Light and electron microscopy of the kidney tissues on viral haemorrhagic disease in rabbits (토끼의 바이러스성(性) 출혈증(出血症) 감염시(感染時) 신장(腎臟)의 광학(光學) 및 전자현미경적(電子顯微鏡的) 관찰(觀察))

  • In order to observe the pathological changes of kidney in rabbits infected with the viral haemorrhagic disease, the kidney tissues from the 91 rabbits infected with the viral haemorrhagic disease were examined by light and electron microscopy. The results observed were as follows: 1. On light microscopic observation, the kidney lesions were identified as haemorrhagic glomerular necrosis(33.0%), membranous glomerulonephritis(20.9%), thrombotic glomerulopathy(19.8%), membranoproliferative glomerulonephritis(8.8%), mesangial proliferative glomerulonephritis(8.8%) ischemic acute tubular necrosis(7.7%), and acute serous glomerulitis(6.6%). 2. On electron microscopic observation, cytoplasmic degeneration of mesangial cells, and irregular thickening of basement membranes with electron dense granular materials were observed. In podocytes swelling of mitochondria, dilatation of endoplasmic reticulum and extensive fusion of foot processes were also observed. Nonenveloped round icosahedral picornaviral particles with a diameter of 28~33nm were detected in the cytoplasm of degenerative endothelial cells, polymorphonuclear leucoytes, and monocytes.

A Study on Self-efficacy, Coping, and Compliance in Patients with Kidney Transplantation (신장 이식 환자의 자기효능감, 대처 행동, 치료지시 이행에 관한 연구)

  • Purpose: The purpose was to examine the self-efficacy, coping, and compliance in patients with kidney transplantation. Methods: Participants consisted of 300 outpatients who underwent kidney transplantation and regularly visited hospital for health check-up. A tool developed by Ahn (2000) was used for measuring self-efficacy. A modified version of the Jalowiec Coping Scale (Jalowiec, 1987) by Hwang (2004) was used for measuring coping, and a tool developed by Ryu, Kim, and Kang (2003) was used for compliance. Data were analyzed using SPSS program version 21.0 ANOVA, Pearson's correlation coefficient, and Scheff$\acute{e}$'s test for post-hoc test. Results: Coping shows significant differences according to marital status and education. Coping was used more often among patients with ABO incompatible transplantation than those with ABO compatible. Differences in compliance were significant according to donor type, ABO incompatible, period after transplantation, and admission after the transplantation. The management of life style, stress, nutrition, and exercise in self-efficacy and compliance had lower scores than the others. Conclusion: There are significant correlations between self-efficacy, coping, and compliance in patients with kidney transplant which might be helpful for health care professionals in taking care of these population.

The Effects of an Individual Educational Program on Self-care Knowledge and Self-care Behavior in Kidney Transplantation Patients (신장이식환자에 대한 개별교육프로그램이 자가간호 지식과 자가간호수행에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effects of an individual educational program on self-care knowledge and self-care behavior in kidney transplantation patients. Methods: The kidney transplantation patients were recruited from a transplantation center, at a university hospital located in Korea. Data were collected from June 1, 2010 to January 31, 2011. The research design was a nonequivalent one group pre-post test. Forty-two subjects were participated two times individually in an educational program given by the researcher. Results: After the intervention, the participants showed a significant increase in self-care knowledge(t=-4.10, p=.000) and self-care behavior (t=-6.07, p=.000) than before the intervention. Conclusion: This results suggest that the program developed in this study can be considered an effective nursing intervention for health promotion, prevention of complication and self-care behavior in kidney transplantation patients.

  • Chronic kidney disease-mineral bone disorder (CKD-MBD) is a systemic disorder of mineral and bone metabolism caused by CKD. Patients with early-stage CKD who present with disordered regulation of bone and mineral metabolism may be asymptomatic. However, if untreated, the condition can be a significant barrier in achieving optimal bone strength, linear growth, and cardiovascular health in pediatric patients with CKD. Thus, the current study evaluated the definition, pathogenesis, diagnosis, and management of pediatric CKD-MBD.

BK polyomavirus-associated nephropathy

  • BK polyomavirus (BKPyV) is a ubiquitous virus residing in the kidney tubules and is clinically significant only in immunocompromised patients. In clinical practice, BKPyV is a causative pathogen of BKPyV-associated nephropathy (BKVAN) in kidney allograft recipients or hemorrhagic cystitis of hematopoietic stem cell transplant recipients. Currently, there is no effective treatment for BKVAN; therefore, careful monitoring and prudent modification of immunosuppression are necessary to prevent BKVAN. In this article, the epidemiology, pathophysiology, and current management strategies for BKVAN are reviewed.

  • Chronic kidney disease (CKD) causes numerous changes that destabilize homeostasis, of which anemia is one of its important complications. Anemia significantly reduces the quality of life in children with CKD and plays a crucial role in the progression of cardiovascular disease such as left ventricular hypertrophy, a major cause of mortality in those with advanced CKD. The treatment of anemia is a pivotal factor in reducing morbidity and mortality rates in children with CKD, representing one of the methods for enhancing patients' quality of life.

Light and Electron Microscopy of Gill and Kidney on Adaptation of Tilapia(Oreochromis niloticus) in the Various Salinities (틸라피아의 해수순치시(海水馴致時) 아가미와 신장(腎臟)의 광학(光學) 및 전자현미경적(電子顯微鏡的) 관찰(觀察))

  • This study was taken to examine the light microscopic and ultrastructural changes of gill and kidney of female tilapia{Oreochromis niloticus) adapted in 0%o, 10%o, 20%o, and 30%o salt concentrations, respectively, by light, scanning and transmission electron microscope. The results obtained in these experiments were summarized as follows: Gill chloride cell hyperplasia, gill lamellar epithelial separation, kidney glomerular shrinkage, blood congestion in kidneys and deposition of hyalin droplets in kidney glomeruli, tubules were the histological alterations in Oreochromis niloticus. Incidence and severity of gill chloride cell hyperplasia rapidly increased together with increase of salinity, and the number of chloride cells in gill lamellae rapidly increased in response to high external NaCl concentrations. The ultrastructure by scanning electron microscope(SEM) indicated that the gill secondary lamella of tilapia(Oreochromis niloticus) exposed to seawater, were characterized by rough convoluted surfaces during the adaptation. Transmission electron microscopy(TEM) indicated that mitochondria in chloride cells exposed to seawater, were both large and elongate and contained well-developed cristae. TEM also showed the increased chloride cells exposed to seawater. The presence of two mitochondria-rich cell types is discussed with regard to their possible role in the hypoosmoregulatory changes which occur during seawater-adaptation. Most Oreochromis niloticus adapted in seawater had an occasional glomerulus completely filling Bowman's capsule in kidney, and glomerular shrinkage was occurred higher in kidney tissues of individuals living in 10%o, 20%o, 30%o of seawater than in those living in 0%o of freshwater, and blood congestion was occurred severer in kidney tissues of individuals living 20%o, 30%o of seawater than in those living in 10%o of seawater. There were decreases in the glomerular area and the nuclear area in the main segments of the nephron, and that the nuclear areas of the nephron cells in seawater-adapted tilapia were of smaller size than those from freshwater-adapted fish. Our findings demonstrated that Oreochromis niloticus tolerated moderately saline environment and the increased body weight living in 30%o was relatively higher than that living in 10%o in spite of histopathological changes.

Risk Factors for the Progression of Chronic Kidney Disease in Children

  • Chronic kidney disease (CKD) in children is associated with various complications, including poor growth and development, mineral bone disorder, cardiovascular disease, kidney failure, and mortality. Slowing down the progression of CKD is important since CKD is often not curable. Prospective cohort studies have been conducted to understand the progression and outcomes of CKD in children, and these studies have identified non-modifiable and modifiable risk factors. Recognition of known risk factors and early intervention are important to delay the progression of kidney function decline in children.

Structural Equation Modeling of Self-Care Behaviors in Kidney Transplant Patients Based on Self-Determination Theory (자기결정성이론에 근거한 신장이식환자의 자가간호행위 구조모형)

  • Purpose: The purpose of this study was to test a hypothesis explaining direct and indirect relationships among the factors affecting self-care behaviors of kidney transplant patients, based on self-determination theory. Methods: Data were collected from 222 outpatients with kidney transplantation. The endogenous and exogenous variables of the hypothetical model consisted of healthcare provider's autonomy support, duration after kidney transplantation, basic psychological need satisfaction, autonomous and controlled motivation, depression, and self-care behaviors. Collected data were analyzed using SPSS/WIN 24.0 and AMOS 24.0. Results: The hypothetical model demonstrated a good fit: RMSEA=.06, SRMR=.04, TLI=.94, CFI=.97. Statistically significant explanatory variables for the self-care behaviors of kidney transplant patients were duration after transplantation and basic psychological need satisfaction. Healthcare provider's autonomy support was indirectly significant, while autonomous motivation, controlled motivation and depression were not statistically significant for self-care behaviors. The variables accounted for 59.5% of the self-care behaviors of kidney transplant patients. Conclusion: It is necessary to develop an autonomy support program for healthcare providers to enhance the self-care behaviors of kidney transplant patients. Preventing the deterioration of self-care behaviors will be possible by conducting this program at one year and six years post-transplantation. In addition, the results suggest the need to developing personalized autonomy support programs for healthcare providers that can meet the basic psychological need satisfaction of kidney transplant patients.

Effects of Cervus elaphus for herb-acupuncture solution on Antioxidation in Rat's kidney (녹용(鹿茸) 약침(藥針) 제제(製劑)가 흰쥐 신장(腎臟) 조직(組織)의 항산화(抗酸化) 작용(作用)에 미치는 영향(影響))

  • Cervus elaphus for herb-acupuncture solution (CEHAS) was tested for the effects of free radical generating enzyme and lipid peroxidation in rat's kidney. In vitro, levels of lipid peroxide in tissues of kidney were proportionally decreased to concentration of extracts prepared from CEHAS. They were much more decreased, when lipid peroxidation was induced with ferrous iron (FeII). Also, enzyme activities of xinthine oxidase were decreased. The ratio of type conversion of xanthine oxidase was lowered, two. But, it was not seen changes on enzyme activities of aldehyde oxidase. These results suggest that CEHAS decrease the activities of free radical generating enzymes such as xanthine oxidase which form lipid peroxide.
