Effects of Aluminum Compound on Kidney Tissues of Rats (알루미늄 화합물이 흰쥐의 신장조직에 미치는 영향)

  • This study was performed to investigate the effects of aluminium treatment on renal damage in rats. Seventy five male Sprague-Dawley strains were divided into five groups; the control, 250 ppm $AlCl_3$, group, 500 ppm AlCl3 group, 250 ppm $Al_2(SO_4)_3$, group, 500 ppm $Al_2(SO_4)_3$ group and kept on the diet for 2 weeks. The body weight gain was more increased in $AlCl_3$, treated of rats but decreased in $Al_2(SO_4)_3$, treated rats than the control, on based in histophysiological examination, in $AlCl_3$, treated group, thyrodization of kidney tubular lumen and interstitial inflammatory cell infiltration, abscess formation were demonstrated and in $Al_2(SO_4)_3$, treated animals destruction of glomeruli and necrosis in kidney cortex by inflammatory cells were shownd. On the other hand, blue-pigmented cast was noted in tubules of $AlCl_3$ administration group whereas focal aluminium accumulation was noted at interstitial tissue in kidney of $Al_2(SO_4)_3$, administration group.

Insurance risk analysis of kidney donors (신장 공여자의 보험의학적 위험분석)

  • Background: The kidney transplantation is increasing. The kidney donation of a living donor is more common in Korea than in other countries. Underwriters may encounter a case of a kidney donor. So we need to determine medical risk for a living kidney donor. Methods: Comparative mortality figures were calculated from a source article using mortality analysis methods. Results: Mortality Ratio of a living kidney donor was estimated to 106%, and Excess Death Rate was 0.89 per 1000. Discussion: A healthy kidney donor is quite within standard, even better in terms of medical risk.

  • Chronic kidney disease, the presence of structural and functional abnormalities in the kidneys, is associated with a lower quality of life and increased morbidity and mortality in children. Genetic etiologies account for a substantial proportion of pediatric chronic kidney disease. With recent advances in genetic testing techniques, an increasing number of genetic causes of kidney disease continue to be found. Genetic testing is recommended in children with steroid-resistant nephrotic syndrome, congenital malformations of the kidney and urinary tract, cystic disease, or kidney disease with extrarenal manifestations. Diagnostic yields differ according to the category of clinical diagnosis and the choice of test. Here, we review the characteristics of genetic testing modalities and the implications of genetic testing in clinical genetic diagnostics.

  • Type II membranoproliferative glomerulonephritis (Dense deposit disease) is an acquired primary glomerular disease characterized by electron microscopic evidence of a continuous dense membrane deposition replacing the lamina densa. It is a subtype of idiopathic membra- noproliferative glomerulonephritis, and was described as a separate entity by Berger and Galle in 1963. It frequently occurs in older chilren and young adults and the clinical course is variable, but is generally progressive. The presenting feature is nephrotic syndrome in many patients, and proteinuria and hematuria are also seen frequently. The purpose of this paper is to present a case of DDD (Dense deposit disease) from a 10 year old boy who was diagnosed as a acute poststreptococcal glomurulonephritis with protenuria, hematuria, and facial edema by renal biopsy 4 years ago. Research Related to Chronic Kidney Disease in BMI >25 Overweight Children (체질량 지수 25 이상의 과체중군에서 신장질환과 관련된 인자들에 대한 연구)

  • Purpose : Chronic kidney disease (CKD) and obesity are the worldwide public health problem. Obesity is an already well-established risk factor for CKD. The objective of this study is to evaluate the relationship between high BMI and increased risk for nephropathy by clinical data. Methods : Study group were 26 patients who had $BMI{\geq}25\;kg/m^2$ and control group were 49 patients with BMI<$25\;kg/m^2$. Both groups received renal biopsy in Kyung Hee Medical Center between 2003. Jan.-2007. Dec. BMI was calculated from measured weight and height when they were admitted to the hospital. We collected laboratory data such as CBC and blood chemistry. Results : Our hypothesis was that overweight and obesity are associated with incidence and progression of CKD. From kidney biopsy, we found IgAN 17, MesPGN 5, HSPN 2, Intestitial nephritis 1, IgMN 1 (total 26) in the study group whereas IgAN 22, MesPGN 17, HSPN 3, MGN 3, benign hematuria 2, MPGN 1, Intestitial nephritis 1, (total 49) were found in the control group. There was no significant difference between the two groups. Overweight patients demonstrated significantly higher platelet, TG, ALT, and uric acid level compared to control group. Conclusion : We identified a significant relationship between overweight and development of CKD. These results suggest that overweight children have an increased risk for CKD than those who are not obese. So, we should pay attention to children with overweight who have CKD and earlier weight management is crucial to prevent aggravation of CKD.

Development of Postoperative Self Care Mobile App for Kidney Transplantation Patients (신장이식 환자의 수술 후 자가간호 어플리케이션 개발)

  • The purpose of this research was to develop a self-care application for kidney transplantation patients based on a review of previous literatures and the results of a survey that evaluated the needs of patients. The research proceeded in ADDIE order of analysis, design, development, implementation, and evaluation stages. In the analysis phase, interviews were conducted on over 5 kidney transplantation patients. Moreover, related applications and literatures were reviewed to develop application contents. App-based postoperative self-care program composed of the following: Health teaching, measurement, checklist, views, alarms, and App information. For the evaluation phase, a survey was conducted on 9 experts and 5 patients, using a smartphone application. SPSS/WIN 21.0 program was used for data analysis. Descriptive statistics were used to analyze the validity and suitability of data obtained from experts and users. Content was validated using CVI. Expert assessment of application for the self-care after kidney transplant showed 3.5 out of 4. Patient assessment showed 3.7 out of 4. We determined that a self-care application for patients that underwent kidney transplant is helpful. Moreover, a future study is necessary to test and verify the effects of using this application on self-care and self-care knowledge.

Evaluation of Cerebral Blood Flow Using Arterial Spin Labeling in Patients with Chronic Kidney Disease (만성 콩팥병 환자들에서 동맥 스핀 표지 기법을 이용한 뇌 관류상태의 평가)

  • Purpose This study aimed to compare the brain perfusion status of patients with chronic kidney disease to a normal control group to identify any significant differences. Materials and Methods The perfusion state of the brain was measured by MRI using the arterial spin labeling technique in 36 patients undergoing hemodialysis due to chronic kidney disease and 36 normal controls. Images were then analyzed in a voxel-wise manner to detect brain areas showing significant perfusion differences between the two groups. Results Patients with chronic kidney disease showed increased perfusion in the form of large clusters across the right fronto-parieto-temporal lobe and the left parieto-occipital lobe. In addition, perfusion increased in the bilateral thalami, midbrain, pons, and cerebellum (p < 0.01, familywise error corrected). Conclusion Brain perfusion appears to increase in patients with chronic kidney disease compared to normal controls. Uremic toxicity is thought to be the cause of this increase as it can cause damage to the microscopic blood vessels and their surrounding structures.

  • 부항은 열 또는 음압(陰壓)장치에 의하여 부항단지 안에 음압을 조성하여 피부에 붙임으로써 피를 뽑거나 울혈(鬱血)을 일으키며 물리적 자극을 주어 병을 치료한다. 부항으로 얻어지는 물리적인 자극은 혈액순환을 촉진하고, 죽은피를 빼냄으로써 혈관을 자극하고 그로인해 다양한 효과를 얻는다. 따라서 본 논문에서는 신장에 해당하는 명문혈을 자극하여 신장과 관련된 음성분석 요소의 변화를 측정하였다. 이를 위해 신장에 이상이 없는 피실험자 10명을 선정하고 신장에 해당하는 명문혈을 자극하기 전과 후의 음성을 수집하였다. 실험은 음성분석 요소 중 신장과 관련된 1 Formant Bandwidth를 적용하여 신장 명문혈 자극 전과 후의 변화를 측정, 분석하였다. 실험 결과, 90%의 피실험자가 값이 감소하는 현상을 보였으며, 이를 통해 명문혈 자극에 따른 신장과 음성신호와의 상관성을 분석할 수 있었다.

A Study on Stress, Hardiness, and Self-care in Kidney Transplant Patients (신장이식환자의 스트레스, 강인성, 자가간호에 대한 연구)

  • Purpose: This study was performed to identify relationships among stress, hardiness, and the level of self-care in kidney transplant patients and use the results of this study as baseline data for developing self-care promotion programs. Method: Subjects were 41 patients who undergone a kidney transplantation surgery at least one month before and were receiving regular follow-up care via OPD. Four researchers interviewed each subjects by filling out questionnaires. Results: The mean score of the level of stress was 2.32(SD=0.54) on a 4 point scale and the mean score of hardiness was 4.68(SD=0.62) on a 6 point scale. The mean score of self-care among the subjects was 3.83(SD=0.55). There was statistically a significant positive relationship between selfcare and hardiness(r=0.42). Especially, relationships between self-care and commitment (r=.51) and control(r=.36) as the subdimensions of hardiness were significant. The item suggested as most stressful was economic burden and the item suggested as least stressful was sexual activity. Conclusion: Although transplant patients have various stressors, the positive self-care activities of the patients could be increased if the commitment and control were enhanced by proper intervention programs.

A Female Infant with Segmental Multicystic Dysplastic Kidney (분절형 다낭성 이형성신 영아 보고 1례)

  • There have only been 35 pediatric cases and one adult case reported on segmental multicystic dysplastic kidney (MCDK) from our search in PubMed, including 19 cases detected antenatally. There is little documentation of segmental MCDK, particularly concerning its natural history. Segmental MCDK can be presented atypically, making diagnosis more difficult. We report an another case with segmental MCDK. Multicystic abdominal mass detected on antenatal sonogram in this infant was diagnosed as segmental MCDK by renal ultrasonography and computed tomography. If a definitive diagnosis of segmental MCDK can be made on imaging, surgery is not required for a diagnostic biopsy.
