Competencies of Nurses in a Maternal Early Childhood Sustained Home-Visiting Program (영유아 가정방문간호를 위한 간호사의 역량)

  • Purpose: Competency is a key foundation of the nursing education curriculum and a baseline element of nursing practice. The purpose of this study was to explore the competencies of community health nurses in a maternal early childhood sustained home-visiting program based on nurses' field experiences. Methods: The participants were 21 nurses who had more than 1 year of experience in this program. Reflective interviews were performed; lasting 1 hour per nurse, on August 18, 2016, and the collected data were analyzed using qualitative directed content analysis. Results: Twenty-four themes were extracted for 9 competencies. The major themes included implementing a long-term relationship-based approach, providing client-led service rather than expert-led service, helping mothers with psychosocial difficulties, and applying skills in a practical manner in the home environment. Conclusion: In order to develop a home-visiting program as part of maternal-early childhood nursing practice, nurses should develop competencies that help them make significant and positive interventions. Results indicate that the curriculum for community health nurses should be changed to improve competencies in building relationships with clients and to focus on the application of skills in specific cases and in clinical practice.

Parenting Stress, Parenting Guilt Feelings, Separation Anxiety and Alternative Care in Hospital-Nurses (종합병원 간호사의 양육스트레스, 양육죄책감, 격리 불안 및 대리양육)

  • The purpose of this study was to investigate on parenting stress, parenting guilt feelings and separation anxiety and alternative care in nurses. Data were collected from nurses with preschooler at 2 hospitals from August 2013 to January 2014. Self reported questionnaires were used to collect data from 163 nurses. There were significant differences according to the age, satisfaction of work place and co-worker, spouse support of parenting in parenting stress, parenting guilt feelings and separation anxiety in common. The majority type of alternative care was surrogate foster home when they went to work. However, there were significant differences according to type of alternative care in parenting stress, parenting guilt feelings and separation anxiety. Particularly, those who use child day-care center and surrogate foster home showed more parenting guilt feelings and separation anxiety in Scheffe post-hoc comparison. As a result, it is needed to be improvement of working conditions and policy for nurses' parenting support to continue work.

Introduce and Promote the Home-based Hospice and Palliative Care (가정호스피스·완화의료 제도 도입을 위한 국민 인식도 조사)

  • Purpose: This study was conducted to understand public perception of home-based hospice and identify related factors. Methods: Between August 19, 2014 and August 30, 2014, data were collected using an E-mail questionnaire that was filled by 1,500 adults who were over 20 years of age. Data were analyzed using descriptive statistics, ${\chi}^2$-test and logistic regression. Results: Among the respondents, 15.9% were aware of home-based hospice care, and 61.3% were willing to receive home-based hospice care. The factors that influenced the participants' willingness to use home-based hospice services included residential district, religion and private health insurance. Respondents who lived in Seoul (OR: 1.56, 95% CI: 1.04~2.33), Gwangju/Jeolla province (OR: 2.02, 95% CI: 1.23~3.32), Busan/Ulsan/South Gyeongsang province (OR: 1.81, 95% CI: 1.17~2.82) were more well-aware of home-based hospice care than those who lived in Incheon/Gyeonggi province. The faithful were more informed about the services than those without non-faithful participants (Roman Catholics (OR: 2.03, 95% CI: 1.30~3.17), Protestants (OR: 1.76, 95% CI: 1.22~2.53). Participants who had a private health insurance plan knew more about the services than those without one (OR: 1.45, 95% CI: 1.03~2.04). Conclusion: First, it is necessary to improve perception of the public and healthcare providers regarding home-based hospice care. The government should review a measure to institutionalize operation of a palliative care team at hospitals and community home-based hospice care centers.

Analysis of Hospital-based Home Care Service Utilization Using National Health Insurance Claim Data from 2008 to 2017 (건강보험 자료를 이용한 전국 의료기관 가정간호 실시 및 이용 현황 분석: 2008년-2017년)

  • Purpose: The purpose of this study is to analyze the status quo of hospital-based home care utilization in Korea from 2008 to 2017. Methods: Data from a total of 1,396 medical institutions, 350,390 patients, and 3,563 home care nurses were analyzed using claim data from the Health Insurance Review & Assessment Service. Results: The number of hospital-based home care agencies decreased from 177 in 2008 to 115 in 2014. This number started to increase in 2015 and reached 179 in 2017. The number of hospital-based home care patients declined from 35,056 in 2008 to a low of 26,848 in 2013. This number started to increase in 2014 and reached 67,863 in 2017. Essential hypertension was the most common disease among hospital-based home care patients from 2008 to 2015. The number of hospital-based home care visits declined from about 500,000 in 2008 to a low of 362,000 in 2013. This number started to increase in 2014 and reached 658,000 in 2017. Conclusion: It is necessary to vigilantly monitor hospital-based home care agencies, patients, and the utilization of services. This may help establish platforms for providing community and home-based nursing services for the super-aged society in Korea.

A Study on the Recognition on Home Care Services by Nurses, Doctors and General Employees (일 종합병원의 가정간호사업에 대한 의사, 간호사 및 일반직원의 인식 조사)

  • Purpose: The purpose of this study was to provide the data for improving home care services through investigating the recognition of home care services by nurses, doctors and general employees. Method: The data were collected from 231 nurses, 103 doctors and 157 general employees who were working at a general hospital in Seoul from Sept 11 to Sept 24, 2006 and analyzed using descriptive statistics and ANOVA. Results: The recognition on necessity of home care services was lower in doctors(M=2.73, SD=.39) and general employees (M=2.83, SD=.31) than nurses(M=3.13, SD=.39). In terms of the recognition on content of home care services, the affirmative percentages of most items were lower in doctors and general employee than nurses. The recognition on effects of home care services was also lower in doctors(M=2.90, SD=.32) and general employees(M=2.99, SD=.31) than nurses(M=3.26, SD=.35). Conclusion: This study indicates that the continuous educations and advertisements on home care services are necessary to facilitate home care services in general hospitals.

A Study on Recognition Regarding Hospital-Based Home Care Service: With the Subject of the Study Selected among Physicians and Nurses in a Hospital (병원중심 가정간호사업에 대한 인식 조사연구 -의사, 간호사를 중심으로-)

  • Purpose: This study is to investigate the recognition regarding Hospital - Based Home Care Service among physicians and nurses in a G University Hospital. Method: Data were gathered from 92 physicians and 257 nurses. working at G University Hospital. from January 15 to January 22. 2003 by questionnaire. The data were analyzed by using the SPSS/PC +. Result: 1) As to the previous information about a hospital based home care service. those who have been familiar to it were 85.9% of the physicians. and 98.8% of the nurses. 55.4% of the physicians and 55.1% of the nurses responded that they are willing to refer their patients to the home health care only if their patients and families want to. 47.8% of the physicians and 44.4% of the nurses perceived the present cost of a hospital based home care service to be moderate. 2) Most of the physicians and nurses reported that the most available service was 'wound dressing' (98.4%, 92.6%) and the least available service was 'incision and drainage'(1l7.4%, 42.8%). 3) As to the necessity of home nursing business and the anticipated effects. nurses perceived higher than physicians($3.46\pm.74$). and both showed a significant difference according to age of subjects. Conclusion: For the stabilization and successful implementation of a hospital based home care service. it should be accompanied with education programs about home care for physicians. in particular junior staff.

A Survey on the Job Stress of Home Care Nurses (가정전문간호사의 업무 스트레스)

  • Purpose: The purpose of this study was to investigate the job stress in home care nurses. Method: The subjects were 139 home care nurses of hospital in an urban cities. The data were collected from 26 Man to 27 June by self report adherence questionnaire. Job stress was measured using CHN-PWRSQ(Community Health Nurses Perceptions of Work-Related Stressors Questionnaire). Result: The average score of job stress was 60.8. There were significantly high levels of job stress in those who have less than 10 years office period, those who have less than 2000000 won monthly salary than in their counterparts. With regard to the level of job stress by item, 'driving long distance and traffic jam', 'risk of traffic accident', 'bad weather condition', 'uncooperative family members', and 'uncooperative client/patient' were higher than other items. The age and office period were negatively correlated with job stress. Conclusion: The average score of job stress was 60.8. Workload and Job Satisfaction of Home Health care Nurses in Korea (한국 가정전문간호사의 업무량과 직무만족도)

  • Purpose: This study aimed to identify the workload of home healthcare nurses and their job satisfaction. Methods: The research was conducted on 87 home healthcare nurses in nationwide medical institutions. Results: Number of visits was correlated with direct nursing hours. The monthly average number of visits was 108.84 ; the average number of patients was 45.66; the average daily working hours were $8.95{\pm}0.95$, of which $5.43{\pm}1.06 $ were indirect nursing hours, and $3.57{\pm}1.05$ were direct nursing hours. The average nursing time by severity condition was 22.25 minutes for group 1 patients, 28.04 minutes for group 2, 34.97 minutes for group 3, and 42.88 minutes for group 4. Of possible scores out of 5 for the job satisfaction of home healthcare nurses, their satisfaction was low for their welfare ($2.68{\pm}1.12$), workload ($2.90{\pm}0.88$), and enough time to work ($2.98{\pm}0.90$). Conclusion: The severity of patient's conditions influenced the workload of home healthcare nurses. Although workload did not correlate significantly with job satisfaction, it is necessary to devise a measure for manpower so that home healthcare nurses can secure sufficient nursing hours when they visit clients.

A Study of Role Awareness, Role Conflict and Job Satisfaction for Hospital-based Home Care Nurses (병원중심 가정전문간호사의 역할인식, 역할갈등 및 직무만족)

  • Purpose: This study is a descriptive research conducted for looking into the role awareness level. role conflicts, and job satisfaction among professional home care nurses in the field of hospital based home care nursing service. Method: 154 specialized home care nurses in 63 hospitals and clinics that are implementing home care programs were subject to the study. Result: 1. The level of role awareness among professional home care nurses was $4.47\pm0.41$ on average out of 5. 2. The average score for role conflicts was $3.87\pm0.61$ out of 5. 3. In terms of job satisfaction level. $3.33\pm0.71$ was scored out of 5. 4. It has been discovered that there are statistically significant positive correlations among role awareness level, role conflicts(r= .224, P= .005), and job satisfaction level(r= .166, P=.040), while a negative correlation was detected between role conflicts and job satisfaction level(r=-.210, P= .009). Conclusion: The level of role awareness among professional home care nurses in the field of hospital based home care nursing service was very high and job satisfaction level was high when the level of home care nursing antecedents was high and the organization structure of working place was independent. But the level of role conflicts coming out of the weakness of job-supporting system was high. So the realistic alternative such as the improvement of working environment by administrative, institutional support, is required to make up for this problem in the future.

A Study of Factors Influencing Morale of Hospital based Home Care Nurses (병원중심 가정전문간호사의 직무관련 사기(士氣) 정도)

  • Purpose: The purpose of this study was to examine morale and related factors of hospital based home care nurses. Method : The subjects were 159 home care nurses from 94 hospitals and clinics which were operating home care programs around the country. Data were collected for 40days from March 14, to April 24, 2005. The questionnaire consisted of 51 items including 12 general variables and 39 items of nurses' morale. The reliability of the questionnaire by Cronbach's ${\alpha}$, was .88. Result : The level of the morale was found as mean score 2.69 in 4 point scale The high ranks of morale were self actualization($3.05{\pm}0.43$) and job satisfaction($3.03{\pm}0.43$), the factors which showed lower points were evaluation of work ($2.47{\pm}0.53$), welfare($2.42{\pm}0.42$), promotion system ($2.35{\pm}0.45$) and wages($2.23{\pm}0.54$). The level of morale according to the general variables were significantly different in such variables ; home care nursing antecedent(p = .000), motivation for job selection(p= .030), intention to quit the job(P= .000). Variables of intention to quit the job(15.7%) and home care nursing antecedent; 6.7%(p=.001) showed 22.4% of explanatory persuasion effect on level of morale. Conclusion : To improve a morale of home care nurses, the arrangement of nursing department should be consider nurse's aptitude and interest and allow them to have longer period of work in that part. Also wages, promotion system and welfare should be reformed as relevant as their career.
