A Study on the Relationship with Nicotine Dependence and Mental Health -Focusing on SCL-90-R (Symptom Checklist-90-Revision)- (니코틴 의존도와 정신 건강 수준의 연관성에 대한 조사 연구 -간이 정신진단 검사 중심으로-)

  • Objectives: The purpose of this study is to investigate the relationship of nicotine dependence and mental health. Methods: The subjects of this study were 153 male smokers in their 20s, 30s, 40s and 50s who visited Wonmi Public Health Center. They were evaluated with Fagerstorm's test for nicotine dependence (FTND) and Symptom Checklist-90-Revision (SCL-90-R). Using the FTND score, the participants were divided into three groups; mild nicotine dependent group, moderate group and severe group. Results: In the SCL-90-R score, there were statistically significant differences in the characteristics of Somatization, Obsessive-Compulsive, Interpersonal Sensitivity, Depression, Global Severity Index (GSI) and Positive Symptom Test (PST) among the three groups. Comparing the values of FTND and the amounts of Daily Smoking to the values of Somatization, Obsessive-Compulsive, Interpersonal Sensitivity, Depression and GSI in this study resulted in positive correlations, thereby demonstrating a statistical significance. Conclusions: The above results suggest that there are significant relations between nicotine dependence and the level of mental health. These results can be used for planning the smoking cessation program and mental health promoting program based on Korean traditional medicine.

Significance of Follow-up Blood Test after Health Examination (건강진단 후 추구 관리의 현황 및 의의)

  • After a health examination, abnormal patients were classified into two groups. One needs clinical treatment immediately, while the other one needs check-up in 2~3 month after for blood tests and preventive medical study. To discover the status and importance of the follow-up study in a health program, blood test upon health examination was compared to the test from the follow-up. We examined patients of ages from 20's to 70's. Among these patients, most of them were between 40's and 50's, and in male patients the follow-up test were more freguent than female. We examined blood obtained from January 2007 to December 2008. We analyzed and compared initial and the follow up blood test. Lipid profile, liver test, thyroid function tests (TFTs), glucose fasting, and HbA1c were examined from the blood tests. We also surveyed their distribution and status. The value of the follow up blood tests was significantly decreased with the most striking differences in lipid profiles and liver test. Follow up study is able to improve life style of patients through doctor's advice in a health program. Therefore, we can conclude that the follow up blood test is also quite critical.

A study of participation in health-related decision and self-care competency in patients with chronic disease (만성질환자의 건강관련 의사결정에 대한 참여 정도와 자가간호역량에 관한 연구)

  • This study investigated the correlation between the participation in health-related decisions and self-care competency and the factors influencing self-care competence of patients with chronic disease. The participants were 518 patients with chronic illnesses who were admitted to a public health center. Using a structured-questionnaire, the data was collected from January 1, 2019 to 31, 2019. The data was analyzed using descriptive statistics, t-tests, ANOVA, Scheffé tests, Pearson correlation coefficients and multiple linear regression with the IBM SPSS/WIN 22.0 program. The mean scores on the participation in health-related decisions and self-care competency were 3.62±0.20 and 4.57±0.34, respectively. The main variables that distinguish significant differences between participation in healthcare decisions and self-care capabilities are gender, age, education level, diagnosis, and subjective health status. Self-care competency was positively correlated with the participation in health-related decisions (r=.66, p=.043). Factors influencing self-care competence were education (β=-2.23, p<.001), health concerns (β=0.14, p=.003), subjective health statues (β=2.57, p=.011), and age (β=-0.36, p=.048). The input variables explained .56 of self-care competence. The results of the study indicate that approaches focusing on enhancing the participation in health-related decisions could potentially improve self-care competency of patients with chronic disease.

Analysis of the factors of dental hygiene plans influencing patients of the dental hygiene program based on dental hygiene process (치위생과정에 근거한 구강건강관리프로그램 대상자의 치위생계획의 영향요인 분석)

  • Objectives: This study aims to recognize the importance of dental hygiene process diagnosis of dental hygiene process which can comprehensively grasp the patient's problem and to use it as a basis for establishing the patient's preventive treatment plan. Methods: This study did survey to 443 patients who received treatment based on the oral health care program from a dental clinic in Busan from January 2015 to January 2017. Data analysis was performed using IBM SPSS Statistics (Version 21.0), and statistical significance level was set at ${\alpha}=0.05$. Binary logistic regression analysis was performed to the dental hygiene problems affecting the dental hygiene plan. Results: There were significant differences in dental hygiene problems between male and female respondents on various dental problems such as dental plaque deposition, attrition, stain, dental fear, possibility of jaw joint disorder, food pressing, possibility of malocclusion. There were also significant differences in dental hygiene plans between male and female respondents in air-Jet, non-smoking education, and sealant. The most common dental hygiene plan was scaling, The problem of stain showed that the scaling plan was 0.20 times less. The explanatory power of the model was 43.5%, and the Hosmer and Lemeshow tests were 0.345. Conclusions: Therefore, if we continue to study the factors affecting the dental hygiene problems and the plan, we can reduce the burden of the dental hygienists applying the dental hygiene process in the dental clinic. And, it is expected that the oral health care program using the dental hygiene process will spread to the dental clinic as an excellent oral preventive program.

An Influence of Academic Stress on the Life-Style Behaviors and Mental Health of Junior High School Students (중학생의 학업스트레스 정도에 따른 건강관련 생활습관 및 정신건강 비교)

  • The purpose of this study was to investigate effects of academic stress levels on life-style behaviors and mental health among the junior high school students in Korea. A convenient sample drawn from the 842 students from 8 junior high schools in Seoul and Kyunggi-Do area was investigated with a survey instrument. Female students showed higher academic stress levels compared to male students, especially in terms of the stress related to teachers, class time, and peer relationships. For the male students, higher stress levels were related to the higher risk factors of smoking, drinking and inhaling drug abuse. Students who had higher academic stress levels spent more time in using computer and playing digital games compared to those who had lower academic stress. In addition, higher academic stress levels were significantly related to risk of mental health, especially depression. These results suggest the urgent needs of preventive or educational program to support students with academic stress resolve in healthy manners such as playing sports or music or club activities.

Health behaviors and quality of life by life cycle of hypertensive patients (고혈압환자의 생애주기별 건강행태와 삶의 질)

  • The purpose of this study was to identify the relation of health behaviors and the quality of life by life cycle of hypertensive patients. This study was conducted by analyzing secondary data with data from the 2017 Community Health Survey. The subjects were total of 62,056 who were diagnosed with hypertension and analyzed using the SPSS WIN 20.0 program. As a result of the study, As a result of the study, first, hypertension treatment and drug compliance were significantly lower in adulthood than in middle-aged and older adults. Second, it was confirmed that the quality of life score by life cycle was the highest in adulthood and decreased as age increased. Third, the quality of life according to health behaviors differed according to drinking, exercise, and low-salt diet throughout the life cycle. Considering the results presented in this study, it is suggested that health education by life cycle is essential for lifelong health management of hypertensive patients. In particular, it is considered that a convergence approach such as education and medical approaches will be needed to develop measures to increase drug compliance in adult hypertensive patients and to have healthy health behaviors to improve quality of life.

Relationship between Arterial Stiffness as Measured by the Cardio-Ankle Vascular Index with Body Mass Index in Healthy Elderly Subjects (건강한 노인에서 심장-발목 혈관지수로 측정한 동맥 경직도와 체질량 지수간의 상관관계)

  • An inverse correlation between obesity and arterial stiffness has been reported, but there are no consistent results in elderly subjects. This study examined the relationship between the arterial stiffness measured by the CAVI (cardio-ankle vascular index) and BMI (body mass index) in healthy elderly people. This study included 629 healthy elderly people aged 65 and over who underwent health examinations at a general hospital in Gyeonggi from July 2018 to June 2019. Metabolic syndrome was diagnosed using the criteria of the Adult Treatment Panel (ATP) III of the US National Cholesterol Education Program (NCEP). Among the criteria of NCEP-ATP III, the waist circumference and obesity criteria were based on the WHO criteria. All subjects underwent a biochemical blood test and an assessment of the CAVI. In both men and women, the CAVI was lower in the obese group than in the normal weight group. Gender (P=0.047), age (P<0.001), BMI (P<0.001), and waist circumference (P=0.008) were factors affecting the CAVI. Gender, high blood pressure, and hyperglycemia were independent positive predictors of the CAVI levels, while obesity was a negative predictor. Therefore, the CAVI and BMI showed an inverse correlation. In conclusion, there was an inverse correlation between the CAVI and BMI in the elderly, and obesity was a negative predictor of the CAVI.

  • The increasing regular medical check up rate for early diagnosis in disease has increasing sleep endoscopy rate because of reduction with discomfort. The purpose of this study was to determine the number of recovery bed as increasing sleep endoscopy rate using check up time, waiting time & recovery time at a general hospital in Seoul. This study was analyzed using ARENA 10.0 program. At present and as increasing of sleep endoscopy rate 10%, 20% was increased recovery time, waiting time & the rate of bed inflection. So at present, the number of recovery bed has to increase for client's safety and as increasing of sleep endoscopy rate 10%, 20% has to increase 3 and then waiting time decreased in 2 minutes.

A Study on the Three-Year Weight Changes of Workers at a Manufacturing Company and the Comparison of Metabolic Syndrome Diagnosis Components: Focused on the Data of Korean National Health Screening (2015~2017) (일개 제조업 근로자의 3년간 체중변화와 대사증후군 진단 구성요소의 비교 분석: 국가건강검진(2015~2017년) 자료를 중심으로)

  • Purpose: This study aims to provide preliminary data for weight management, and prevention and management of cardiovascular and cerebrovascular diseases. We examined the effect of changes in the weight of workers at a manufacturing company over three years on their metabolic syndrome and metabolic syndrome diagnosis components. Methods: Necessary data were collected from the questionnaire and the results of the Korean National Health Screening of 2015 and 2017, which included 228 workers at a manufacturing company in G region. The collected data were analyzed using the SPSS/WIN 23.0 program. ANCOVA was used to examine the differences in the metabolic syndrome diagnosis components according to weight change. In addition, multiple logistic regression analysis was used to obtain the odds ratios of metabolic syndrome and metabolic syndrome analysis component, based on the weight changes in the normal weight group and the obesity group. Results: Waist measure, systolic blood pressure, and blood pressure were found to have significant effects based on participants' weight change over three years. These factors increased with a larger increase in weight at a statistically significant level. This study analyzed the weight changes of the normal weight group and the obesity group considering the data from the National Health Screening of 2015, and found that the risk of metabolic syndrome increased at a statistically significant level as body weight increased; thus, the obesity group showed a higher risk in this regard. It was also found that waist measure, fasting blood sugar, and high-density low cholesterol increased at a statistically significant level as body weight increased. Conclusion: Health administrators need to recognize the importance of workers' weight management, select an intensive management group based on a time series analysis of weight changes, and develop and implement programs to manage the metabolic syndrome diagnosis components.

Synergistic Effect of Forest Environment and Therapeutic Program for the Treatment of Depression (산림활동이 우울증 환자들의 호전에 미치는 영향: 산림치유 프로그램 집단과 병원 프로그램 집단, 산림욕 집단, 대조군 비교 연구)

  • This study was conducted to evaluate the effect of forest environment and therapeutic program to the patients with major depressive disorder in antidepressant medication. The first group participated the four sessions of therapeutic program in the forest environment ("forest therapy"), the second group did in the program in a hospital environment, the third group did merely walking in the forest ("forest bath"), and the last group was controls. All the participants was diagnosed with major depressive disorder and had been taking antidepressant medication longer than 3 months. Depressive symptoms were measured by the Hamilton Rating Scales for Depression(HRSD), Montgomery-Asberg Depressin Rating Scales(MADRS), Beck Depression Inventory(BDI), and general health perception was measured by Short Form Health Survey Questionnaire(SF-36). Heart rate variability(HRV) were also examined to observe the physiological parameters before and after the program. In the results, HRSD score of forest program group was significantly lower than controls after the program. MADRS score of forest therapy group and hospital program group was also significantly lower than controls after 4 sessions of the program. The remission rate defined as below 7 points in HRSD was higher in the forest therapy group, hospital program group, forest bath group, and controls in order. These results reveals that the therapeutic program performed in forest environment may improve unremitted depressive symptoms of patients with major depressive disorder. Although not significant, the forest therapy program showed better outcome than hosptial program and forest bath.
