자이로스코프의 이해와 응용

  • 강직성은 우주 공간에 대해 일정한 방향을 유지하려고 하는 성질로 뉴턴의 제1법칙인 관성력을 이용한 한 형태라고 할 수 있다. 강직성은 자체 회전축의 방향을 변화시켜려는 외력에 대항하여 자이로의 지지대를 기울여도 자이로의 축을 원위치로 유지하게 하는 성질이다. 이 성질은 3개의 변수인 로터의 질량, 반경, 회전속도에 좌우된다. 강직성을 이용하여 만든 자이로를 3축 자이로 혹은 Free Mounted Gyro라 하며 3축 자이로는 항공기 계기에서 자세기준, 방향기준을 설정하는데 이용된다.

Effects of Combined Postural Correction Exercises on Selected Muscle Activity and Stiffness in Children With Cerebral Spastic Diplegia: Case Study (융합적 자세교정치료가 뇌성경직양측마비 아동의 선택된 근육 활성도와 강직도에 미치는 영향: 사례연구)

  • The aim of this study was to investigate the short-term effects of combined postural correction exercises (CPCE) on muscle stiffness and muscle activity in children with cerebral spastic diplegia(CSD). The subjects were 3 children (2 male and 1 female) with CSD from 4 to 7 years old. The measurement sites were upper trapezius muscle(UT), rectus abdominis muscle(RA), and gluteus maximus muscle(Gmax), muscle stiffness and muscle activity were measured using Myoton and surface EMG, and the symmetry of both sides was also examined. As a result, children with severe compensatory action showed decreased muscle stiffness and muscle activity in UT and RA and increase in Gmax after CPCE intervention. Especially, the left and right symmetry of the muscles was decreased. In this study, despite some limitations, CPCE have shown a positive effect in posture correction of children with CSD.

  • Ankylosing spondylitis causes ankylosis of the spine due to ossification of the articular cartilage and ligaments around the vertebral body as well as the sacroiliac joint. This pathophysiology limits joint movement and, in many cases, causes pain and deformity of the spine. If this disease is left untreated, it ultimately causes ankylosis and ossification of the whole-body joints. The symptoms generally develop before age 30 years, and the gradual progression of the disease adversely affects the physical function, professional ability, and quality of life. This increases the likelihood of developing psychiatric disorders, such as depression. The authors are aware of this severity and introduce recent trends and studies to prevent surgical treatment with various medications before systemic ossification. This paper presents various surgical treatments and complications in patients who were unable to prevent progression and underwent surgical treatment.

Dynamic Behavior of Direct Fixation Track on Yeongjong Grand Bridge (영종대교 강직결 궤도구조의 동적거동에 관한 연구)

  • In this study, field measurements were performed to analyze the effects of train types (AREX, AREX Express, KTX, KTX-Sancheon) and train speeds on the dynamic behavior of the direct fixation track structure on Yeongjong grand bridge by bridge type (truss bridge, suspension bridge). Based on field measurement results, the track impact factor and train running stability (coefficient of derailment, Rate of wheel load reduction, lateral displacement of rail head) are compared with domestic and foreign standards and regulations to influence the dynamic behavior of direct fixation track. As a result, the differences in the dynamic behavior of the direct fixation tracks by the type of bridges of Yeongjong bridge are not significant, but it was analyzed that these were more directly affected by the magnitude of the train load. Therefore, it is necessary to establish the reinforcement plan of the direct fixation track structure on Yeongjong grand bridge in consideration of the increase of the track impact factor and dynamic track force.

Arthroscopic Adhesiolysis for Postoperative Arthrofibrosis of the Knee (수술후 발생한 슬관절 강직의 관절경적 유리술)

  • We retrospectively reviewed the results of arthroscopic adhesiolysis on postoperative knee stiffness in order to observe the clinical improvement and to evaluate the prognostic factors. Arthroscopic adhesiolysis without extraarticular procedure was performed in 31 knees displaying persistent flexion or extension loss. The causes of arthrofobrosis were previous ligament surgeries in 17 knees, surgery for a fracture involving the articular surface of the knee in 10 cases and other causes in four cases. The average range of motion was $60^{\circ}(range,\;14^{\circ}-74^{\circ})$ preoperatively, and improved by $120^{\circ}(range,\;7^{\circ}-127^{\circ})$ immediately following the procedure. The range of motion at the final follow-up (average 34 months) was $129^{\circ}(range,\;3^{\circ}-132^{\circ})$. In the 17 patients with arthrofibrosis fellowing ligament surgery, the range of motion was improved from $65^{\circ}$ preoperatively to $135^{\circ}$ at final follow-up. The improvement in function and motion was achieved during the first postoperative year. In the 10 patients with arthrofibosis following intraarticular fractures, the range of motion was improved from $60^{\circ}$ preoperatively to $125^{\circ}$ at the final follow-up, and most of the increase in motion was achieved within the first 6 months. Patients who suffered from a limitation of motion for less than 7 months gained an average $70^{\circ}$ improvement in total range of motion following arthroscopic surgery. However, the total range of motion in patients with a duration of symptoms greater than 7 months improved by an average $49^{\circ}$ postoperatively. In conclusion, arthroscopic adhesiolysis without incisional procedure is an effective therapeutic modality in arthrofibrosis of intraarticular origin. Improved outcomes can be expected in stiffness after ligament surgery and a symptom duration of arthrofibrosis less than 7 months.
