진통제-근육이완제 복용 병행 척추교정과 진통제-근육이완제 복용이 만성허리통증 환자의 통증, 허리 관절가동범위, 장애 지수에 미치는 효과 비교 (Comparison of the Effects of Spinal Manipulation Combined with Painkiller and Muscle Relaxant to Painkiller and Muscle Relaxant Alone on Pain, Lumbar Range of Motion, and Disability Index in Patients with Chronic Low Back Pain)

  • Purpose : This study aimed to compare the effects of spinal manipulation combined with medication on low back pain (LBP), range of motion, and disability in patients with chronic LBP. Methods : Twenty patients with chronic LBP were included in this study. The participants were randomly assigned to the spinal manipulation with medication group (n=10) or the medication only group (n=10). The intervention group received spinal manipulation for 15 minutes, twice a week, and took medication twice a day for eight weeks. The control group received the medication twice daily for eight weeks. Pain intensity assessed using the visual analog scale (VAS), range of motion, and disability due to LBP assessed using the Oswestry disability index were measured before and after the intervention. Results : The intervention group showed a significant improvement in pain intensity compared to the control group (p<.05), and the intervention and control groups significantly improved low back pain after the intervention (p<.05). The intervention group showed a significant improvement in the range of motion in flexion, extension, right lateral flexion, left lateral flexion, and right rotation (p<.05). The intervention group also showed a significant improvement in the change of disability in total score, pain intensity, personal care, lifting and standing compared to the control group (p<.05). Conclusion : This study showed that the combination of spinal manipulation and medication can benefit patients with chronic LBP, as evidenced by significant improvements in pain intensity, ROM, and disability. These findings suggest that utilizing both spinal manipulation and medication can positively affect individuals with chronic LBP. The results of this study should be applied in clinical settings to optimize treatment outcomes in patients with chronic LBP.

정강종아리 관절가동술이 외측 발목염좌 환자의 통증, 관절가동범위 및 균형에 미치는 영향 (Effects of Tibiofibular Joint Mobilization on Range of Motion, Balance, and Pain in Patients with Lateral Ankle Sprain)

  • Background: Ankle sprains are a common clinical ankle disorder and alternations in tibiofibular joint biomechanics along with the talus are thought to contribute to its occurrence. During ankle joint dorsi flexion, proper movement requires the talus to glide posteriorly. Due to the wider front of the talus head, achieving the end range of dorsi flexion necessitates both superior and posterior glide of the distal fibula and anterior glide of the proximal fibula. The purpose of this study was to investigate the effects of tibiofibular joint mobilization on pain, range of motion, and balance in patients with lateral ankle sprains. Methods: Participants were randomly assigned to a control group (n=33) or an experimental group (n=31). Both groups underwent ankle joint mobilization three times a week for two weeks. Additionally, the experimental group received proximal and distal tibiofibular joint mobilization three times a week for two weeks. Measurements were obtained pre-intervention and post-intervention (after 2 weeks). Results: Evaluation parameters included the visual analog scale score (VAS), range of motion (ROM), and one-leg standing test (OLS). Post-intervention, both groups demonstrated significantly improved results for all assessments (p<.01). A significant intergroup difference was observed only in the ROM (p<.01) and OLS (p<.05). Conclusion: Our findings suggest that tibiofibular joint mobilization, combined with ankle joint mobilization, may be beneficial in enhancing outcomes for individuals with lateral ankle sprains.

근에너지 기법과 스트레칭 기법의 발목관절 적용이 발등굽힘 제한이 있는 만성 뇌졸중 환자의 발목 관절가동범위, 균형 능력, 보행 능력에 미치는 영향 비교 (Comparison of the Effects of Applying Muscle Energy Techniques Versus Stretching Techniques to the Ankle Joint on Ankle Joint Range of Motion, Balance Ability and Gait Ability of Chronic Stroke Patients with Limited Ankle Dorsiflexion)

  • PURPOSE: This study compared the effect of the muscle energy technique (MET) and stretching technique on ankle dorsiflexion passive range of motion, balance, and gait ability of stroke patients with limited ankle dorsiflexion. METHODS: Forty-four post-stroke patients participated. The participants were randomized into the MET group (METG; n = 22) and the stretching group (STG; n = 22). The METG was subjected to the MET to relax the dorsiflexion, while the STG was subjected to the dorsiflexion stretching technique. Both groups completed standard neurological physical therapy for 30 min per session. The intervention was conducted five times a week over 3 weeks for a total of 15 times. All participants underwent ankle dorsiflexion passive range of motion measurement and Berg Balance Scale score determination and completed a 10-m walking test and the timed up and go test before and after the intervention. RESULTS: After the 3-week intervention, both groups showed significant improvement after the intervention (p < .05). METG participants showed greater improvements in ankle dorsiflexion passive range of motion and 10-m walking test results compared to STG participants (p < .05). CONCLUSION: Both interventions improved ankle dorsiflexion passive range of motion, balance, and gait ability in stroke patients with limited ankle dorsiflexion. Moreover, the MET was superior to ankle dorsiflexion passive range of motion on the 10-m walking test.

  • Purpose: This study aimed to investigate the effects of ankle stretching with intrinsic muscles on ankle range of motion, static and dynamic balance, and gait speed in chronic stroke patients. Methods: The participants were 20 chronic stroke patients, divided into two groups, 10 in the ankle stretching with intrinsic muscles group and 10 in the slant board exercise group. Both groups performed their respective interventions once daily, with three sets per session, five times a week for four weeks. Ankle range of motion, dynamic and static balance, and gait speed were measured before and after the intervention. An independent t-test was used to compare the results between the two groups before and after the intervention. Results: Both groups showed significant improvements in ankle range of motion, dynamic and static balance, and gait speed after the intervention (p < .05). The ankle stretching with intrinsic muscles group showed more significant improvements in ankle range of motion, dynamic and static balance, and gait speed after the intervention compared to the slant board group (p < .05). These results suggest that ankle stretching with intrinsic muscles may be more effective than the slant board for certain outcomes. Conclusion: Ankle stretching with intrinsic muscles is effective in improving ankle range of motion, dynamic and static balance, and walking speed.

엉덩관절-우세 넙다리뒤근 운동과 무릎관절-우세 넙다리뒤근 운동이 넙다리뒤근 근력과 관절가동범위, 기능적 수행 수준에 미치는 영향 비교: 무작위 중재 시험 (Comparison of Effect of Hip-dominant vs Knee-dominant Hamstring Strengthening Exercises on the Muscle Strength, Range of Movement, and Functional Performance Level: A Randomized Intervention Trial)

  • Background: The most common form of injury in soccer is a hamstring strain. Eccentric bias exercises are crucial for reducing injury risk and improving sprinting performance. The purpose of this study was to compare a 4-week hip-dominant hamstring strengthening exercise program (HDE) with a knee-dominant hamstring strengthening exercise program (KDE) in youth soccer players. Method: The sample (n=31) was randomly divided into two groups: HDE (n=17) and KDE (n=14), with soccer players in HDE performing hip joint-dominant hamstring strengthening exercises and those in KDE performing knee joint-dominant hamstring strengthening exercises. Each intervention was conducted three times per week for four weeks. The outcome measures included the hamstring strength test (isometric strength test, eccentric strength test), knee joint range of motion (90/90 active knee extension; AKE) test, lower limb function test Y-balance test and Agility T-test. Results: Following the intervention, both groups showed significant improvement in isometric and eccentric hamstring strength on both dominant and non-dominant sides (p<.05). The AKE test showed significant improvement in the dominant leg (p<.05) and non-dominant leg only in KDE. The agility test significantly improved in HDE (p<.05). There was a slight improvement in the Y-balance test score; however, the difference was not statistically significant. There were no differences between the two groups in the values before, and after the intervention for all items. Conclusion: The hip-dominant and knee-dominant hamstring strengthening exercise should be coordinated and used based on the environment during hamstring injury prevention training.

멀리간 기법과 멕켄지 기법이 역학적 목 통증을 가진 아급성 환자의 통증 및 관절가동범위에 미치는 효과 비교 (Comparative Effect of Mulligan Technique and Mckenzie Technique on Pain & Range of Motion in Sub-Acute Patients with Mechanical Neck Pain)

  • Background: Neck pain is recognized as one of the common and important musculoskeletal disorders in modern society. There are many studies proving that manual therapy is one of the effective treatment methods for treating neck pain. This study compared the treatment effects of the Mulligan technique and the McKenzie technique which are widely used in clinical practice to treat neck pain, through changes in neck pain and range of motion. Methods: Forty-five sub-acute patients with mechanical neck pain were randomly divided into groups as Mulligan technique group (n=22) and McKenzie technique group (n=23). The two groups were treated 8 times for 4 weeks. The participants in both groups received home exercise program. Pain (Visual Analog Scale) and range of motion were measured before and after the treatment program. Results: In both group, the pain intensity and range of motion in all direction improved significantly (p<.001). The Mulligan technique was superior to the McKenzie technique in neck extension, lateral flexion, and rotation (p<.01), but there was no difference in neck flexion and pain intensity. Conclusion: Both the Mulligan technique and McKenzie technique were effective on neck pain and ROM in patients with mechanical neck pain. The Mulligan technique is more useful than the McKenzie technique to improve neck extension, lateral flexion, and rotation.

  • Background: This study aimed to investigate the effects of joint mobilization with movement (MWM) on pain, ankle joint range of motion, balance, and gait in patients who underwent total knee arthroplasty (TKA) and exhibited ankle joint inversion. Methods: We divided 35 patients who had undergone TKA into experimental and control groups. The intervention involved a 40-min session three times a week over 4 weeks. The control group received general physical therapy, ankle pumping exercises, Q-setting exercises, knee joint range of motion exercises, and gravity-controlled gait training. In addition to these treatments, the experimental group received MWM to evaluate knee pain, ankle joint angle, balance, and gait pre- and post-intervention. Results: Both groups experienced a decrease in pain levels, with no significant difference between the groups. Both groups also showed a significant decrease in ankle joint inversion angle, with notable differences between them. Additionally, both groups significantly increased their mean dorsiflexion angles and balance, with a significant difference observed between the groups. Although walking decreased in both groups, there was no significant difference observed between them. Conclusion: The group that received MWM treatment showed significant effects on inversion angle, dorsiflexion angle, balance, and gait. These results provide valuable insights into the potential benefits of MWM as a post-TKA intervention, while highlighting the importance of long-term follow-up studies on post-TKA lower limb alignment interventions.

  • Background: This study aimed to determine the effects of pectoralis minor stretching, lower trapezius strengthening exercise with serratus anterior strengthening exercise to find an effective therapeutic exercise for alignment, shoulder range of motion, muscle imbalance of rounded shoulder patient. Methods: This study targeted on a total of 28 male subjects, aged 20s and 30s, and randomly divided the subjects into each group, with 14 subjects each. The experimental group performed pectoralis minor stretching, lower trapezius strengthening exercise, and serratus anterior strengthening exercise, while the control group performed pectoralis minor stretching and lower trapezius strengthening exercise. Results: both group showed an increase in active range of motion for shoulder flexion, a muscle activity of the serratus anterior muscle and the lower trapezius muscle, a decrease in muscle activity of the upper trapezius muscle, and a significant improvement in scapular alignment after the intervention. In the comparison between group, the experimental group showed greater increased shoulder joint flexion range of motion, serratus anterior muscle activity, and decreased upper trapezius muscle activity than the control group. Conclusion: Performing serratus anterior strengthening exercises with pectoralis minor stretching and lower trapezius strengthening exercises is effective for improving the active range of motion in shoulder joint flexion and muscle imbalance

정상 성인의 등뼈 가동성 운동이 등뼈가동범위 및 가슴 우리 확장에 즉각적으로 미치는 영향 (Immediate Effects of Thoracic Mobility Exercises on the Thoracic Range of Motion and Chest Expansion in Healthy Adults)

  • PURPOSE: This study examined the immediate effects of thoracic mobility exercises on the thoracic range of motion and chest expansion to provide data on thoracic mobility exercises in adults. METHODS: Adults were assigned randomly to two groups: the group that performed thoracic mobility exercises (TME group; n=10) or the group that performed thoracic mobility exercises using balance tools (TMEB group; n=10). The exercises were performed for 10 min in three different positions. The range of motion and chest expansion were measured before and after the exercise. RESULTS: The within-group comparisons before and after the intervention showed significant differences in the range of motion measurements for lateral flexion and rotation in the TME and TMEB groups (p < .05); difference in measurements of chest expansion was observed only in the TMEB group (p < .05). No significant differences in the range of motion for flexion and extension were observed in either group or during chest expansion in the TME group (p > .05). The between-group comparisons showed a significant difference in left lateral flexion (p < .05), whereas no other measures differed significantly (p > .05). CONCLUSION: The TME and TMEB groups showed significant increases in the thoracic range of motion; the TMEB group showed a significant increase in chest expansion.

주관절에서 이소성 골형성의 재발방지를 위한 예방적 방사선 치료 (Prophylactic Radiotherapy to Prevent the Recurrence of Heterotopic Ossification after Surgical Intervention of the Elbow)

  • 목적: 주관절의 이소성 골형성은 수술이나 외상 후에 발생하는 흔한 합병증으로 알려져 있다. 본 연구는 주관절에서 발생하는 이소성 골형성의 재발 방지를 위해 예방적 방사선 치료를 시행한 환자들을 대상으로 후향적 분석을 시행하였다. 대상 및 방법: 주관절에 이소성 골형성으로 진단된 45명의 환자가 수술 후 방사선 치료를 시행 받았다. 이소성 골형성의 주된 원인은 골절과 외상이었고 수술 전 주된 증상은 가동관절범위의 제한이었다. 방사선 치료는 대부분의 환자에서 수술 후 2일 안에 시작하였고 1일 조사선량을 4 Gy씩 2회에 걸쳐 총 8 Gy를 조사하였다. 29명의 환자에서 $1{\sim}8$개월 동안 NSAID를 투약 받았다. 치료 후($60{\sim}145^{\circ}$) 측정한 가동관절범위에서 치료 전($0{\sim}135^{\circ}$)보다 통계적으로 유의한 향상을 보였고(p=0.028), MEPI를 이용한 기능지수 평가에서도 치료 전($15{\sim}90$)보다 치료 후($80{\sim}100$)에서 통계적으로 유의하게 증가하였다. 추적 관찰 시 방사선 사진의 판독이 가능했던 34명의 환자 중 2명에서 경미한 이소성 골형성의 재발이 있는 것으로 나타났다. 치료 후 합병증은 관찰되지 않았다. 결 론: 주관절의 이소성 골형성의 경우 수술 후 예방적 방사선 치료를 시행하는 것은 재발 방지를 위해 유용한 치료로 선택될 수 있고, 향후 더 많은 환자를 대상으로 연구가 진행되어야 할 것으로 생각된다.
