선천성 장애인의 재활서비스 비용에 영향을 미치는 요인 (Factors Affecting the Cost of Rehabilitation Services for People with Congenital Disabilities)

  • 본 연구는 선천성 장애인의 재활서비스 비용에 영향을 미치는 요인을 파악함으로써 재활서비스 이용의 접근성을 높이기 위한 기초자료를 제공하는데 목적이 있다. 연구대상은 2020년 장애인실태조사 자료에서 선천성 장애인 712명이며, 자료분석은 SPSS WIN 26.0를 이용하여 위계적 다중회귀분석을 실시하였다. 연구결과는 19-40세, 중산층, 가구소득, 바우처 사용, 기관 서비스 만족도, 자폐성 장애유형이 재활서비스 비용에 영향을 미쳤으며, 이중 바우처 사용이 재활서비스 비용에 가장 큰 영향을 미쳤다. 따라서 선천성 장애인의 재활서비스 비용을 완화하기 위해 바우처 사업의 대상을 개선하고 전 생애에 걸친 재활서비스 지원체계 구축을 제안하였다.

알러지 질환 대학생의 삶의 만족도에 관한 연구 (Life Satisfaction of College Students with Allergic disease)

  • 본 연구는 알러지 대학생의 삶의 만족도에 대한 영향요인의 성별 차이를 생태학적 접근을 통해 실시하였다. 2015년도 지역사회 건강조사 자료를 이용하여 알러지 질환을 의사로부터 진단받은 대학생 1,862명을 연구대상자로 선정하였다. 연구결과, 남학생의 삶의 만족도는 알러지 질환정도, 주간 아침식사일수, 친구와의 네트워크, 친척과의 활동, 종교적 활동, 자연환경과 의료서비스 여건에 대한 만족도로 7.9% (Adj R2=.079, F=12.550, p<.001)을 설명하였다. 여학생은 주간 아침식사일수, 친척과의 활동과 자연환경만족도로 삶의 만족도 3.0% (Adj R2=.030, F=10.282, p<.001)을 설명하였다. 알러지 질환 대학생의 삶의 만족도 향상을 위해서는 아침식사와 자연환경 만족도 향상을 위한 방안마련이 요구된다.

입원환자의 사회인구학적 요인 및 의료이용 특성과 환자만족$\cdot$불만족간의 관련성 (Inpatient Satisfaction and Dissatisfaction in Relation to Socio-demographics and Utilization Characteristics)

  • Purpose: This paper reports a study exploring factors related to patient satisfaction and dissatisfaction with inpatient care. Method: A cross-sectional study design was used, employing data from the National Health and Nutrition Survey conducted in 2001. Socio-demographic factors, utilization, self-rated health status, and disease characteristics were assessed by employing univariate comparisons and multivariate logistic regression analyses. Result: Out of 37,769 respondents, 1,043 aged 20 years and over had been admitted to a hospital or clinic at least once during the past year. About a quarter of the respondents were discharged from tertiary hospitals and $21\%$ from clinics. The majority of patients ($58\%$) were satisfied with inpatient care received, whereas $11\%$ were dissatisfied. Greater satisfaction was found in patients aged 45-64 years and those having formal education, discharge from tertiary hospitals, national health insurance as a payer, medical expenses not being burdensome, good self-rated health status, and neoplasm. Living in non-metropolitan urban areas, shorter length of stay, and musculoskeletal diseases were associated with greater dissatisfaction. Conclusion: Different factors were related to patient satisfaction and dissatisfaction with care. Those factors need to be taken into account when evaluating and comparing satisfaction levels between health care institutions.

장기요양서비스 노인 주 부양가족의 가족부담감, 가족지지, 우울, 만족도 평가 (Relationship between Family Burden, Family Support Depression, and Satisfaction of the Elderly Caregiver)

  • Purpose: This study was aimed to explain the relationship of family burden, family support, depression and satisfaction among caregivers of the elderly. Methods: The study population were 126 caregivers of 5 elderly residing in institutionalized nursing homes in Seoul and Kyunggi. The research tool of this study was a structured questionnaire on family burden, family support, depression and satisfaction. The data were collected from March 13 to April 30, 2008, and analyzed by Cronbach's alpha, descriptive statistics, t-test, ANOVA and Pearson's correlation coefficient and multiple regression by using the SPSS/WIN 14.0 program. Results: The major findings of this study were as follows: a significant positive correlation between family support and depression (r=.65, p=.001) was found. A significant positive correlation between family support and satisfaction (r=.68, p=.001) was perceived. Significant factors influencing Satisfaction were Family Support, which explained 38.2% of the variance of Satisfaction of Elderly Caregiver. Conclusion: This study suggested that we should develop programs and policies to increase the satisfaction of caregivers for their family support. In conclusion, the study was done to give suggestions to improve caregiver satisfaction of the aged and to serve as a basis for policy strategies by examining the current conditions of the nursing facilities.

조산원과 병원 분만 산모의 재원일 수, 의료비용, 산후불편감과 의료서비스 만족도 비교 (Length of Stay, Health Care Cost, Postpartum Discomfort, and Satisfaction with Medical Service in Puerperas Giving Birth in Midwifery Clinic and Hospitals)

  • Purpose: To determine traits related to pregnancy and delivery, length of stay, health care cost, postpartum discomfort, and satisfaction with medical service of puerperas giving birth in midwifery clinic and hospitals. Methods: This study used a comparative survey design. Data were collected from a total of 140 postpartum mothers composed of 70 mothers who gave births in two hospitals and another 70 mothers who delivered in one midwifery clinic. Results: Delivery in midwifery clinic had higher Apgar score at 1 minute and 5 minutes after birth than hospital. Those who delivered in midwifery clinic had shorter stay in the clinic, fewer health care cost, less postpartum discomfort in physical, environmental, social, and cultural areas, higher satisfaction with medical services than those who delivered in hospitals. Conclusion: Results of this study can be used as a basis for studies on giving birth in midwifery clinic and hospitals. They might increase the autonomy of women in giving birth with positive effect on the delivery experience of the mother and her spouse.

일 지역사회의 가정간호서비스 내용 및 만족도 조사 (A Study on Home Care Service and the Level of Client Satisfaction in Community)

  • Purpose : This study was done to examine the home care service provided by home care nurses and the level of client satisfaction in community. Method : Data were collected from 120 clients who received the home care service at the community home care service center in Pusan from 2th May. 2003 to 31th May 2003. The tool for measurement of satisfaction was composed of 16 items and was 4 score scale by Kim. Data were analyzed by using SPSS/WIN 10.5 program. Result: 1) 67.5% among 120 clients was over sixty years old and 30.6% of clients received home visits 20 times by home care nurse. 2) The total number of home care services was 15,783. And most of the clients received therapeutic nursing care. 3) The mean score of satisfaction on provided home care services was 3.31, among 16 items, "The home care nurses kept up the secret of patients and family" was the highest($3.67{\pm}0.48$). the total level of satisfaction of home care services was very high, with mean score $3.31{\pm}0.32$. Conclusion : The satisfaction level of home care services was very high. so we can expect that the prospect of home care services is very challengeable. Therefore we should try to expand the service recipients with promotional education to assure close ties with the public health centers for the continuous home care service linking and make more efforts to improve the quality of the service.

교육전담간호사가 주도하는 교육 프로그램에 대한 간호·간병통합서비스 병동 간호사의 교육 만족도 측정 도구 개발 및 타당도 검정 (Development and Validation of a Education Nurse Specialist-Led Education Satisfaction Scale for Nurses Working at Comprehensive Nursing Care Service Wards)

  • Purpose: The purpose of this study was to develop and validate a education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service wards. Methods: A methodological study was conducted. A total of 237 nurses working at comprehensive nursing care service wards in a general hospital participated in this study. The scale was developed through literature reviews, in-depth interview, development of preliminary items, verification of content validity, development of secondary items, evaluation of construct validity, and extraction of final items. Analysis included exploratory factor analysis, pearson's analysis, and reliability analysis using cronbach's α. Results: The education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service ward consisted of 21 items. Two factors (critical thinking disposition and clinical competency) were identified which explained 63.5% of the total variance. Cronbach's α of each factors were >.95. Conclusion: The results suggested that the education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service wards demonstrated acceptable validity and reliability. Items of the instrument can assess the level of satisfaction with regards to education led by education nurse specialist among nurses working at comprehensive nursing care service wards.

간호·간병통합서비스 병동 간호사와 일반 병동 간호사의 위임 준비성, 비판적 사고성향, 의사소통 능력이 직무만족에 미치는 영향 (Effects of Preparedness towards Delegation, Critical Thinking Disposition, and Communicative Competence on Job Satisfaction: A Comparison of Comprehensive Nursing Care Service Ward Nurses and General Ward Nurses)

  • Purpose: This study aimed to identify the effects of preparedness towards delegation, critical thinking disposition, and communicative competence on job satisfaction among comprehensive nursing care service ward nurses and general ward nurses. Methods: The participants in this study were 218 nurses from two university hospitals. Data were collected using self-administered questionnaires and were analyzed using SPSS Statistics/WIN 21.0, including descriptive statistics, x2 test, Fisher's exact test, t-test, ANOVA, Scheffeé test, Pearson's correlation coefficient, and multiple linear regression analysis. Results: For comprehensive nursing care service ward nurses, the significant influencing factors on job satisfaction were age (β=-.37, p=.041), marital status (β=.35, p=.001), critical thinking disposition (β=.29, p=.003), and communicative competence (β=.28, p=.002) with the explanatory power was 39.0% (F=12.00, p<.001). On the other hand, for general ward nurses, the significant influencing factors on job satisfaction were advanced degree (β=.29, p=.047), critical thinking disposition (β=.23, p=.040), and communicative competence (β=.42, p<.001) with the explanatory power was 33.0% (F=11.78, p<.001). Conclusion: The results of this study suggest the importance of developing and applying education programs to improve critical thinking disposition and communicative competence of nurses. This would enhance the comprehensive nursing service and increase overall nursing manpower resources. In addition, further studies are needed to improve nursing work environments and to develop strategies for enhancing preparedness towards delegation, which would increase the job satisfaction of nurses in the comprehensive nursing care service wards.

지역사회 통합돌봄 제도 연구 동향 분석: 주제 범위 문헌 고찰 (Research Trends in integrated Community Care Systems: A Scoping review)

  • 본 연구는 국내 지역사회 통합돌봄 제도에 관한 연구 동향을 파악하고 향후 연구 방향을 제시하고자 수행되었다. 주제 범위 문헌고찰방법으로 JBI방법론적 지침에 따라 진행하였다. 문헌검색은 5개의 DB(RISS, DBpia, NDSL, KISS, NAL)에서 실시하여 최종분석에는 34편의 논문이 선정되었다. 지역사회 통합돌봄과 관련된 이용자와 제공자 측면으로 분류하여 분석하였으며 이용자 측면 결과는 서비스 만족도, 신체적 건강상태와 관련된 변수가 가장 많이 사용되었고 긍정적인 결과가 나타났다. 제공자 측면 결과는 서비스 제공으로 인한 문제점과 개선점을 주로 연구하였으며 기관 간 협업체계 활성화의 필요성과 서비스 수행 인력 부족이 많은 연구에서 나타났다. 본 연구결과를 토대로 향후 지역사회 통합돌봄 제도의 확장에 있어 대상자 선정기준 확립 뿐 아니라 서비스 제공자의 전문성 확보, 지역 간 및 기관 간 협업체계 및 서비스 편차해소를 위한 대책마련이 필요할 것이다.

  • Purpose: This study attempted to identify differences in elders' health status, quality of life, and satisfaction with customized home visiting health service depending on connection to volunteerism. Methods: A total of 400 subjects participated in this research. Data were collected from May to August of 2009 and the measurement tool used for this study was the house visiting health service recording sheet recommended by the Ministry of Health. Results: According to the results of this research on elders' health status in customized home visiting health service depending on connection to volunteerism, elders' connected to volunteerism positively showed a high level in functional health status areas such as daily life performance ability, instrumental daily life performance ability and Joint exercise capacity, and in the quality of life area. Conclusion: The results of this study can be utilized as complementary information when resources and networks are used for the effective management of house visiting health service subjects.
