Since the first report of coronary artery bypass grafting (CABG) in patients with end-stage renal disease in 1974, numerous reports have documented the feasibility of CABG in patients with chronic renal diseases. Patients with chronic renal failure often have comorbid disorders such as hypertension, and diabetes mellitus, each with their own complications and associated impact on both short and long-term survivals. In addition, infection and sepsis have been identified as significant causes of morbidity and mortality in most series of patients with end-stage renal disease undergoing cardiac surgical procedure. As a result of these and other factors such as perioperative volume and electrolyte disturbances, patients with chronic renal failures are at an increased risk of complication and mortality after CABG. We report 3 cases of "Off-pump" CABG in the chronic renal failure patients. patients.
This study deals with the changes in the concentrations of blood glucose and electrolytes during open heart surgery. Blood glucose and electrolytes in connection with age, disease and anesthetic period were measured in 25 patients about to undergo heart surgery which were performed between June 1986 and August 1986 in Yeungnam University Hospital. Because glucose solution is commonly used as priming solution, and the priming solution is hyperglycemic and hyperosmolar, glucose level of priming solution in this study was adjusted to 100-200mg% level to minimize hyperglycemic and hyperosmolar effect. The following results were obtained. 1. Glucose level of priming solution before extracorporeal circulation was $151.6{\pm}3.13mg%$. 2. With body cooling, blood glucose level was elevated. As duration of extracorporeal circulation is prolonged, blood glucose level was elevated more, but no difference between age and diseases were observed. On warming, blood glucose level was progressively lowered. 3. Despite the low serum potassium level during by pass, the potassium level was progressively elevated following by-pass, cut the serum potassium level was low compared to control values. Elevated calcium level was maintained during by pass.
Thromboelastography(TEG) enables a global assessment of hemostatic function to be made from a single blood sample, documenting the interaction of platelets with protein coagulation cascade from the time of the initial platelet-fibrin interaction, through platelet aggregation, clot strengthening and fibrin cross linking to eventual clot Iysis. Thirty-five patients(mean age 34$\pm$ 12) undergoing open heart surgery from April 1st, 1996 to August 31th, 1996 were investigated at preoperatively and immediate, one hour, and 24 hours after cessation of cardiopulmonary bypass using TEG. Comparisons were made between classic hematological indices and TEG data. There were statistically significant correlation between maximal amplitude(MA) and platelet count before CPB, activating clotting time(ACT) and TEG date(R time, K time and a angle) at 24-hour after CPB. The data on the predictive accuracy for postoperative bleeding at 24-hour after CPB, the TEG was significantly better than ACT(57%) or the coagulation profiles(43%) as a predictor of postoperative bleeding, with an accuracy rate of 100% (P=0.0043). In conclusion, TEG seems to be easy to use, clinically accurate, cost effective and provides data which can effectively manage a patient's hemostasis.
Purpose: To examine the effects of the environmental nursing intervention using eye-shields and earplugs on the incidence of delirium, environmental stress and sleep among the patients with open-heart surgery in the post-cardiac ICU(intensive care units). Method: This study was a non-equivalent control group non-synchronized design. The subjects consisted of 93 patients who received ICU care after the cardiac surgery. Among those, 32 subjects was assigned in one experimental group with eye-shields, 31 in the other experimental group with earplugs, and 30 in the control group. ICU delirium was measured by CAM-ICU, environmental stress by ICUESS, and sleep by two sleep scales developed by Oh, et al. and Kim. The data were analyzed by the SPSS 13.0 program including frequency, percentage, t-test, $x^2$ test, ANOVA and ANCOVA. Results: There are no statistically significant differences in the incidence of delirium among three groups (p=.139). The ICU environmental stress scores from the subjects of two experimental groups demonstrated lower (F=6.731, p=.002) than the control group. Also, the degrees of sleep pattern (p=.000, p=.000) and sleep satisfaction (p=.000, p=.000) were greater than those of the control group. Conclusion: Although the environmental nursing intervention using eye-shields and earplugs did not demonstrate the effects in decreasing the incidence of ICU delirium, it has affirmative effects in decreasing environmental stress and improving the quality sleep among the patients with open-heart surgery while they stay in ICU. Further nursing implications are discussed.
A Clinical study was carried out to observe changes in concentration of serum and urinary potassium during and after open-heart surgery under extracorporeal circulation with Heart-Lung-Machine. The patients studied in the present work included eight cases of congenital heart anomalies and seven cases of acquired valvular diseases of the heart. Blood and urine samples were collected at intervals of 30 minutes during open-heart surgery and at intervals of 8 hours until 48 hours after the operation to determine concentration of serum and urinary potassium. 1.While the concentration of urinary potassium was not changed during the operation, the concentration of urinary potassium was rapidly increased after the operation. Eight hours after the operation, the concentration of urinary potassium was increased to the highest value of 86.97±43.96 mEq/L and was gradually decreased thereafter. The concentration of urinary potassium, however, still remained elevated even 48 hours after the operation, the concentration being greater than preoperative state by 19.90 mEq/L. 2.The concentration of urinary potassium after the operation was higher in the group who went through 90 minutes of extracorporeal circulation [$108.71{\pm}94.71mEq/L$] than in the group who went through 60 minutes of extracorporeal circulation [$86.58{\pm}50.87mEq/L$]. The Increased level of urinary potas-sium observed In the group who went through 90 minutes of extracorporeal circulation continued for longer period of time as compared with that in the group who went through 60 minutes of extracorporeal circulation. 3.The increased level of urinary potassium was observed in the both groups of patients with or without diuretics plus digitalis therapy. However, the increased level of urinary potassium appeared to con-tinue for longer period of time in the group of patients receiving diuretics plus digitalis therapy prior to the operation than in the group of patients receiving no diuretics plus digitalis therapy.
Activities of serum glutamic pyruvic transaminase [SGPT], serum glutamic oxaloacetic transaminase [SGOT] and serum lactic dehydrogenase [LDH] were determined during and after surgery In a total of 18 patients with heart diseases Including 8 cases of congenital heart anomaly and 10 cases of acquired valvular diseases who undergone open heart surgery under cardiopulmonary bypass. The results obtained are as follows: 1]The average value of the activity of SGPT before the surgery was $27.94{\pm}12.24$ units. The value was increased following the operation and reached to the maximum value of $43.83{\pm}37.02$ units 2 days after the operation, which was slightly deviated from the normal range. The enzyme activity returned to the normal range of $32.82{\pm}23.61$ units 4 days after the operation. 2]The average value of the activity of SGOT before the surgery was observed to be $30.5{\pm}11.29$ units. The value increased during the operation and reached to the maximum value of $95.56{\pm}43.38$ units. 2 days after the operation, the enzyme activity was gradually decreased thereafter and returned to the normal range 6 days after the operation. 3]The average value of the activity of serum LDH before the surgery was found to be $258.0{\pm}88.96$ units. The value was increased during the operation and reached to the maximum value of $930.55{\pm} 258.23$ units 2 days after the operation. The enzyme activity was gradually decreased thereafter and returned to normal range 7 days after the operation. 4]It was observed that the longer the time required for the extracorporeal circulation in the open heart surgery became, the greater the changed in activity of SGOT, SGPT and serum LDH were found.
During 1986, 111 cases of open heart surgery were performed at Yeungnam University Hospital consisting 88 cases of congenital heart disease and 23 cases of acquired heart disease. Among 88 congenital heart disease, 72 were acyanotic group and 16 were cyanotic. Common congenital heart diseases were ventricular septal defect(51%), atrial septal defect(18%) and Tetralogy of Fallot(16%). Among 23 acquired heart disease, 22 cases were valvular heart disease and one was dissecting aortic aneurysm. Three cases of the postoperative death were present resulting 2.7% of surgical mortality rate.
From December of 1994 to April of 1995, we, SHUH Department of Pediatric Thoracic and Cardiovascular Surgery, studied effects of aprotinin. 95 patients were randomly divided into two groups : group I (n=47) with aprotinin and group ll (n=48) without aprotinin. Aprotinin was given as one shot injection to cardiopulmonary bypass perfusion solution with dose of 50,000 KIUikg. Laboratory data such as hemoglobin, hematocrit, BUH, creatinine, fibrinogen, electrolyte concentration, aPTT, PT, and AT R was checked preoperatively, 5 minutes after anesthesia, 5 minutes and 35 minutes after CPB circulation, and 5 minutes, 3 hours, and 24 hours after reperfusion. Also, chest-tube drainage, transfused amount of RBC, platelet concentrate, and fresh frozen plasma within first 24 hours postoperatively were checked and analyzed after transition nn body weight demension. Only RBC transfused postoperatively had statistical significance with P value of less than 0.001. Others had no difference statistical wise. Postoperative side effects of aprotinin was not detected weeks after the surgery and there was no reoperated patient due to postoperative bleeding.
Even though some authors have reported on the advantages of early total correction of complex heart disease, for low birth weight premature neonates, most surgeons prefer a multi-step approach to early total correction due to the many problems, such as the technical problems, the cardiopulmonary bypass management and etc. We report here on a successful case of early one-stage total repair of coarctation of the aorta and a ventricular septal defect in a 1,250 gram premature neonate.
In order to assess the correlation of the myocardial damage and the duration of cardiopulmonary bypass, measurement of creatine kinase [CK], lactate dehydrogenase [LDH], asparatate aminotransferase [AST], and MB band of CK [CK-MB] were carried out on the first, third, fifth, seventh, and ninth day in 44 patients following open heart surgery [POD 1,3,5,7,9]. And the patients were divided into three groups according to the duration of aortic cross clamp time [ACT]: Group I [ACT< 60 minutes. n=19], Group II [60 minutes < ACT< 90 minutes, n=7] and Group III[90 minutes > ACT, n=18]. 1. The leakage of CK in total patients increased to the highest level at POD 1, with rapid decrease and recovery at POD 7. The leakage of CK in Group III were greater than in Group I from POD 1 to POD 3 [P < 0.01]. The recovery time of CK level was shorter in Group I [POD 3] than in Group II and III [POD 7]. 2. The serum levels of LDH in total patients increased to the highest level at POD 1, with slow recovery until POD 9. The levels of LDH in Group III were higher than in Group I until POD 9 [P < 0.005]. The levels of LDH in Group I and II recovered but not in Group III. 3. The serum levels of AST in total patients increased to the highest level at POD 1, with rapid decrease and recovery at POD 7. The levels of AST in Group III were greater than in Group I from POD 1 to POD 5 [P < 0.05]. The recovery time of AST level was shorter in Group I and II [POD 5] than in Group III [POD 7]. 4. The positive cases for CK-MB in 36 patients were 22 [61.1 %] as a whole, 5[41.6%] in Group I, 4[57.1 %] in Group II, 13[76.4 %] in Group III at POD 1, and a case in each group at POD 3, and only a case in Group Ill at POD 5. It is concluded that the myocardial injury was closely related with the duration of cardiopulmonary bypass in open heart surgery.
