make the contour of upper anterior teeth.

Fracture Strength of IPS Empress Crown : The Effect of Incisal Reduction and Axial Inclination on Lower Canine (IPS Empress 도재관의 파절강도 : 하악 견치에서 절단연 삭제량과 축면 경사도에 따른 영향)

  • The purpose of this study was to compare the fracture strength of the IPS Empress ceramic crown according to the incisal reduction(2.0mm, 2.5mm, 3.0mm) and axial inclination ($4^{\circ}$, $8^{\circ}$, $12^{\circ}$) of the lower canine. After 10 metal dies were made for each group, the IPS Empress ceramic crowns were fabricated and each crown was cemented on each metal die with resin cement. The cemented crowns mounted on the testing jig were inclined 30 degrees and a universal testing machine was used to measure the fracture strength. The results of this study were as follows : 1. The fracture strength of the ceramic crown with 3.0mm depth and $12^{\circ}$ inclination was the highest(1377N). Crowns of 2.0mm depth and $4^{\circ}$ inclination had the lowest strength (731 N). 2. There were no significant differences of the fracture strength by axial inclination in same incisal reduction group. 3. The fracture mode of the crowns was similar. Most of fracture lines began at the loading area and extended through proximal surface perpendicular to the margin irrespective of incisal reduction.

Three-Dimensional Finite element analysis of Canine and Carnassial for Observation of Dog Bite forces (개(犬)의 교합력 관찰을 위한 견치와 열육치의 3차원 유한요소 분석)

  • Purpose: This study is for the prosthesis of dogs. Observe the occlusal relation between the dog's canine and carnassial teeth. The strength and the direction of the occlusal by 3D FEM analysis. Methods: The mandibular canine and carnassial of dogs were tested. The dog's skull was contact point confirmed by dental CAD. The skull of the dog was 3D modeled by CT. The 3D model was analyzed by ABAQUS. Opening and closing movement has been a force of 100N, 200N, 300N, 500N, 1000N, 1,500N. The peak von Mises stress distribution was confirmed. Results: As occlusal force increased, stress appeared to 1.34 MPa, 3.32 MPa, 5.00 MPa, 6.19 MPa, 5.58 MPa, 5.47 MPa in left canine. and Stress was seen at 2.10 MPa, 3.08 MPa, 3.89 MPa, 5.50 MPa, 7.04 MPa, 7.18 MPa in the right canine. Stress appeared at 2.41 MPa, 3.53 MPa, 5.15 MPa, 7.28 MPa, 31.26 MPa, 67.22 MPa in the left carnassial. and Stress was seen at 1.57 MPa, 2.96 MPa, 3.76 MPa, 6.01 MPa, 20.94 MPa, 64.38 MPa in the right carnassial. Conclusion: Peak von Mises stress values were found at the peak of the canine, the buccal of the central cusp of the carnassial, and the occlusal surface of the distal cusp.

Surgical orthodontic treatment of skeletal Class III malocclusion using mini-implant: correction of horizontal and vertical dental compensation (Mini-implant를 이용한 III급 부정교합의 수술교정치료: 수평, 수직적 치성 보상의 조절)

  • Treatment of skeletal Class III malocclusion with mini-implant anchorage is discussed in relation to vertical control of the maxillary posterior dentoalveolar region and horizontal control of mandibular anterior teeth. A midpalatal mini-implant provided anchorage for intruding the maxillary posterior teeth. Mandibular mini-Implant implants were used to bring about labioversion of mandibular anterior teeth. After mandibular setback surgery, improvement of the facial profile was obtained both horizontally and vertically, Total treatment time was 11 months. Stable occlusion was maintained after 18 months of retention, The effectiveness and efficacy of mini-implants for the treatment of skeletal Class III malocclusion are also discussed.

The frog appliance for upper molar distalization: a case report (Frog appliance를 이용한 상악 대구치의 원심 이동: 증례 보고)

  • The purpose of this article was to evaluate the effects of a new upper molar distalization system, the Frog Appliance, on dentofacial structures in a Class II, division 1 patient. An 11-year-old girl was referred to our clinic for orthodontic treatment. She had a mild skeletal Class II malocclusion with Class II molar and canine relationship on both sides. The treatment plan included distalization of the upper first molars bilaterally followed by full fixed appliance therapy. For the upper molar distalization, a new system, the Frog Appliance, was constructed and applied. Lateral cephalometric radiographs were used to evaluate the treatment results. Distalization of the upper first molars was achieved in four months successfully, and Class I molar relationship was obtained. Total treatment time was 16 months. According to the results of the cephalometric evaluation, a nearly bodily distal molar movement with a slight anchorage loss was attained. In conclusion, the Frog Appliance was found to be a simple, ef ective, non-invasive, and compliance-free intraoral distalization appliance for achieving bilateral molar distalization.

TUNNEL TRACTION OF IMPACTED MAXILLARY CANINES (매복된 견치의 Tunnel을 통한 교정적 견인)

  • The impacted maxillary canine is a common problem of which all dental practitioners should be aware. The surgical orthodontic treatment of impacted canines is replaced correction position in dental arch without periodontal damage. Many treatment possibilities have been considered for this goal; window procedure, apically positioned flap, closed eruption technique and tunnel traction. Prognosis for these treatment may be very uncertain in many case(infraosseous impacted tooth). Other steps are required to achieve a satisfactory periodontal outcome. Satisfactory results could be expected if the physiologic eruption pattern is restored by tunnel traction, because permanent tooth erupts through the gingiva near the crest of the ridge so that periodontal damage is reduced. This article report that the surgical orthodontic treatment using tunnel traction is obtained proper position and reduced periodontal damage in facially impacted maxillary canines.

  • Nevoid basal cell carcinoma syndrome(NBCCS) is a autosomal dominant disorder, and its major manifestations are multiple basal cell carcinoma, keratocystic odontogenic tumor, rib anomalies, palmer and plantar pits, calcification of the falx cerebri. Keratocystic odontogenic tumor(KCOT) is defined as intraosseous tumor of odontogenic origin with a characteristic lining of parakeratinized stratified squamous epithelium and potential aggressive behavior. We report a case of a 3-year-old patient with nevoid basal cell carcinoma syndrome who initially presented with unilocular keratocystic odontogenic tumor in maxillary canine region. Keratocystic odontogenic tumor was treated by enucleation, and periodic follow-up check will be required for early diagnosis of additional diseases related with this syndrome.

  • Impaction of maxillary incisor is rare than the third molar and canine, but its rate is higher than the other anterior teeth due to frequent mesiodens, trauma and variation of root formation (root dilaceration etc.). It is often observed in the dental age of about eight years and over. It will be occurred that the space loss, midline deviation and cyst formation due to the impaction of maxillary incisor. So it is important to evaluate the precise location of impacted tooth and to make appropriate treatment plan. Treatment would be surgical extraction or expectation for spontaneous eruption. If the impacted tooth has no pathologic change and development of the root is favorable, orthodontic traction is recommended for recovery of function and esthetics. In these cases, we performed orthodontic traction for the eruption of impacted maxillary incisors, and obtained satisfactory results.

THE EFFECTS OF CLEFT ON MESIODISTAL DIMENSIONS OF PERMANENT TEETH IN UNILATERAL CLEFT LIP AND PALATE PATIENTS (순구개열이 영구치 근원심 폭경에 미치는 영향)

  • The purpose of this study was to investigate the effects of cleft on mesiodistal dimensions of permanent teeth in unilateral cleft lip and palate patients. Mesiodistal dimensions of permanent teeth were measured to the nearest 0.01mm on plaster models of 50 subjects with unilateral complete cleft lip and palate, 10 siblings and 50 Controls. The results were as follows : 1. Tooth size discrepancy in the cleft group was significant in all regions except maxillary cuspid, mandibular cuspid and mandibular first premolar. 2. Some of the mesiodistal dimensions of the teeth on the cleft side were significantly smaller than those of their antimeres on the non-cleft side in the cleft group. 3. A comparison of mesiodistal dimensions of the teeth for the right and left sides of the control group showed no statisically significant differences excepts maxillary lateral incisor. 4. Asymmetries of mesiodistal dimensions of the teeth in the sibling group was not found except maxillary first molar.
