노인여성의 사망률 개선을 위한 밴드 서킷 트레이닝의 중재 효과 (The Moderating Effects of Band Circuit Training for the Mortality Improvement of Elderly Women)

  • 본 연구는 노인여성의 사망률 개선을 위한 밴드 서킷 트레이닝의 중재 효과를 구명하여 노인들의 삶의 질 향상을 위한 효과적인 운동 중재 프로그램을 제공하는 데 목적이 있다. 본 연구의 목적을 달성하기 위해서 Y시 소재 만 65세 이상 여성을 대상으로 8주간 밴드 서킷 트레이닝 중재가 노인여성의 밴드 서킷 트레이닝 중재가 노인여성의 Senior Fitness Test(SFT)와 Sitting Rising Test(SRT)에 미치는 영향을 구명하여 다음과 같은 결론을 얻었다. 밴드 서킷 트레이닝 중재에 따른 노인여성의 기능적 체력(의자에 앉았다 일어서기, 2kg 덤벨 들기, 2분 제자리 걷기, 의자에 앉아 앞으로 숙이기, 등 뒤로 손잡기, 2.44m 왕복걷기)와 Sitting Rising Test(SRT)는 측정시기와 집단 간 통계적으로 유의한 상호작용효과가 나타났으며, 실험집단에서 밴드 서킷 트레이닝 중재 후 긍정적으로 개선된 것으로 나타났다. 이상의 연구결과를 종합해 볼 때 밴드 서킷 트레이닝 중재는 노인들의 사망률과 관련이 있는 기능적 체력과 SRT에 긍정적 영향을 미치는 것으로 나타나 노인들의 사망률 개선을 통해서 삶의 질 향상을 위한 효과적인 운동 중재 프로그램으로 적용 가능하다고 생각된다.

  • 본 연구의 목적은 70세 이상의 여성고령자들에게 재가운동 프로그램(home support exercise program)을 16주간 주3회씩 실시하여 생활기능 체력과 건강관련 삶의 질(SF-36)에 대한 주관적 평가에서의 변화를 살펴보고자 하였다. 실험군(총17명, 평균나이:79.4)과 통제군(총17명, 평균나이:80.7)은 부산시 연제구에 거주하며 신체적 외상이나 거동에 불편함이 없고, 연구 참여에 동의한 여성 고령자로 구성하였다. SAS 9.2 통계 패키지를 활용하여 집단간 기술통계와 동일성 검증 및 사전측정을 공변량으로 한 공분산 분석을 실시하였다. 그 결과는 다음과 같다. 첫째, 실험군이 통제군에 비해 덤벨 들기, 일어섰다 앉기, 2.24m 왕복달리기, 2분간 제자리 걷기 항목에서 통계적으로 유의하게(p<.001) 증가한 것으로 나타났다. 둘째, 유연성을 측정하는 등 뒤 손잡기 항목에서는 실험군에서 유의할 만한 변화가 없는 것으로 제시되었다. 이러한 점은 재가운동프로그램에 유연성을 향상시킬 수 있는 운동항목을 추가하여 보완할 필요성이 있음을 시사하는 것이었다. 셋째, SF-36을 통한 건강관련 삶의 질에 대한 주관적 평가에서는 8개의 모든 하위요인에서 실험군과 통제군간에 유의할 만한 차이가 없는 것으로 제시되었다. 다만 통증과 활력 하위요인에서 운동을 처방받은 실험군이 통제군에 비해 p<.10 수준에서 향상된 것으로 나타나 긍정적인 변화를 미세하게 시사하였다.

보행시 신발 밑창 형태가 하지 근활성도에 미치는 영향 (The Effect of Form of Outsole on Lower Leg Electromyography during Gait)

  • 이 연구에서는 최근 국내에서 개발된 보행용 기능성 신발 중 걷기 효율성을 변화시키는 복합적 기능성 신발(double rocker sole;CDR)과 negative-heel rocker sole(NR)형태 신발의 근육 활성도를 비교하였다. 연구대상은 하지정형 외과적 질환이 없는 40-50대 중년여성 11명을 대상(연령 $47.33{\pm}4.4$, 신장 $158.00{\pm}3.3$cm, 체중 $59.50{\pm}6.1$, BMI $23.80{\pm}1.8kg/m^2$)으로 하였다. 연구결과 좌,우측의 내측광근, 비복근, 반건양근에서는 근활성도의 수준은 통계적으로 유의한 차이가 나타나지 않았지만, CDR신발이 전경골근에서 통계적으로 유의하게 높은 결과가 나타났다(p<.05). 걷기운 동시 동일한 조건속에 운동을 실시할 경우 신발 밑창의 변화를 통해 인위적으로 운동량을 증가시킬 수 있을 것이다.

일주일간의 간헐적 좌식차단의 혈관기능 개선 효과 (Seven Days Breaking Up Prolonged Sitting Improves Systemic Endothelial Function in Sedentary Men)

  • PURPOSE: To examine the cumulative (7 days) effect of breaking up prolonged sitting on systemic endothelial function in sedentary men. METHODS: Thirty sedentary men ($33.93{\pm}5.72years$) participated in two randomized 7 days sitting trial (Sit group (control) vs. Breaks group). The protocol of Breaks group is as follows: 4-minute of moderate-intensity marching in place (walking) every 1 hour during business hour (total: 8 breaks/day). Assessment of brachial artery endothelial function using flow-mediated dilation (FMD) and arterial stiffness indices (augmentation index, arterial pressure and pulse wave velocity) were measured before and after 7 days treatment. RESULTS: Brachial artery FMD significantly increased after 7 days breaking up prolonged sitting treatment (Breaks groups, $9.65{\pm}2.61$ to $9.62{\pm}2.6%$) compared with 7 days prolonged sitting (Sit group, $8.37{\pm}3.41$ to $10.11{\pm}3.75%$) (interaction effect, p=.004). Arterial pressure (AP) significantly increased after treatment (Breaks group, $2.75{\pm}2.19$ to $2.38{\pm}1.63mmHg$, p=.002) in Sit group but there was no change (Sit group, $1.00{\pm}3.18$ to $2.50{\pm}9.23mmHg$) in Breaks groups (interaction effect, p=.008). CONCLUSIONS: These finding show that 7 days regular breaking up prolonged sitting improve in FMD, compared with prolonged sitting. Therefore, regular breaking up prolonged sitting may improve systemic endothelial function in sedentary men.

모래사장 위 맨발걷기와 운동화걷기가 허리통증이 있는 노인의 통증, 장애, 운동기능, 수면만족도, 삶의 질에 미치는 효과 비교 (A Comparison of the Effects of Barefoot Walking and Sneakers Walking on a Sandy Beach on Pain, Disability, Motor Function, Sleep Satisfaction, and Quality of Life in the Elderly with Low Back Pain)

  • Purpose : The purpose of this research was to investigate the effects of barefoot walking on the sandy beach on pain intensity, disability, motor function, sleep satisfaction, and quality of life in the elderly with low back pain (LBP). Methods : A single-blinded randomized controlled trial was conducted involving 32 elderly with LBP who underwent sand walking barefoot (intervention, n=16) or with sneakers (control, n = 16). Both walking methods were carried out for 30 minutes per day, three times a week for four weeks. Pain intensity and disability were assessed using the visual analog scale and Oswestry disability index, respectively. Balance and gait were evaluated using the Berg balance scale and timed up and go test, respectively. Sleep satisfaction was quantified with the Verran and Snyder-Halpern sleep scale. Finally, quality of life was assessed through the WHO quality of life scale's abbreviated Korean version. Results : Compared with the control group, the intervention group showed significant differences in pain intensity (p=.005), disability due to LBP (p=.002), static balance ability (p=.003), dynamic balance ability (p=.002), and sleep satisfaction (p=.017). There was no significant difference in the quality of life between the two groups. Conclusion : Barefoot walking on a sandy beach is significantly effective in improving pain, disability due to LBP, balance ability, and sleep satisfaction in the elderly with LBP. Further studies with larger sample sizes and longer intervention periods must to be conducted to generalize using barefoot walking in LBP management.

요양병원 입원 치매노인을 위한 율동적 걷기프로그램이 보행, 인지 및 낙상위험에 미치는 효과 (Effects of Walking Program with Dance on Gait, Cognition, and Risk of Falls of Elderly with Dementia in a Long-term Care Hospital)

  • Purpose: This study was conducted to examine the effects walking program with dance on gait, cognition, and risk of falls of elderly with dementia in a long-term care hospital. Methods: Subjects consisted of 42 elderly with dementia in a long-term care hospital (21 elderly with dementia in an experimental group and 21 elders with dementia in a control group). Data were collected from March 14 to April 30, 2016. Subjects in the experimental group performed walking program with dance (three times a week, 30-50 minutes session, 18 sessions). Data were analyzed using descriptive statistics, Chi-square test, Fisher's exact test, independent t-test, paired t-test, repeated measures ANOVA with the SPSS/WIN 21.0. Results: The participants in the experimental group showed significantly increased scores of cadence (F= 8.36, p= .007) and improved scores of cognition (F= 9.95, p= .003) compared the control group. Conclusion: The findings indicate walking program with dance is recommend a regular exercise program to enhance the cadence associated with walking speed and cognitive function elderly.

평지를 걸어갈 때 하지운동과 작용하는 하중에 대한 생체역학적 해석 (A Biomechanical Analysis of Lower Extremity Kinematics and Kinetics During Level Walking)

  • A two-dimensional biomechanical model was developed in order to calculated the lower extremity kinematics and kinetics during level walking. This model consists of three segments : the thigh, calf, and foot. Each segment was assumed to be a rigid body ; its motion to be planar in the sagittal plane. Five young males were involved in the gait experiment and their anthropometric data were measured for the calculation of segmental masses and moments of inertial. Six markers were used to obtain the kinematic data of the right lower extremity for at least three trials of walking at 1.0m/s, and simultaneously a Kistler force plate was used to obtain the foot-floor reaction data. Based on the experimental data acquired for the stance phase of the right foot, calculated vertical joint forces reached up to 0.91, 1.05, and 1.11 BW(body weight) at the hip, the knee, the ankle joints, respectively. The flexion-extension moments reached up to 69.7, 52.3, and 98.8 Nm in magnitude at the corresponding three joints. It was found that the calculated joint loadings of a subject were statistically the same for all his three trials, but not the same for all five subjects involved in the gait study.

  • 충남대학교 부속동물병원에 내원(來院)한 우측후지(右側後肢)의 부전골에 골절상(骨折傷)을 받은 6개월금(個月今)의 꽃사슴에 대하여 대퇴골절단술(大腿骨切斷術)을 실시한 증례(症例)를 보고한다. 마취는 체중 kg당 1.2mg의 xylazine을 근육주사하여 실시하였으며, 수술방법은 개에서의 대퇴골절단술(大腿骨切斷術)과 같은 방법(方法)을 시도하였다. 수술직후 일어서기, 걷기, 달리기등의 운동에 불편을 보였지만 시간이 경과함에 따라 숙달되는 것을 관찰할 수 있었으며, 절단수술후(切斷手術後) 현재(現在)까지 (약(約) 6개월(個月)) 병적(病的) 미후(微候)는 인정되지 않았다.

거울을 이용한 뒤로 걷기 운동이 뇌졸중 환자의 균형과 보행능력에 미치는 영향 (The Effect of Backward Walking Exercise using a Mirror on Balance and Gait in Patients with Stroke)

  • PURPOSE: This study aimed to identify the effect of a backward walking exercise using a mirror on balance and gait in patients with stroke. METHODS: Twenty subjects with post-stroke hemiparesis volunteered to participate in this study. The experimental and control groups performed backward walking exercise plus conventional therapy or conventional physical therapy, respectively, for 6 weeks. Assessment instruments included the Berg Balance Scale (BBS), timed up and go test (TUG), and 10-m walking test (10MWT). Evaluation was performed before and after the 6-week training period. We conducted a paired t-test to compare the within-group changes before and after the intervention. An independent t-test was used to compare between-group differences. The statistical significance level was set at ${\alpha}=.05$ for all variables. RESULTS: The experimental group showed a significant within-group change in the BBS, TUG, and 10MWT (p<.05). The control group also showed a significant change (p<.05). A significant difference was observed between the experimental and control groups with regard to changes in the BBS, TUG, and 10MWT results after the interventions (p<.05). CONCLUSION: This study demonstrated that backward walking exercise using a mirror may be valuable for future research. Further studies with a wider range of pathology and longer experiment duration are required to validate the results of the present study.

뒤로 걷기 운동에 따른 뇌졸중 편마비 환자의 균형능력 및 보행능력의 변화 (Changes in Balance and Gait Following Backward Walking Exercise in Hemiplegic Stroke Patients)

  • PURPOSE: The purpose of this study was to identify whether backward walking exercise was more effective than conventional physical therapy for balance and gait in hemiplegic stroke patients. METHODS: Eighteen patients with chronic stroke were randomly assigned to the experimental (n=9) or control (n=9) group. The experimental and control group performed backward walking exercise and conventional physical therapy, respectively, for 8 weeks. Stability Index (SI) and Weight Distribution Index (WDI) during standing were assessed using the Tetrax Balance System. The Timed Up and Go (TUG) test and Korean version of the Berg Balance Scale (K-BBS) were used to evaluate balance and fall risk. Walking speed, stride length, and step length on the affected side were measured using the 10-Meter Walk and ink foot printing tests. Wilcoxon signed-rank and Mann-Whitney U tests were used for within- and between- group comparisons, respectively. RESULTS: The experimental group showed significantly higher changes in SI (p<.01), WDI (p<.01), TUG (p<.001), and BBS score (p<.001) following intervention compared with the control group. The experimental group also showed significantly greater improvements in walking speed (p<.01), stride length (p<.001), and step length on the affected side (p<.001) after intervention compared with the control group. CONCLUSION: Backward walking exercise is an effective intervention to improve balance and gait in hemiplegic stroke patients.
