Development of an Associative Value Knowledge Base based on UMLS & LOINC Database for Semantic Medical Information Integration. (의미적 의료정보 통합을 위한 UMLS와 LOINC DB 기반의 연관 값 지식베이스 개발)

  • 최근 다양한 의료정보 시스템이 개발되어, 그 사용이 급증하고 있다. 이 들 각각의 의료정보 시스템에서 발생, 축적된 의료정보는 분산 이질의 형태를 가지며, 또한 같은 의미를 갖는 의료정보가 각기 다른 구조와 용어로 기술되어 축적되는 것이 일반적이다. 이와 같이 개별적으로 개발, 활용되어 온 의료정보를 웹 상에서 통합하여, 단일화 된 의료정보 검색 기능을 제공하기 위해서는 이들 의료정보의 의미적 연관성을 고려한 정보의 통합, 검색 기술의 개발이 필수적이다. 본 논문에서는 의미적 의료정보의 통합을 위한 UMLS와 LOINC 데이터베이스 기반의 연관 값 지식베이스의 설계 및 개발 방식을 제안한다. 웹 상에 존재하는 각종 분산 이질 형태의 의료정보는 XML을 공통 데이터 구조로 하여 통합되며, 정보 통합의 과정에서 연관 값 지식베이스를 참조하여 의미적 관련도가 높은 의료정보(구조 정보와 내용 정보)는 상호 연결되어, 진정한 의미의 정보 통합을 구현하게 된다. 지식베이스는 용어별로 식별자, 요소명, 연관값, 복수형, 동의어, 한글 이름 등의 필드틀 가지며, 현재 상담, 처방, 보험, 의료용어, 증상, 임상결과 등 적용분야 별로 작성된 연관 값 지식베이스가 구현되어 있다.

  • 최근 코로나 19에 대한 세계적인 팬데믹 선언에 의해 의료서비스의 변화가 오고 있다. 특히, 국내 법제도적으로 묶여 있던 원격 서비스에 대한 재검토가 되고 있는 실정이다. 본 논문에서 제안하는 커넥티드 라디올로지 케어 시스템은 모바일 의료영상진단기기를 기반으로 의료사각지대에 있는 환자들의 영상촬영과 이에 대한 판독 서비스를 제공하기 위한 시스템이다. 제안한 시스템은 의료환경에 적용하기 위해 환자의 개인정보 보호를 위한 방법과 절차가 반드시 포함되어야 한다. 이를 위해 전체 시스템 구조와 익명화 처리과정을 보인다. 그리고 끝으로 구축된 시스템의 수행과정을 보인다.

  • Healthcare costs are continuously increasing due to longer life expectancy and providing global healthcare services through medical tourism is new service growth engine for Korea. Several countries have well established programs and infrastructure dedicated to medical tourism. South Korea is attempting to become a major player in this domain by undertaking broad initiatives. The success of medical tourism is greatly impacted by easy access to two types of information, namely, medical and travel information. The National Health Insurance System in Korea collects huge amount of clinical and financial information from all hospitals. However, this information does not get used effectively in health and travel information systems to support medical tourism. This paper provide clear process map of medical tourism to understand how the patient and information process both medical and tourism fields also describe the need of customer and service provider. In this paper, we develop a medical tourism service system that will promote information exchange and service delivery.

  • This study investigated the differences in how medical institutions are chosen according to the motivation of medical service utilization(minor disease, major disease, chronic disease management, cosmetic). Importance order of selection for the minor disease were medical skill, kindness, rapidness, proximity, importance order of selection for major disease and cosmetic were medical skill, facilities, reputation, medical fee, importance order of selection for chronic disease management were medical skill, facilities, kindness, medical fee. From this medical study skill was a more important factor in medical institutions irrespective of motivation, subjects with the major disease tend to consider the selection of the medical institutions more important, except proximity. Medical users select medical institutions according to the motivation of visiting, and there were differences in same motivation according to socioeconomic status and information searching behavior.

  • Ubiquitous health care system, which is one of the developing solution technologies of IT, BT and NT, could give us new medical environments in future. Implementing health information systems can be complex, expensive and frustrating. Healthcare professionals seeking to acquire or upgrade systems do not have convenient, reliable way of specifying a level of adherence to communication standards sufficient to achieve truly efficient interoperability. Great progress has been made in establishing such standards-DICOM, IHE and HL7, notably, are now highly advanced. In this paper, there are suggestions that provide a test bed, demonstration and specification of how standards such a IHE, HL7, ASTM can be used to provide an integrated environment.

  • Unexpected results caused by medical service are defined as malpractice, and the doctor must bear the responsibilities following the medical treatment. Malpractice disputes are disputes between patients, who are seen as the only victim in this way, and medical personnel who do not admit to the charges. Unfortunately, in reality, an official approach to mutual understanding and communication in the instance of such disputes does not exit. Based on this background, this study was attempted to prevent similar forms of medical disputes from occurrence and provide fundamental data to prepare by analyzing the medical disputes precedent. Results of the study are as follows: First, For type of medical institution, hospitals accounted for the most part as 62.9%. Second, Among total medical disputes, the cases surgery accounted for 27.8%, violation of duty care accounted for 20.6% and that of medical treatments 11.3%. Third, For a mean agreed amount by medical department, it was the highest in obstetrics & gynecology as 38,384,000 won. In conclusion, the most desirable method of dispute resolution is to prevent a cause of dispute to the root.

  • Used medical equipment utilized in medical technology, most are not human or environmental risks likely to in normal medical technology. Therefore, the risk is close to zero so that medical devices are classified into Class 1 medical device, such as little or no risk by treating. It can be minimized without arising unnecessary and waste of time. As handlers in the medical device vendors are expanding the area of the business, we can uplift operators' commitment to the business and for the people's choice. In this paper, our research are presented to improve the legal and regulatory directions for the distribution of used medical devicesin in order to establish and promote a fair deal of diversity.
