  • Multiple rice body formation is a complication of chronic bursitis frequently associated with seronegative rheumatoid arthritis or tuberculosis. It resembles synovial chondromatosis on imaging and clinically. We report on a pathologically diagnosed multiple rice body formation in subacromial and subdeltoid bursitis in a 44-year-old man who was treated by surgical removal and bursectomy. At 16 months after the removal, range of motion of affected shoulder was normal. No evidence of recurrence of rice body in plain X-ray and ultrasonography. Multiple rice body formed in chronic subacromial and subdeltoid bursitis could be treated with surgical removal and bursectomy successfully.

  • Purpose: To evaluate the bursoscopic findings of refractory primary adhesive capsulitis of the shoulder and to determine the clinical correlation. Materials and Methods: Arthroscopic capsular release was performed in 21 refractory adhesive capsulitis patients who had not responded to conservative treatment. The VAS for pain, range of motion, UCLA score was checked before and after surgery, and at the last follow up. Results: Bursitis, mild friction and impingement on coracoacromial ligament, as well as adhesion were observed. The function and pain improved earlier than after simple acromiohumeral joint capsular release. Conclusion: Subacromial bursoscopy helps rule out a hidden secondary frozen shoulder or secondary changes with primary adhesion. In addition, it has the advantage of being both a treatment and procedure for making a prognosis.

  • Purpose: To investigate the effectiveness of injection therapy in the treatment of subacromial bursitis. Materials and Methods: A total of 38 patients with sonographic confirmation of subacromial bursitis were recruited into this study. The shoulder abduction range of motion and visual analog scale (VAS) were compared before injections and 3 wks after the completion of injections. Results: The shoulder range of abduction before injection was $77.89{\pm}14.17$ degrees and improved to $148.68{\pm}13.39$ degrees 3 wks after the injection treatments (P<0.05). VAS before injection was $6.8{\pm}1.4$ and improved to $1.4{\pm}0.8$ 3 wks after the injection treatments (P<0.05). Conclusions: Ultrasound may be used as an adjuvant tool in guiding the needle accurately into the inflamed subacromial bursa. The ultrasound-guided injection technique can result in significant improvement in shoulder abduction range of motion in treating patients with subacromial bursitis.

  • Multiple rice bodies are a rare disorder that is most commonly observed in chronic rheumatoid arthritis patients and as a complication of chronic inflammation in the bursa. However, it can occur in the absence of an underlying systemic disorder. Although it resembles synovial chondromatosis clinically and on imaging, the condition can be discriminated by an analysis of the radiographic and MR appearances. We encountered a case of multiple rice body formation with subacromial bursitis on the shoulder of a 37-year old man suffering from pain and motion limitation. The patient was treated by arthroscopic removal of the multiple rice bodies and a subacromial bursectomy. We present this case with a review of the relevant literature.

  • Numerous small fibrinous rice bodies are a common finding in joints afflicted with rheumatoid arthritis(RA) or seronegative arthropathy, Subacromial and subdeltoid bursitis of the shoulder associated with multiple rice body formation is a rare occurrence. To our knowledge, this is the first report to describe the arthroscopic treatment of massive rice bodies in both glenohumeral joint and subacromial-subdeltoid bursae combined with full thickness of rotator cuff in RA. Besides, the MR appearance of subacromial-subdeltoid bursal rice bodies have been previously described in only few recent reports. Therefore, we also describe the MR appearances subacromial-subdeltoid bursae associated with multiple rice bodies in RA.

  • The rotator cuff is situated in a potential tight subacromial space and undergoes senescent structural changes commonly observed in other joints of the body. When the cuff fails, spontaneous healing of the torn tendon is not expected to occur, and multiple factors may be responsible. Its fibers are under tension and typically retract on tearing. The subacromial bursal inflammation and alterations in normal glenohumeral kinematics have been considered in the development of symptoms. Controversy continues to exist concerning the pathogenesis of rotator cuff disease. The heterogeneity of the disorder, as well as the notion that rotator cuff disease may not actually represent a continuum of the same process, but rather, is a compilation of independent disorders, may partly explain the differing viewpoints on its origin. Two contrasting pathogenetic mechanisms have been extensively described and include vascular, or intrinsic, causes and impingement, or extrinsic, factors. Other etiologies have also been reported that include trauma, congenital or developmental factors, and instability. For successful treatment of the rotator cuff diseases, it is essential to understand the structure and function of rotator cuff and to clarify the pathogenesis and natural history of its disorder.

  • Objectives This study aims to analyze clinical studies on subacromial-subdeltoid (SA-SD) bursitis and propose future directions for clinical research on SA-SD bursitis. Methods We searched eight databases to investigate research trends in Korean medicine treatments for SA-SD bursitis up to November 2023. A total of thirty-one studies were included in this analysis. Results Among the included studies, there were six case reports from Korea, fifteen case reports, and ten randomized controlled trials from abroad. Various interventions were uesd, including acupuncture, pharmacopuncture, manual therapy, electro-acupuncture, cupping, physiotherapy, acupotomy, warm-acupuncture, herbal medicine, and moxibustion. The evaluation tools used in these studies included the total effective rate, visual analogue scale, range of motion, and numeral rating scale. Conclusions This study provides an overview of the research trends in Korean medicine treatments for SA-SD bursitis. However, given the low quality and small sample sizes of the studies, the evidence supporting effect of Korean medicine treatments for SA-SD bursitis was insufficient. Further clinical trials and systematic reviews are required.
