Women and Children's Health Care in Korea: Status and Strategies (한국의 여성과 어린이 건강 - 실태와 전략 -)

  • 여성건강은 개인 뿐만 아니라 가족, 사회, 국가적으로 대단히 중요하다. 왜냐하면 미래사회의 자원인 인간을 출산하고 양육하는 것과 직접적으로 관련이 있기 때문이다. 여성은 스스로 자신의 몸을 지키고 건강행위를 위한 결정을 할 수 있어야 한다. 특히 출산과 관련하여 여성은 자신의 건강을 유지 증진 할 수 있는 능력이 있어야 한다. 한나라의 발전수준은 모자보건의 수준으로 결정하며 영아 사망률과 모성사망률은 국가 발전의 수준을 대표한다. 본 연구는 한국의 여성과 어린이 건강 실태를 살펴보았다. 모성사망률과 영아사망률은 여성의 산전간호 수진률의 증가와 영아 예방접종률의 증가로 점차 감소하고 있다. 그러나 모유수유률의 감소와 영아의 사고발생률이 증가하는 것은 자라나는 어린이의 건강을 위협한다. 어린이 사고는 환경과 돌보는 어른들의 부주의에 의해서 발생되는 경우가 대부분이므로 어린이 사고를 방지하고 모유수유률을 증가시키는 예방전략이 필요하다. 여성의 흡연, 음주, 약물의 사용이 증가하는 것과 여성폭력이 증가하는 것은 여성건강을 위협하는 중요한 요인이다. 또한 식습관 장애와 같은 잘못된 건강행위는 여성의 건강을 위협할 뿐 아니라 자녀출산에도 부정적인 영향을 미친다. 여성과 어린이의 건강을 유지 증진하기 위해서는 여성이 여성으로서의 인식을 가지고 건강관리기관을 찾고 건강관리를 할 수 있는 능력을 가져야 한다. 그러므로 간호사를 비롯한 의료인은 여성이 스스로 판단하고 결정하고 자가간호 할 수 있는 힘과 의지와 정보를 제공해야 한다. 이를 위해 여성건강전문간호사가 필요하다. 또한 국가와 사회단체(NGO)가 함께 여성건강이 국가적인 사업임을 인식하고 지원과 관심을 기울여야 한다.

A Study on the Oral Health Status and Oral Health Awareness of Pregnant Women (임산부의 구강건강상태 및 구강보건인식 실태)

  • 본 논문은 임신의 시기가 내분비계의 변화로 인한 프로게스테론의 과잉분비가 발생하여 혈관의 투과성을 항진시켜 구강내의 잇몸 부종, 잇몸 출혈 등을 유발하고, 입덧으로 인한 메쓰꺼움, 몸의 무거움과 나른함, 음식 기호변화, 간식횟수의 증가 등을 겪게 되어 신체뿐 아니라 구강 환경 또한 변화하게 된다는 여러 관련 자료들을 근거로 임산부의 구강 건강상태와 구강보건인식을 파악하고자 2008년 12월 15일 ~ 2009년 1월 30일까지 180명의 임산부를 대상으로 구강건강상태와 구강보건인식에 대한 조사를 실시하였다. 설문의 결과 임산부의 생활수준, 분만경험, 임신시기에 따라 생활수준이 좋을수록, 분만경험이 없는 경우에서, 4-6개월 임신시기 시 구강건강상태에 대하여 유의미한 차이가 나타났다. 건강한 구강건강을 위한 우선 행위인 잇솔질에서의 교육 수준별, 생활 수준별, 임신 시기별로 식사 후 3회 정도의 잇솔질의 횟수가 유의미한 차이의 결과를 나타내었으며, 잇솔질시 오심과 구토로 잇솔질이 어려운 임산부들의 구강상태 청결을 위해 사용되어지는 보조구강위생용품으로는 치실, 구강 양치액, 혀 클리너, 치간 칫솔이 사용된다는 결과가 나타났다. 이러한 결과의 자료들은 임신 시기 중 가장 고민된다고 답하여준 구강건강 부분에서의 입덧으로 인한 잇솔질의 어려움, 잇몸출혈, 충치, 치석제거, 구강질환 치료의 적정시기, 구취 등의 부분을 임산부 대상의 구강보건교육 시 기존의 자료에 조금 더 중점적으로 다루어져야 하는 부분임을 확인할 수 있었다.

Effect of Self-Rated Health Awareness and Oral Health Care Behavior on Dental Fear in Some Areas Adult Patients (일부지역 성인 환자의 주관적 건강인식과 구강건강관리행태가 치과공포감에 미치는 영향)

  • The purpose of this study was to examine the oral health care and self-rated health status of adults visiting dental clinics in the region of Busan and influential factors for their dental fear from January to May, 2017, in an effort to obtain some information on how to relieve fear. As for dental fear, adults who were in their 30s, who were homemakers and whose monthly income ranged from 4 to 4.99 million won. In regard to fear according to oral health care, dental fear caused by the treatment avoidance factor was stronger among the adults who didn't receive dental checkups and who brushed their teeth in the wrong way. Dental fear that was attributed to the physiological reaction factor and the stimuli-inducing factor was severer among the adults who didn't receive dental checkups and who had no scaling experience. Overall fear was severer among the adults who didn't receive dental checkups and who changed their toothbrushes every four or more months. The factors that affected dental fear were self-rated health status, self-rated oral health status, gender, age, whether to receive dental checkups on a regular basis or not, and oral health education experience. Therefore in order to alleviate dental fear, self-rated health status should be improved, and the kind of system that encourages regular dental checkups and provides oral health education should be prepared.

A Study on Effects of Subjective Perception to Nutrient Intake and Mental Health of Korean Adolescents: Using the Korea National Health and Nutrition Examination Survey (청소년들의 체형인식에 따른 영양소 섭취실태 및 정신건강 - 국민건강영양조사 자료를 이용하여 -)

  • The objective of this study was to investigate the effects of the subjective perception to the nutrient intake and mental health of Korean adolescents by the residual method. This study was based on data from the 5th and 6th Korea National Health and Nutrition Examination Survey (2010-2014) of the 2,961 total subjects(male 1,548, female 1,413, aged 12 to 19 years). The nutrient intake was collected by using the 24 hr recall method and mental health and BMI were collected by the health interview survey and physical examination research. An analysis of the relationship between the subjective perception and BMI showed that the proportion of those with a distorted perception of their body type was the highest among adolescents with normal weight(p for trend, <.001). Especially, regarding the subjective body type, the proportion of adolescents who perceived themselves overweight was significantly higher among females as compared to that among males (p for trend, <.001). An analysis of nutrient intake status against the subjective perception showed that the group of adolescents who perceived themselves overweight had the lowest scores on the index of nutritional quality (INQ), nutritional adequacy ratio (NAR) and mean adequacy ratio (MAR) (p for trend, <.001). With respect to the daily intake of major nutrients, adolescents who perceived themselves overweight consumed the lowest amounts of total energy (p for trend, <.01) and carbohydrate (p for trend,<.001). An analysis of the mental health status of the adolescents showed that those who perceived themselves overweight had the highest levels of stress recognition and depression (p<0.01). In conclusion, the adolescents' subjective perception is associated with their nutrient intake and mental health. Therefore, some educational programs are desirable to help the adolescents to have their desirable subjective perception and to increase their satisfaction with their body type.

A Convergence Study on Factors Affecting Physical-Health Status of College Students (대학생의 신체적 건강상태 영향요인에 관한 융합연구)

  • This is a convergence study to investigate the factors affecting the physical health status of college students and to present a strategy for effective program development. The participants of this study were 200 college students in a region and collected data by self-reported questionnaire. Data were analyzed by descriptive statistics, t-test, ANOVA, correlation analysis and stepwise multiple regression analysis. The results were as follows: 1) The physical-health status of the college students was positively correlated with the subjective perception of health status(${\beta}=-.323$, p<.01), health awareness(${\beta}=-.278$, p<.01), gender(${\beta}=-.197$, p<.01), and breakfast habit(${\beta}=-.192$, p<.01). And the explanatory power($R_2$) by 4 variables was 33.1%. As a result, it was confirmed that strengthening subjective perception, positive reinforcement of health awareness, and habit of breakfast were important for improvement of physical health status. Therefore, it is necessary to develop a health promotion program based on the factors influencing on the physical health status and to analyze its application effect.

Novice Mothers' Perception, Experiences, and Needs on Healthy Pregnancy (초보어머니의 건강한 임신에 대한 인식과 경험 및 요구)

  • The purpose of the study was to find out the novice mothers' perceptions, experiences, and need on healthy pregnancy. For the research purpose, we conducted the focus group interview with 10 novice mothers who were pregnant of their first child or raising first child under 12 months old. The results of the study were as follows. First, novice mothers recognized 'healthy pregnancies' as 'physically healthy pregnancies for mothers and fetuses,' 'pregnancies where mothers were mentally healthy and happy,' and 'pregnancy planned and prepared with husband together.' Second, novice mothers experienced 'examination of obstetrics and gynecology,' 'management of health and daily life,' and 'preparation of realistic conditions' to prepare for pregnancy and childbirth. Third, difficulties during pregnancy were due to 'sickness and physical changes' and 'psychological and emotional changes.' Fourth, the needs on support for healthy pregnancy were 'pre-education on mental and physical changes caused by pregnancy' and 'support for pregnancy and counseling in advance.'
