Accuracy of Astigmatic Correction Using Toric Intraocular Lens by Position and Size of Corneal Incision (각막절개 위치와 크기에 따른 난시교정인공수정체의 난시교정의 정확성)

  • Purpose: To assess the accuracy of toric intraocular lens (IOL) implantation by the location and size of the corneal incision. Methods: We retrospectively reviewed the medical records of 98 patients (98 eyes) who underwent phacoemulsification with toric IOL implantation from January 2014 to March 2017. The patients were divided into two groups: group 1 got an incision of the superior side of the cornea (n = 54) and group 2 received an incision on the temporal side of the eye (n = 44). For both groups, incisions were made at their steep corneal astigmatism axises. Each group was further divided into subgroups for whom different sized blades were employed (2.75 vs. 2.2 mm widths). We measured the refractive index and autokeratometric parameters. We postoperatively assessed residual astigmatism and any reduction thereof. Results: In both groups, uncorrected and best-corrected visual acuity, refraction cylinder astigmatism, and autokeratometric astigmatism improved statistically. Between two groups, corneal astigmatism decrease was not significant. Residual astigmatism also showed no significant differences between the two. Patients in both groups treated using 2.75 mm wide blades exhibited greater increases in corneal astigmatism. Conclusions: During cataract surgery, precise correction of astigmatism via toric IOL implantation is possible when surgically induced astigmatism is minimized by careful choice of the location and size of the corneal incision.

The Changes of The Eye and a Correlation Depending on Watching a Smartphone and taking in Alcohol (스마트폰 시청과 알코올 섭취에 따른 눈의 변화와 상관관계)

  • Purpose: This article tried to find the changes of the eye and a correlation depending on watching a smartphone and taking in alcohol. Methods: This paper conducted the refraction inspection, the radius of corneal curvature, tonometry and the corneal thickness measurement and analysis for 31 persons (18 males and 13 females) who have healthy drinking habit, not having the medical history of taking medicine related with disorders of the eye, a mental disease, systemic disease and alcohol metabolism. Results: Of respirations depending on watching a smartphone, the alcohol concentration was significantly decreased in 15 minutes, 30 minutes, 45 minutes, 60 minutes and the convalescent stage. (p<0.01, p<0.001) As the result of comparing the refraction inspection after and before watching a smartphone when drinking alcohol, there was a significant difference in the cylindrical power and the axis. (p=0.005, p=0.001) The change of intraocular pressure depending on watching a smartphone after drinking alcohol was decreased significantly from 30 minutes (p<0.001), and watching a smartphone didn't have any significant effect on the corneal thickness depending on the elapsed time from when drinking alcohol. Conclusions: Watching a smartphone and taking in alcohol had a significant effect on the cylindrical power and the axis. Watching a smartphone and taking in alcohol which may cause the visual function-strain are the factors that need to consider before the refraction inspection.

Comparison of Anterior Segment Features between Groups with or without Glaucoma in Pseudoexfoliation Syndrome (거짓비늘증후군에서 개방각녹내장 병합 유무에 따른 전안부 소견과 생체 계측치에 대한 비교)

  • Purpose: To investigate the factors associated with glaucoma in patients with pseudoexfoliation syndrome by comparing features of the anterior segments and ocular biometry according to the presence or absence of open-angle glaucoma in pseudoexfoliation syndrome. Methods: We analyzed 96 patients (115 eyes) diagnosed as having pseudoexfoliation syndrome in this study. The patients were divided into two groups of simple pseudoexfoliation syndrome (64 patients, 76 eyes) and pseudoexfoliation glaucoma (32 patients, 39 eyes). We compared the age, sex, underlying disease, location of pseudoexfoliative material, iris change, degree of nuclear cataract, pupil dilatation, corneal endothelial cell counts, central corneal thickness, anterior chamber depth, axial length, corneal curvature, and intraocular pressure (IOP). Results: There were no significant differences between the two groups in terms of age (p = 0.694), sex (p = 0.161), diabetes (p = 0.440), hypertension (p = 0.238), pseudoexfoliative material observed in anterior capsule (p = 0.700), pupillary margin (p = 0.210), iris depigmentation (p = 0.526), pupillary ruff loss (p = 0.708), degree of nuclear cataract (p = 0.617), pupil dilatation (p = 0.526), central corneal thickness (p = 0.097), anterior chamber depth (p = 0.283), axial length (p = 0.095), or horizontal and vertical corneal curvature (p = 0.066 and 0.306, respectively). In pseudoexfoliation glaucoma, significantly higher IOP (p = 0.026), a high frequency of membrane formation (p = 0.047), and decreased corneal endothelial cell counts (p = 0.048) were observed. Conclusions: Pseudoexfoliation syndrome with open-angle glaucoma was shown to be associated with high IOP, decreased corneal endothelial cell counts, and a high frequency of membrane formation. Therefore, when such changes are observed in pseudoexfoliation syndrome patients, a higher risk of open-angle glaucoma should be recognized, and careful attentionis required accordingly.

Relationship of Refractive and Anatomical Changes on Eyes after Alcohol Ingestion (알코올 섭취에 의한 눈의 굴절변화와 해부학적 변화와의 관계)

  • Purpose: This study was to investigate the relevance between refractive and anatomical changes temporarily on the eyes after alcohol ingestion. Methods: Eight subjects (16 eyes) which were $24.5{\pm}1.5$ aged males drunk the alcohol of 0.42 g per kg of body weight within 30 minutes. Refractive errors, the radius of corneal curvature, corneal thickness, pupillary size, intraocular pressure, and the length of the ocular axis at 1 h, 4 h, and 24 h after alcohol ingestion were compared with them of non-alcoholic state. Results: At 1 h after alcohol ingestion, breath alcohol concentration was the highest (p<0.001), more negative spherical power was needed (p<0.05) for correction, pupillary diameter was decreased (p<0.05), intraocular pressure was decreased (p<0.001), and the length of the ocular axis was increased compared with each one of non-alcoholic state. At 4 h after alcohol ingestion, all anatomical changes were the same tendency as at 1 h after alcohol ingestion. But at 24 h after alcohol ingestion, both refractive changes and anatomical changes were not significant compared with them of non-alcoholic state. Conclusions: Temporary changes of refractive error after alcohol ingestion may be related with decrease of intraocular pressure and increase of the length of ocular axis.

The Change in Corneal Eccentricity on the Correction of Refractive Error using Reverse Geometry Lens (역기하렌즈(Reverse Geometry Lens)의 굴절교정시 각막 편심률(Eccentricity)의 변화)

  • Purpose: In this study we investigated the correlation between the effect of myopia correction and the change of corneal eccentricity using reverse geometry lens. Methods: The 23 students (46 eyes) continuously wearing reverse geometry lens during 3 months were divided into Group I and Group II by different parameter fitting methods of wearing Reverse Geometry Lens. We measured a corneal eccentricity for Group I and Group II at $10^{\circ}$, $20^{\circ}$, and $30^{\circ}$ positions from corneal apex before wearing reverse geometry lens, 1 week, 1 month, and 3 months after wearing reverse geometry lens. We also measured an uncorrected visual acuity, a spherical equivalent, and a corneal radius and analyzed the correlation between them and the change of corneal eccentricity using statistical significance test. Results: There were the statistical significances of a change of corneal eccentricity (p=0.03, t=-2.29) for Group I and Group II at 10 position from corneal apex in a week after wearing reverse geometry lens, but were not those (p>0.05) in 1 month, and 3 months after wearing reverse geometry lens. There were the statistical significances of correlation between the change of corneal eccentricity and a corrected visual acuity, and a corneal radius, respectively. Particularly, the high correlation between the change of corneal eccentricity and a corrected visual acuity (r=-0.36, p=0.00, t=6.5), and a spherical equivalent (r=-0.72, p=0.00, t=-70.5) for Group II in a week after wearing reverse geometry lens showed. Conclusions: We knew from these results that the high correlation between the effect of myopia correction and the change of corneal eccentricity in a week after wearing reverse geometry lens represented.

Analysis on Forces Acting on the Contact Lens Fitted on the Cornea (콘택트 렌즈에 작용하는 힘의 해석)

  • A mathematical model is proposed to analyze the force; acting on the hard contact lens fitted on the cornea. The model incorporates the nonlinear equations and their numerical solution program, based on the formulations of surface tension force arising from the capillary action in the tear-film layer between the lens and cornea. The model simulates how the adhesion between lens and cornea varies according to the base curves and diameters of the lenses. When the spherical lens is fitted on the spherical cornea it is to rotate downward due to the weight of lens itself until it reaches an equilibrium position along the cornea where the counter(upward) moment caused by net force between the upper and lower portion of the periphery of lens. It is found that both the adhesion and displacement of lens along the cornea, where the gravity of lens balances the capillary-induced upward force, increases rapidly as the base curve of lens increases, i.e., as the lens gets flatter, while the increase in the diameter of lenses has resulted in the less increase in the rotation and adhesion. With the base curve and diameters of lenses being remained constant the increase in surface tension of tear film yields the increase in the adhesion between the cornea and lens while the initial rotation of lens is inversely proportional to the surface tension of the tear film.

The Clinical Study on the Visual Acuity and Cornea of ez-NANOsence II RGP Contact Lens (ez NANOsence II RGP 콘택트렌즈의 시력과 각막에 미치는 임상적 연구)

  • The purpose of this study was to perform a clinical test using ez NANOsence II RGP contact lenses for the effects of visual acuity and cornea on subjects with abnormal refraction status. One hundred twenty one adults (twenty nine males, ninety two females, range = 17 to 43, mean = 22.86) were recorded. The subject's history including, the symptoms from previous lens wear, were studied. The subjects were observed, both pre and post lens wear, for any symptoms and signs of change of the eye. The Visual acuity and the binocular status were tested at the far distance using the contact lens. The refraction test was performed on the naked eye using the objective method (Topcon KR-8100, Japan). The Stereopsis test was performed at the near distance after contact lens wear by the Titmus fly (Stereo Optical Co., U.S.A) and TNO (TECH, The netherlands). The contrast sensitivity diagnosis was performed at 1m distance after contact lens wear by the contrast sensitivity chart (pelli-Robertson, USA). The corneal topography was analyzed on the naked eye after lens wear by ORB scan (Bausch Lomb, U.S.A.). The ultra structure of surface on the contact lens was observed using SEM (JMS-5800, Japan). The chemical component's of the contact lens was analyzed by EDS program. The results of this study were as follows: 1. The longer of contact lens wear period, The subjects have acquired the most improved visual function. 2. Subjects had experienced few side effects wearing the contact lenses. 3. The lenses were easy to use by the subjects. 4. The longer of contact lens wear period, the power of cornea had more decrease. but the base curve of cornea had more increase and corneal astigmatism was decrease (p<0.01). Also, the thickness of corneal center was few decrease. 5. Longer periods of using the contact lens showed stereopsis and contrast sensitivity at more normal values. 6 The corneal topographical after lens wear showed most subjects with similar morphology for different wear periods. 7. The surface ultrastructure of the new and used contact lens was the similar fine shape. 8. The chemical component's of in the new and used contact appeared to have similar results. In conclusion, this study showed that the surface ultrastructure and chemical component's of the new and used contact lens are similar. In addition, the subjects had improved the quality of vision and few experienced any side effects during long periods of contact lens wear. Also they have decrease of corneal astigmatism during the long period of lens wear. Our test has showed that the chemical composition and fine structure of contact lens have related to the visual function on contact lens wearer. In this paper, we suggested that ez NANOsence II RGP contact lenses had a moderate effect for correcting vision of abnormal refraction eye.

The Study of the Corneal Diopter Power and Corneal Radius on the Contact Lens Wear of the Korean Female in the Twenties Age (20대 한국여성의 콘택트 렌즈 착용자에 대한 각막 굴절력과 곡률반경에 대한 연구)

  • An analysis on the corneal diopter power and corneal radius has been carried out the Korean female in the twenties age. The results were as follows. The most contact lens wears have the early stage of the twenties age, but the increase of aging have a decrease. The mean corneal diopter power on the right eye have 44.04 diopter, but left eye have 43.92 diopter. The mean of corneal radius on the right eye have the 7.66 mm, but left eye have the 7.69 mm. As a results, In increasing of the age, Most mean corneal diopter power have increase, but the mean of corneal radius have not change.

Predicting Powers of Spherical Rigid Gas-permeable Lenses Prescription (구면 RGP 렌즈의 처방 굴절력 예측)

  • Purpose: Usefulness in predicting the power of spherical rigid gas-pearmeable (RGP) lenses prescription using dioptric power matrices and arithmetic calculations was evaluated in this study. Noncycloplegic refractive errors and over-refractions were performed on 110 eyes of 55 subjects (36 males and 19 females, aged $24.60{\pm}1.55$years) in twenties objectively with an auto-refractometer (with keratometer) and subjectively. Tear lenses were calculated from keratometric readings and base curves of RGP lenses, and the power of RGP lenses were computed by a dioptric power matrix and an arithmetic calculation from the manifest refraction and the tear lens, and were compared with those by over-refractions in terms of spherical (Sph), spherical quivalent (SE) and astigmatic power. Results: The mean difference (MD) and 95% limits of agreement (LOA=$MD{\pm}1.96SD$) were better for SE (0.26D, $0.26{\pm}0.70D$) than for Sph (0.61D, $0.61{\pm}0.86D$). The mean difference and agreement of the cylindrical power between matrix and arithmetic calculation (-0.13D, $-0.13{\pm}0.53D$) were better than between the others (-0.24D, $0.24{\pm}0.84D$ between matrix and over-refraction; -0.12D, $0.12{\pm}1.00D$ between arithmetic calculation and over-refraction). The fitness of spherical RGP lenses were 54.5% for matrix, 66.4% for arithmetic calculation and 91.8% for over-refraction. Arithmetic calculation was close to the over-refraction. Conclusions: In predicting indications and powers of spherical RGP lens fitting, although there are the differences of axis between total (spectacle) astigmatism and corneal astigmatism, Spherical equivalent using an arithmetic calculation provides a more useful application than using a dioptric power matrix.

The Efficacy and Effect of Reverse Geometry Contact Lens on Cornea (역기하학 렌즈의 유효성과 각막에 미치는영향)

  • Object of this research is to estimate the effect of myopia correction and safety on reverse geometry contact lens fitting in school children. This research include 53(106eyes) schoolchildren among 7 to 18 years who has low to moderate myopia(-1.00D~-5.00D) and prescribed reverse geometry contact lens for purpose on orthokeratology between January to July 2004 and had 3months full follow up examination. They were tested for slit lamp examinations, BUT(Break up time), direct ophthalmoscopy, retinoscopy, uncorrected visual acuity, best corrected visual acuity, autorefraction, autokeratometry and corneal topography in each examination(1day, 1week, 2weeks, 1, 2, and 3months) of before-and-after lens wearing to find out the effect of myopic correction and side effect. The results came out as follow. The average of uncorrected visual acuity was $0.0938{\pm}0.378$ before lens wear and $0.3136{\pm}0.283$ after 1day lens wear, and there was fast improvement after 1week($0.7925{\pm}0.301$) and little improvement after 2weeks period but still they shows better uncorrected visual acuity(p<0.01). The result of this study, the reverse geometry lens is very useful to correct refractive error and control the progression of myopia temporally among low to moderate myopic patient. The side effects were relatively rare but further study should be necessary with long term lens wear effect on eye health. For the lens prescription, the clinical fitting process had higher rate of success with consideration of eccentricity and corneal topography.
