A comparison of the Effects of Intravenous Fluid Warming and Skin Surface Warming on Peri-operative Body Temperature and Acid Base Balance of Elderly Patients with Abdominal Surgery (수액가온요법과 피부가온요법이 개복술 환자의 저체온 예방에 미치는 효과 비교)

  • Purpose: The purpose of this study was to compare the effects of intravenous fluid warming and skin surface warming on peri-operative body temperature and acid base balance of abdominal surgical patients under general anesthesia. Method: Data collection was performed from January 4th, to May 31, 2004. The intravenous fluid warming(IFW) group(30 elderly patients) was warmed through an IV line by an Animec set to $37^{\circ}C$. The skin surface warming(SSW) group(30 elderly patients) was warmed by a circulating-water blanket set to $38^{\circ}C$ under the back and a 60W heating lamp 40 cm above the chest. The warming continued from induction of general anesthesia to two hours after completion of surgery. Collected data was analyzed using Repeated Measures ANOVA, and Bonferroni methods. Results: SSW was more effective than IFW in preventing hypothermia(p= .043), preventing a decrease of $HCO_3{^-}$(p= .000) and preventing base excess(p= .000) respectively. However, there was no difference in pH between the SSW and IFW(p= .401) groups. Conclusion: We conclude that skin surface warming is more effective in preventing hypothermia, and $HCO_3{^-}$ and base excess during general anesthesia, and returning to normal body temperature after surgery than intravenous fluid warming; however, skin surface warming wasn't able to sustain a normal body temperature in elderly patients undergoing abdominal surgery under general anesthesia.

Lung Lobe Torsion with Concurrent Intestinal Lymphangiectasia in a Maltese Dog (Maltese 개에서 림프관 확장증을 동반한 폐염전 발생 증례)

  • A 4-year-old intact male, Maltese dog weighing 2.2 kg was presented for evaluation of tachypnea and hyperthermia. On initial evaluation, the dog was dyspneic, but alert and responsive. Muffled heart sound was noted with auscultation of the right hemithorax. The radiographic findings were consolidation of the right middle lung lobe, pleural effusion, and abdominal fluid accumulation. Thoracic computed tomography confirmed a contrast-enhancing mass within the right middle lung lobe. Surgical exploration of the thoracic cavity was performed using a right fifth intercostal thoracotomy. Examination of the right lung lobe revealed $360^{\circ}C$ torsion of the right middle lung lobe at the level of the hilus, in a clockwise direction. Lung lobectomy was performed using a thoracoabdominal stapling device with 2.5-mm vascular staples. Full-thickness intestinal biopsy by exploratory laparotomy was taken from the jejunum for histological assessment. Histopathologic examination of the full-thickness intestinal biopsy revealed distended lacteal located within the submucosa of the jejunum. Intestinal lymphangiectasia was resolved with prednisone and low fat diets. Subsequent communication with the owner revealed that the patient was in good health 6 months post-discharge.

  • Gastrointestinal involvement in Henoch-Sch$\"{o}$nlein purpura (HSP) is common. However, both acute pancreatitis and acute renal failure appear to be very rare complications of HSP. We describe a case of HSP with acute pancreatitis and hypovolemic acute renal failure in a 7-year-old girl who presented with a vasculitic purpuric rash involving the lower extremities, abdominal pain, hematochezia, vomiting, and oliguria. Laboratory findings showed increased serum levels of amylase, lipase, and creatinine. An abdominal CT scan revealed diffuse enlargement of the head and body of the pancreas. The patient was successfully managed with conservative treatment, including corticosteroids, and then her pancreatic enzymes and renal function returned to normal. Acute pancreatitis should be differentiated from other causes of acute abdomen in HSP to avoid unnecessary surgery.

Colonic Leiomyoma in Two Dogs (두 마리의 개에서 결장 평활근종의 증례)

  • Two dogs were referred with the clinical signs of depression and constipation, respectively. One dog with depression had hypoglycemia on biochemistry. On survey abdominal radiographs, a soft tissue density round mass with calcified spots was identified cranial to the urinary bladder. On ultrasonographs, a solitary oval heterogeneous complex-echoic mass with well-defined margin was found. On surgery, the mass was pedunculated and originated from the colonic wall. The resected mass was confirmed as colonic leiomyoma. The other dog with constipation had normal CBC and biochemistry values. On survey abdominal radiographs, there was a soft tissue density round mass dorsal to the descending colon and rectum. The descending colon was displaced ventrally and narrowed by the mass. On ultrasonographs, a well-defined round heteroechoic mass compressed the colon. The mass was also pedunculated and originated from the colonic wall on surgery and confirmed as colonic leiomyoma. Two dogs recovered without any clinical signs after surgery and showed permanent resolution of the clinical signs, respectively.

False Positive of F-18 FDG-PET/CT due to Activated Charcoal Granuloma from Intraperitoneal Chemotherapy: A Case Report (복강 내 화학요법에 이용된 활성화 탄소 육아종에 의한 F-18 FDG PET/CT의 위양성 소견: 증례)

  • F-18 FDG-PET/CT could be used to evaluate the surveillance of recurrent stomach cancer, but some cases reported as false-positives. The authors found an activated charcoal granuloma from intraperitoneal chemotherapy by using a curative resection and mitomycin C for stomach cancer. A mass behind the right colon that showed on CT 6 months after an operation in a 46-year-old male patient had no progression in size, but 36 months after the operation, an increase was seen on F-18 FDG-PET/CT, and a metastatic tumor was suspected. The tumor was resected by an explorative laparotomy and was diagnosed as being an activated charcoal granuloma based on the histologic finding. Based on this case, we should be reminded of the possibility of a false-positive on analysis of F-18 FDG-PET/CT caused by an activated charcoal granuloma in a patient who has intraperitoneal chemotherapy.

Successful Removal of Intravenous Leiomyomatosis with Extension into Inferior Vena Cava and Right Atrium (하대정맥, 우심실에 연장된 정맥내 평활근종증의 성공적 절제)

  • Abdominal tumors that can grow through vascular lumen and spread to the right heart are rare. Intravenous leiomyomatosis is a rare tumor that originates from the uterus and spreads through the vessels. Although histologically benign, tumor extension with mechanical obstruction of the inferior vena cava, right cardiac cavities, or even the pulmonary artery, may occasionally result in fatal outcome. The best treatment is complete surgical resection of the entire tumor using cardiopulmonary bypass and total circulation arrest, We report a case of intravenous leiomyomatosis of the uterus that showed intravascular growth up to the right atrium. The patient underwent successful resection of the tumor by one-stage cardiotomy with laparotomy.

  • The operation for esophageal cancer is both complex and challenging, and may be associated with significant morbidity and mortality compared to other oncologic surgeries. Minimally invasive surgeries have been applied on various kinds of surgery to enhance better recovery with minimal surgical complications. But for the esophageal cancer, it has not been actively applied yet. With improvement in instrumentations and increasing experience with endoscopic surgical techniques, minimally invasive surgical approaches to esophageal cancer are being explored to determine feasibility, results and potential advantages. We experienced eight cases of minimally invasive surgery for esophageal cancer and report here focusing on surgical techniques and tips.

Recovery of In Vivo Matured Oocytes from a Bitch with Hydrometra (자궁수종이 있는 개에서 체내 성숙 난자의 회수)

  • One year old mixed-breed bitch was examined to retrieve in vivo matured oocytes. Laparotomy was performed 72 hr after ovulation determined by serum progesterone concentration, and abnormally enlarged left uterus horn was found. Both ovaries had eight corpus lutea, and a total 16 in vivo matured oocytes having perivitelline space within $25{\mu}m$, polar body, and metaphase II nucleus were recovered by flushing oviducts. This is the first study to confirm in vivo maturation of oocytes from a bitch with hydrometra, which suggests that oocytes recovered from canids with reproductive disease could be valuable sources for assisted reproductive technologies.

Duodenal Duplication Cyst in a Korean Native Cattle (한우 송아지에서 발생한 십이지장의 중복낭종)

  • A congenital duplication cyst, which presents with symptoms of gastric outlet obstruction, is an unusual disorder in humans and a rare condition in animals. A 5-day-old male Korean native cattle was presented with anorexia and no defecation. On exploratory laparotomy, an oval shaped mass ($16{\times}7$ cm) was identified on the antimesenteric border of the proximal duodenum, with no communication between the intestinal lumen and the mass itself. Surgical correction for gastrointestinal tract patency was performed. The calf recovered well and was clinically normal three months after surgery. This case is the first report of a duodenal duplication cyst in a Korean native cattle.

Uterine Torsion Associated with Hydrometra and Pyometra in a Domestic Rabbit (애완 토끼에서 자궁수종, 자궁축농증, 자궁염전의 병발 증례)

  • A 1.5-year-old, female domestic rabbit was diagnosed with uterine torsion accompanying hydrometra of the right uterine horn and pyometra of the left horn. The exploratory laparotomy revealed the torsion of bilateral uterine horns in the region of the uterine cervix. No growth was detected in fluid collected from the right horn of the uterus; however, Gram-positive micrococcus and bacilli were cultured from the left horn. The pathophysiology of the simultaneous occurrence of these disorders was suspected considering that abdominal distension progressed over 4 months and the rabbit's condition deteriorated abruptly within 2 days, hydrometra was the initial disease and then infection to the left uterine horn and torsion developed later due to the large volume of fluid within the uterus. This report is the first case showing uterine torsion accompanying hydrometra and pyometra in each uterine horn.
