Effect of Auriculotemporal Nerve Block Anesthesia on Manual Reduction of Disc Displacement without Reduction of the Temporomandibular Joint (악관절의 비정복성관절원판변위의 수조작 정복에 대한 이개측두신경 전달마취의 효과)

  • Aim: Disc displacement without reduction of the temporomandibular joint (TMJ) has been managed by mandibular manipulation to reduce the displaced disc but with a low success rate. The purpose of this study was to determine whether auriculotemporal nerve block anesthesia had an effect on the reduction of the displaced disc and to analyze the factors that influenced the result. Methods: 112 patients were diagnosed with disc displacement without reduction and treated by mandibular manipulation. Disc was recaptured in 35 patients. Among the 77 patients with whom disc recapture had failed, the auriculotemporal nerve was blocked with a local anesthetic in the 49 patients (mean $age \;{\pm}\; SD\; =\; 34.4\;{\pm}\; 15.1$; male 24, female 25) and then mandibular manipulation was performed again. Factors including age, elapsed time from the onset, and opening amount were analyzed in association with disc reduction rate with the auriculotemporal nerve block. Results: Among 49 patients who did not respond to manipulation only, manual reduction with auriculotemporal nerve block anesthesia was successful in 19 patients (38.8%). Maximum unassisted opening amount significantly increased in the 19 patients with successful recapture of the disc ($mean \;{\pm}\; SD\; =\; 46.1 \;{\pm}\; 4.5\; mm$), in contrast to the limited opening amount of the 49 patients before local anesthesia of the auriculotemporal nerve ($mean \;{\pm}\; SD\; =\; 25.7 \;{\pm}\; 6.0\; mm$). Age, elapsed time after the onset, and preoperative opening amount were not associated with the reduction rate. Conclusion: The results of this study suggest that auriculotemporal nerve block anesthesia increases the reduction rate of the disc displacement without reduction of the TMJ when combined with mandibular manipulation, and such anesthesia should be applied at the first stage of manual treatment of disc displacement without reduction.

  • In this paper, the transformed QMSA to load a capacitor without limitation of the electric force on QMSA(Quarter-wavelength Microstrip Antenna) is designed. Bandwidth of the designed and manufactured antenna is 5.7% at the resonant frequency of 2.0 GHz and the resonant frequency and bandwidth versus change of any arbitrary feed point is observed. Since the size of wide slot width between the left and right parallel plate to load a capacitor is very wide bandwidth, will be suitable for very wide bandwidth communication. The radiation pattern characteristics of the designed antenna based on the dipole structure and the aperture structure analysis method. As calculation results, relative backward radation is - 5 dB.

  • Length of staircase get longer and evacuation load get heavier in proportion to the building height. Therefore evacuation time could be considered proportional to square of building height. While smoke spreads vertically so rapidly, evacuation speed is too slow, therefore the reasonable measure for the safe of egress route is urgent. Existing pressurizing smoke control system of vertical exit route is seriously limited in function under the stack effect in winter. Stack effect in winter could be overcome with the methods of opening the staircase outward or letting airflow be free between staircase and outside.

  • 52 Dental students without masticatory problems were selected for this study. They were trained on several mandibular position and mandibular movement exercise, that is, rest position, light bite, tapping, hinge opening, habitual opening, opening limitation, stretch exercise, resistance exercise and clenching. The objectives of this study was to investigate the effects of mandibular movement exercise, especially stretch and resistance, on the experimentally guided limited mouth opening. Muscle activity of the anterior temporalis and the masseter on above mentioned position or exercise were recorded with bioelectric processor EM2(Myotronics, U.S.A.) and the data were processed with SPSS. The obtained results were as follows : 1. Activity of the muscles at rest position were decreased with mandibular movement exercise. 2. Forceful mouth opening on opening limitation increased muscle activity greatly, especially of the masseter. 3. On opening limitation, stretch or resistance exercise was very efficient for decrease of muscle activities. 4. There were no difference of muscle activity between on hinge opening and on habitual opening. Therefore, for muscle relaxation, the two movement exercise can be used interchangeably.

  • By considering the definition and application of resolution as well as the concept and theory of SAR, the essential contents of the SAR design and analysis are described. This paper is to compare and analyze the resolution performance capability of three techniques for achieving azimuth resolution such as the real aperture, the unfocused and the focused techniques, through the simulation. Simulation is performed to make the restricted conditions for the unfocused technique that can be implemented by the less commputing load of signal processingand the lower cost. Through the mission analysis, the use of SAR image can be applied for estimation of whole situation at the regional area in the field of military demands for tactical purpose as well as civilian demands for the damage of disaster. RPV and sall or medium aircraft are selected to carry the SAR for these purposes and the proper resolution turns ou 5~15 m. The trade-off study of variables through the simulations results in the proper conditions such that range is less 3, 000 m, Wavelength is 1~10 m, and the raw signals and results processed by three techniques for two point targets are exhibited undr such conditions. Therefore, at some points, the result of this paper si proposed for useful applications of unforcused technique in the restricted conditions except the identification of the small target at a long range re- quired for high resolution.

Comparative Analysis : The Patterns of Temporomandibular Disorder among Adolescents (청소년 측두하악장애 환자의 임상적 연구)

  • A retrospective study was conducted to determine the patterns of Temporomandibular Disorder(TMD) among adolescents (12-19 years) over last ten years. 174 patients who had visited the Oral medicine of Pusan National University Dental Hospital in 2000 and 491 patients in 2008 were found on chart review. The results obtained were as follows : 1. The number of adolescent patients was significantly increased in 2008 than in 2000, especially in male. 2. Bruxing, clenching, holding habits and bad sleep hygiene were highly increased more in 2008 than in 2000. 3. Osteoarthritis was significantly increased in 2008 than in 2000 and anterior disc dislocation without reduction was slightly increased. 4. The ratio of male to female adolescent patients with osteoarthritis was remarkably increased. Among the patients who had holding, bruxing, clenching habits, significantly increased osteoarthritis found to be common. This could imply holding, bruxing, clenching habits are prominently associated with increasing risk of developing osteoarthritis.

CORRECTION OF MICROSTOMIA BY BILATERAL COMMISSUROPLASTY USING "OVER AND OUT" BUCCAL MUCOSA FLAPS: REPORT OF A CASE (협점막 외전 피판을 이용한 양측성 구각성형술에 의한 소구증의 교정 1예)

  • Microstomia can be occurred as a result of direct injury to tissues such as chemical, thermal and electrical burns, and animal bites. It also may be secondary to contracture of burned perioral skin, or may result from scarring after reconstructive lip surgery. Narrowing of the oral aperture is not only disfiguring, but also limiting the oral access needed for introduction of food, insertion of dentures, oral hygiene, and dental treatment. Limited mouth opening may also interfere with mastication and speech. Few reports exist regarding correction of microstomia and reconstruction of the corners of the mouth. A 16-year-old girl with a bilateral cleft lip and palate presented with the limited mouth opening (approximately 20 mm), the esthetic problem due to the small lip, and the cleft lip-nasal deformity. The microstomia was corrected by bilateral commissuroplasty using "over and out" buccal mucosa flaps proposed by Converse. The intercommissure distance was increased from the preoperative 40 mm to the postoperative 60 mm. The one-year postoperative intercommissure distance was 54 mm, because the 6 mm relapse was occurred. The bilateral commissuroplasty using "over and out" buccal mucosa flap could increase the width and general size of the oral aperture and improve the lip appearance.

Factors Affecting the Appropriateness of Forest Land Transaction Price and Officially Assessed Land Price in Six Districts in Southern and Northern Parts of Han River in Seoul (서울시 강남·강북권 6개구(區) 임야 거래가격과 공시지가 적정성에 영향을 미치는 요인)

  • The purpose of this study is to identify the key factors affecting official land values and the appropriateness of the assessed land price, to find out what determines the real estate price and to assess the appropriateness of the valuation. This study explored whether actual transaction prices of forest land located in six Gu districts in southern and northern parts of Han river are appropriate using independent sample t-analysis and logistic regression analysis. Results showed that regional differences and shape were adequate for development restriction areas, whether biotope was designated, whether or not to be preserved, differences in pitch, and differences in use, and differences in bearing and approach. Thorough analysis of unique factors that determine forest land prices must be carried out in advance and the findings should be applied to the examination and assessment of official land values. The forest land appraisal system is closely related to the public's economic activity, thus it is necessary to apply forest land value determinants considered to be significant by market participants to the forestland appraisal system. I look forward to seeing variables related to the appropriateness of forest land transactions drawn from this study being used as indices for settlement of forest land transaction orders and market stabilization.

  • Treacher Collins syndrome(TCS) is an autosomal craniofacial development disorder which results from mutations in the gene TCOF1. Major features include midface hypoplasia, micrognathia, microtia, conductive hearing loss. Oral manifestations are characterized by cleft palate, shortened soft palate, malocclusion, anterior open bite and enamel hypoplasia. The purpose of this presentation is to describe the interesting aspects of dental treatment of a patient with TCS. A 6-year-old boy with TCS visited Seoul National University Dental Hospital for dental caries. Multiple caries was observed from clinical and radiographic examination. Because of multiple caries and behavior management ploblem, dental treatment under general anesthesia was planned. Treatment of posterior teeth was performed and some primary teeth were extracted. General anesthesia was induced and maintained with sevoflurane, nitrous oxide and oxygen. Under general anesthesia, successful dental procedure was done. Considering behavior management problem and medical condition of patient with TCS, general anesthesia can be useful.

  • Osteoarthritis in patients with temporomandibular disorders(TMDs) induces pain, limitation of mouth opening, occlusal problems, and most commonly affects their life quality. Control method and progressive process of osteoarthritis are being extensively researched. The researchers focus on histologic changes, synovial changes, muscular and ligamental changes and observed reaction to pain. Therefore most of them developed the animal model for osteoarthritis in TMD patients. In this study, we applied several methods which induces osteoarthritis of temporomandibular joint(TMJ) in rats or mice. For locally induce osteoarthritis in TMJ, Monosodium iodoacetate(MIA) or interleukin-$1{\alpha}$(IL-$1{\alpha}$) were injected into TMJ joint space for 5 or 3 weeks. Other groups are chosen for osteoarthritis under systemic control including hormonal changes and aging. To observe cellular change, increased collagen, degenerative bony destruction and distribution of proteoglycans (PGs), safranin-O staining and Masson's trichrome staining were used.
