미인증 종합병원간호사의 환자안전관리 중요성 인식, 환자안전역량이 환자안전간호활동에 미치는 영향 (Effects of perceptions of the importance of patient safety management and patient safety competency on patient safety management activities among nurses at unaccredited general hospitals)

  • Purpose: This descriptive research study attempted to determine how general hospital nurses' awareness of the importance of patient safety management and patient safety competency affected patient safety management activities. Methods: From September 13 to 26, 2022, a survey was administered to 230 ward nurses who provided direct care to patients at five non-accredited general hospitals being evaluated for accreditation located in metropolitan cities. The collected data were analyzed with descriptive statistics, the t-test, one-way analysis, the Scheffé test, Pearson correlation coefficients, and hierarchical regression using SPSS for Windows version 26.0. Results: In total, 221 (96.1%) respondents were female. The average age was 32.2 years, and the average clinical experience was 3.5 years; 196 participants (85.2%) were general nurses. Patient safety competency (β = .44, p < .001), awareness of the importance of safety management (β = .31, p < .001), and medication error experience (β = -.15, p = .002) all had statistically significant associations with patient safety management activities. The explanatory power of these variables for patient safety management activities was 50.7%. Conclusion: This study confirmed that patient safety competency, awareness of the importance of patient safety management, and experience with medication errors significantly influenced patient safety management activities.

노인 당뇨환자의 당뇨 자가관리와 관련요인 (Factors Associated with the Diabetes Self-care Activities of the Elderly with Type 2 Diabetes)

  • 본 연구는 노인 당뇨환자의 당뇨 자가 관리 정도를 파악하고 관련 요인을 확인하기 위해 구조화된 설문지를 이용해 시행된 서술적 조사연구이다. 연구의 대상자는 당뇨로 진단받고 S시의 일개 종합 전문 의료기관에 외래진료를 위해 방문한 65세 이상의 노인 당뇨환자 264명이다. 연구결과 대상자의 당뇨 자가관리 행동 일수는 일반적 당뇨식이일수가 1주일 중 5.5일로 가장 많았고 특수 당뇨치료 식이 일수가 3.3일로 가장 낮았다. 일반적 특성과 질병관련 특성에 따라 당뇨 자가관리 행동의 차이가 있는 것으로 나타났으며 당뇨 자가관리 행동은 우울과 음의 상관관계가(r=-.278, p=.033), 일상생활 수행능력(r=.310, p=.013) 및 도구적 일상생활 수행능력(r=.415, p=.001)과 양의 상관관계가 있는 것으로 확인되었다. 본 연구결과의 의의는 노인 당뇨환자의 자가관리정도와 관련 요인을 확인한 결과를 바탕으로 자가관리 향상을 위한 간호중재 개발의 기초자료를 제시한 것 이다. 본 연구결과를 바탕으로 심리적 변인(우울)과 기능적 역량(ADL, IADL)에 따른 노인 당뇨 자가관리 중재접근이 필요할 것이다. 또한 환자 수준에 맞춘 개별화된 당뇨 자가관리 교육 및 관리도구 개발을 위한 추후 확대 연구의 시행을 제언한다.

병동선임간호장교의 간호관리역량 격차분석과 원인조사 (An Analysis of Nursing Managerial Competencies;Military Hospital Head Nurses)

  • The purpose of this study was to identify the gap between need-level and demonstration-level in nursing managerial competencies. In addition, the study proposes solutions to narrow this gap. The results of this study are as follows : 1) The mean score for need-level of each item was 4.0, and for demonstration-level, 3.5. This indicates that military hospital head nurses demonstrate a higher level of managerial competencies than the moderate level on all items. But items which were related to resource/ cost/ information managament, staff development management and professionalism management got relatively low ratings in the need-level. 2) The mean score for need-level of each category was 4.14, and for demonstration-level, 3.53. Categories on the individual dimension got a higher rating than categories on the group or organization dimension in both need-level and demonstration level. 3) The gap between need-level and demonstration-level appeared in all items(p<.05) and categories(p<.001). Although the gap was relatively low, it indicates that it is essential to plan a developmemt program for all nursing management competencies for military hospital head nurses. 4) There were significant differences in the gap between need-level and demonstration-level according to specific characteristics of the subjects. The gap did not appear in many categories on the individual dimension where the number of nursing staff was more than 10, a major grade, ICU head nurse or for head nurses having a long career. 5) Need-level and demonstration-level showed a difference according to specific characteristics of the subjects, because need-level and demonstration-level were higher where the number of nursing staff was more than 10, a major grade, and for ICU or Medical ward head nurses. The categories which showed need-level difference and demonstration-level differences according to specific characteristics of the subjects existed almostly completely in the group and organization dimension. Gap-level differences according to the number of hospital bed existed in only two categories. 6) The general causes of the gap were indicated to be 'Knowledge/ skill/ experience deficit', 'Limitation of rules and systems/ Inappropriate organizational environment' for most items, categories, and dimensions. The results of this study indicate that extensive competency developing strategies must be developed, because a gap was found in all items and categories. Specially, there is a need to concentrate attention on competencies in the group and organizational dimension which had a wider gap level. And it is important to take an individual approach according to the cause for each gap. In order to produce effective competency developing strategies, top managers must have sinsights into the importance of nursing staff development and nursing officer's efforts to develop themselves must be achieved. Further multi-dimensional(education, personnel-policy, nursing/ organizational environment) solutions to the gap must be developed and utilized.

미래지향적 역량교육을 위한 전문대학생 핵심역량 진단 도구 고도화 개발: Y대학 사례를 중심으로 (Advanced development of the core competency diagnosis tool for college students for future-oriented competency education: Focusing on the case of Y University)

  • 본 연구의 목적은 미래사회가 요구하는 인재를 양성하기 위한 역량 중심 교육 실행을 위하여 전문대학생이 갖추어야 할 핵심역량 진단 도구를 고도화 개발하고 타당도와 신뢰도를 검증하는 것이다. 이를 위해 선행연구 분석과 구성원 의견수렴, FGI, 전문가 자문을 통해 기존 진단 도구를 분석하고 예비문항을 도출하였다. 도출된 문항은 총 46문항이었으며 이는 내용 타당도로 검증하였다. 이후 재학생 중 희망자 380명을 대상으로 예비조사를 실시하였다. 구성 타당도 검증을 위해서 AMOS 18.0을 사용하여 탐색적 요인분석을 시행하였다. 그 결과 6개 핵심역량으로 구성된 30개 최종 문항을 도출하였다. 핵심역량 진단 도구는 재학생의 역량을 진단하여 향후 미래지향적 역량교육의 실행과 평가, 질 관리 도구로 적극적으로 활용될 것으로 기대된다.

역량강화 건강관리 프로그램이 도시독거노인의 자아존중감과 삶의 질에 미치는 효과 (The Effect of an Empowerment Health Management Program on Self-esteem and the Quality of Life for the Urban Elderly)

  • Objectives: The purpose was to test the effect of an empowerment health management program on self-esteem and the quality of life for the elderly who live in cities. Methods: Forty-three elders were randomly assigned to either the experimental group that received the intervention to participate in one year's empowerment practice (one home visiting per week and phone counselling) (n=21) or to the control group that got home visiting only (n=22). The data was analyzed using ANCOVA. Results: There was no significant difference in the self-esteem score between the two groups after the intervention. But the quality of life score of the experimental group was significantly different from that of the control group after the intervention. Conclusions: The health management intervention using empowerment for the elderly living alone with chronic diseases in urban areas had positive effects on the quality of life. Therefore this intervention can be broadly applied to other elders living alone with chronic diseases in urban areas.

방문건강관리사업 대상 허약노인의 건강증진행위, 주관적 건강상태, 사회참여 및 역량강화 (Health Promotion Behavior, Perceived Health Status, Social Participation and Empowerment in Frail Elderly Receiving Home Visiting Services)

  • Purpose: This research was conducted to identify methods of inducing health promotion behavior, perceived health status, social participation and empowerment in the frail elderly receiving home visiting services. Methods: The subjects were 255 frail elders aged over 65 registered in the home visiting services of five public health centers in Daegu. Data were collected from June 9 to August 10, 2015. Data were analyzed by descriptive statistics, t-test, ANOVA, Scheffe's test and Pearson's correlation coefficient using SPSS Win 18.0. Results: The mean health promotion behavior, perceived health status, social participation and empowerment scores were 2.56 (${\pm}0.33$), 7.11 (${\pm}1.98$), 2.60 (${\pm}0.69$) and 2.90 (${\pm}0.29$), respectively. There was significant difference in health promotion behavior by client classification and life satisfaction. There were significant differences in perceived health status by life satisfaction, social participation by religion and client classification and empowerment by past jobs. Health promotion behavior, perceived health status, social participation and empowerment were positively correlated. Conclusion: An integrative health care program that includes these significant variables of subjects is essential to management and prevention of deterioration of frailty in elderly.

간호학생의 재난인식, 재난준비도 및 자아탄력성에 관한 융합연구 (A Convergence Study on Disaster Awareness, Disaster Preparedness, and Ego-resilience of Nursing Students)

  • 본 연구는 간호학생들의 재난인식, 재난준비도 및 자아탄력성을 확인하고, 변수들간의 상관관계를 확인하기 위한 서술적 상관관계 연구이다. 연구대상자는 국내 2개 도시의 4개 대학에 재학 중인 간호학생 300명이었다. 자료수집은 2019년 4월 1일부터 4월 30일까지 시행되었고, 구조화된 자가보고식 설문지 작성을 통해 수집하였다. 자료분석은 SPSS 22.0 windows program을 이용하여, T-test, ANOVA, Pearson's correlation coefficient를 이용하였다. 연구결과, 재난인식, 재난준비도 및 자아탄력성 간의 유의한 양의 상관관계를 확인할 수 있었다. 이에 본 연구에서는 간호학생들의 재난인식, 재난준비도 및 자아탄력성을 향상시키고, 재난 관리 역량을 증진시킬 수 있는 융합 교육 프로그램의 개발을 제안한다.

혈액투석환자의 희망과 자가간호역량과의 관계 (A Study on the Relationship between Hope and Self-Care Agency in Hemodialysis Patients)

  • This study examined and identified the relationship between the level of hope and self-care agency in hemodialysis patients. The purpose of the study was to reveal that hope is an essential factor to enhance self-care agency and to prepare the basic materials to help nurse the patients. The subjects for this study consisted of 108 persons who undergoing hemodialysis from 2 hospital, 2 hemodialysis center in Pusan, Korea. The degree of hope was measured by the questionnaire that has been developed by Miller. The self-care agency was measured by the questionnaire that has been developed by Kearney B.Y & Fleisher B.J. The collected data were analyzed by descriptive statistics and t-test. one-way ANOVA and Pearson's Correlation Coefficient with SPSS WIN program. Results were obtained as follows: 1) The mean scores of hope were $131.77{\pm}25.77$(range 67-195). The rate of hope classified said that the lowest hope was 2.8% and low hope was 27.8%. 2) The mean scores of self-care agency were $143.78{\pm}23.63$(range 74-187). The degree of self-care agency classified said that the rate of self-care agency deficiency was 0.9% and low self-care agency was 12.0%. 3) The hope was significantly related to self-care agency. That is, the higher the hope score was, the higher the level of self-care agency score(r=0.668,p=0.001) was. In this result, there were hemodialysis patients who have very low hope score and self-care agency score. In this cases self-care agency can be increased by encouraging the patients to be more hopeful. Conclusively nurses should access the degree of hope and self-care agency of hemodialysis patients individually and apply the hope therapy in order that the patient have higher hope than before.

병원 간호단위관리자의 핵심직무 ­- 핵심역량 매트릭스 개발 (Development of the Core Task and Competency Matrix for Unit Managers)

  • Purpose: The aim of this study was to develop the nursing management core task and competency matrix for hospital unit managers. The perceived level of importance and performance of identified core competencies by unit managers were also investigated. Methods: Literature review and expert survey identified nursing management core task and competencies. Subsequently, the core task and competency matrix was developed and validated by expert panel. A survey of 196 nurse managers from 3 cities identified perceived importance and performance of core competiences. Results: Thirty-eight nursing management core task and thirty-seven nursing management core competencies were identified comprising five categories; Clinical practice knowledge, Evidence-based practice, Employee development, Strategic planning and Initiative. Based on the core task and competencies, the task and competency matrix for unit managers was developed. In the analysis of importance and performance of core competencies, the mean score of importance ($3.50{\pm}0.30$) was higher than the mean score of performance ($3.03{\pm}0.34$). Conclusion: The development of core task and competencies for unit managers in hospitals provides a guide for the development and evaluation of programs designed to increase competence of unit managers.

A Comparison of the Clinical Competence, Knowledge of Patient Safety Management and Confidence of Patient Safety Management according to Clinical Practice Experience of Nursing Students

  • 본 연구의 목적은 간호대학생의 임상실습 경험에 따른 임상수행능력, 환자안전관리지식 및 환자안전관리 수행자신감의 차이를 비교하기 위함이다. 간호대학생 3학년을 대상으로 임상실습을 경험한 73명, 임상실습을 경험하지 못한 35명으로 총 108명 분석하였다. 분석방법으로는 동질성 검정은 X2-test, 두 그룹간의 차이비교는 independent t-test로 분석하였다. 연구결과로 두 그룹간의 임상수행능력(t=.88, p=.377), 환자안전관리지식(t=-.29, p=.773) 및 환자안전관리 수행자신감(t=1.11 p=.267)의 차이는 통계적으로 유의하지 않았다. 두 그룹에서 각 변수의 하위영역별 가장 점수가 낮은 항목을 살펴보면, 첫째, 임상수행능력에서는 간호과정에 대한 영역에서 대체로 점수가 낮았고, 둘째, 환자안전관리 지식은 근접오류에 대한 개념에 대한 지식 측정 영역에서 가장 낮았으며, 셋째, 환자안전관리 수행자신감에서는 오류 발생 시 사건 보고서 작성에 대한 수행 자신감 영역에서 점수가 가장 낮은 것으로 확인되었다. 따라서 간호대학생의 임상수행능력의 질적 향상을 위해서 임상실습교육환경 개선이 절실하게 요구되며, 간호대학생의 환자안전관리 역량 증진 및 환자안전관리에 대한 올바른 태도 함양을 위해 전략적인 교육지침의 개발이 필요하다.
